2.Association between prodromal headache and propensity of rebleeding after aneurysmal subarachnoid hemorrhage

Objective To investigate the association between prodromal headache and the propensity of rebleeding after aneurysmal subarachnoid hemorrhage.Method Prodromal headache was defined as a sudden attack of severe headache lasting at least one hour with or without accompanying symptoms,not leading to a diagnosis of SAH 4 weeks before the confirmation of subarachnoid hemorrhage.Two hundred and nine patients with aneurymal subarachnoid hemorrhage were enrolled in this research.Age,gender,Hunt-Hess grade,D-dimer, (FBG),site and number of aneurysms and prodromal headache were recorded.Patients were divided into rebleeding group and non-rebleeding group.Comparing the above survey components between two groups was carried out by using case control study method.Age,aneurysm number,D-dimer and fibrinogen of the two groups were analyzed by t test.Prodromal headache,gender,site of aneurysms were analyzed by chi square test.Hunt- Hess grade was analyzed by rank test.Results Twenty-two(10.5%)patients had rebleeding.In the rebleeding group,10 patients(45.5%)had prodromal headache,while in the non-rebleeding group(n=187),21 patients (11.2%)had prodromal headache.Patients with prodromal headache had greater propensity of rebleeding compared with those without prodromal headache(P

4.Surgical treatment of short upper lip

Objective In order to cure some patients with the following characteristics which appear only when smiling: elevation and shortening of the upper lip, a transverse furrow below the co-lumella, and increased exposure of the maxillary gums, we reported a new surgical procedure to cor-rect this deformities. Methods By means of a transfixion incision extended laterally along the vestibu-lar or the upper gingivolabial sulcus incision, part of the depressor septi and orbicularis muscle were excised, and extensive separation were made to lower the upper lip. Results 27 cases of short upper lip were treated. Among the total, the transfixion incisions were made in 9 cases, and the upper gingi-volabial sulcus incisions were made in 18 cases. These procedures achieved lowering the upper lip, lighting or disappearing the transverse furrow, and covering the exaggerated exposure of the gums when patients smiled. At the same time the rhinoplasties were done in 6 cases. Through the above-mentioned we were able to correct the deformities on rhino-gingivolabial part only when the faces moved, achieved some kind of beauty for patients, and obtained satisfactory results. Conclusions Some patients have added defects .that are exported only when they begin to make facial movements. The cause of these characteristics is hypertrophy of the depressor septi nasi muscles. We describe the technique that is not only to correct the defects but also to enhance the patient's smile. It is worth to recommend for clinical application.
5.Simultaneous determination of five iridoids in gentianae macrophyllae radix and their local variety by HPLC.

This study aims to establish a new method for quality evaluation of Gentianae Macrophyllae Radix by simultaneous determination of five iridoids (loganic acid, 6'-O-beta-D-glucopyranosylgentiopicroside, swertiamarin, gentiopicroside, sweroside), and to detect five iridoids in the root of eight species (Gentiana macrophylla, G. straminea, G. crassicaulis, G. dahurica, G. robusta, G. waltonii, G. lhassica, and G. tibetica). The separation was carried out on a Shiseido SPOLAR C18 (4.6 mm x 250 mm, 5 microm) column eluted with mobile phase of water containing 0.04% formic acid (A) and acetonitrile (B) in a gradient program. The flow rate was 0.8 mL x min(-1). The detect wavelength was set at 240 nm. The column temperature was kept at 30 degrees C. The volume of injection was 5 microL. The five iridoids were well separated with ideal linear correlations. The average recoveries were 97.35% - 106.23%. All the five iridoids were detected in the root of eight species. The contents of same species changed in a somewhat wider range. The contents in root of G. dahurica were lower than that in other species.

7.Placement of a Long Intestinal Tube in Patients with Early Postoperative Small Bowel Obstruction under Fluoroscopic Guidance.

