Attention deficit hyperactivity disorder is a common neurodevelopmental disorder characterized by persistent attention deficit, hyperactivity, and impulsive behaviors that are not consistent with developmental age.Academic and vocational difficulties, social exclusion, and delinquent behaviors are manifested in daily life.It is also commonly accompanied by psychiatric problems.At the same time, mental problems are common, and the overall quality of life is greatly affected, placing a heavy burden on society as well as the family.International attention deficit hyperactivity disorder experts have developed a common and comprehensive International Classification of Functioning, Disability and Health core set of classifications for assessing individual functioning in attention deficit hyperactivity disorder.
2.Analysis of dose-related factors in small intestine exposure to preoperative radiotherapy for rectal cancer

Objective:To analyze the related factors affecting the dose to the small intestine exposure to preoperative radiotherapy for rectal cancer, aiming to provide reference for alleviating the adverse reactions of radiotherapy for rectal cancer.Methods:Medical record data and radiotherapy plan information of 138 rectal cancer patients who received intensity-modulated arc radiotherapy at Sun Yat-sen University Cancer Center from May 2021 to February 2023 were retrospectively analyzed. Tumor staging, tumor location, gender, age, planned bladder volume, body mass index (BMI), and small intestine irradiation dose volume were subject to Spearman correlation analysis. Further grouping and comparison were conducted based on the correlation results. Independent sample non parametric tests were used for inter group comparison.Results:The main factors related to the small intestine irradiation dose volume were tumor location, gender, planned bladder volume, and BMI. Tumor location was weakly correlated with the small intestine V 5 Gy-V 45 Gy. Gender was weakly correlated with the small intestine V 30 Gy-V 45 Gy. Planned bladder volume was weakly negatively correlated with the small intestine V 20 Gy-V 45 Gy. BMI was weakly negatively correlated with the small intestine V 10 Gy-V 45 Gy. Grouping comparison analysis showed that the small intestine V 5 Gy-V 45 Gy of rectal cancer patients in the low position group was significantly smaller than those in the middle and high position groups (both P<0.05), and there was no significant difference between the middle and high position groups ( P>0.05). Female rectal cancer patients had higher V 30 Gy-V 45 Gy levels than male counterparts ( P<0.05). The small intestine V 20 Gy and V 25 Gy levels in the planned bladder volume <200 ml group were significantly higher than those in the 200-400 ml and >400 ml groups (all P<0.05), whereas there was no difference between the 200-400 ml and >400 ml groups ( P>0.05). The small intestine V 30 Gy-V 45 Gy levels in the 200-400 ml group were significantly lower than those in the <200 ml group, but higher than those in the >400 ml group, and the differences were statistically significant (all P<0.05). Regarding BMI comparison among groups, the small intestine V 15 Gy-V 45 Gy in the low body weight group was significantly higher than those in the other three groups (all P<0.05). There were no significant differences among the normal, overweight, and obese groups (all P>0.05). Conclusion:In preoperative radiotherapy for rectal cancer, more attention should be paid to the dose to the small intestine in patients with middle and high position rectal cancer, female patients, and patients with low body weight.
3.Genotyping of Mycoplasma pneumoniae and macrolide antibiotic resistance gene mutations in children with Mycoplasma pneumoniae pneumonia

