1.PTEN mRNA expression in proliferative LECs of the rabbit

· AIM: To observe the effects of PTEN and the tumor suppressor gene in proliferative LECs of the rabbit.·METHODS:Forty-two white rabbits were randomly divided into test group (36 rabbits) and control group (6rabbits). The transparent lenses of treated rabbits were operated with extra capsular cortex extraction, and the controls were kept untouched. The rabbits were sacririced at 1d, 3d, 1wk, 2wK, 1mo and 2mo after surgery.Immunohistochemistry was applied to detect proliferative cellular nuclei antigen (PCNA) as well as hybridization in situ and reverse-transcriptase polymerase chain reactions (RT-PCR) were applied to detect phosphatase as well as tensin homology deleted on chromosome 10(PTEN) mRNA in lens equartor.·RESULTS:PCNA expression increased significantly to a high level at 1wk, began to reduce 2wk later, and recovered to the normal level 1 or 2mo after the surgery.PTEN mRNA expressed positively in normal rabbit LECs.The relative PTEN mRNA contents reduced greatly at 1dafter operation and remained low level at 3d. It began to increase slightly at 1wk, kept rising at 2wk and regained the normal expression after 1 or 2mo. There was inverse correlation between the PTEN mRNA and PCNA expression.·CONCLUSION: PTEN mRNA expresses positively in normal rabbit LECs plasma. PTEN participates in LECs proliferation and correlates with lens proliferative conditions.
2.Kidney cancer stem cells and epigenetic regulation

Targeted therapeutic strategy for cancer stem cells (CSCs) is the key to prevent tumor relapse and metastasis. The im-portant roles of epigenetic regulation on the development of stem cells and gene reprogram of somatic cells suggest that this process may remarkably affect the occurrence and development of CSCs. The epigenome, which comprises DNA methylation, histone modifica-tions, chromatin structures, and non-coding RNAs, controls gene expression patterns. In renal cell carcinoma (RCC), aberrant changes occur in the epigenome. To date, cells with CSC properties from RCC have been successfully isolated using different methods, such as sorting using the Hoechst 33342 side population, forming tumor spheroid, and sorting CD105 cell surface biomarker. According to the progress in genetic studies on RCC, in addition to DNA sequence, the abnormality in the regulatory mechanism has considerable func-tions in tumor progression. Epigenetic changes may be integral to the behavior of cancer progenitor cells and their progeny. Knowledge on epigenetics in renal tumorigenesis process is beneficial in the development of new therapeutic modalities and may deliver new prog-nostic and early diagnostic markers. This paper reviews the latest development in the study of RCC stem cells and the underlying mech-anisms of epigenetic regulation on the development of CSCs in RCC.
3.Sequencing analysis of 102 cases of rare thalassemia gene mutations

Objective To discover the mutations of rare thalassemia genes by sequencing of α and β-globin genes,to understand the frequency of rare mutations and to enrich thalassemia gene mutation spectrum in Chinese population.Methods For the cases of phenotype and genotype inconsistent,the 1st generation of sequencing was performed for α or β-globin gene coding region sequence analysis.Results A total of 102 patients with rare thalassemia gene mutations were found by sequencing,including 79 cases of β-thalassemia with 35 kinds of mutant types,and 23 cases of α-thalassemia with 11 kinds of mutant types.Conclusion The thalassemia gene sequencing could reveal rare mutations in genes,identify the genotype of patients,provide important support for prenatal diagnosis of rare thalassemia families,and reduce the missing rate and birth rate of children with thalassemia.
4.Curative effect of pranoprofen eye drops with tobramycin and dexamethasone ophthalmic suspension for cataract patients after surgery

