2.Studies on Content Assay and Fingerprints of Intravenous Emulsion Containing Garlic Oil

OBJECTIVE: To establish and a method for determination of allicin and the fingerprints of intravenous emulsion containing garlic oil. METHODS:GC method was applied to determine allicin in preparation and the fingerprints of intravenous emulsion containing garlic oil. The separation was performed on HP-5 (30 m?320 ?m?0.25 ?m) column with the flow rate of 1.0 mL?min-1 and inlet temperature of 250 ℃. The column temperature was 60~220 ℃ and rose by temperature-programmed method with N2 as carrier gas. The temperature of FID detector was 300 ℃ with furan as internal standard. RESULTS: The linear ranges of allicin were 0.05~2.4 mg?mL-1(r=0.999 8). The average recovery were 102.5% (RSD=0.41%,n=9). There were 17 mutual peaks in the fingerprints of intravenous emulsion containing garlic oil. The RSD of intra-day relative retention time and relative peak area were less than 0.05% and 8% respectively. After preparation setting for 48 h at room temperature, the RSD of relative retention time and relative peak areas were less than 0.08% and 15% respectively. The RSD of them in the replicate test were less than 0.04% and 8%, respectively. CONCLUSION: The method is accurate, reliable, simple and available for quality control of intravenous emulsion containing garlic oil
3.Minimally invasive treatment for acute biliary pancreatitis

Objective To discuss clinical effects of combined use of duodenoscopy and laparoscopy in the treatment of acute billiary pancreatitis(ABP).Methods The clinical data of 94 ABP patients who underwent minimally invasive treatment from February 2001 to Feburary 2006 were retrospectively reviewed.Among 94 ABP patients,59 patients had gallbladder stones were given laparoscopic cholecystectomy(LC)alone;14 patients had common bile duct stones received endoscopic nasobiliary drainage(ENBD),combined endoscopic sphincterotomy(EST)and LC;21 patients had both gallbladder and common bile duct stones received combined EST and LC.Results Postoperatively,in the whole group,only one patient had recurrent pancreatitis,one patient had hemobilia,and both cases followed ERCP+EST;two cases had lung infection,and one case had infection of abdominal incision.All of the 5 cases with postoperative compllcations were successfully treated by conservative therapy.The effective rate for the whole group was 100%.Conclusions Combined use of duodenoscopy and laparoscopy is significantly effective for treatment of acute biliary pancreatitis and this minimally invasive treatment is the ideal therapy for acute biliary pancreatitis.
4.Effect of Compatibility of Radix Astragali and Radix Angelicae Sinensis in Different Proportion on Hemorheology and Contents of IL-4, IFN-γ in Blood of Qi Deficiency and Blood Stasis Rat Model

Objective To observe the effects of the compatibility of Radix Astragali and Radix Angelicae Sinensis in different proportion on hemodynamic and balance of Th1/Th2 cells of Qi deficiency and blood stasis rat model. Methods Qi deficiency and blood stasis rats were caused by restricted diet, forced swimming and norepinephrine subcutaneous injecting, and treated by compatibility of Radix Astragali and Radix Angelicae Sinensis in different proportion of 5∶1, 3∶1, 1∶1 and 1∶2 for 21 days. The indexes of hemorheology were detected with hemorheological analyser, and the level of IFN-γand IL-4 in serum were tested by ELISA. Results Compared with model group, the compatibility of Radix Astragali and Radix Angelicae Sinensis in proportion of 1∶1, 3∶1 and 5∶1 groups reduced the low shear blood viscosity. The spleen index of model group decreased, and compatibility of Radix Astragali and Radix Angelicae Sinensis in 5∶1 proportion group increased spleen index. The level of IL-4 increased and IFN-γ decreased in the serum of model group, the compatibility of Radix Astragali and Radix Angelicae Sinensis in proportion of 3∶1 group increased the level of IFN-γ. Astragalus angelica 5∶1 group decreased the level of IL-4 and increased the level of IFN-γ. Conclusion The compatibility of Radix Astragali and Radix Angelicae Sinensis can improve hemorheology, adjust the balance of Th1/Th2 cells of Qi deficiency and blood stasis rats. The effects were better when Radix Astragali’s dosage greater than that of Radix Angelicae Sinensis, and the group of 5∶1 proportion was the best.
5.Expression of PEDF and VEGF in corneal neovascularization after corneal alkali burn

