1.A STUDY OF NEUROENDOCRINE CELLS IN THE LUNG OF HUMAN FETUSES

The neuroendoerine cells (NE cell) of the lung have been studied in 61 human fetuses by silver stain, immunocytochemical method and electron microscopic technique. The NE cells are mainly located in epithelium of conducting portion with various appearance. Some NE cells are neuron like with several processes. The 5-HT positive NE cells are often found in the epithelium of the primitive alveoli with pyramidal, squamous and tadpole-like appearance. Based upon their ultrastructure, the NE cells are rich in organelles and dense core vesicles (DCV). The NEB is often covered by a layer of cuboidal cells, but no nerve fibers are found among the cells in the NEB.The argyrophil cells usually appear in the 12th week, and reach the maximum number by 20th week. Before the 19th week, the argyrophil cells in the upper segment of the conducting portion are more numerous than those in the lower segment, while after 19th week the condition is just reverse. A regression curve made from the number of argyrophil cells and the thickness of intrapulmonary arteries in different fetal ages shows positive correlation.

Primary aldosteronism is caused by adrenocortical lesions characterized with autonomous secretion of aldosterone and inhibited renin-angiotensin system.Multidisciplinary Team (MDT) integrates the advantages of multiple clinical specialists,and is important for improving the diagnosis and treatment of primary aldosteronism.Here,we briefly reviewed the role of MDT in primary aldosteronism.
3.Enlightenment of Yin Fire Theory on the TCM Treatment of Brittle Diabetes

[Objective] To explore the relationship between brittle diabetes and yin fire, so as to more comprehensively understand fragile diabetes from TCM, broaden the clinical diagnosis and treatment, and improve the clinical curative effect. [Methods] The theory of yin-fire theory was summarized, and the relationship between brittle diabetes and yin-fire was expounded from the aspects of academic origin, pathological basis, clinical manifestation and so on, proposed treatment principles and prescriptions.[Result] The yin fire theory originated from Nei Jing. Yin and yang do not rise, wet muddy bets, endogenous fire, the fire is too weak. Fever is the reason for the formation of diabetes, Yin fire is an important reason for fluctuations in blood sugar; brittle diabetes expelling Bu Zhong Yi Qi, lifting fine, Xie Yin fire, from Dongyuan Qingshu Yiqi Decoction to the treatment of fragile diabetes sugar made a good effect. [Conclusion] The theory of yin fire has important practical significance for diabetes, especially brittle diabetes. It is a new idea and method to treat brittle diabetes by the thought and method.
4.Clinical study of treatment of locally advanced non-small cell lung cancer by intensity modulated irradiation combined with Pemetrexed and Cisplatin

Objective To observe the therapeutic effect and toxicity of chemoradiation of locally advanced non-small cell lung cancer by intensity modulated irradiation combined with pemetrexed and cisplatin. Methods Fourty-two patients presented with Ⅲ - stage non-small cell lung cancer(Ⅲ、 25 patients, ⅢB 17 patients)received concurrent chemoradiotherapy. Intensity modulated irradiation technique was used to the total dose of 66 Gy and concurrent chemotherapy consisted of pemetrexed 500 mg/m2 on Day 1 and cisplatin 75 mg/m2 on Day 1 by intravenous infusion once every 3 weeks at the initiation of radiation.Patients received 4 cycles of chemotherapy. Results Thirty-four patients finished the whole of therapeutic schedule. And 2 patients received radiation with total dose of 54 Gy, 2 patients 56 Gy;3 patients received 2 cycles of chemotherapy, 1 patients 3 cycles of chemotherapy. Total effective rate was 79%. There were 2 patients with ≥3 grade marrow depression, 3 patients with 3 grade radiation esophagitis, 4 patients with ≥2 radiation pneumonitis, and 1 patient with 3 grade mucositis. The 1-year survival rate was 65%.Conclusion Recent effect was favourable and toxicity was tolerable for chemoradiation of locally advanced non-small cell lung cancer by intensity modulated irradiation combined with pemetrexed and cisplatin.
5.Clinical observation of fluorouracil implanting to abdominal cavity during operation of advanced colonic cancer patients

Objective To explore the safety of fluoreuracil implanting to abdominal cavity during operation of advanced colonic cancer patients, search for new approach for preventing from local relapse of colonic cancer. Methods Divide 60 patients accepting radical colonic cancer surgery into 2 groups, treating group and control group. Treating group are implanted of fluorouracil along the wounded surface of tumor and draining path of lymph nodes. To observe the curing of wounded surface, stomal leak ,wound infection, intestinal obstruction and hospital day. Results There is no significant differences between treating group and control group in stomal leak,wound infection, intestinal obstruction and hospital day (P>0.05). Conclusion It is safe to implant fluoruuracil during radical colonic surgery, which is an effective path of regional chemotherapy after operation.
6.Paroxysmal nocturnal hemoglobinuria with cerebral venous sinus thrombosis:a case report and literature review

