Objective To study the anatomic relationship between the thoracic transverse process and adjacent bony structures and its clinical significance for thoracic screw fixation.Methods The present anatomic measurement used 45 dry cadaveric specimens of the normal adult thoracic vertebrae.We measured the distances from the horizontal midline of the transverse process to the superior,middle and inferior margius of the pedicle,the height of the anterolateral transverse process sheltered by ribs,and the position at the horizontal midline of the transverse process corresponding to the vertebral body.Results The horizontal midlines margius of the transverse processes of T1 to T10 are localized in the plane ranging from the superior to the inferior margins of the pedicle.The midlines of the transverse process are closer at T6 and T7 while more apart at T9 and T10.From T1 to T8,the anterolateral transverse process is totally or mostly sheltered by ribs,but the shelter is much less at T9 and T10.The horizontal midline of the transverse process corresponds to the upper 1/3 or middle 1/3 of the vertebral body.Conclusion Thoracic screw fixation across the transverse process to the vertebral body is safe,reliable and feasible in clinic.

Objective To study the features and values of ultrasound and Helical computed tomography (CT) in diagnosis of cystic renal cell carcinoma. Methods Twenty patients were preoperatively examined by B-mode ultrasound, color Doppler ultrasound and helical CT. Results All the cases were confirmed by operation and pathology,including 12 cases on the left side and 8 cases on the right side. On B-mode ultrasound images, all lesions appeared as unilocular or multilocular cyst in 5 cases, intratumoral septum in 10 cases,intratumoral nodule in 4 cases,solid-like mass in 1 case. Color blood flow was showed within tumors in 18 patients,in which 17 cases showed the arterial spectrum with a mean resistance index of 0.66. On enhanced CT scans, all the lesions appeared as the early uneven enhancement(19 cases), septal enhancement(6 cases) and mural nodular enhancement(4 cases) respectively. The accuracy of conventional ultrasound combined with color Doppler ultrasound was 85%, while the accuracy of the enhanced CT was 80%.Conclusions Conventional ultrasound combined with color Doppler ultrasound and enhanced CT plays an important role in diagnosis of cystic renal cell carcinoma.

Objective To determine the effect of Vibrio vulnificus cytolysin (VVC) inducing ap-optosis in human umbilical vein endothelial cell(HUVEC) and its possible mechanism. Methods The en-tire vvhA gene that encoding VVC from V. vulnificus strain GTC333 was amplified by PCR and sequenced af-ter T-A cloning. E. coli BL21DE3pET-42a-vvhA, a prokaryotic expression system of the vvhA gene, was then con-structed. Ni-NTA affinity chromatography was applied to purify the target recombinant protein rVVC, and SDS-PAGE plus Bio-Rad Agarose Image Analyzor were used to measure the output of rVVC and to determine the purity of rVVC extract. The activity of rVVC dissolving rabbit erythrocytes was detected by hemolysis test. DPNH chromotometry and TphBNa chromotometry were performed to examine the contents of LDH and K+ in the supernatants of rVVC-treated HUVEC cultures, respectively. The effect of rVVC inducing apepto-sis of HUVEC was detected by flow cytometry, rVVC was labeled with FITC and the location of FITC-labe-ling rVVC in HUVEC was observed by laser canfocal microscopy. Results The cloned whA gene had 96.09% and 98.26% similarities of nucleotide and amino acid sequences compared to the corresponding se-quences in GenBank. rVVC, with a dosage of 1 μg/ml, could dissolve rabbit erythrocytes (P<0.01). 10 μg/ml rVVC was able to promote the increases of K+ content (P<0.01) but no change of LDH content could be found in the cell supernatants. HUVEC was apoptotic after the cell was treated with 1～100 μg/ml of rVVC for 2 h. In the 5～240 min duration of co-incubation of FITC-labeling rVVC and HUVEC, the rV-VC gradually moved from surface to inner side of the membrane and then entered the cytoplasms. When FITC-labeling rVVC treated HUVEC for 30 min, most of the rVVC was found to be intracellular location. Conclusion rVVC has cytolytic activity. VVC has an ability to enter HUVEC and causes injury of HUVEC via inducing apoptosis, which may be the major pathogenic mechanism of VVC.

