1.Analysis of drug resistance of Klebsiella pneumoniae lower respiratory tract infection in patients with coexisting pulmonary tuberculosis and diabetes

Objective To investigate the drug resistance characteristics of Klebsiella pneumoniae lower respiratory tract infection in the patients with coexisting pulmonary tuberculosis (TB) and diabetes mellitus (DM ) to provide a basis for the prevention and treatment of Klebsiella pneumoniae infections .Methods The lower respiratory tract specimens were collected from the patients with coexisting pulmonary TB and DM in the Shenzhen Hospital of Peking University from January 2009 to May 2015 .The bacteri‐al species was identified by using the France Bio‐Merieux Vitek2‐Compact automatic microorganism identification instrument ,the drug susceptibility tests adopted the disk method (K‐B) ,and extended spectrum β lactamase (ESBLs) detection was conducted ,the drug susceptibility test results were judged according to the CLSI standards .The data were analyzed by the WHONET 5 .6 and SPSS18 .0 software .Results 139 strains of Klebsiella pneumoniae were isolated from the lower respiratory tract specimens in the patients with coexisting pulmonary TB and DM ,the ESBLs producing rate was up to 46 .0% .The drug susceptibility testing results showed that the resistance of non‐ESBLs producing and ESBLs producing Klebsiella pneumoniae to cefoperazone / sulbactam ,pip‐eracillin/tazobactam ,imipenem ,meropenem ,cefotetan and amikacin was lower and less than 15 .0% ,the resistance of ESBLs‐produ‐cing strains to most antibacterial drugs was significantly higher than that of non‐ESBLs producing strains (P< 0 .05) .Conclusion Klebsiella pneumoniae isolated from the patients with coexisting pulmonary TB and DM complicating lower respiratory tract infec ‐tion in this hospital has the high detection rate of ESBLs producing strains and strong drug resistance ,therefore clinic should strengthen the ESBLs detection and drug resistance monitoring for preventing the generation of multi‐drug resistance .In the treat‐ment of its infection ,the first choice is containing enzyme inhibitors and carbapenems antibacterial drugs .
2.Analysis of endemic status of schistosomiasis in Gaoyou City from 1970 to 2009

Objective To explore the changing rule of schistosomiasis endemic situation in Gaoyou City,so as to provide the evidences for schistosomiasis control in lake and marshland regions. Methods The endemic data of schistosomiasis of Gaoy?ou City from 1970 to 2009 were collected to analyze the endemic patterns of schistosomiasis in different control stages compre?hensively. Results In the first years of infection control stage(1970-1975),transmission control stage(1976-1994)and transmission interruption stage(1995-2009),the prevalence rates of schistosomiasis were 4.20%,0.80%and 0 in human and 3.00%,0.51%and 0 in cattle respectively. In 1984,the positive rate of crowd stool tests fell to 0.04%,which was reduced by 99.05%compared with that in 1970. From 1970 to 1980,the positive rates of stool tests of people and livestock were positively correlated significantly(r=0.67,P<0.05). After 1985,no local infection patients or cattle were found. No schistosome infect?ed Oncomelania hupensis snails were found from 1970 to 2009. Conclusion The comprehensive control measures have been insisted in Gaoyou City in different prevention and control stages. Although the snail situation is undulate,the prevalence of schistosomiasis steadily declines and the interruption of transmission has achieved.
3.Further study on possibility of diffusion of Oncomelania hupensis with water in river channels of the east route of South-to-North Water Diversion Project

Objective To study the drifting law of floats and potential risks of Oncomelania hupensis diffusion in the water diversion rivers of the east route of South?to?North Water Diversion Project. Methods The O. hupensis snails in the river chan?nels were monitored by the salvage method and snail luring method with rice straw curtains,and the diffusion possibility of snails along with water was assessed through the drift test of floats with GPS. Results In the flood seasons from 2006 to 2013, totally 8 338.0 kg of floats were salvaged,and 2 100 rice straw curtains were put into water in the Li Canal and Jinbao shipping channel,but no Oncomelania snails were found. The drift test of floats with GPS before water diversion showed that the flow ve?locity on water surface(northbound)was 0.45 m/s,the average drift velocity of the floats was 0.56-0.60 m/s,and the average drift distances each time were 999.70- 1 995.50 m in the Gaoshui River section,while there were no obvious drift in Jinbao shipping channel section. During the water diversion period,the flow velocity on water surface(northbound)was 0.45 m/s,the average drift velocity of the floats was 0.35-0.41m/s,and the average drift distances each time were 1 248.06-1 289.44 m in the Gaoshui River,while in Jinbao shipping channel section,the flow velocity on water surface was 0.28 m/s,the average drift velocity of the floats was 0.25- 0.27 m/s,and the average drift distances each time were 477.76- 496.38 m. The drift test showed that the floats gradually closed to the river bank as affected by water flow,wind direction and ship waves,when blocked by the reeds,water plants or other obstacles,and they would stopped and could not continue to drift without outside help. Con?clusions There are no Oncomelania snails found in the river channels of the east route of South?to?North Water Diversion Proj?ect. The drifting distance of the floating debris along with the water is restricted by the flow rate and shore environment.
4.Correction of the complex rigid talipes equinovarus deformities with the Ilizarov technique

Objective To evaluate the clinical results of the Ilizarov technique for the treatment of the complex rigid talipes equinovarus deformities.Methods From July 2005 to July 2011,28 patients (41 feet) with rigid talipes equinovarus deformities which had been corrected with the Ilizarov technique were retrospectively analyzed,including 18 males (26 feet) and 10 females (15 feet) with an average age of 15.3 years.According to the classification system proposed by Diméglio,31 feet were categorized as grade Ⅲ,and 10 as grade Ⅳ.We performed corrections with a soft tissue release in 23 feet,and with a limited osteotomy in 18,and then a Ilizarov external fixator was applied.Anteroposterior and lateral X-rays were taken to compare the pre and postoperative data in terms of the angle of plantarflexion and dorsiflexion,the range of motion of the ankle joint,radiological measurements of the talocalcaneal angle.Results All the 28 patients achieved an outpatient follow-up,with an average of 25 months.All patients achieved a plantigrade foot with an almost normal appearance as the fixator was removed after applied for an average of 5.1 months (range,2-14).At the preoperative and final follow-up respectively,the angle of dorsiflexion of the foot was -45.0°±12.0° and 9.5°±5.5°,the angle of plantarflexion was 67.0°±14.0° and 45.5°±7.8°,talocalcaneal angle was 6.5°±4.5° and 22.5°±5.5° in anteroposterior radiograph and 5.5°±11.0° and 40.6°±8.5° in lateral radiograph.Spastic ischemia occurred in one foot and relieved by a slower distraction rate.Wire-hole infections occurred in 5 feet and treated by dressing changs,wire tract altering and antibiotic therapy,finally the infections were controlled.Deformity relapsed in one foot three months after the device was removed,then corrected with an additional fixator application and has not recurred till the final follow-up.Toe contracture and residual deformity occurred in 5 feet and 3 feet,respectively.Conclusion The Ilizarov technique is an effective method for correction of complex rigid talipes equinovarus deformities,with which the appearance and function of the foot could be kept as much as possible,and without impact on food development.
