1.Analysis of influencing factors of failure of internal fixation after closed reduction and internal fixation for intertrochanteric fractures in the elderly

Objective To explore the process of clinical treatment of senile intertrochanteric fractures in patients with closed reduction and internal fixation,to analyze the related factors leading to operation failure,and to lay the foundation for the clinical treatment in Department of Orthopedics.Methods 100 elderly patients with intertro-chanteric fracture were treated with closed reduction and internal fixation,and the related factors leading to the failure of the operation were analyzed.Results Through the statistics of all patients,7 cases were found in patients with internal fixation failure.The internal fixation failure rates of the patients with 4 ~6 class Singh index,basic diseases, TAD >25mm,and the using of hormone drugs were significantly higher than those of the patients with 1 -3 class Singh index,basic diseases,TAD≤25mm,and no using hormone drugs,the differences were statistically significant (χ2 =6.246,4.532,4.532,6.439,all P <0.05).And the fracture type,Singh index,basic diseases,TAD >25mm and Cleveland partition were internal fixation failure risk factors,the difference was statistically significant(all P <0.05).Conclusion In the application of closed reduction and internal fixation for the treatment of intertrochanteric fractures in the elderly patients,the fracture type,Singh index,basic disease,TAD >25mm and Cleveland partition can be used as the risk factors of internal fixation failure.

3.Disease burden of occupational pneumoconiosis in Jinhua City from 2009 to 2021

Objective:
To investigate the disease burden of occupational pneumoconiosis from 2009 to 2021 in Jinhua City, Zhejiang Province, so as to provide insights into formulating occupational pneumoconiosis prevention and control measures.
Methods:
Data on occupational pneumoconiosis in Jinhua City from 2009 to 2021 were collected through Occupational Disease and Health Hazard Monitoring Information System including demographic characteristics, disability level, age, pneumoconiosis type and stage, and analyzed years live dwith disability (YLD), years of life lost (YLL) and disability adjusted life years (DALY) by different genders, pneumoconiosis stages, pneumoconiosis types, ages and disability levels.
Results:
A total of 244 occupational pneumoconiosis cases were diagnosed in Jinhua City from 2009 to 2021, of which 225 cases were male, accounting for 92.21%. The median age of onset was 44.50 (interquartile range, 23.00) years. There were 229 deaths, with a median age of 78.00 (interquartile range, 13.00) years. The DALY was 3 791.88 person-years, YLD was 2 428.21 person-years (64.04%) and YLL was 1 363.67 person-years (35.96%). The YLD was 3 647.8 person-years in men, which was higher than 144.08 person-years in women (P<0.05). The YLD of asbestosis cases was lower than that of silicosis, coal workers' pneumoconiosis and other pneumoconiosis (P<0.05). The YLL was not statistically significant in the stage and type of occupational pneumoconiosis (P>0.05). The YLD was higher among cases at ages of less than 40 years, and lower among cases at ages of 60 to 69 years and 70 years and over (P<0.05); the YLL was lower among cases at ages of 70 years and over (P<0.05). The cases with second/third level of disability had the highest YLD, followed by the cases with fourth/fifth level of disability, and the cases with sixth/seventh level of disability had the lowest YLD (P<0.05).
Conclusions
The disease burden of occupational pneumoconiosis in Jinhua City from 2009 to 2021 is mainly induced by disability, and the disease burden caused by silicosis is the highest. With the progress of pneumoconiosis stages and disability levels, the life loss continues to increase.
4.Protective effects of nitidine chloride on rats during myocardial ischemia/reperfusion

