OBJECTIVE:To establish the quality standard of Compound Yiganling dropping pills.METHODS:Fructus Silybi and Fructus Schisandrae Chinensis were identified by TLC.The content of Silybin was determined by HPLC.RESULTS:The qualitative identification was of high resolution and free of interference of negative substance.The linear ranges of Silybin was 0.09~0.54 mg?mL-1,and the average recovery was 97.38%(RSD=1.19%,n=6).CONCLUSION:The method can be used for the quality control of Compound Yiganling dropping pills.
2.Relationship between serum parathyriod hormone level and renal anemia in patients with chronic kidney dis-ease

Objective To explore the relationship of serum parathyriod hormone ( PTH) level with renal ane-mia in patients with chronic kidney disease (CKD).Methods Serum levels of PTH,Hb,Hct,BUN,Cr were observed in 126 patients with CKD at stages 3-5,the changes of 42 patients in maintenance hemodialysis(MHD) with recombi-nant human erythropoietin (rhEPO) and calcitriol-1.25(OH)2D3 treatment for three months were determined. Results The serum PTH level was significantly increased in CKD patients at stage 3 and more in CKD patients at stage 4 and 5.There were positive relationship between blood PTH and BUN ,Cr(P<0.01),but a negative relation-ship between PTH and Ccr,Hb(P <0.01).42 patients of MHD treated with rhEPO,1.25(OH)2D3 after three months,according to the changes of HCT ,the effective rate was 73.8% and the ineffective rate was 26.2%.The PTH,Hb,HCT of the effective group had statistically significant differences compared with the ineffective group ( all P<0.01).Hb and Hct significantly increased and PTH significantly decreased in the effective group .Conclusion CKD patients had decreased renal function ,elevated blood PTH .High serum PTH might aggravate renal anemia and fail to respond to rhEPO.The high serum PTH corrected by using 1.25(OH)2D3 might improve renal anemia,with high plasma PTH correcting .
3.Comparison of surgical efficacies of transurethal enucleation of prostate with transurethal resection of prostate

Objective To compare the surgical effects of transurethal enucleation of prostate (TUEP)with transurethal resection of prostate (TURP) by a retrospective analysis of clinical data.Methods The patients in this study were divided into the groups of TUEP (enrolled in 2010 and 2011,n =77) and TURP (enrolled in 2009,n =27) with prostate larger than 60 g and smaller than 60 g respectively.Comparisons were made between the two groups in operation time,blood loss volume and weight of resected prostate.Results In the group of patients with prostate larger than 60 g,there was no significant difference in prostate weight ((88.5 ± 9.2) g vs.(107.0 ± 15.30) g,P =0.255),operation time ((91.5 ± 8.8) min vs.(118.3 ± 20.2)min,P =0.083),and weight of resected prostate ((48.0 ± 4.6) g vs.(58.4 ± 5.4) g,P =0.32) between the TUEP (53 patients) and the TURP (12 patients) groups.There was significant difference in blood loss ((110.0 ± 16.4) ml vs.(193.3 ± 22.3) ml,P =0.011) between the two groups.In the group of patients with prostate smaller than 60 g,there was no significant difference in prostate weight ((43.1 ± 3.2) g vs.(36.8 ± 3.4) g,P =0.072),operation time ((62.7 ±6.8) min vs.(69.3 ±6.2) min,P =0.431),blood loss ((56 ± 5) ml vs.(110±20) ml,P=0.082),and weight of resected prostate ((26.3 ±2.4) g vs.(23.6 ±2.1) g,P =0.291) between the TUEP (24 patients) and the TURP (15 patients) groups.Conclusion Compared with TURP,TUEP has the advantages of less blood loss in the treatment of patients with prostate larger than 60 g.With the improvement of surgeon' s experience and development of operation techniques,TURP will be replaced by TUEP.In the treatment of patients with prostate smaller than 60 g,the operation modality can be chosen by the surgeon based on his experience and proficiency.

