1.PTEN mRNA expression in proliferative LECs of the rabbit

· AIM: To observe the effects of PTEN and the tumor suppressor gene in proliferative LECs of the rabbit.·METHODS:Forty-two white rabbits were randomly divided into test group (36 rabbits) and control group (6rabbits). The transparent lenses of treated rabbits were operated with extra capsular cortex extraction, and the controls were kept untouched. The rabbits were sacririced at 1d, 3d, 1wk, 2wK, 1mo and 2mo after surgery.Immunohistochemistry was applied to detect proliferative cellular nuclei antigen (PCNA) as well as hybridization in situ and reverse-transcriptase polymerase chain reactions (RT-PCR) were applied to detect phosphatase as well as tensin homology deleted on chromosome 10(PTEN) mRNA in lens equartor.·RESULTS:PCNA expression increased significantly to a high level at 1wk, began to reduce 2wk later, and recovered to the normal level 1 or 2mo after the surgery.PTEN mRNA expressed positively in normal rabbit LECs.The relative PTEN mRNA contents reduced greatly at 1dafter operation and remained low level at 3d. It began to increase slightly at 1wk, kept rising at 2wk and regained the normal expression after 1 or 2mo. There was inverse correlation between the PTEN mRNA and PCNA expression.·CONCLUSION: PTEN mRNA expresses positively in normal rabbit LECs plasma. PTEN participates in LECs proliferation and correlates with lens proliferative conditions.
2.Kidney cancer stem cells and epigenetic regulation

Targeted therapeutic strategy for cancer stem cells (CSCs) is the key to prevent tumor relapse and metastasis. The im-portant roles of epigenetic regulation on the development of stem cells and gene reprogram of somatic cells suggest that this process may remarkably affect the occurrence and development of CSCs. The epigenome, which comprises DNA methylation, histone modifica-tions, chromatin structures, and non-coding RNAs, controls gene expression patterns. In renal cell carcinoma (RCC), aberrant changes occur in the epigenome. To date, cells with CSC properties from RCC have been successfully isolated using different methods, such as sorting using the Hoechst 33342 side population, forming tumor spheroid, and sorting CD105 cell surface biomarker. According to the progress in genetic studies on RCC, in addition to DNA sequence, the abnormality in the regulatory mechanism has considerable func-tions in tumor progression. Epigenetic changes may be integral to the behavior of cancer progenitor cells and their progeny. Knowledge on epigenetics in renal tumorigenesis process is beneficial in the development of new therapeutic modalities and may deliver new prog-nostic and early diagnostic markers. This paper reviews the latest development in the study of RCC stem cells and the underlying mech-anisms of epigenetic regulation on the development of CSCs in RCC.
3.Sequencing analysis of 102 cases of rare thalassemia gene mutations

Objective To discover the mutations of rare thalassemia genes by sequencing of α and β-globin genes,to understand the frequency of rare mutations and to enrich thalassemia gene mutation spectrum in Chinese population.Methods For the cases of phenotype and genotype inconsistent,the 1st generation of sequencing was performed for α or β-globin gene coding region sequence analysis.Results A total of 102 patients with rare thalassemia gene mutations were found by sequencing,including 79 cases of β-thalassemia with 35 kinds of mutant types,and 23 cases of α-thalassemia with 11 kinds of mutant types.Conclusion The thalassemia gene sequencing could reveal rare mutations in genes,identify the genotype of patients,provide important support for prenatal diagnosis of rare thalassemia families,and reduce the missing rate and birth rate of children with thalassemia.
4.The morphological investigation on AcrySof Natural's protective function from acute retinal light injury

AIM: To investigate whether the Acrysof Natural has the protective function for retina from blue light in morphology.METHODS: Fresh porcine eye cups were formed in vitro. Blue light beam between 420-450nm spectrum eradiated the porcine retina and cultured RPE cells in 30J/cm2 and 40J/cm2, respectively. The adjacent region in 3mm diameter was eradiated in various ways: exposed directly to light, through AcrySof one piece IOL, PMMA IOL,AcrySof Natural IOL, and without light. Then the eye cups were cultured for 48h. Lastly, tissue and cell structure were observed with light microscope and transmission electron microscope (TEM).RESULTS: In the retinal region without light, the structure of every layer was clear, cells in neuroepithelial layer arrayed in rule, some bubble presented in external granular layer and internal granular layer, RPE cells were compact, and the color of pigment article was coincident. In the region with direct blue light and that with 30J/cm2+Acrysof one-piece/ pMMlA, cells on photoreceptor and external granular layer were lost partially, bubble increased, RPE cells were with different sizes, and cell edema, cell lost and pigment article cluster could be seen.. In region with 30J/cm2+Natural, a little disorganization could be seen comparing to that without light, but more normal than those with Acrysof and direct eradia tion. When the power was 40J/cm2, the situation was similar to that with 30J/cm2 but more severe.CONCLUSION: ① the blue light intensity in 30J/cm2 and 40J/cm2 could both induce the acute retinal light injury, ② AcrySof one piece IOL and other PMMA IOLhave no obvious effect no retina comparing to direct eradiation, ③AcrySof Natural can weaken the injury of blue light to some extent.
5.Curative effect of pranoprofen eye drops with tobramycin and dexamethasone ophthalmic suspension for cataract patients after surgery