Objective:To analyze the epidemiological characteristics of Mycoplasma pneumoniae (MP) infection and its genotyping in children with community-acquired pneumonia (CAP), as well as macrolide resistance and gene mutation genotyping, in order to provide evidence for the prevention and treatment of Mycoplasma pneumoniae pneumonia (MPP) in children.Methods:MP positive cases in 620 hospitalized children at Maternal and Child Health Hospital of Sanshui District, Foshan City with CAP were detected.P1-RFLP genotyping was performed for the MP positive cases.The distribution of the MP positive, type Ⅰ and type Ⅱ in different years and different genders, ages as well as seasons were analyzed.The mutations of macrolide resistance genes in MP were detected.The differences of A2063G and A2064G mutations of the drug-resistant mutant gene in each year, gender, age and season were analyzed.Results:Among 620 children with CAP, 198 were MP positive, and the infection rate was 31.94%.There was little difference among the years.The infection rate was higher in female than that in male.The infection rate gradually increased with the increase of age, and the highest infection rates were found in pre-school age and school age.The infection rates in summer and autumn were significantly higher than those in spring and winter.Among 198 children, P1-RFLP classification showed that 157 (79.29%) cases were P1-Ⅰ and 41 (20.71%) cases were P1-Ⅱ.There was no significant difference in the distribution of type Ⅰ and type Ⅱ in each year, gender, age as well as season.A total of 143 cases were tested for mutation of macrolide resistance gene, in 125 of them, MP gene mutation resulted in drug resistance, and the overall drug resistance rate was 87.41%.MP gene mutation led to drug resistance in 125 children, 66 (52.80%) cases had A2063G mutation and 53 (42.40%) cases had A2064G mutation.There was no significant difference between two types in each year, gender, age and season.Conclusion:MP infection rate of CAP among children in our hospital is 31.94%, and is more common in femal, and the infection rates are highest in pre-school age and school age, with summer and autumn as the season of high incidence.The P1-RFLP typing showes no significant differences among the years, genders, ages and seasons.The drug resistance gene mutation among children is high.Time, gender, age and season does not affect drug resistance gene mutation.
4.Application effect of responsibility nursing for cerebral thrombosis patients

Objective To analyze effect of responsibility nursing for cerebral thrombosis patients.Methods A total of 76 patients with cerebral thrombosis were randomly divided into two groups according to the coin-tossing method.The control group (n =37) was given routine nursing,and the observation group (n =39) was given responsibility system.The nursing effect and recurrent rate of disease of two groups were compared.Results The standardized rates of language function,body function,and satisfaction and the rate of nursing were 87.2%,82.1%,and 94.9%,respectively,which were significantly higher than 62.2%,56.8%,73.0% in the control group (P <0.05).The hospital stay in the observation group was significantly shorter than that of the control group [(16.8 ± 5.0) d vs.(26.4 ± 5.3) d,P < 0.05].The activity scores of daily living in the two groups were significantly higher than that on admission (P < 0.05),and the observation group was higher than the control group (P < 0.05).After one year follow-up,the relapse rate of cerebral thrombosis in observation group was significantly lower than that in control group (5.1% vs.24.3 %,P <0.05).Conclusion Responsibility management can not only establish a good relationship between nurses and patients,but also improve quality of nursing,promote patients'language function,limb function and activities of daily living,shorten the course of the disease,and prevent the recurrence of disease,so it is worth promoting.

6.Analysis of non-tumor diseases affecting the diagnosis and treatment of cancer patients

Objective:To investigate the types of non-tumor diseases in patients with cancer, and to explore the effects of those dis-eases on the diagnosis and treatment of cancer patients. Methods:We collected the medical records of cancer patients from January 2013 to December 2017 in Peking University Cancer Hospital, and screened for non-tumor diseases. The clinical records of the patients in this group were analyzed retrospectively, and the effects of those diseases on the diagnosis and treatment of tumors were dis-cussed. Results:Of the 1,323 cases of inter-hospital consultation, 1,153 cases of non-tumor disease (87.2%) were selected. There were 773 men (67.0%) and 380 women (33.0%) included. The median age was 62 (14-90) years. The primary tumor types included lung can-cer, gastric cancer, lymphoma, colorectal cancer, esophageal cancer, breast cancer, malignant melanoma, liver cancer, cholangiocarci-noma/gallbladder cancer, pancreatic cancer, and other tumors. Non-neoplastic diseases included cardiovascular disease in 356 cases (30.9%), respiratory system disease (17.0%) in 196 cases, digestive system disease in 107 cases (9.3%), skin and venereal diseases in 81 cases (7.0%), nervous system lesions (6.4%) in 74 cases, urinary system disease in 72 cases (6.2%), blood disease in 70 cases (6.1%), en-docrine and metabolic diseases in 47 cases (4.1%), autoimmune disease in 23 cases (2.0%), and other diseases (11.0%) in 127 cases. Impact on tumor diagnosis and treatment was as follows:direct, 771 cases (66.9%);no influence, 313 cases (27.1%);and uncertain, 69 cases (6.0%). Conclusions:Cardiovascular disease is a major non-tumor disease associated with cancer. Non-neoplastic diseases are important factors affecting the diagnosis and treatment plans of cancer.
7.Relationship between serum osteocalcin levels and glucolipid metabolism in elderly type 2 diabetic patients with non-alcoholic fatty liver disease