AIM:To explore the application effect of pranoprofen combined with tobramycin and dexamethasone ophthalmic suspension in cataract extraction after IOL implantation.METHODS:Retrospective analysis of the clinical data of cataract patients treated from May 2015 to June 2016 in our hospital.According to the treatment methods, patients were divided into tobramycin and dexamethasone ophthalmic suspension with pranoprofen group (combined treatment group) and pranoprofen treatment group.The difference of visual acuity, intraocular pressure, anterior chamber flash change, satisfaction rate and symptom score before and after treatment in two groups were observed.RESULTS:The vision of the two groups before treatment had no difference(P>0.05).At 1wk and 1mo after treatment, the visual acuity of the two groups all improved compared with before treatment, there was no significant difference between the two groups (P>0.05).Before treatment, the anterior chamber flare of the two groups had no difference(P>0.05).At 1wk and 1mo after treatment, the anterior chamber flare of combined treatment group was lower, the difference was statistically different (t=2.435, 1.864;P<0.05).The symptom score of the two groups before treatment had no difference(P>0.05).After treatment, the symptoms and signs of combined treatment group was significantly lower than that of pranoprofen group (t=2.586, 7.820;P<0.05).The satisfaction rate of the combined treatment group patients was 100%, significantly higher than pranoprofen group.CONCLUSION:Pranoprofen and tobradex in the treatment of cataract extraction and intraocular lens implantation has good application effect, can significantly improve the patient's symptoms and signs.
5.The morphological investigation on AcrySof Natural's protective function from acute retinal light injury

AIM: To investigate whether the Acrysof Natural has the protective function for retina from blue light in morphology.METHODS: Fresh porcine eye cups were formed in vitro. Blue light beam between 420-450nm spectrum eradiated the porcine retina and cultured RPE cells in 30J/cm2 and 40J/cm2, respectively. The adjacent region in 3mm diameter was eradiated in various ways: exposed directly to light, through AcrySof one piece IOL, PMMA IOL,AcrySof Natural IOL, and without light. Then the eye cups were cultured for 48h. Lastly, tissue and cell structure were observed with light microscope and transmission electron microscope (TEM).RESULTS: In the retinal region without light, the structure of every layer was clear, cells in neuroepithelial layer arrayed in rule, some bubble presented in external granular layer and internal granular layer, RPE cells were compact, and the color of pigment article was coincident. In the region with direct blue light and that with 30J/cm2+Acrysof one-piece/ pMMlA, cells on photoreceptor and external granular layer were lost partially, bubble increased, RPE cells were with different sizes, and cell edema, cell lost and pigment article cluster could be seen.. In region with 30J/cm2+Natural, a little disorganization could be seen comparing to that without light, but more normal than those with Acrysof and direct eradia tion. When the power was 40J/cm2, the situation was similar to that with 30J/cm2 but more severe.CONCLUSION: ① the blue light intensity in 30J/cm2 and 40J/cm2 could both induce the acute retinal light injury, ② AcrySof one piece IOL and other PMMA IOLhave no obvious effect no retina comparing to direct eradiation, ③AcrySof Natural can weaken the injury of blue light to some extent.
6.Analysis of risk factors and pathogens for stroke associated pneumonia in intensive care unit

