Reconstructing thumb opposition function using modified transfer of proper extensor tendon of the fifth digit is one of the methods to treat old median nerve injury-induced thumb opposition dysfunction.To explore the long-term effect of this method,97 patients undergoing modified transfer of proper extensor tendon of the fifth digit to reconstruct thumb opposition function in Department of Hand Surgery,Affiliated Hospital of Beihua University between January 2003 and December 2007 were retrospectively analyzed,and 21 were followed up.The thumb opposition function postoperatively was evaluated using Changing of Hand Function Evaluation Standard.After reconstructing thumb opposition function,length of thumb (T),distance moving to (mo) and moving away from (Mo) centre of palm in opposition,i.e.during closing and leaving centre of palm distance between middle point of thumb interdigit transverse striation and middle point of middle finger MP joint transverse striation.In addition,their ratios were calculated.The patients were followed-up for 7.5 years (ranging 1-14 years).It is a good therapy to reconstruct thumb opposition function using modified transfer of proper extensor tendon of the fifth digit.

3.Long Intergenic Non-Protein Coding RNA 665 Regulates Viability, Apoptosis, and Autophagy via the MiR-186-5p/MAP4K3 Axis in Hepatocellular Carcinoma

PURPOSE: Long intergenic non-protein coding RNA 665 (LINC00665) plays a vital role in the development of cancer. Its function in hepatocellular carcinoma (HCC), however, remains largely unknown. MATERIALS AND METHODS: The expressions of LINC00665, miR-186-5p, and MAP4K3 were determined by qRT-PCR. Cell viability and apoptosis were evaluated by MTT and flow cytometry, respectively. Autophagic puncta formation was observed by fluorescence microscopy. Bioinformatics analysis, luciferase reporter assay, RNA immunoprecipitation, and RNA pulldown were performed to identify associations among LINC00665, miR-186-5p, and MAP4K3. Western blot was utilized to examine the expressions of MAP4K3, Beclin-1, and LC3. Tumor growth was evaluated in a xenograft model. RESULTS: Elevations in LINC00665 were observed in HCC tissues and cells. The overall survival of HCC patients with high levels of LINC00665 was shorter than those with low levels. In vitro, LINC00665 depletion inhibited viability and induced apoptosis and autophagy. miR-186-5p interacted with LINC00665 and was downregulated in HCC tissues and cells. Upregulation of miR-186-5p inhibited viability and induced apoptosis and autophagy, which were attenuated by upregulation of LINC00665. MAP4K3 was found to possess binding sites with miR-186-5p and was upregulated in HCC tissues and cells. MAP4K3 depletion inhibited viability and induced apoptosis and autophagy, which were attenuated by miR-186-5p inhibitor. In vivo, miR-186-5p expression was negatively correlated with LINC00665 or MAP4K3 in HCC tissues, while LINC00665 was positively correlated with MAP4K3. LINC00665 knockdown suppressed tumor growth. CONCLUSION: LINC00665 was involved in cell viability, apoptosis, and autophagy in HCC via miR-186-5p/MAP4K3 axis, which may provide a new approach for HCC treatment.

4.the Value of serum cystatin C and Homocysteine in different stage of renal damage in type 2 diabetes

Objective To investigate the value of serum α1 acid glycoprotein(AAG) and Homocysteine(Hcy) in different stage of renal damage in type 2 diabetes(T2DM ) .Methods a total of 128 patients with T2DM were recruited from January 2013 to Octo‐ber 2013 ,According to 24 h urinary albumin excretion rate (UAER) were divided into the first group ,the second and third groups , and choose 80 healthy cases over the same period as the normal control group .Serum AAG ,Hcy ,creatinine (Cr) and blood urea ni‐trogen (BUN) were detected ,the test results were analyzed statistically .Results There were significant differences in type 2 diabe‐tes mellitus group ,early diabetic nephropathy group ,clinical diabetic nephropathy group of the serum AAG ,Hcy levels(P<0 .05) , there were positively correlated (r=0 .673 ,P<0 .05) between UAER and serum AAG levels ,and were positively correlated (r=0 .35 ,P<0 .05) between UAER and Hcy levels .Conclusion Combined detection of serum AAG ,Hcy have a significant for early diagnosis of diabetic nephropathy and assessing the degree of renal injury in diabetes .
5.Serum cystatin C and urinary albumin were measured on the diagnosis of early renal function injury

Objective To investigate the serum cystatin C (CysC) and urinary albumin excretion (mALB) joint detection in di‐agnosis of early diabetic nephropathy injury .Methods 120 cases of determination of CysC and microalbuminuria in type 2 diabetic patients and 40 healthy people and related clinical and biochemical parameters were measured ,and the various factors by correlation analysis ,the determination of differences in early renal damage in diabetes and healthy people .Results the early diabetic nephropa‐thy group ,diabetic nephropathy group serum cystatin C and urinary microalbumin is higher than the normal control group ,CysC and mALB level had significant difference ,the difference was statistically significant (P<0 .05) ,the mALB was positively correla‐ted with serum CysC levels (r=0 .662 ,P<0 .05) ,and no obvious correlation of serum Cr ,BUN levels and mALB .Conclusion trace albumin serum cystatin C and urine is reflecting early renal function injury sensitive and reliable index ,serum cystatin C com‐bined detection of urine micro albumin contributes to kidney disease early diagnosis ,treatment and prognosis judgement .
6.Application of Systems Software for Hospital Infection Control