Objective To explore the effect of situational clinical probation fourth-step method in Nursing Managementteaching in clinic.Methods We introduced innovative teaching method in clinicsituational clinical probation fourth-step method in Nursing Management.Firstly,it took students to learn the construction of the management structure in clinic (observation),then interview management persons (interview),and playing roles in management activities (role-play),at last interactive discussion (seminar).Results Through the SCP fourth-step learning,the nursing students' learning ability was strengthened,the students' ability to innovate and the learning interest in nursing management were inspired.Conclusions Application of situational clinical probation fourth-step method in Nursing Managementteaching in clinic shows better effect,which should be widely applied in clinic practice.
5.Effect of early CRRT on correlation kidney injury in patients with severe acute pancreatitis

Objective To investigate the effects of early continuous renal replacement thempy(CRRT)on correlation kidney injury and prognosis in patients with severe acute pancreatitis(SAP).Methods According to the digital table,40 SAP patients were randomly divided into two groups:the control group(21 cases)and CRRT treat-ment group(19 cases).The levels of serum creatinine,urea nitrogen,IL -1,IL -6,TNF -α,the APACHEⅡscore, the incidence of mechanical ventilation were compared between the two groups.Results The levels of serum creati-nine,urea nitrogen were significantly lower in the CRRT group than those in the control group in day 3(t =2.836, 2.952,P =0.003,0.004);The levers of IL -1,IL -6,TNF -αwere significantly lower in the CRRT group than those in the control group in day 3(t =2.376,2.414,2.197,P =0.351,0.028,0.042);The APACHE II score,inci-dence of mechanical ventilation,the fatality rate in the CRRT group were significantly lower than those in the control group in day 3[(20.16 ±5.23)points vs.(13.83 ±4.48)points,14 cases(66.7%)vs.6 cases(31.6%),8 cases (38.1%)vs.2 cases(14.3%),t =4.572,χ2 =4.912,4.043;P =0.0329,0.027,0.044].Conclusion Early CRRT therapy can eliminate the IL -1,IL -6 and TNF -αin SAP patients,can reduce the incidences of AKI,which may provide more clinical benefits in the early phase of SAP.
6.Impact of mild cognitive impairment on elderly well-being: the mediating effect of self-efficacy

Objective:To explore the effect of mild cognitive impairment on the subjective well-being of the elderly and the mediating role of self-efficacy in it. To provide an effective basis for health management and clinical intervention in the elderly with mild cognitive impairment.Methods:A total of 164 patients with mild cognitive impairment from December 2018 to June 2019 were surveyed by the General Self-efficacy Scale, the Montreal Cognitive Assessment Scale, and the Memorial University of Newfoundland Scale of Happiness Scale.Results:The overall score of self-efficacy was 21.41±3.98, and subjective well-being was 26.44±7.35. There was a positive correlation among cognitive function, self-efficacy and subjective well-being, except that there was no significant correlation between self-efficacy and cognitive function( r values were 0.263-0.687, P<0.01). Self-efficacy played a part in mediating between cognitive function and subjective well-being in patients with mild cognitive impairment, accounting for 37.8% of the total effect. Conclusions:Mild cognitive impairment can also have a negative impact on the subjective well-being of the elderly. Among them, self-efficacy plays a particularly important role in mediating. Interventions about self-efficacy on the elderly with mild cognitive impairment may have more significant effects.
7.Swimming training facilitate the improvement of microcirculations in cochlea after chronic hypoxia with ligustrazine in guinea pigs