Objective To explore the relationship between serum osteocalcin levels and glucolipid metabolism in elderly type 2 diabetic patients with non-alcoholic fatty liver disease (NAFLD).Methods Data collected from 97 patients with type 2 diabetes mellitus(T2DM)admitted to the Department of Geriatric Endocrinology of the First Affiliated Hospital of Zhengzhou University from June 2014 to April 2015 were retrospectively analyzed.Patients were divided into the T2DM group(type 2 diabetic patients without NAFLD,n= 47)and the NAFLD group(T2DM patients with NAFLD,N = 50).Healthy elderly subjects (n = 30)from the same period served as the control group.Body mass index(BMl),osteocalcin,fasting blood glucose,fasting insulin,homeostasis model assessment for insulin secretion index (HOMA-β) and insulin resistance (HOMA-IR),glycosylated hemoglobin(HbA1c),total cholesterol,triglyceride,high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL C)were compared between the 3 groups.Results Levels of fasting blood glucose,fasting insulin,HbAlc,total cholesterol,triglyceride,LDL-C and HOMA IR were higher,while levels of HDL-C,HOMA-β and osteocalcin were lower in the T2DM and NAFLD groups than in the control group(all P<<0.05).Levels of BMI,fasting glucose,fasting insulin,HbAlc,total cholesterol,triglyceride,LDL-C and HOMA-IR were higher and levels of osteocalcin were lower in the T2DM group than in the NAFLD group(all P<0.05).Pearson correlation analysis showed that the osteocalcin level was negatively correlated with fasting blood glucose,HbA1C,HOMA IR and BMI(r=-0.701,0.442,-0.337 and 0.543,P<0.05 or P<0.01),and positively correlated with HOMA-β (r = 0.341,P < 0.05) in the NAFLD group.With serum osteocalcin as the dependent variable,multiple linear regression results showed that fasting blood glucose was an independent influencing factor for serum osteocalcin(β=-1.57,P<0.05)in the fatty liver group.Conclusions Serum osteocalcin levels significantly decrease in elderly T2DM patients with NAFLD,are closely correlated with glucolipid metabolism,and may have some important clinical significance in the prevention and treatment of NAFLD in elderly patients with type 2 diabetes.
8.Clinical characteristics and therapy analysis of Graves ophthalmopathy in children and adolescents