Paroxysmalnocturnalhemoglobinuria(PNH)isadefectdiseaseofacquiredclonal hematopoietic stem cel s. It can be expressed as hemolytic anemia, hemoglobinuria, and venous thrombosis. Cerebral venous sinus thrombosis (CVST) is a rare but serious complication of PNH. Here we report a PNH patient with CVST and reviewthe relevant literature. For patients who have the risk factors for CVST and neurological symptoms, such as headache and increased intracranial pressure, should early conduct brain imaging examination and make the diagnosis clear, and give an active treatment in the aspects of anticoagulation, dehydration, eliminating the causes of disease, and controling complications.
7.Effects of prenatal ethanol exposure on the neuronal number in visual cortex of off spring mouse

Aim To study ethanol-induced changes in the development and neuronal number of visual cortex in C57BL /6 mice. Methods Female mice were fed with ethanol during pregnancy . The neuron density (ND) and cortical thickness (CT) in visual cortex of off spring mice were measured at either P0, P7 and P14 with hematoxylin and eosin (H.E) and Nissl staining. Results Embryonic death and malformationswere found in the ethanol-treated groups. Malformations, such as microcephaly,anencephaly and myeloschisis with spinabifida, etc were found in late-term embryos. The malformation rate was 12%. Compared with control group, the development of visual cortex in ethanol-treated groups was delayed, and its lamination was in disorder. The neuron polarity was disturbed. Neuron loss was found after ethanol exposure. At various ages, the neuron density in ethanol-treated groups was lower than that in control group(P

Objective To study ethanol-induced the neuroapoptosis of visual cortex in offspring mice. Methods Pregnant female mice were fed by intubating alcohol daily,beginning on E5(embryonic,E) and continuing through the pup's birth.The neuroapoptosis in P0,P7 and P14 visual cortex was visualized by Caspase-3 activity immunohistochemistry and Terminal deoxynucleotidyl transferase biotin-dUTP nick end labeling(TUNEL) staining. Results Usually,the pup's birth days would delay one or two days after ethanol exposure.Moreover,ethanol induced reabsorption of fetus and malformations,such as microcephaly,anencephaly and myeloschisis with spinabifida and so on,were found in the study.Apoptosis index in ethanol treatment groups was obviously higher than that in control at either P0,P7 or P14(P
9.Value of pathological features in defining the longitudinal margin of the clinical target volume in radiotherapy of esophageal squamous-cell carcinoma

Objective To study the characteristics of microscopic spread of esophageal squamous-cell carcinoma (ESCC) and the influence of clinicopathological features on it to help define the clinical target volume (CTV) margin in radiotherapy.Methods Sixty-four surgical specimens of ESCC were observed for longitudinal microscopic spread per centimeter both proximally and distally from the tumor.The shrinkage ratio of each specimen was calculated and used for tissue incision.Results The further the distance beyond the tumor, the lower the incidence there was of microscopic spread.Positive rates of microscopic spread in group 3 cm of proximal and distal were 4.8% and 6.9%, respectively, and in group 4 cm were both 3.6%.Tumors longer than 5 cm in length,with poorer differentiation, lymph nodes metastasis and more aggressive phase had higher positive rates (79.3% vs 45.7%,77.4% vs 45.5%,76.0% vs 51.2%,70.5% vs 40.0%,χ2=7.52,6.86,3.91,5.36;P=0.006,0.009,0.042,0.021).Differentiation and tumor length were main factors contributing to microscopic spread (χ2=0.19,4.82;P=0.020,0.017).Conclusions To cover 95% of the microscopic spread,a margin of 3.0 cm proximal and 4.0 cm distal beyond gross tumor volume is needed and as to 90%, a margin of 3.0 cm both proximal and distal is needed.Moreover, the influence of pathological features should be taken into account.
10.Correlation study between brain N-acetylaspartate and executive function in alcoholics

Objective To investigate the changes of the N-acetylaspartate(NAA) concentrations in different brain regions and executive function skills in alcohol dependence,and to study the relationship between NAA levels and cognitive functions in subjects.Methods 49 male,non-smoking,alcohol-dependent patients and 45 healthy control subjects were measured with Proton 1H Magnetic resonance spectroscopy (MRS) and Wisconsin Card Sorting Test (WCST).Results Alcoholics had lower NAA/Cr ratios in prefrontal grey matter(GM) (1.59± 0.13) and white matter(WM) (1.58±0.12) regions and performed poorly on executive function tests compared to controls (P<0.001).NAA/Cr in left prefrontal regions positively correlated with certain parameters of EF testing (number of correct responses 30.37± 3.73,perseverative errors 11.49± 3.39,random errors 6.18± 2.64,categories completed 2.08± 1.59)in alcoholic group (P<0.01).NAA/Cr in prefrontal WM regions correlated with certain parameters of EF testing in alcoholic group (number of correct responses r=0.379,categories completed r=0.433,P< 0.05).Conclusion Long-term,chronic alcoholism will damage neuronal viability and cognitive functions,which suggests that NAA concentrations can reflect the extent of damage of cognitive functions with decreased levels reflecting neuronal loss.
