OBJECTIVE:To investigate the effects of erythropoietin (EPO) combined with hyperbaric oxygen on neurologic function recovery and cerebral tissue metabolism of hypoxic-ischemic encephalopathy (HIE). METHODS:A total of 47 HIE neo-nates in our hospital during Sept. 2012-Apr. 2015 were selected and divided into observation group (25 cases) and control group (22 cases)according to random number table. Both groups received routine symptomatic supportive treatment as reducing intracrani-al pressure,correcting acidosis,maintaining electrolyte balance and controlling seizures. Control group was given hyperbaric oxy-gen therapy,compressing 15 min,inhaling 30 min,decompressing 15 min,qd. Observation group was additionally given rhEPO injection(CHO cell)200 U/kg,subcutaneously at the first time,intravenous injection from the second time,qd. Both groups were treated for consecutive 10 d. NBNA score,neural reflex recovery time,consciousness recovery time,the serum levels of NSE, MBP and S100B protein were observed in 2 groups before and after treatment. The occurrence of ADR was recorded. RESULTS:Before treatment,there was no statistical significance in NBNA score,serum levels of NSE,MBP and S100B protein between 2 groups (P>0.05). Five and ten days after treatment,NBNA scores of 2 groups were increased significantly compared to before treatment,and the observation group was significantly higher than the control group at corresponding period,with statistical signifi-cance (P<0.05). The serum levels of NSE and S100B protein in 2 groups were decreased significantly compared to before treat-ment,and the observation group was significantly lower than control group at corresponding period,with statistical significance (P<0.05). After 5 days of treatment,serum levels of MBP in 2 groups were increased significantly compared to before treatment, with statistical significance(P<0.05),but there was no statistical significance between 2 groups(P>0.05). After 10 days of treat-ment,serum levels of MBP in 2 groups were decreased significantly,and the observation group was significantly lower than con-trol group,with statistical significance (P<0.05). After treat-ment,neural reflex recovery time and consciousness recovery time of observation group were significantly shorter than con-trol group,with statistical significance (P<0.05). There was no statistical significance in the incidence of ADR between observation group (8.00%) and control group (9.09%)(P>0.05). CONCLUSIONS:For HIE neonates,EPO combined with hyperbaric oxygen can effectively promote neurologic function recovery and cerebral tissue metabolism with good safety.

Objective To evaluate the safety and clinical efficacy of finasteride in treating patients with benign prostatic hyperplasia (BPH) during a 14-year period in a hospital.Methods Forty-one patients with BPH receiving finasteride 5 mg daily for the treatment from December 1994 to Febrary 2009 were included in the study. The base line and the end of study data of nocturia, prostate volume, serum creatine, complete blood count and serum prostatic specific antigen (PSA) were recorded. The acute urinary retention, surgical treatment and drug adverse reaction (prostate cancer or breast cancer) during the observation periods were recorded as well.ResultsAll the 41 cases took finasteride regularly for long-term medical therapy of BPH. At the end of this study, the average age of patients was (87.9±5.4) years old and the average duration of treatment was (141.1±27.1) months. The numbers of nocturia were 1.8±1.5 and 3.2±1.3 pre- and post- treatment, respectively (t= -4.52,P<0. 05). Before and at the end of the study, the prostate volumes were (44.9±26.6) ml and (42.8±31.3) ml, respectively(t=0. 33,P>0.05). Stratified study showed that, compared with the baseline data, the prostate volume was increased by 17.3 ％ in patients with prostate volume <25 ml(t= -0. 88 ,P>0. 05) ; the prostate volume was decreased by 17.2％ in patients with prostate volume of 25-40 ml(t=2.59,P<0.05); the prostate volumes were (63.3±28. 9) ml and (62.6±36.5) ml pre- and post-treatment in patients with prostate volume > 40 ml, and there was no significant change(t= 0.07, P>0. 05). Before and after the treatment, the serum creatine levels were (96.8±18. 6) mol/L and (86.45±32. 3) mol/L, respectively(t= 1.79, P>0. 05) ; the white blood cell counts were (6.4±1.5) × 109 L and (6.0±1.7) ×109 L, respectively (t= 1.13,P>0. 05) ; and the PSA levels were (1.2±2.0) μg/L and (1.4±1.7) μg/L, respectively (t=-0. 49,P>0. 05). Three cases (7.3％) occurred acute urinary retention. There was no prostate cancer and breast cancer case, and no new adverse event occurred during long-time use of finasteride. Conclusions This retrospective study has demonstrated that the clinical progress of BPH can be controlled effectively by long-term administration of finasteride.

