1.Effect analysis of high filtration rate continuous renal replacement therapy in severe acute pancreatitis patients

Objective To analysis the effect of high filtration rate continuous renal replacement therapy in severe acute pancreatitis pa-tients. Methods According to the admission time,56 severe acute pancreatitis patients were divided into the observation group ( from Jan. 2012 to Mar. 2013,26 cases) and the control group (from Apr. 2010 to Dec. 2011,30 cases). The control group was given CRRT for hemopu-rification while the observation group was given high filtration rate CRRT. The vital signs of rescue period,treatment effect,inflammatory medi-um level,fatality rate and prognosis of the two groups were compared. Results There was no significant change in terms of temperature after operation in the two groups. Pulse and breathing in the observation group were lower than that of the control group with a significant difference (P<0. 05). APACHEⅡ and ALT of the observation group were lower than that of the control group 24 h after the operation,and the differ-ence is of statistical significance(P<0. 05). Conclusion It would get a satisfactory effect to use high filtration rate CRRT in severe acute pancreatitis patients since it can obtain better treatment effect with more stable vital signs, lower inflammatory medium level and lower fatality rate.
2.Signal Amplification Effect of Non-fluorescent Zinc Sulfide Nanoparticles Cluster for Detection of Trace Proteinsbiological Molecules

Cation exchange (CX) reaction for the non-fluorescent ZnS nanocrystal clusters (NCCs) can be used to detect trace biomolecules . Nano clusters synthetized by hydrothermal synthesis are porous. So they can quickly release large amounts of Zn2+ from through cation exchange ( CX) reaction and nano cluster, generate fluorescent signal under the action of zinc reagent to detect fluorescence. The relationship between the release efficiency, target binding force of Zn2+ and its average diamete was investigated when the average diameter was 44 nm, 86 nm and 144 nm in this experiment. Results showed that the smallest nano cluster exhibited the highest cation exchange efficiency, and 71 percent of Zn2+ closed could be released by microwave radiation within 2 min. When the sandwich method of NCCs of 44-nm was used to detect immunoglobulin E (IgE) in a sandwich assay, the limit of detection (LOD) was 5 ng / L, which was 1000 times lower than that of ELISA. It turns out that CX for the ZnS NCCs is superior to the conventional signaling strategies in its high amplification efficiency, robustness, and biocompatibility.

Objective To investigate the clinical value of homocysteine (Hcy) ,cystatin C (Cys C) and high‐sensitivity C‐reactive protein (hs‐CRP) in atherosclerosis (AS) .Methods Totally 100 cases of newly diagnosed patients of AS were choosed as the ob‐servation group ,36 cases condition improved markedly of AS after treatment as the treatment group ,and 120 cases of normal medi‐cal groups as the control group ,the Hcy ,Cys C and hs‐CRP in all subjects were detected respectively .Results Hcy ,Cys C and hs‐CRP levels of observation group were significantly higher than those of normal groups ,and the difference was statistically signifi‐cant (P< 0 .05) .While the Cys C and hs‐CRP levels of treatment group were significantly lower than the those of observation group ,the difference was also statistically significant (P< 0 .05) .Conclusion The detection of Serum Hcy ,Cys C and hs‐CRP might be help to the diagnosis ,treatment evaluation and prognosis of AS .
4.Evaluation on the intervention of Nitroglycerin on ~(99m)TC-MIBI myocardial tomography imaging for myocardial viability