BACKGROUND: Studies indicated that lipid peroxidation due to increase of free radical is the key factor of ischemia/reperfusion injury.Shinyleaf pricklyash root extracts, rutaceae plant, is bitter in taste, no stimulation, which has the effects of promoting qi, relieving pain, promoting blood circulation by removing blood stasis, dispelling wind and dredging collaterals and antioxidation.OBJECTIVE: To observe the influence of nitidine chloride on myocardial ischemia/reperfusion injury in rats and analyze its mechanism.DESIGN: Randomized controlled study.SETTING: Departmentof Pharmacology and Department of Chemistry,Guangxi Medical University.MATERIALS: A total of 60 healthy Wistar rats were selected, half male and half female, with the body mass of 250-300 g. Nitidine chloride was provided by Department of Chemistry, Guangxi Medical University, batch number 20050609. MS4000U biological signal quantitative record analysis system, 722N evident spectrophotometer, hydrochloric acid verapamil (batch number 020701, 2 mL in each), malondialdehyde (MDA) and superoxide dismutase (SOD) kit were purchased from Guangzhou Longfeida Technology Co., Ltd., Shanghai Precision Scientific Instruments Corporation, Shanghai Harvest Pharmaceutical Co, Ltd. and Nanjing Jiancheng Bioengineering Institute, respectively. Hitachi 7170A full automatic biochemistry analyzer was also applied.METHODS: The experiment was performed at the Department of Pharmacology and Department of Chemistry, Guangxi Medical University between June 2004 and May 2006. ①Totally 60 healthy Wistar rats with normal ECG (half male and half female) were randomly divided into 6 groups:sham operation group, model group, 2, 1, 0.5 mg/kg nitidine chloride groups, positive control group with 10 rats in each group. The rats in the sham operation group received threading without deligation, and 90 minutes later the experiment was accomplished. Other 50 rats received left anterior descending branch of coronary artery deligation, ischemia for 30 minutes reperfusion for 60 minutes. 2 mg/kg verapamil, 2,1,0.5 mg/kg, 5 mL/kgnitidine chloride, saline of the same volume were injected into femoral vein in rats of the positive control group, different doses nitidine chloride groups and model group, respectively 10 minutes before deligating left anterior descending branch of coronary artery. ②Monitoring was conducted successively with standard limb Ⅱ lead ECG when performing reperfusion. Type,incidence rate and duration of cardiac arrhythmia were recorded within 60minutes. Change of ST segment was also recorded after reperfusion for 15minutes and 60 minutes. ③At the end of experiment, serum myocardial enzymology indexes were measured wi th full automatic biochemistry analyzer.MDA content and SOD activity in myocardial tissues were examined with thiobarbituric acid(TBA) method and xanthine oxidase (XOD) method, respectively. ④Measurement data and enumeration data between two groups were compared with t test and x2 test. MAIN OUTCOME MEASURES: Occurrence of cardiac arrhythmia, ECG ST segment elevation, change of serum myocardial enzymology indexes, MDA content and SOD activity of myocardial tissues in rats of each group.RESULTS: A total of 60 rats were involved in the result analysis. ①Degree of cardiac arrhythmia and ECG ST segment elevation of rats: The emergency time of cardiac arrhythmia in 1 and 2 mg/kg nitidine chloride groups was significantly later than that in the model group (P < 0.05,0.01). The duration of cardiac arrhythmia in the 1 and 2 mg/kg nitidine chloride groups and positive control group was obviously shorter than that in the model group (P < 0.05-0.01). The incidence rates of various kinds of cardiac arrhythmia were markedly less than those in the model group (P < 0.01). The degree of ST segment elevation at reperfusion for 15 and 60 minutes was remarkably lower than that in the model group (P < 0.05-0.01). ②Serum myocardial enzyme level: It was significantly higher in the model than the sham operation group after 60-minute myocardial ischemia and reperfusion (P?.01). Activity of myocardial enzyme in the 1 and 2 mg/kg nitidine chloride groups was remarkably lower than that in the model group (P < 0.01,P < 0.05). The level of myocardial enzyme decreased with the increase of nitidine chloride. It was lower significantly in the positive control group than the model group (P < 0.05-0.01 ). ③SOD activity of myocardial tissues: It was markedly lower in the model group than the sham operation group after 60-minute myocardialischemia and reperfusion (P < 0.01); It was dramatically higher than in the 1 and 2 mg/kg nitidine chloride groups than the model group (P < 0.01). The activity also increased with the increase of nitidine chloride. ④MDA content of myocardial tissues: It was distinctly higher in the model group than the sham operation group after myocardial ischemia reperftsion for 60 minutes (P < 0.01). It was remarkably lower in the 1 and 2 mg/kg nitidine chloride groups than the model group (P < 0.01). The content decreased with the increase of nitidine chloride. It was obviously lower in the positive control group than the model group (P < 0.05 ).CONCLUSION: ①1 and 2 mg/kg nitidine chloride can reduce the incidence rate of cardiac arrhythmia in rats with myocardial ischemia and reperfusion, postpone the emergence time of cardiac arrhythmia and shorten its duration, decrease the degree of ST segment elevation after reperfusion for 15 minutes and 60 minutes, which have similar effect with verapamil.② 1 and 2 mg/kg nitidine chloride can reduce the release of myocardial enzyme, relieve the severity of oxygen-derived free radicals injury, and has the effect of protecting myocardial injury during ischemia-reperfusion, in which represents a dose-dependent effect.
5.Health risk assessment of chemical pollution in rural drinking water in Jinhua