OBJECTIVETo investigate the placement of a long tube into the small intestine under fluoroscopic guidance and to evaluate its decompression effect on early postoperative small bowel obstruction (EPSBO).
METHODSFifty-four patients with EPSBO requiring decompression between April 2010 and July 2014 were enrolled in the study. Insertion of a long tube was guided by fluoroscopy. We first used the guide wire to pass the pylorus and then used the 10 Fr feeding tube as an exchangeable tube to put the superstiff wire into the duodenum. Finally the long tube could be passed over the guide wire through the pylorus into the intestine. The total procedure time, the radiation exposure time, and the incidence of complications were evaluated.
RESULTSThe long tubes passed into the jejunum on initial insertion for all patients, so the success rate of this technique was 100%. The long tube was inserted into ileum in 18 patients. The mean total procedure time was 34.4 ± 8.6 minutes, and the mean radiation exposure time 18.9 ± 6.8 minutes. A total of 47 patients (87%) experienced full recovery following long-tube decompression and without the need for surgical intervention.
CONCLUSIONSUsing the wire-exchange technique, it is easy to place a long tube into the small bowel under fluoroscopic guidance. This decompression method is safe and effective for management of EPSBO.

Protein tyrosine phosphatase (PTP) 1B is a potential target for the treatment of diabetes and obesity. We have previously identified the benzoyl sulfathiazole derivative II as a non-competitive PTP1B inhibitor with in vivo insulin sensitizing effects. Preliminary SAR study on this compound series has been carried out herein, and thirteen new compounds have been designed and synthesized. Among them, compound 10 exhibited potent inhibition against human recombinant PTP1B with the IC50 value of 3.97 micromol x L(-1), and is comparable to that of compound II.

9.Sacrococcygeal gap injection for the treatment of failed back surgery syndrome.

OBJECTIVETo explore the clinical effect of the sacrococcygeal space injection for the treatment of failed back surgery syndrome.
METHODSFrom July 1998 to October 2012,47 patients with failed back surgery syndrome were treated and included 39 males and 8 females with an average age of 61.5 years old ranging from 35 to 89 years old. Among them,41 patients experienced one time of operation, 6 patients with twice of operation. Forty-one patients underwent single,bilateral fenestration or central laminectomy decompression, discectomy. Six patients underwent total laminectomy discectomy and inter body fusion and pedicle screw fixation. All patients were examined by X-ray plain film, CT or MRI before treatment. The anticoagulation was discontinuation before treatment. The needle was put into the sacrococcygeal gap at prone position in the sense of frustration,suction without cerebrospinal fluid and blood,with injection of Mailuoning (Chinese characters: see text) 15 ml. The pain was assessed by VAS before and after treatment. The Oswestry low back pain disability index and survival quality interference degree were evaluated.
RESULTSAt 1 month after treatment,the pain VAS decreased from 59.24 +/- 17.35 before treatment to 19.19 +/- 11.19 after treatment (P < 0.05); The Oswestry low back pain disability index decreased from (41.35 +/- 9.87)% before treatment to (23.17 +/- 17.56)% after treatment (P < 0.05); The survival quality interference degree decreased from 6.5 +/- 2.2 before treatment to 2.6 +/- 1.4 after treatment (P < 0.05).
CONCLUSIONThe sacrococcygeal gap injection for treatment of failed back surgery syndrome has advantages of simple, safe, fewer complications, and low treatment cost.

10.The influence of the presents of target and watching ways on the level and amplitude of accommodative fluctuation

Background Researches showed that as the non-optical factors,cognitive has certain influence on the regulating system.So accurately experimental design is one of the key steps that evaluates the non-optical factors on regulating system.Objective The present study was to investigate the influence of presenting pattern of target and watching way on the level and amplitude of accommodative fluctuate and to analyze the effect of focus gaze of cognitive on regulating system and the relationship between focus gaze condition under near work and the development of myopia.Methods This study complied with Declaration of Helsinki,and the permission of Ethic Committee and written informed consent was obtained before entering in this trial.Thirty healthy volunteers were included with the mean age (24.80 ± 1.98) years old,equivalent refractive diopter (-1.92 ± 2.02) D and mean cylinder (-0.19±0.58) D.The presenting pattern of the targets was designed as focus gaze and relaxed gaze.The accommodative response and accommodative fluctuation in the complete corrected right eyes for the different targets at the 40 cm under the gazing state was recorded with Grand Seiko WAM 5500 automatic infrared refractor in the experiment.Results The mean accommodative response value was (1.86±0.26) D under the focus gaze and (1.27±0.39) D under the relax gaze,showing a statistically significant difference (t=-8.052,P=0.000).The mean fluctuate value was(0.17±0.06) D under the focus gaze,with a significant lowing in comparison with (0.28±0.17) D under the relax gaze (t =3.600,P =0.001).Conclusions These results demonstrate that the different presenting patterns of sighting target and watching ways of the subjects affect accommodation system.The accommodative response was relatively more accurate with a smaller microwavc moving under the focus gaze condition.