Objective To analyze the incidence,risk factors and pathogens of stroke associated pneumonia (SAP) in patients with acute stroke in the intensive care unit (ICU).Methods One hundred and forty-two patients with acute stroke admitted in ICU from January 2012 to December 2013 were retrospectively studied.The data of medical history of patients,treatment,prognosis,and pathogens of SAP were collected.Data were analyzed by t test,Mann-Whitney U test,Pearson x2 test and muhivariable logistic regression.Results Of 142 patients,94 (66.2%) were contracted SAP of which 54.3% were early-onset pneumonia (EOP≤72 h) and 45.7% were late-onset pneumonia (LOP >72 h).The most common pathogens isolated from EOP were Staphylococcus aureus and Klebsiella pneumonia,while the most common pathogens isolated from LOP were Acinetobacter baumanii,Staphylococcus aureus and Pseudomonas aeruginosa.Multivariate logistic regression analysis demonstrated that hemorrhagic apoplexy,history of stroke,higher APACHE score,dysphagia,prolonged use of mechanical ventilation,prolonged stay in ICU,and hyperglycemia were the independent risk factors of SAP,and the odds ratios (OR) with 95% confidence intervals (CI) were 10.917 (1.834-60.959),15.223 (1.947-96.969),1.607 (1.253-2.062),5.321 (1.225-26.519),1.809 (1.208-2.709),1.391 (1.085-1.783),1.534 (1.l01-2.138),respectively.While plasma albumin level was negatively associated with SAP (OR =0.809,95% CI:0.674-0.971).The common risk factors of EOP and LOP were higher APACHE score and prolonged use of of mechanical ventilation.The independent risk factors of EOP were dysphagia (OR =4.331,95% CI:1.330-14.098),history of stroke (OR =13.690,95% CI:2.198-85.277) and chronic bronchitis (OR =12.907,95% CI:1.203-138.542),While those of LOP were prolonged stay in ICU (OR =1.687,95 % CI:1.131-2.517),hemorrhagic apoplexy (OR =21.657,95% CI:1.559-106.752) and low plasma albumin level (OR =0.782,95% CI:0.637-0.961).There was no significant difference in mortality between EOP (49%) and LOP (44.2%) (P > 0.05),but the mortality of SAP was significantly higher than that of non-SAP group.Conclusions The incidence rate and mortality of SAP are quite high in ICU.The pathogens and risk factors are different between EOP and LOP.This observation results suggest it is important to identify high-risk stroke patients,and to develop a novel treatment strategy and prophylactic measures facilitating limiting the complications of stroke.
7.Outcome of posterior pedicle screw fixation combined with bucking bar technique in treatment of C2 pedicle fractures

Objective To evaluate the efficacy of pedicle screw fixation combined with bucking bar technique in treatment of C2 pedicle fractures.Methods A retrospective case series study was conducted for 26 cases of C2 pedicle fractures undergone posterior pedicle screw fixation after putting the bucking bar on the anterior surface of C2 via oral cavity from April 2004 to April 2014.There were 19 males and 7 females,with a mean age of 42 years (range,27-71 years).Bilateral C2 pedicle screw placement status,operation time and intraoperative blood loss were recorded.American Spinal Injury Association (ASIA) impairment scale and Visual Analogue Scale (VAS) were used for outcome evaluation.Fracture reduction and symptoms such as sore throat,expectoration,hoarse voice,dysphagia and condition of posterior pharyngeal wall were detected after operation.Results Pedicle fixation of C2 pedicle fractures was completed successfully in 21 cases,repeat C2 pedicle fixation took place in three,and C1-3 fixation was conducted for two.Operation time was (83.2 ± 16.6) min and blood loss was (171.2 ± 115.5)ml.One of the cases experiencing repeat manipulation for screw insertion had transient body balance disorder caused by cerebellar ischemia and recovered within 2 days.X-ray and CT showed complete reduction in 17 cases and incomplete reduction in nine cases.Bony fusion was obtained 6 months after operation.Twenty cases were followed up for 3-46 months (mean,26 months).Final follow-up showed anatomical reduction and bony healing in 19 cases,while malunion in other three cases.ASIA grade improved from C to D in one case,whereas the rest showed a complete neurological recovery (ASIA grade E).VAS improved from preoperative 3-8 points [(5.2 ± 2.5) points] to 0-4 points [(1.6 ±1.5) points] at the final follow-up (P < 0.05).Mild swollen sore throat occurred in four cases after operation.Laryngoscopy showed injury to the posterior pharyngeal wall mucosa (Grade Ⅰ in six cases and Grade Ⅱ in two cases),which were healed within 5-7 days after operation.Conclusion For C2 pedicle fractures,pedicle screw fixation combined with bucking bar technique via oral cavity benefits fracture reduction and stability without apparent impairment to the soft tissue of pharyngeal mucosa.
8.Synthesis, biological activity and molecular docking research of N-{(4-oxo-thiochroman-3-yl)phenyl-methyl}acetamide derivatives as α-glucosidase inhibitors.