Objective:
To understand the health risk of drinking water in rural areas of Jinhua and to provide evidence for water sanitary management in rural areas.
Methods:
Totally 2 032 samples of drinking water in rural areas of Jinhua were collected from 2016 to 2018. According to GB/T 5750-2006 Standard Examination Methods for Drinking Water,five chemical carcinogens(As,Cd,Cr6+,CHCl3 and CCl4)and twelve non-carcinogenic chemicals(Pb,Hg,Se、CN-、F-、NO3-、Al、Fe、Mn、Cu、Zn and NH3-N)were detected. The health risk assessment in rural drinking water was conducted by United States Environmental Protection Agency(USEPA)model.
Results:
The total health risk,total carcinogenic risk and total non-carcinogenic risk of rural drinking water caused by the seventeen chemicals were 34.8×10-6/a,34.80×10-6/a and 6.65×10-9/a,respectively. The carcinogenic risk of five chemical carcinogens accounted for 99.98% of the total health risk,and the carcinogenic risk of Cr6+ accounted for 89.95% of the total health risk. The total health risk of the fully processed,partially processed and unprocessed water samples were 31.68×10-6/a,34.78×10-6/a and 34.77×10-6/a,respectively. The total health risk of finished water and peripheral water were 34.79×10-6/a and 34.82×10-6/a.
Conclusion
The health risk of drinking water in rural areas of Jinhuacaused by chemicals is low. The hexavalent chromium has the highest health risk and need more attention to be paid on.
6.Effect of thyroxin on neuronal apoptosis,serum NSE and IL-6 in rats with severe traumatic brain injury

Objective To explore the protective effect of thyroxin on severe traumatic brain injury of brain tissue by observing the effect of thyroxin on neuronal apoptosis,serum neuronal specific enolase(NSE),interleukin-6 (IL-6) and serum FT3 and FT4.Methods A total of 90 SD rats was randomly divided into control group,model group,low dosage of levothyroxine sodium tablets group,moderate dosage of levothyroxine sodium tablets group and high dosage of levothyroxine sodium tablets group,18 rats in each group.The animal model was reproduced by referring to Feeney's free fall impact modeling.Intragastric administration was performed at 6 h after injury.The levels of neuronal apoptosis and serum NSE,IL-6,FT3 and FT4 were detected by TUNEL method,ELISA method and radioimmunoassay at 24,72,168 h after intragastric administration.Results (1) After severe traumatic brain injury,the levels of serum FT3 and FT4 were under the normal and the level of FT4 was decreased to the lowest at 168 h.Thyroxine could increase the levels of FT3 and FT4.(2) Significant neuronal apoptosis was observed in rats with severe craniocerebral injury,and the apoptosis continued until 168 h.Moderate and high dose of thyroxine could improve neuronal apoptosis within 24 h,while low dose of thyroxine changed within 168 h.(3) The levels of serum NSE and IL-6 were increased significantly in rats after severe traumatic brain injury until 168 h,and they could be decreased by moderate and high dose of thyroxine within 72 h.Conclusion Exogenous thyroxine can protect brain tissue in rats with severe traumatic brain injury.
7.Chronic health risk assessment on metal elements in atmospheric PM2.5 in Wucheng District