AIM:To explore the application effect of pranoprofen combined with tobramycin and dexamethasone ophthalmic suspension in cataract extraction after IOL implantation.METHODS:Retrospective analysis of the clinical data of cataract patients treated from May 2015 to June 2016 in our hospital.According to the treatment methods, patients were divided into tobramycin and dexamethasone ophthalmic suspension with pranoprofen group (combined treatment group) and pranoprofen treatment group.The difference of visual acuity, intraocular pressure, anterior chamber flash change, satisfaction rate and symptom score before and after treatment in two groups were observed.RESULTS:The vision of the two groups before treatment had no difference(P>0.05).At 1wk and 1mo after treatment, the visual acuity of the two groups all improved compared with before treatment, there was no significant difference between the two groups (P>0.05).Before treatment, the anterior chamber flare of the two groups had no difference(P>0.05).At 1wk and 1mo after treatment, the anterior chamber flare of combined treatment group was lower, the difference was statistically different (t=2.435, 1.864;P<0.05).The symptom score of the two groups before treatment had no difference(P>0.05).After treatment, the symptoms and signs of combined treatment group was significantly lower than that of pranoprofen group (t=2.586, 7.820;P<0.05).The satisfaction rate of the combined treatment group patients was 100%, significantly higher than pranoprofen group.CONCLUSION:Pranoprofen and tobradex in the treatment of cataract extraction and intraocular lens implantation has good application effect, can significantly improve the patient's symptoms and signs.
6.Clinical curative effect analysis of the intravitreal injection of anti VEGF antibody combined with Ex -press glaucoma drainage device for neovascular glaucoma

?AIM: To observe the clinical curative effect of the intravitreal injection of anti-VEGF antibody combined with the implantation of Ex-press glaucoma drainage device for neovascular glaucoma ( NG) .?METHODS:A retrospective analysis of 20 patients with NG, who got the intravitreal injection of anti -VEGF antibody combined with the implantation of Ex-press. The visual acuity, intraocular pressure ( IOP ) , iris neovascularization fade and intraoperative and postoperative complications were observed at 1wk, 1, 3 and 6mo postoperatively.?RESULTS:The average IOPs of 20 patients were 47 ± 5.6mmHg, 13.4 ±3.6mmHg, 15.3 ±4.2mmHg, 16.9 ± 5.3mmHg and 18.7 ±6.9mmHg preoperatively and postoperatively 1wk, 1mo, 3mo and 6mo with statistical difference (P<0.05).The intraoperative and postoperative complications of the implantation of Ex-press mainly included early shallow anterior chamber, drainage tube obstruction, filtering bleb scarring. There were 8 eyes with filtering bleb scarring with normal IOP.?CONCLUSION: The intravitreal injection of anti-VEGF antibody combined with implantation of Ex -press is effective for NG, which can significantly reduce the IOP.
7.Effects of estrogen and androgen on lens oxidative damage after ovariectomy in rat