ObjectiveTo explore the effect of the swimming training on the microcirculations in cochlea in guinea pigs after chronic hypoxia.Methods28 guinea pigs were divided into normal control group (group A) which were outside of cabin and chronic hypoxia model group (group B) randomly. 4 weeks after, the model group were divided into the model control group (group B1), medical treatment group (group B2) and swimming training with medical treatment group (group B3). The blood flow of the cochlea (CBF) were measured using laser Doppler flow. The spiral ligament spreaded-slice optical lens was used to observe the vascular veins, capillary appearance and red blood cells (RBC) counts.Resultsthe CBF of group A was(98.075±5.08)% ,while that of group B1 was(86.80±2.12)%(P<0.01 vs group A), that of group B2 was (89.14±4.12)%(P<0.05 vs group B1), that of group B3 was (91.18±5.02)%(P>0.05 vs group B2, but P<0.01 vs group B1).ConclusionSwimming training can facilitate the improvement of the swelling on the thin blood vessel and the microcirculation in cochlea under chronic hypoxia environment with ligustrazine in guinea pigs.
8.Risk factors of hyperamylasemia in diabetic ketoacidosis

Risk factors of hyperamylasemia in 152 diabetic ketoacidosis(DKA)patients who had no acute pancreatitis were examined.Serum levels of hemodiastase,natrium,kalium,chlorine,calcium,phosphonium,creatinine,carbon dioxide combining power,osmotic pressure,blood glucose,blood lipids and uric acid were measured.With hemodiastase and other 14 factors as independent variables,the multiple linear regression analysis was performed and the partial regression coefficient was estimated.Blood glucose,serum natrium,osmotic pressure and triglyceride entered the regression equation,and their partial coefficients were 10.26,10.35,2.21 and 8.00,respectively.The constant quantitv was -2162.06.Compared with amylase-normal group,hyperamylasemia group showed statistically significant difference in blood glucose,serum natrium,osmotic pressure and triglyceride levels.Higher blood glucose and triglyceride might be the primary causes of hyperamylasemia.
9.Microsurgery treatment with small craniotomy lateral fissure approach for hypertensive basal ganglia intracerebral hemorrhage

Objective To investigate the efficacy of microsurgery treatment with small craniotomy lateral fissure approach for hyperten-sive basal ganglia intracranial hemorrhage. Methods From December 2013 to February 2015, 30 patients with hypertensive basal ganglia intracranial hemorrhage ( blood loss within 30~60 mL) were analyzed respectively. Summarized the methods and techniques of this microsur-gery treatment and analyzed its advantages and disadvantages compared with the traditional operation. Results The hematomas were excluded more than 90% which were proved by reexamination after opreation. There was one case of temporal lobe infarction,but there was no re-bleed case and death case. The recovery of consciousness is good, and the limb function and language function were improved by early rehabilitation treatment. Conclusion Microsurgery treatment with small craniotomy lateral fissure approach for hypertensive basal ganglia intracerebral hemorrhage has the advantages of minimal invasion,complete hematoma removal and low complication and disability rates, which is worthy of promotion and application.
10.Clinicopathological analysis of seven cases of pemphigoid nodularis

Objective To analyze clinical characteristics of pemphigoid nodularis. Methods Seven cases of pemphigoid nodularis were included in this retrospective study. The clinicopathological features of pemphigoid nodularis were retrospectively analyzed, including patients′gender, age at onset, clinical manifestations, treatment and follow?up. Results Of the 7 patients, 4 were female and 3 were male, with the median age at onset being 59 years. Clinical manifestations mainly included markedly pruritic prurigo?like lesions and nodular lesions with or without blisters. The most common misdiagnoses were nodular prurigo and eczema. Pathology showed epidermal hyperplasia and subepidermal clefts, collagen hyperplasia in the papillary dermis, and perivascular infiltration of lymphocytes and eosinophils in the superficial dermis in all the patients. Direct immunofluorescence assay showed linear deposition of IgG and C3 in the basement membrane zone, and indirect immunofluorescence was positive in 2 patients. Glucocorticoids combined with immunosuppressants were effective in all the patients. Conclusions Pemphigoid nodularis is easy to be misdiagnosed, and immunopathology is helpful for its diagnosis. Glucocorticoids combined with immunosuppressants appear to be effective for its treatment.