Objective To study the value of the intervention of Nitroglycerin(NTG) on ~(99m)Tc-MIBI myocardial tomography imaging in the estimation of myocardial viability.Method According to the analysis of autologous electrocardiogram(ECG),a total of 66 patients with unstable angina(UA) was divided into group A and group B.The patients in group A were without old myocardial infarction and those of group B were with old myocardial infarction.The patients in the two groups were respectively underwent the resting ~(99m)Tc-MIBI myocardial tomography imaging and the NTG administration intervened the next day.The imaging was collected and tested by the computer.Results Of 594 myocardial segments in the 66 cases,242 segments(40.7%) in testing myocardial imaging were proved to be perfusion abnormal,while after the intervention of NTG administration,the perfusion of 114 segments(47.1%) had been improved according to the imaging.Conclusion The intervention of NTG administration on ~(99m)Tc-MIBI myocardial tomography imaging for myocardial viability is simple,safe,objective and accurate,which provides a forceful means for the post-operative evaluation and the selection of indication before the rebuilding of coronary artery of the UA patients.
5.Rescue stenting after failure of mechanical thrombectomy for acute cerebral large artery occlusive infarction

Objective To investigate the efficacy and safety of rescue stenting after failure of mechanical thrombectomy for acute cerebral large artery occlusive infarction. Methods A total of 29 patients with acute cerebral large artery occlusive infarction who failed mechanical recanalization, admitted to our hospital from January 2016 to March 2018, were chosen in our study; 18 patients accepted rescue stenting (stenting group) and 11 patients did not accept rescue stenting (non-stenting group). Comparative analyses of final vascular recanalization rate, complication rate, and clinical outcomes in the stenting and non-stenting groups were performed. Results The final recanalization rates of the stenting group and non-stenting group were 88.9% (16/18) and 36.4% (4/11), respectively, and the good prognosis rates were 55.6% (10/18) and 18.2% (2/11), respectively; the differences were statistically significant between the two groups (P<0.05). The incidence of symptomatic intracranial hemorrhage (11.1% [2/18] vs. 18.2% [2/11]) and mortality (22.2% [4/18] vs. 45.5% [5/11]) showed no significant differences among the two groups (P>0.05). Conclusion Rescue stenting after mechanical recanalization of acute cerebral large artery occlusive infarction can significantly improve the clinical prognosis without increasing risk of intracranial hemorrhage.
6.Preliminary experience of endovascular revascularization for chronic long segment internal carotid artery occlusion

Objective To explore the feasibility,safety and effect of endovascular revascularization for chronic long segment internal carotid artery occlusion. Methods The cases of chronic long segment internal carotid artery occlusion who were treated by endovascular revascularization in our center from May 2015 to April 2017 were reviewed. Eleven cases met the inclusion criteria:the duration of the occlusion was more than three weeks and the segment of the occlusion was beyond the petrosal segment of internal carotid arteries from the initial segment. All of the cases had the related symptoms and had declining cerebral perfusion. The analysis index included:baseline information,radiological information,perioperative results, clinical follow-up and imaging follow-up. The imaging follow-up index were the re-stenosis or re-occlusion of the revascularized artery. Results The occlusion was recanalized successfully in 9 of 11 patients,the two procedures were abandoned after repeated attempts and the guide wire could not reach the true lumen when navigating in the cavernous segment. Six cases of the nine successfully recanalized cases accepted perfusion-weighted imaging scan. Cerebral perfusion of all the cases were improved. Thrombus shifting was observed in one case and occluded a subbranch,mechanical thrombectomy was performed successfully,the case was recovered well without sequela. No symptomatic stroke or death was happened in the perioperative period. All of the nine cases who successfully recanalized acquired clinical follow-up,median follow-up time was ten months(4—28 months). No ischemic stroke and death happened after the procedures. Seven of nine cases improved in the clinical symptoms. Five cases accepted the imaging follow-up. The meantime was six months. No re-occlusion was happened. Conclusions Endovascular revascularization for chronic long segment internal carotid artery occlusion is feasible,safe,and short-term effective. More clinical research is needed to verify the long-term effect.
7.Effect of anesthetic sensitivity to propofol after biliary decompression in common bile duct ligation rats