AIM: To analyze the effect of human amniotic homogenate extract on corneal neovascularization after corneal alkali burn in the process of pigment epithelium derived factor (PEDF) and vascular endothelial growth factor (VEGF) expression and the effect of corneal neovascularization.METHODS: Totally 32 patients with corneal alkali burn were selected from June 2015 to June 2016 in Foshan,and were randomly divided into Group A and Group B,with a total of 37 eyes.Group A of 17 cases,with a total of 19 eyes,were treated with 40mg/L human amniotic homogenate extract;Group B (n=15),and 18 eyes,treated with 3g/L prednisolone eye drops.In the treatment of 1,4,7,14,21 and 28d at different time points,we observed the growth of corneal neovascularization,and detected the expression of PEDF and VEGF during angiogenesis.RESULTS: Group A of patients in the use of human amniotic homogenate extract after the treatment,the expression level of PEDF was significantly higher than that in Group B(P=0.001),after 28d treatment,the expression level of PEDF reached 0.721±0.314.While patients in Group B the expression level of PEDF was only 0.538±0.253.Two groups had significant difference between the expression level of PEDF (P<0.05).The expression level of VEGF in Group A was lower than in Group B at different time points in the test.After the treatment of 28d patients in the Group A,the expression level of VEGF was 0.152±0.020,in Group B the expression level of VEGF was0.302±0.031.Two groups of patients with VEGF expression level between the differences were statistically significant (P<0.05).The patients number in Group A with corneal neovascularization was significantly lower than that in Group B,the difference was statistically significant (P<0.05).CONCLUSION: Human amniotic homogenate extract can increase the expression of PEDF in corneal neovascularization after corneal alkali burn,inhibit the expression of VEGF and the proliferation of corneal neovascularization.
6.Effect of ionizing radiation on expressions of regulatory T cells and related molecules in mice spleen

Objective To observe the changes in expressions of spleen regulatory T cells (Tregs)and the related factor forkhead box protein-3 (Foxp3) after irradiation with different doses of X-ray in mice at different times,and to elaborate the effects of X-rays on regulatory T cells and Foxp3.Methods 112male ICR mice were randomly divided into 2 groups and irradiated by X-rays at the doses of 0.075 and 2 Gy,respectively.The mice were killed at 0,4,8,16,24,48,and 72 h post-irradiation and the spleens removed.Flow cytometry was used to detect the percentage of CD4 + CD25 + Treg and protein expression of (Foxp3),and RT-PCR was used to exmiamine the mRNA expression of Fox3.Results Compared with those before irradiation,the CD4 + CD25 + Treg positive rates began to increase and peaked at 8 h post-irradiation with 0.075 Gy at 8,16,24,72 h(t = 8.73,10.55,4.21,4.65 ,P < 0.05) and 2 Gy at 8,16,48,72 h(t = 4.65,4.28,3.71,2.88,P < 0.05),and then slightly decreased,but still remained at high levels.The mRNA protein levels of Fox3 did not change significantly after exposure to the dose of 0.075 Gy,but began to significantly increase at 8 h after exposure to the dose of 2 Gy.However,the Foxp3 protein level began to increase 4 h post-irradiation,peaked at 16 h,and then slightly decreased,but still ramained at high levels (t =2.59,3.37,3.70,3.20,P<0.05).Conclusions The changes in expressions of Tregs and Foxp3 after high- and low-dose X-ray irradiation may be used to explain the differences in immune effects induced by ionizing radiation at different doses.
7.Application of pancreatic duct guide wire and transpancreatic septotomy with precutting techniques in dififcult endoscopic retrograde cholangiopancreatography