OBJECTIVE:To improve information management of hospital infections.METHODS:The hospital infection monitoring and management software developed by the hospital where the authors work was applied to monitor the risk factors of hospital infections at real time and selectively.RESULTS:The software could be applied to investigate the situation of antibacterial use in hospital,evaluate the rationality of drug action,monitor the incidence of hospital infections,and analyze the risk factors of hospital infections.CONCLUSIONS:The hospital infection monitoring and management software is con-venient and easy to operate,which can enhance the efficiency and effect of the management of hospital infection.
7.Effects of jaw tracking technique on the absorb dose of PTV and OARs in intensity-modulated radiation therapy for nasopharyngeal carcinoma

Objective To study the effects of using jaw tracking technique with Smart LMC algorithm on the absorbing dose of planning target volume (PTV) and organs at risk (OARs) in dynamic intensity-modulated radiation therapy (IMRT) for nasopharyngeal carcinoma (NPC).Methods Field fluencies of 10 cases of NPC patients were optimized using DVO algorithm on Eclipse TPS (11.0),and according to the same optimal fluence,MLC operation files were calculated using jaw tracking technique and jaw fixing technique respectively,dose distribution was calculated with AAA algorithm and jaw tracking IMRT plan (JT-IMRT) and jaw fixing IMRT plan (JF-IMRT) were generated respectively.Collimators' position at the plan implementation was observed,and the total number of plans' monitor units (MU),the dose of PTV,the absorb dose of OARs,and the actual fluence verification pass rate were compared.Results The collimators' opening gap distances in 166 control points of the JT-IMRT reduced in both X and Y directions in the field,compared to that of the JF-IMRT.Total number of the JT-IMRT's MU increased by 3.59%-11.63%.There was no statistical significant difference between the doses of the PTV.Statistical significance was found in the differences between maximum dose (Dmax) of brainstem,spinal cord,crystal,optic nerve,the mean dose (Dmean) and D50% of parotid and their decreased values after therapy (t=5.70-8.66,P<0.05).The actual fluence verification pass rate of the JT-IMRT was higher than that of the JF-IMRT.There was a significant difference between the results (t=5.18,P<0.05).Conclusions The JT-IMRT plan of the smart LMC algorithm is more tolerant to the radiation leakage between inter-and intra-leaf.The dose of OARs is lower,while the dose calculation precision and the verification pass rate are higher,the actual radiation dose is more accurate and reliable.Therefore it is more suitable for clinical applications.
8.Therapeutic Observation of Acupuncture for Malignant Pleural Effusion of Moderate Volume

Objective To observe the clinical efficacy of acupuncture in treating malignant pleural effusion (MPE).Method Eighteen MPE patients confirmed by histopathological and/or cytological examinations were treated by acupuncture by selecting Yunmen (LU 2), Qimen (LR 14), Zhangmen (LR 13), Jingmen (GB 25), Guanyuan (CV 4), Zhongji (CV 3), Shuidao (ST 28), and Guilai (ST 29) as the major points. The treatment was given once every other day, 3 times a week, 4 weeks as a treatment course, and the therapeutic efficacy was evaluated 1 course later.Result Of the 18 patients, 6 got complete remission, 9 got partial remission, 2 achieved stable condition, while 1 didn’t respond to the treatment, and the total effective rate was 83.3%. Of the 5 cases with encapsulated pleural effusion, 2 got complete remission and 3 got partial remission, and the total effective rate was 100.0%. The amount of pleural effusion (anteroposterior diameter of the fluid), comprehensive score of symptoms, item scores of the symptoms, and Karnofsky Performance Status (KPS) score were changed significantly after treatment (P<0.01).Conclusion Acupuncture is an effective method in treating MPE of moderate volume, and it can swiftly improve the symptoms and promote the quality of life.
9.Changes of serum UHRF1 levels in perioperative patients with esophageal squamous cell carcinoma

Objective To detect serum ubiquitin-like with PHD and ring finger domains 1 (UHRF1) levels in the patients with esophageal squamous cell carcinoma (ESCC),and analyze their differences among different clinicopathological features subgroups and changes during the perioperative period.Methods Serum samples from 130 preoperative ESCC patients,62 patients 1 week after operation,14 patients 1 week and 2 weeks after operation and 67 healthy controls,and clinicopathological data from ESCC patients were collected.Serum UHRF1 levels were detected by ELISA,and the differences among different groups were analyzed with independent t test,paired t test or one-way ANOVA.Results Serum UHRF1 levels in 130 preoperative ESCC patients were significantly higher than that in healthy controls (t =7.680,P < 0.01),and they were related to the ESCC patients' tumor size,differentiation degree,tumor invasion,lymph nodes metastasis and pTNM stage (P < 0.05),but unrelated to the patients' age,gender,tumor location and types (P > 0.05).Serum UHRF1 levels in 62 postoperative patients were significantly lower than that before operation (t =5.530,P < 0.01),but similar to that in healthy controls (t =1.622,P > 0.05).The serum UHRF1 levels before operation,1 week after operation and 2 weeks after operation in 14 ESCC patients decreased gradually (F =7.595,P < 0.01).Conclusion Serum UHRF1 levels may be a potential biomarker for dynamically monitoring perioperative ESCC patients.