OBJECTIVE: To explore the correlation of mitochondrial DNA (mtDNA) variants and coronary heart disease (CHD) in a Chinese pedigree and the possible molecular mechanisms. METHODS: A Chinese pedigree featuring matrilineal inheritance of CHD who visited Hangzhou First People's Hospital in May 2022 was selected as the study subject. Clinical data of the proband and her affected relatives was collected. By sequencing the mtDNA of the proband and her pedigree members, candidate variants were identified through comparison with wild type mitochondrial genes. Conservative analysis among various species was conducted, and bioinformatics software was used to predict the impact of variants on the secondary structure of tRNA. Real-time PCR was carried out to determine the copy number of mtDNA, and a transmitochondrial cell line was established for analyzing the mitochondrial functions, including membrane potential and ATP level. RESULTS: This pedigree had contained thirty-two members from four generations. Among ten maternal members, four had CHD, which yielded a penetrance rate of 40%. Sequence analysis of proband and her matrilineal relatives revealed the presence of a novel m.4420A>T variant and a m.10463T>C variant, both of which were highly conserved among various species. Structurally, the m.4420A>T variant had occurred at position 22 in the D-arm of tRNAMet, which disrupted the 13T-22A base-pairing, while the m.10463T>C variant was located at position 67 in the acceptor arm of tRNAArg, a position critical for steady-state level of the tRNA. Functional analysis revealed that patients with the m.4420A>T and m.10463T>C variants exhibited much fewer copy number of mtDNA and lower mitochondrial membrane potential (MMP) and ATP contents (P < 0.05), which were decreased by approximately 50.47%, 39.6% and 47.4%, respectively. CONCLUSION Mitochondrial tRNAMet 4420A>T and tRNAArg 10463T>C variants may underlay the maternally transmitted CHD in this pedigree, which had shown variation in mtDNA homogeneity, age of onset, clinical phenotype and other differences, suggesting that nuclear genes, environmental factors and mitochondrial genetic background have certain influence on the pathogenesis of CHD.

Objective:To analyzed effect of stratified positive psychological intervention based on Self-reporting Inventory (SCL-90) in patients with digestive tract tumors.Methods:Totally 78 patients with digestive tract cancer from January 2019 to February 2020 were divided into observation group (78 cases) and control group (76 cases), the control group given disease knowledge education, psychological intervention, diet and exercise management, and complication prevention etc routine nursing intervention, the observation group combined with stratified positive psychological intervention based on SCL-90 assessment strategy. Cancer-related fatigue, hope level and nurse satisfaction were compared between two groups.Results:6 months after intervention, the observation group cognitive fatigue, mental fatigue, physical fatigue, the influence of fatigue on life, cancer-related fatigue score were respectively (7.24±1.21), (7.58±1.16), (5.16±1.24), (7.65±1.42), (27.63±4.32) points. The control group were respectively (8.35±1.32), (8.02±1.12), (5.87±1.16), (8.72±1.56), (30.96±4.56) points. There were statistically significant of difference between the two groups ( t values were 2.265-5.442, P<0.05 or 0.01). Observation group Grade Ⅲ cancer-related fatigue total score were respectively (27.34±4.32), (27.65±4.25), (27.63±4.41) points, and the control group were respectively (29.56±5.12), (30.23±4.35), (40.77±5.45) points. There were statistically significant of difference between the two groups ( t values were 8.528, 2.163, 5.812, P<0.05 or 0.01). Observation group attitudes towards reality and the future, maintain close relationships with others, adopt positive action, hope level total score were respectively (10.35±2.01), (9.87±1.43), (10.16±1.42), (30.28±5.20) points, and control group were respectively (9.12±1.45), (9.15±1.32), (9.24±1.36), (27.51±4.36) points. There were statistically significant of difference between the two groups ( t values were 3.245-4.104, P<0.05 or 0.01). Observation group second and third grade hope level total score were respectively (30.24±5.20), (31.87±5.45) points, control group were respectively (27.24±4.23), (19.41±3.52) points. There were statistically significant of difference between the two groups ( t values were 2.271, 5.644, P<0.05 or 0.01). 6 months after intervention, The observation group nurse satisfaction 94.87% (74/78) higher than that control group 82.89% (63/76) ( χ2 value was 5.623, P=0.021) . Conclusion:Stratified positive psychological intervention based on SCL-90 assessment strategy help to ensure the balance of the intervention effect, alleviate cancer-related fatigue, Promote development of hope levels, and improve nurse satisfaction.