In order to develop potent antidiabetic agents that have inhibitory effect to a-glucosidase, twelve β-acetamido ketone derivatives such as N-{[(substituted-4-oxo-thiochroman-3-yl)phenyl]-methyl}acetamide are designed and synthesized through one-pot Dakin-West reaction. Their chemical structures are confirmed by 1H NMR, 13C NMR, IR and HR-MS. In vitro α-glucosidase inhibition assays of compounds 4a-41 were carried out using glucose oxidase method. The result indicated that most of them possess inhibitory activity in vitro. Compound 4k showed the most potent inhibitory activity with 87.3% inhibition of α-glucosidase at the concentration of 5.39 mmol x L(-1). The structure-activity relationship of these β-acetamido ketone derivatives was discussed preliminarily. Moreover, the molecular docking method was used to study the interaction mode of compound 4k and α-glucosidase. Our results will be helpful for designing of α-glucosidase inhibitors in the future.
9.Screening and Identification of Predicted Epitopes of HLA-A2-restricted Cytotoxic T Lymphocytes Derived from the HPV16 E7 Antigen

Objective To screen and identify the predicted epitopes of synthesized predicted HLA-A2-restricted cytotoxic T lymphocytes (CTLs) derived from HPV16 E7 antigen. Methods The predicted epitopes of HLA-A2-restricted CTLs derived from HPV16 E7 antigen were synthesized and purified with Standard Fmoc assays, and the standard 51Cr release assay was used to determine their activities to induce specific CTL. Results Two epitopes of HLA-A2-restricted CTLs, namely E711-19 (YMLDLQPET) and E749-57 (RAHYNIVTF) derived from HPV16 E7 antigen were identified. Conclusion E711-19 (YMLDLQPET) and E749-57 (RAHYNIVTF) have antigenicity, and may be the candidates for development of peptide vaccine in the treatment of HPV infections.
10.The value of MCV, MCH and HbA2 in laboratory screening of thalassemia

ObjectivesTo explore the roles of mean corpuscular volume(MCV),mean corpuscular hemoglobin(MCH) and hemoglobin A2 (HbA2) in the laboratory screening of thalassemia,and to find optimal screening modality for different conditions.Methods From September 2008 to May 2011,1384 subjects underwent thalassemia screening at Department of Obstertrics andGynecology of Nanfang Hospital.Of them,1036 cases were diagnosed with thalassemia (408 α-thalassemia,608 β-thalassemia,and 20 αβ compound thalassemia,thalassemia group) and 348 without thalassemia,non-thalassemia group.All subjects were screened respectively for MCV,MCH and HbA2.Analyses were performed in all subjects to assess the sensitivity,specificity,positive predictive value,negative predictive value and diagnostic accuracy respectively associated with MCV,MCH and HbA2 alone,combination of MCV and MCH,and combination of MCV,MCH and HbA2.Results( 1 ) In the thalassemia group,the sensitivity of MCV alone was 92.9% (379/408) for α thalassemia,99.3% (604/608) for β thalassemia and 100.0%(20/20) for αβ compound thalassemia.In the non-thalassemia group,the specificity of MCV alone was 75.0% (261/348).(2) In the thalassemia group,the sensitivity of MCH alone was 92.9% (379/408) in α thalassemia,99.0% (602/608) in β thalassemia and 100.0% (20/20) in αβ compound thalassemia.In the non-thalassemia group,the specificity of MCH alone was 72.7 % (253/348).(3) The sensitivity of Hb A2 alone was 67.4% (275/408) for α thalassemia,97.5% (593/608) for 3 thalassemia,and 100% (20/20) for α3 compound thalassemia while it's specificity was 72.4% (252/348) in the non-thalassemia group.(4)With positive indexes of MCV,MCH and MCV + MCH,when HbA2 > 3.5% it had a high value in [β-thalassemia screening,but when HbA2 < 2.5% it had little value in α-thalassemia screening.(5) As a single marker,MCV and MCH had better sensitivity,specificity,positive predictive value,negative predictive value and diagnosis accuracy than HbA2.MCV + MCH was the best for overall screening,but for [β thalassemia screening,MCV + MCH + HbA2 was the best.ConclusionsMCV and MCH are suitable for epidemic screening in a large population,physical examination and premarital check-up.Hb electrophoresis andthalassemiagenediagnosisarerecommendedforsubjectswithpositiveMCVandMCH indexes.Diagnoses of α and β-thalassemia gene are recommended for pregnant women with positive MCV and MCH indexes.