Background Epidemiological investigation in human has been conclusive. In postmenopausal women,the incidence of cataract is higher than men at the same age. In addition,hormone replacement therapy may protect against the development of cataract. However,this role of androgen is not clear. Objective This study was to explore the effects of estrogen and androgen on anti-oxidative ability of lens after ovariectomy. Methods Fifty-six three-month-old clean female Wistar rats were randomly divided into normal control group,sham operation group, castration group,estrogen eyedrops group;estrogen injection group;androgen eyedrops group;androgen injection group and 8 rats for each. Ovariectomy was performed in the rats of castration group and gonadal hormone application group, and estradiol benzoate solution or testosterone propionate solution were utilized topically or systemly in 5 months after ovariectomy for 6 weeks respectively. Only abdominal cut was curried out in sham operation group. The lenses of rats were examined weekly under the slit lamp. The serum estrogen and androgen levels of rats were detected before,after operation and 6 weeks following the administration of gonadal hormone. The contents of superoxide dismutase( SOD) , glutathione( GSH) ,malondialdehyde( MDA) and water-soluble protein ( WSP) in rat lens homogenate were detected at the end of the experiment. Utilization of animals complied with the Regulation for the Administration of Affair Concerning Experimental Animals by State Science and Technology Commission. Results No opacity of lenses was found during the experiment duration in various groups. The serum estradiol levels of rats in sham group were insignificantly different from normal groups in various time points( P>0. 05). The evident decline of serum estradiol was detected in the rats of castration group and gonadal hormone application groups compared with sham group in 5 months after operation( all P<0. 01). However,at the sixth weeks after the system use of estradiol or testosterone,the serum estradiol levels were significantly higher than the castration group and topical application groups of gonadal hormone(P<0. 01). The contents of SOD,GSH and WSP in lenses were considerably increased,but the MDA level in lenses was decreased after system use of estrogen ( P<0. 01). The activity of SOD and GSH were lower after system use of testosterone in comparison with castration rats ( P < 0. 05 ). Conclusion Estrogen can protect lens against oxidation damage. However, androgen, to a certain extent, may contribute to the development of oxidative damage in OVX female rats.
8.Effects of the adjustive tractor in treatment of cervical dislocation

Objective To observe the clinical effects of the adjustive tractor for cervical dislocation.Methods Forty-seven patients were included between September 2007 and November 2011.There were 36 males and 11 females with age ranged from 7 to 62 (mean,35 years).The mean interval from injury to admission was 8.6 h (range,0.5-72 h).There were atlanto-occipital dislocation in 2 cases,C1.2 in 2 cases,C2.3 dislocation in 5 cases,C3.4 dislocation in 2,C4.5 in 9,C5.6 in 13,C6.7 in 14.Thirty cases were complicated by fracture.No facet locking occurred in sixteen cases.Facet locking was found in ten cases and bilateral facet locking was in 21 cases.After reduction,brace or internal fixation followed.According to American Spinal Injury Association (ASIA) spinal function impairment scale standard,there were 4 cases in level A,10 cases in level B,18 in C,10 in D,and 5 in E.According to Japanese Orthopaedic Association (JOA) spinal function rating standard,the mean JOA score was 9 (range,2-14).Results All 47 cases were reduced successfully without neuronal function aggravation.Traction power ranged from 7 to 60 kg (mean,25.6 kg),the mean time of traction was 8 min (range,3-10 min).The mean follow-up was 38 (range,6-48) months.All the patients achieved normal alignment and intervertebral height.The intervertebral body fusion was observed in all of cases,the mean fusion time was 3.3 months (range,3-6 months).One patient who experienced nonunion of vertebral arch fracture didn't receive further treatment because of absence of symptoms.At last follow-up,there were 3 cases in level A,1 in level B,4 in C,8 in D,and 31 in E according to ASIA scale.The mean JOA score was 12 (range,2-17).Conclusion The adjustive tractor is simple and safe for prompt reduction.
9.Analysis of prenatal diagnosis results of 206 thalassemia families

Objective To summarize the geographical distribution,phenotype and genotype data of 206 thalassemia families underwent prenatal diagnosis to provide information for clinical genetic counseling and avoid the birth of severe thalassemia children.MethodsTotally,206 thalassemia families were collected from Southern Medical University Nanfang Hospital from January 2008 to December 2009.Genomic DNA was extracted from peripheral blood,villus,amniotic fluid or cord blood from the couples or the fetuses.Gap-polymerase chain reaction (gap-PCR) and reverse dot blot (RDB) technology were used to detect the common α and β-thalassemia mutations.DNA sequencing was used to detect the rare mutations.Follow-up visit were done half a year after the fetuses were born. Results The 206 thalassemia families came from 12 provinces and areas across China,including Heilongjiang province.Mutations detected in α-thalassemia families included --SEA/,-α3.7/,-α4.2/,αCS α/ and αQS α/,which were all included in the testing kit. While there were 4 kinds of β-thalassemia mutations,Gγ+ (A γδβ)0,-28(A→C),CD54-58(-TATGGGCAACCCT) and CD37(G→A),could not be identified with routine testing kit. The 57 α-thalassemia families consisted of 11(19.3%) severe thalassemia,induding 8 Bart's hydrops syndrome and 3 Hb H disease,26(45.6%) heterozygote and 20(35.1%) normal infants,and the 149 β-thalassemia majors families consisted of 28 (18.8%) severe thalassemia,82(55.0%) heterozygote and 39 (26.2%) normal infants.Among the β-thalassemia heterozygotes,there was one 13-trisomy.Follow-up visit found that babies with Bart ' s hydrops syndrome (n =8),Hb H disease (n =3),β-thalassemia majors (n =28) and β thalassemia heterozygote combined with 13-trisomy(n=1) were aborted.Conclusions Thalassemia was found in some north area other than south of China,which should be paid more attention by clinicians.Gap-PCR and PCR-RDB technology are effective measures for thalassemia prenatal diagnosis in identifying major thalassemia fetuses before their birth,thus reduce the birth rate of thalassemia baby.But missed diagnosis might exist during the screening,so it is necessary to perform DNA sequencing on those patients with positive symptoms and negative common genetic diagnostic results.At the same time,prenatal diagnosis of chromosomal disorders should not be neglected for high-risk families.
10.The levels of angiopoietin-2 in patients with acute respiratory distress syndrome and its value on prognosis

