2.Clinical feature of 9 patients harboring mtDNA G11778 A mutation with Leber hereditary optic neuropathy

AIM: To evaluate the clinical feature of 9 patients harboring mitochondrial DNA ( mtDNA ) G11778A mutation with Leber hereditary optic neuropathy ( LHON) .
●METHODS: Nine LHON patients were enrolled and followed- up between 2012 to 2015 in Shenzhen Eye Hospital, clinical data were collected and analyzed.
●RESULTS: Six cases had maternal inheritance history ( 67%) . Three were sporadic cases. The patients aged from 9 to 43 years old, with average age of (22. 00±9. 42) years. Simultaneous onset with both eyes was in 5 cases (56%). Successively onset was in other 4 cases (44%). The ratio between male and female was 2:1. In the last follow-up, the visual acuity was finger counted in 2 eyes (11%), 0. 01-0. 1 in 12 eyes (67%), 0. 12-0. 4 in 2 eyes (11%),≥0. 4 in 2 eyes (11%). All patients had pale disc and clear boundary. ln the Humphrey visual field examination, 10 eyes had typically cecocentral or paracentral scotoma, 8 eyes had diffuse visual field defect.
●CONCLUSION: ln the 9 LHON patients with mtDNA G11778A mutation, simultaneous onset cases were more than successively onset cases within 1y cases. In most cases, LHON patients kept stable visual acuity. Rare cases had a raise in visual acuity within 1y. Majority patients had typically cecocentral or paracentral visual field scotoma. In the last stage of LHON, visual field present diffused defect. The secondary affected eye was similar performed defect as the former one.

4.Safety and Immunogenicity of Live Attenuated Varicella Virus Vaccine(MAV/06 Strain).

We immunized sixty two healthy subjects with the five different viral titers (300, 500, 1000, 1500 and 2000 plaque forming unit; pfu) of the MAV/06 strain of live attenuated Varicella-zoster virus (VZV) in order to gain sufficient information on safety and immuogenicity as a vaccine strain. Humoral immunity of all vaccine recipients was tested by the fluorescent antibody to membrane antigen (FAMA) assay and Enzyme-linked immunosorbent assay (ELISA) for the quantitative detection of IgG antibody. We tested neutralized antibody in 62 subjects by plaque reduction neutralization test (PRNT50). All of thirty two subjects with initial seronegative response had antibody by FAMA method at four weeks after immunization with four different preparations of dosage. The geometric mean titers (GMTs) of VZV antibody to membrane antigen was 160.9 in 6 subjects with 1500 pfu group; 83.3 in 14 subjects with 1000 pfu group: 116.2 in 7 subjects with 500 pfu groups and 72.0 in 6 subjects with 300 pfu group. Thirty subjects who had VZV antibody at the time before immunization demonstrated elevated antibody titer by FAMA assay and PRNT50 test. Side reactions of the vaccination was not demonstrated in all cases.

5.Treatment of Femoral Neck Fracture (Twenty-nine Cases Treated by Multiple Knowles Pinning and Compression Hip Screw)

It has been emphasized that treatment of hemoral neck fracture is early anatomical reduction and rigid internal fixation to reduce severe complications such as avascular necrosis or non-union.Operative treatment were developed recently with development of radiological technique. This study reviewed twenty-nine patients of femoral neck fracture treated by internal fixation between December 1973 and December 1979. There are eleven casesof Garden's stage I or II fracture with multiple Knowles pinning and eighteen cases (All Gardens stage) treated with compression hip screw, and followed for a minimum of nine months with an average eighteen months. The results were as follows; 1. Eleven cases with minimal displaced fracture (Garden's stage I or II) treated by multiple Knowles pinning were revealed good results. 2. In total eighteen cases treated by compression hip screw, nine cases (Garden's stage I or II) were revealed good results and other nine cases (Garden's stage I or II) were revealed fair or poor results. 3. Eleven cases of mlutiple Knowles pinning and nine case of compressian hip screw used for undisplaced fracture (Garden's stage I or II) were revealed good results without regard to internal fixation devices. 4. Avascular necrosis were four cases in nine cases(44%) of displaced fracture. 5. Non-union were not found in this aeries.