Objective :
To investigate the characteristics and the chronic health risks of metals in fine particulate matter(PM2.5) of Wucheng District,Jinhua .
Methods:
The PM2.5 samples were collected in Wucheng District of Jinhua from the year 2015 to 2017.Chromium(Cr), manganese (Mn),nickel(Ni),arsenic(As),selenium(Se),cadmium(Cd),antimony(Sb),lead(Pb),beryllium(Be),aluminum(Al),thallium(Tl)and mercury(Hg) were measured by inductively coupled plasma mass spectrometry(ICP-MS). The health risks of the metals were assessed based on the standard method recommended by United States Environmental Protection Agency .
Results:
From 2015 to 2017,the average concentration of PM2.5 in Wucheng District was(53.42±26.95)μg/m3,and the average concentration of PM2.5 in each season and each year were significantly different(P<0.05). The median(interquartile range)concentration of metal elements in PM2.5 was 136.66(117.14)ng/m3,with higher concentration of Al,Pb,Mn,Cd and Se. From 2016 to 2017,the average concentration of Cd exceeded the reference limit(5.00 ng/m3). The median (interquartile range) concentration of metal elements in PM2.5 in winter was 175.12(132.45)ng/m3,the highest,and in summer was 92.55(52.81)ng/m3,the lowest. The total hazard quotient(HQ)of nine kinds of metal elements in PM2.5 was 2.90;the HQ of As,Be,Ni,Se,Hg,Al,Mn and Sb were all less than 1,except Cd. The total carcinogenic risk of five kinds of metal elements in PM2.5 was more than 10-6,among which the carcinogenic risk of As and Cd were more than 10-6,and the carcinogenic risk of Be,Pb and Ni were less than 10-6 .
Conclusion
The concentrations of Al,Pb,Mn,Cd and Se in PM2.5 of Wucheng District were higher and the concentration of Cd exceeded the reference limit. The non-carcinogenic and carcinogenic risk of Cd were both higher,the carcinogenic risk of As was also higher.
8.Codon usage bias in the straw mushroom Volvariella volvacea.

We analyzed the whole genome coding sequence of Volvariella volvacea to study the pattern utilization of codons by Codon W 1.4.2. As results, 24 optimal codons were identified. Moreover, the frequency of codons usage was calculated by CUSP program. We compared the frequency of codons usage of V. volvacea with other organisms including 6 modal value species (Homo sapiens, Saccharomys cerevisiae, Arabidopsis thalian, Mus musculus, Danio rerio and Drosophila melanogaster) and 4 edible fungi (Coprinopsis cinerea, Agaricus bisporus, Lentinula edodes and Pleurotus ostreatus). We found that there were less differences in 3 edible fungi (excluding Pleurotus ostreatus) than 6 modal value species, comparing with the frequency of codons usage of V. volvacea. With software SPSS16.0, cluster analysis which showed differences in the size of codon bias, reflects the evolutionary relationships between species, which can be used as a reference of evolutionary relationships of species. This was the first time for analysis the codon preference among the whole coding sequences of edible fungi, serving as theoretical basis to apply genetic engineering of V. volvacea.