Objective To approach the correlation between angiopoietin-2 (Ang-2) levels and degree of lung injury and prognosis and its clinical significance in patients with acute respiratory distress syndrome (ARDS). Methods A prospective observation was conducted. Fifty-three ARDS patients admitted to Department of Critical Care Medicine of Third Affiliated Hospital of Anhui Medical University from January 2012 to March 2014 were enrolled. According to the criteria of the Berlin Definition of ARDS,the patients were divided into mild group (n=15),moderate group(n=22)and severe group(n=16). Meanwhile,ARDS patients were further divided into survival group(n=29)and non-survival group(n=24)according to 28-day outcomes. Twenty cases of non-ARDS patients were served as control. The acute physiology and chronic health evaluationⅡ(APACHEⅡ)score,sequential organ failure assessment(SOFA)score,oxygenation index(PaO2/FiO2),lung injury score(LIS)were recorded within 24 hours after admission. And the plasma levels of Ang-2,interleukin-6(IL-6)and C-reaction protein (CRP)were measured. The independent risk factors of ARDS were analyzed by univariate and multivariable logistic regression. Receiver operating characteristic curve(ROC)was plotted to evaluate the value of Ang-2 in predicting ARDS. Results Compared with non-ARDS group,APACHEⅡ score,SOFA score,LIS score,mortality were significantly increased,PaO2/FiO2 was significantly decreased,and plasma Ang-2,IL-6,CRP were significantly elevated〔APACHEⅡscore:20.7±5.0 vs. 14.1±5.3,SOFA score:7.7±3.5 vs. 3.5±2.1,LIS score:1.69±0.71 vs. 0.28±0.27,PaO2/FiO2(mmHg,1 mmHg=0.133 kPa):159.5±61.3 vs. 394.0±63.2,mortality:45.3%(24/53) vs. 20.0%(4/20),Ang-2(μg/L):4.73(2.59,6.99)vs. 1.22(0.61,1.52),IL-6(ng/L):56.50(27.15,139.90)vs. 13.05(4.38,15.55),CRP(mg/L):95.75(41.74,189.72)vs. 10.56(3.92,21.36),P<0.05 or P<0.01〕. Each index increased or decreased more significantly with the aggravation of the disease. It was shown by correlation analysis that the plasma levels of Ang-2 was significantly positive correlated with IL-6(r=0.468,P=0.000),CRP(r=0.492,P=0.000),APACHEⅡscore(r=0.560,P=0.000),SOFA score(r=0.508,P=0.000)and LIS score (r=0.588,P=0.000),significantly negatively correlated with PaO2/FiO2(r=-0.685,P=0.000). Factors, APACHEⅡ score,LIS score,PaO2/FiO2,Ang-2 and IL-6 founded statistical significance in univariate analysis were analyzed using multivariable logistic regression. High APACHEⅡscore at admission〔odds ratio(OR)=1.316, 95% confidence interval(95%CI)=1.040-1.633,P=0.022〕and increased plasma Ang-2 levels(OR=1.287, 95%CI=1.041-1.760,P=0.038)were the independent prognostic factors for the 28-day mortality in ARDS. The area under the ROC curve of Ang-2 was 0.964,the optimal critical value of Ang-2 was 1.79μg/L,the specificity was 90.0%,and sensitivity was 92.5%. Plasma levels of Ang-2 was better in predicting ARDS than APACHEⅡscore, SOFA score and IL-6. Conclusions The plasma level of Ang-2 was significantly increased in patients with ARDS. The plasma level of Ang-2 was correlated with the severity of acute lung injury and had important prognosis evaluation.