Background Juvenile Graves ophthalmopathy has a low prevalence and few relevant studies.Analyzing and reviewing the clinical features and therapeutic effectiveness of juvenile Graves ophthalmopathy is helpful to its diagnosis and management.Objective This study was to evaluate the clinical characteristics of Graves ophthalmopathy and its management in children and adolescents.Methods The clinical data of 54 eyes from 29 patients with Graves ophthalmopathy who were diagnosed in Shanghai Changzheng Hospital from January 2007 to December 2012 were retrospectively analyzed.The ocular manifestations,thyroid function,CT or MRI testing results were collected,and the activity of Graves ophthalmopathy was scored based on the criteria of CAS.Artificial tears was topically administered in 44 eyes with CAS ≤ 2.In the eyes with CAS ≥ 3,corticosteroids drug was systemically used in 3 patients,and orbital decompression surgery and excision of Müller muscle were performed in 3 eyes of 2 patients respectively.The follow-up was carried out for 1.5-6 years.The treatment outcomes were evaluated according to the reduction of exophthalnos and the improvement of upper eyelid retraction.Results The patients were 5-18 years old with an average age of 12.9 years old.Out of 29 Graves ophthalmopathy patients,5 males and 24 females were included.The initial clinical manifestations were proptosis,eyelid retraction and swelling,and accompanied by conjunctival congestion and hypophasis.The best corrected visual acuity (BCVA) was ≥ 0.8 in all the eyes.CAS scores wcre 0-2 in 48 eyes of 26 patients and ≥ 3 (active Graves ophthalmopathy) in 6 eyes of 3 patients.The increase of orbital adipose volume was exhibited in all the eyes and the enlargement of extraocular muscle was revealed in parts of eyes by CT/MRI.Laboratory examination showed normal thyroid function in 12 patients (41.4%),hyperthyreosis in 15 patients (51.7%) and hypothyroidism in 2 patients (6.9%).The ocular symptom was improved in 20 eyes of 11 patients (37.9%),stabilized in 29 eyes of 16 patients (55.2%) and worsen in4 eyes of 2 patients (6.9%) in following-up duration.Conclusions Graves ophthalmopathy occurs much more in female than in male.The clinical manifestations are mild,with low activity and good prognosis in children and adolescents Graves ophthalmopathy.
9.Retrospective analysis of 4 biochemical indicators on 4042 of urban and rural residents

Objective To understand the abnormal status of alanine aminotransferase (ALT),fasting blood glucose (GLU),to-tal cholesterol (TC)and triglyceride (TG)in urban and rural residents,to provide the basis for making the effective prevention measures,health education and health promotion to community work more targeted and effective.Methods The data of physical ex-amination results of 4 042 residents were analyzed retrospectively,the levels and abnormal rates of ALT,TG,TC,GLU were com-pared between different gender and age.Results Among 4 042 residents,the abnormal rate of male was significant higher than that of female,but the abnormal rates of TG,TC of female were significant higher than those of male(P <0.05).The highest rates of ALT,TG,TC,GLU in male were in 20-<40 years old,40-<60 years old,60-<80 years old,≥80 years old.The highest rates of ALT,TG,TC,GLU in female were in 0-<20 years old,60 -<80 years old,≥ 80 years old,≥80years old.Conclusion The health status of urban and rural residents is not optimistic,the community intervention,health guidance should be strengthened to prevent and control the occurrence and development of diseases and improve the health level of urban and rural residents.
10.Effect of stroke-associated pneumonia on the prognosis of the elderly hospitalized for acute stroke in the intensive care unit

Objective To investigate the risk factors for death within 30 days of stroke associated pneumonia(SAP) in elderly patients of the intensive care unit (ICU).Methods Clinical data of 116 patients with SAP who were admitted to ICU were reviewed.The predicting factors of death within 30 days were analyzed through variable analysis method.Results The hospitalization periods of the SAP group and control group were (27±8) d and (12±5) d,the difference was statistically significant(t =1 1.30,P =0.002).The most common pathogenic bacteria in SAP were Klebsiella pneumoniae,Bauman Acinetobacter,Escherichia coli,and gram negative bacteria.There was significant difference between the two groups in term of mortality rate(37.9%(44/ 116) of SAP group,22.0% (22/100) of control group,x2 =6.423,P =0.011).Logistic regression analysis showed that basic diseases(OR =2.778,95% CI:1.205-6.401),high CRUB-65 score (OR =1.978,95% CI:0.871-11.098),lower Glasgow coma score(GCS) (OR=3.601,95%CI:0.244-9.477),bucking(OR=3.020,95%CI:1.305-10.603),mechanical ventilation(OR=2.654,95%CI:1.176-5.990),shock(OR=2.636,95% CI:1.164-5.969) and high plasma CRP level(OR=2.333,95%CI:1.046-5.206) were risk factors for SAP (all P<0.05).In Cox regression analysis,a GCS score of < 9 was an independent risk factor for 30 d mortality of patientswithSAP(HR=7.23,95%Cl:2.24-20.11,P=0.001).Conclusion SAP is one of the serious complications of the elderly hospitalized for the acute stroke in the ICU,which may affect the prognosis of the patients.