Objective To investigate the application value of pancreatic duct guide wire and transpancreatic septotomy with precutting technique in difficult endoscopic retrograde cholangiopancreatography.Method258 patients who underwent difficult endoscopic retrograde cholangiopancreatography from April 2014 to April 2016 were selected as study subject, 128 patients among them received the technique of pancreatic duct guide wire, the other 130 patients received transpancreatic septotomy with precutting techniques. The success rate, intubation time and incidence of complications were compared between these two methods for cannulation.Result There was no signiifcant difference in preoperative clinical data between the two groups, the success rate did not differ signiifcantly between the two groups (93.75 % vs 93.85 %). Compared with transpancreatic septotomy with precutting techniques group, pancreatic duct guide wire group is less intubation time consuming (5.92 ± 0.69 vs 12.81 ± 3.67) min, the difference was statistically significant (t = -2.27,P < 0.05). 25 patients experienced complications, with 6 cases of acute pancreatitis, 2 cases of biliray tract infection in pancreatic duct guide wire group, and 8 cases of acute pancreatitis, 3 cases of hemorrhage,6 cases of biliray tract infection in transpancreatic septotomy with precutting techniques group. The pancreatic duct guide wire group had a signiifcantly lower incidence of complications (6.25 % vs 13.08 %). the difference was statistically signiifcant (χ2 = 3.27,P < 0.05). The incidence of acute pancreatitis did not differ signiifcantly between the two groups (4.69 % vs 6.15 %).ConclusionsPancreatic duct guide wire and transpancreatic septotomy with precutting techniques both can further improve the success rate of bile duct cannulation with ERCP. The incidence of acute pancreatitis did not differ significantly between two groups. But pancreatic duct guide wire group is less intubation time consuming, and had a significantly lower incidence of complications. Because of the convenience and safety of the pancreatic duct guide wire technique, and the insertion of the pancreatic duct does not increase the risk of postoperative acute pancreatitis. We think that this method is more worthy of Clinical promotion.
8.Stability of Garlic Oil-Hydroxypropyl-?-Cyclodextrin Inclusion Complex

OBJECTIVE:To investigate the stability of garlic oil-hydropropyl-?-cyclodextrin(HP-?-CD)inclusion complex.METHODS:With diallyl trisulfide(DATS)as determination index,the water solution of garlic oil HP-?-CD in-clusion complexes,freeze-drying powders of inclusion complex and the garlic oil injections were respectively subjected to il-lumination,high temperature,dilution tests and accelerated tests,and long-term sample observation tests by gas chro-matography(GC).RESULTS:The content of DATS in freeze-drying powders of inclusion complex had a minimum reduc-tion among the three under conditions of illumination and high temperature.In the accelerated tests and long-term sample observation tests,almost all the determination indexes of freeze-drying powders remained unchanged and a good stability was noted.Within10h,all the different diluents showed no significant effects on the indexes of the inclusion complex.CON-CLUSIONS:As compared with garlic oil injections,the garlic oil inclusion complex had a higher stability,freeze-drying powders of inclusion complex showed the best stability,yet the storing of which still needed to be under a proper temperature and away from light.
9.Experiment and clinical study of contrast-enhanced ultrasonography in evaluating brain injuries

Objective To explore the value of contrast-enhanced ultrasonography(CEUS) in brain injuries. Methods ① Models of graded brain trauma were created in brains of 10 healthy mongrel dogs after the animals were anesthetized and the cranium were removed. Conventional ultrasonography(US) and CEUS were performed to observe the characteristics of traumatic region. ② Thirteen patients with traumatic brain injuries were collected to perform intraoperative ultrasonography. The location, depth, size, internal echo and boundary of traumatic brain injuries were clearly displayed. Eight injury sites were chosen to undergo CEUS. Results ① Twelve injury sites in brains of 10 dogs were detected,and CEUS were peformed in 10 lessions, of which 1 was diagnosed with cerebral contusion, 1 with cerebral contusion and cerebral hemorrhage, 7 with intracerebral hematoma, 2 with ventricular hematoma, 1 with blood vessels in intracerebral hematoma. ② Fifteen lesions were detected by preoperative CT scan, but 18 lessions were detected by intraoperative ultrasound. During operation epidural hematomas in 4 cases were detected. After CEUS,the area of lessions was clearly revealed to be extended, the hemorrhage area was exempt from enhancement while peripheral normal cerebral tissue was homogenously enhanced. Conclusions Conventional ultrasonography could display brain injuries. In case of accurate intraoperative encephalocele, intraoperative untrasound conduces to discover delayed intracranial hematoma. Different traumatic brain injuries had their special CEUS findings.