Objective To investigate if anesthetic sensitivity to propofol will be restored after biliary decompression.Methods Twenty-four adult male SD rats were randomly assigned into 3 groups:sham group (group S),irreversible obstructive jaundice group (group Ⅰ) and reversible obstructive jaundice group (group R).The serum total bilirubin (TBL) and total bile acid (TBA) concentratins were detected in the rat blood samples collected from the caudal vein before and after the operation,3,7,14,21 d respectively.Propofol was administered to measure the time of loss of righting reflex and recovery pre or 7th and 21th day post ligation.Results Serum TBL and TBA in group Ⅰ and serum TBA in group R were significantly higher than that in group S on 3rd,7th,14th,21th day post surgery(P<0.05).Compared with group S,seum TBL in group R were significantly high on 3rd,7th,14th day post-surgery.Serm TBL and TBA in group R were significantly lower than group Ⅰ on 14th,21th day post-surgery (P<0.05).Compared with group S,the time to loss of righting reflex in group I and group R were significantly shortened and the time to recovery were significantly increased on 7th day post-surgery (P<0.05).Conclusion Obstructive jaundice could significantly potentiate the ability of propofol to induce a loss of righting reflex,and the increased anesthesia sensitivity will be restored after biliary decompression.

Objective:
To observe the efficacy and safety between Pegfilgrastim (PEG-rhG-CSF) and Recombinant human granulocyte colony stimulating factor (rhG-CSF) in hematological malignancy after allogeneic hematopoietic stem cell transplantation (allo-HSCT) .
Methods:
157 patients after allo-HSCT were enrolled in this study from June 2015 to November 2016. Two agents of G-CSF were used to stimulate hematopoietic recovery after transplantation. There were 65 cases in PEG-rhG-CSF and 92 cases in rhG-CSF groups. Patients in PEG-rhG-CSF group were given a single subcutaneous dose of 6 mg on the first day and +8 d, while cases in rhG-CSF group were given in dose of 5 μg·kg-1·d-1 by subcutaneous injection from +1 d continuing to neutrophils more than 1.5×109/L, and then the indicators and survival rates in two groups after transplantation were compared.

OBJECTIVES:
To explore the effects of hypoxic and hypobaric conditions on blood gas and erythrocyte-related indicators in rats.
METHODS:
SD male rats were exposed to low-pressure hypoxic conditions simulating an altitude of 6500 m in a small or a large experimental cabin. Abdominal aortic blood samples were collected and blood gas indicators, red blood cells (RBCs) count, and hemoglobin (Hb) content were measured. The effects of exposure to different hypoxia times, different hypoxia modes, normal oxygen recovery after hypoxia, and re-hypoxia after hypoxia preconditioning on blood gas indicators, RBCs count and Hb content were investigated.
RESULTS:
The effect of blood gas indicators was correlated with the length of exposure time of hypoxia and the reoxygenation after leaving the cabin. Hypoxia caused acid-base imbalance and its severity was associated with the duration of hypoxia; hypoxia also led to an increase in RBCs count and Hb content, and the increase was also related to the time exposed to hypoxia. The effects of reoxygenation on acid-base imbalance in rats caged in a small animal cabin were more severe that those in a large experimental cabin. Acetazolamide alleviated the effects of reoxygenation after leaving the cabin. Different hypoxia modes and administration of acetazolamide had little effect on RBCs count and Hb content. Normal oxygen recovery can alleviate the reoxygenation and acid-base imbalance of hypoxic rats after leaving the cabin and improve the increase in red blood cell and hemoglobin content caused by hypoxia. The improvement of hypoxia preconditioning on post hypoxia reoxygenation is not significant, but it can alleviate the acid-base imbalance caused by hypoxia in rats and to some extent improve the increase in red blood cell and hemoglobin content caused by hypoxia.
CONCLUSIONS
Due to excessive ventilation and elevated RBCs count and Hb content after hypoxia reoxygenation, oxygen partial pressure and other oxygenation indicators in hypoxic rats are prone to become abnormal, while blood gas acid-base balance indicators are relatively stable, which are more suitable for evaluating the degree of hypoxia injury and related pharmacological effects in rats.