9.Effects of microRNA-145 on CD44 expression and hepatocellular carcinoma cells

Objective To study the relationship of microRNA-145(miR-145) and CD44 in the hepatocellular carcinoma(HCC).Methods 13 clinical samples of HCC tissues and corresponding normal liver tissues were collected at department of General Surgery,Second Affiliated Hospital of Fujian Medical University from October 2015 to June 2016.The patients with positive expression of CD44 were studied.The expression levels of miR-145 and CD44 in HCC tissues and corresponding normal tissues were detected by real-time quantitative PCR.Western blot was performed to detect the expression of CD44 in HCC cells Hep G2 and SNU423.Biological information methods predicted whether miR-145 and CD44 have a targeted relationship.CD44 expression levels were detected after high expression of miR-145 in HCC cell line with positive expression of CD44.The vector and luciferase reporter genes were constructed to verify the interaction between miR-145 and CD44.The effects of miR-145 on proliferation in HCC cell lines with positive and negative CD44 expression were examined by tetramethylazoazole salt (MTT) assay.Results 8 of the 13 patients showed positive CD44 expression in HCC tissues.Compared with normal liver tissues,the relative expression of miR-145 in HCC tissues was significantly lower (0.998±0.010 vs.0.503±0.046,P<0.05),and the relative expression of CD44 was higher (0.996±0.005 vs.1.878±0.108,P<0.05).Bioinformatics suggested that miR-145 had a targeted relationship with CD44.High expression of miR-145 can significantly reduce the expression level of CD44 mRNA in HCC cell SNU423 (0.941±0.010 vs.0.515±0.021,P<0.05) and down-regulate the expression of CD44 protein.Confirmed by luciferase reporter assay,CD44 is the target gene of miR-145.After transfection with miR-145 mimics,the proliferation of CD44 positive cell SNU423 was significantly inhibited (P<0.05).Conclusions miR-145 can affect the proliferation of CD44 positive HCC cells by regulating the expression of CD44,which may be one of the pathogenesis of HCC.
10.Correction of the complex rigid talipes equinovarus deformities with the Ilizarov technique

Objective To evaluate the clinical results of the Ilizarov technique for the treatment of the complex rigid talipes equinovarus deformities.Methods From July 2005 to July 2011,28 patients (41 feet) with rigid talipes equinovarus deformities which had been corrected with the Ilizarov technique were retrospectively analyzed,including 18 males (26 feet) and 10 females (15 feet) with an average age of 15.3 years.According to the classification system proposed by Diméglio,31 feet were categorized as grade Ⅲ,and 10 as grade Ⅳ.We performed corrections with a soft tissue release in 23 feet,and with a limited osteotomy in 18,and then a Ilizarov external fixator was applied.Anteroposterior and lateral X-rays were taken to compare the pre and postoperative data in terms of the angle of plantarflexion and dorsiflexion,the range of motion of the ankle joint,radiological measurements of the talocalcaneal angle.Results All the 28 patients achieved an outpatient follow-up,with an average of 25 months.All patients achieved a plantigrade foot with an almost normal appearance as the fixator was removed after applied for an average of 5.1 months (range,2-14).At the preoperative and final follow-up respectively,the angle of dorsiflexion of the foot was -45.0°±12.0° and 9.5°±5.5°,the angle of plantarflexion was 67.0°±14.0° and 45.5°±7.8°,talocalcaneal angle was 6.5°±4.5° and 22.5°±5.5° in anteroposterior radiograph and 5.5°±11.0° and 40.6°±8.5° in lateral radiograph.Spastic ischemia occurred in one foot and relieved by a slower distraction rate.Wire-hole infections occurred in 5 feet and treated by dressing changs,wire tract altering and antibiotic therapy,finally the infections were controlled.Deformity relapsed in one foot three months after the device was removed,then corrected with an additional fixator application and has not recurred till the final follow-up.Toe contracture and residual deformity occurred in 5 feet and 3 feet,respectively.Conclusion The Ilizarov technique is an effective method for correction of complex rigid talipes equinovarus deformities,with which the appearance and function of the foot could be kept as much as possible,and without impact on food development.