6.Computerized Tomography in Orthopedic Surgery

The utility of computerized tomogaphy in the study of the anatomy, and pathology of the musculoskeletal system has been the subject of considerable interest since the introduction of CT scanning. It provides an accurate and detailed cross-sectional image of normal anatomical structures and shows the relation of masses to these structures. In order to assess the utility of computed tomography, we analyzed the 92 clinical cases. We divided our experience into three major categories: spinal lesios, pelvic (including hip) lesions, and lesions of extremities. The advent of a new prototype scanning device has made it possible to exam a variety of abonormalities in the orthopedic diseases in a manner not previously possible.

7.Neurophysiologic and Histologic Changes Following Injection of Morphine , Meperidine and Pentazocine on Sciatic Nerve of Rabbits.

Intraspinal(intrathecal, epidural) narcotics administration had been widely used and well estabilished for pain control. The action mechanism of intraspinal narcotics has been well defined, and meperidine and pentazocine have been reported to be used as a sole agent for spinal anesthesia. The authors have already reported good loeal anestheticlike effets of meperidine and pentazocine clinically in patients scheduled for various surgeries, followed by experimen- tal evaluation of toxic effects of two drugs on the sciatic nerve of rats and dogs. This investigation was primarily undertaken to examine the difference in neurophysiological action between meperidine and morphine and also to evaluate early histological changes on aciatic nerve of rabbits within one week after injection of the each narcotics. Adult Korean rabbits were chosen as experimental animals because it is easy to observe neurophysiological activity with responses and to avoid of manipulating trauma. The rabbits were anesthetized using ketamine 10 mg/kg intramuscularly. The sciatic nerve of the rabbits was exposed and stimulated by a nerve stimulator to observe myoneural response as a control and then injected with 0.1% morphine 0.2 mg(Group I), 0.5% meperidine 10 mg(Group 2) and 0.3 % pentazocine 6 mg(Group 3). The sciatic nerve was stimulated for 20 minutes at 5 minutes interval and gait changes were carefully observed in the recovery room to see the myoneural activity, A specimen was taken at 4 and 24 hours, and 1 week after injection. The results were as follows, When the sciatic nerve was stimulated by a nerve stimulator, the normal muscle twitch was observed clearly in Group I with the morphine injection. However, in Group 2 with meperidine and Group 3 with pentazocine injection, muscle twitching decreased gradually and finally disappeared about after 10 minutes, Complete motor paralysis ceased after 60 minutes and muscle reaction returned to normal about 90 minutes after injection, Therafter, myoneural complications were not noticed in the 3 groups for a period of I week. All specimens of the 3 groups were inyestigated under light and with electron microscopic examination and they revealed mild vacuolziations scattered in axons of myelinated and unmyelinated nerves of some of the specimens but these were not significant. As a result of this investigation, we concluded that neurophysiologically, meperidine and pentazocine have local anesthetic-like effect, such as motor paralysis, but not with morphine, and neurohistologically, the above three norcotics have no significant toxic effects on nervous tissues.

8.Effects of Guilin Watermelon Frost on the mRNA expressions of basic fibroblast growth factor in patients with uterine cervical columnar ectopy.

OBJECTIVETo study the molecular biological effects of Guilin Watermelon Frost (GWF) on the mRNA expressions of basic fibroblast growth factor (bFGF) in patients with uterine uterine cervical columnar ectopy.
METHODSOne hundred and sixty patients with uterine cervical columnar ectopy were assigned to two groups by the random digit table. Patients in the treatment group were treated with local spray of GWF, while those in the control group were local applied with bFGF-collagen sponge. The mRNA expressions of bFGF of the uterine tissue were detected in the two groups before and after treatment using RT-PCR.
RESULTSBefore treatment the mRNA expression of bFGF in the uterine cervical columnar ectopy was 0.55 +/- 0.10 in the treatment group and 0.58 +/- 0.13 in the control group, without insignificant difference (P > 0.05). After treatment it significantly increased in the two groups, being 0.82 +/- 0.17 and 0.78 +/- 0.15 respectively, showing statistical difference from before treatment (P < 0.01). But no statistical difference existed between the two groups after treatment (P > 0.05).
CONCLUSIONGWF showed enhancement on the mRNA expressions of bFGF in patients with uterine cervical columnar ectopy.