Objective To observe the extracellular content of dopamine (DA)and expression of D1 receptors in hippocampal dentage gyrus (DG)in the model rats with vascular dementia (VD),and to investigate the relationship between them.Methods 12 male SD rats were randomly divided VD group and sham-operation group,and the VD model was prepared by permanent bilateral carotid occlusion.The extracellular content of DA in the DG was determined by in vivo microdialysis and HPLC,and the expression of D1 receptors was measured by immunehisto-chemistry.Results The DA content in the DG of the rats in VD group was lower than that in sham-operation group (P <0.05).The number of D1 receptor-positive cells in the DG hilus in VD group was increased compared with sham-operation group (P < 0.05),whereas the expression of D1 receptor in DG granule cell layer did not change (P > 0.05).Conclusion The DA content in the hippocampal DG is decreased in the VD rats,and its function may be compensated by the up-regulation of D1 receptors in the DG hilus.

Objective To establish a microbial limit test for levofloxacin hydrochloride gel. Methods Ca2+ was added to neutralize the antibacterial activity of quinolone,and to form the insoluble precipitate by binding with carbomer, therefore destroying the stability of macromolecules in the gel. The microbial limit test for levofloxacin hydrochloride gel was confirmed combining with membrane filtration and plate methods. Results The normal plate counting was used in detecting the mold and yeast colony in the gel, the recovery was more than 70%.Simultaneously neutralization centrifugation and membrane filtration were used in bacterial count,with recovery less than 70%,and controlling bacteria test. Conclusion The method is accurate and practical for microbial limit test of levofloxacin hydrochloride gel, but is risky for bacterial counting and needs further risk assessment.

Objective To report a case of Majocchi's granuloma induced by var. raubitschekii of Trichophyton rubrum, and to evaluate the relationship between deep and superficial fungal infection with genotyping technique. Methods The patient underwent physical, pathological and mycologic examination,which included microscopic observation, fungal culture, and urease reaction. The sequence of intertranscribed spacer (ITS) of rDNA was analysed by PCR and sequencing. Isolates from affected toes and tissue as well as one reference strain and six clinical strains of T. rubrum, were subjected to analysis of the tandem repeat subelement(TRS-1) in nontranscribed spacer(NTS)of rDNA by PCR. Results A 48-year-old female patient presented with a 2-month history of red papules and nodular lesions on the back, buttock and thigh,as well as a 3-year history of onychomycosis which had become more severe after a liver transplantation 9 months before. Pathological and mycologic examinations confirmed the diagnosis of Majocchi's granuloma. The pathogen was identified as var. raubitschekii of T. rubrum by microscopic examination, fungal culture, positive urease reaction and the sequence of ITS. As shown by the amplications of TRS-1 of NTS, the genotypes of strains from affected nails and tissue were identical, but differed from those of other clinical strains of T.rubrum. Conclusions There is a polymorphism in TRS-1 of rDNA NTS of T. rubrum, with the genotypes of isolates from affected nails and tissue being identical, which suggests they are of the same origin.

Objective: Endoscopic measurement mainly depends on visual estimation or open biopsy forceps,which have been proved lacking in accuracy due to the image barrel distortion caused by wide-angle lens in endoscope.This study aimed to validate a new concept of "virtual internal standard" proposed by ourselves and develop a new method for endoscopic measurement based on the concept.Methods: We photographed planar grid graphs printed with standard calibrations with an electronic endoscope at different object distances,obtained the information on the virtual internal standard from each photo at the corresponding object distance,and analyzed the digital characteristics of the barrel distortion photos,including the size of the visual field,the image compression ratio,and the credible area for measurement with the virtual internal standard.Based on the analyses,the method of endoscopic measurement by the virtual internal standard was established and experiments both in vivo and in vitro were conducted.Results: The ratio of the object distance to the visual field was 1 ∶ 2 when the former was between 10 mm and 40 mm.Calculation of the compression ratio showed that the credible area of measurement in the central area was 10 mm at the object distance of 10-15 mm,20 mm at 20-30 mm,and 30 mm at 30-50 mm.The credible area of measurement on various planes accounted for 2/5 to 3/5 of the total image width.There was a tendency of double-curve change in the arrangement of pixel values of the central grids on different planes.The differences between the pixel values per centimeter became slight at the object distance of 35-50 mm.Conclusion: Endoscopic measurement by the virtual internal standard is a objective,accurate,simple and practical method.

Objective To find out about the intention of medical workers with regard to reward for personal services and explore the value of their labor. Methods An intention survey through questionnaires was conducted by means of random sampling among 1 424 medical workers from 6 hospitals in Shanghai with regard to reward for personal services and a model of expected income was obtained via multivariate stepwise regression analysis. Results Only 2.9% of the medical workers felt satisfied with their current total income. Such factors as professional titles, educational backgrounds, working years, and specialties all exerted some impact on the medical workers expected income. The expected income was several times that of the actual income, and there was a large difference between the expected incomes of people at different levels. Conclusion Reward to medical workers for personal services in China needs to be enhanced and reform of the distribution system should keep step with the reform of the personnel system.
