1.Cost-Effectiveness Analysis of Rabeprazole versus Lansoprazole for Peptic Ulcer

OBJECTIVE: To evaluate the cost-effectiveness of Rabeprazole versus Lansoprazole for peptic ulcer. METHODS: A total of 101 patients with peptic ulcer were randomly assigned to receive either Rabeprazole+Clarithromycin sustained release capsules+tinidazole(treatment group) or Lansoprazole+Clarithromycin sustained release capsules+tinidazole(control group) for 14 days. RESULTS: In the treatment group vs. the control group, the effective rate was 96.0% vs. 84.3%(P0.05); the cost was 285.46 vs. 373.87 yuan; the cost-effectiveness ratio was 297.35 vs. 443.50. The incremental cost-effectiveness ratio for the control group was -755.64 as against the treatment group. CONCLUSION: Rabeprazole group is optimal for peptic ulcer in that it is more economical, more effective and safer than Lansoprazole group.
2.Effect of hypoxia on expression of angiopoietin-2 in human retinal pigment epithelium

Background Choroidal neovascularization(CNV)has been descrihed as a main reason of visual loss in a lot of ocular diseases.Researches showed that local hypoxia and retinal pigment epithelial(RPE) cells play an important role in the formation of CNV.A closely relationship of angiopoietin-2 (Ang-2) and angiogenesis has been proved.However,whether the expression of Aog-2 in hypoxic cultured human RPE cells is associated with the pathogenesis of CNV is still below understood.Objective This study was to investigate the effects of hypoxia on expression of Ang-2 in cultured human RPE cells in vitro,and discuss the possible effects of Ang-2 in the formation of CNV.Methods Human RPE cells were cultured and passaged,and 4-7 generation of cells were used in the experiment.The cells were incubated in cultural plate at the density of 5×107 cells/L.The culture medium containing 200 μmol/L CoCl2 was used to establish the hypoxia model of human RPE cells culturecd in vitro for 0.5,1,2,4,6,12and 24 hours,and the RPE cells cultured under normoxia were as controls.Reverse transcription polymerase chain reaction(RT-PCR) was used to detect the expression of Ang-2 mRNA in cultured human RPE cells,and enzymelinked immunosorbnent assay(ELISA) was used to assay the content of Ang-2 protein in supernatant of cultured human RPE cells.Results The survival rate of human RPE cells was 90% after resoscitation.The fourth generation of cells showed the fusiform with the less pigment in them.The Ang-2 mRNA/β-actin mRNA value in human RPE cells was significantly different among various groups(F=1086.30,P=0.00),The Ang-2 mRN A/β-actin mRNA value in hypoxia cultured for 0.5 hours group began to increase and peaked in hypoxia culture for 4-6 hours group,with the significant differences in comparison with normoxia control group(P<0.05).The Ang-2 mRNA/β-actin mRNA value decreased to the baseline level at hypoxia for 24 hours.The ELISA analysis showed that the concentration of Ang-2 protein in supernatant of RPE cells showed significant difference among groups(F=1034.00,P=0.00).The concentration of Ang-2 protein increased at hypoxia culture for 0.5 hours and peaked at 6 hours,showing significant differences in comparison with the control group (P<0.05).Conclusions Hypoxia could significantly up-regulate the expression of Ang-2 in human RPE cells cultured in vitro.Ang-2 expresses highly in the early stage of hypoxia,implying that Ang-2 participates in the formation of CNV.
3.Effect of Sophoramine on Immune Function of Mice

The effects of sophoramine (SA) on immune system in normal mice were studied. SA 15. 20mg/kg reduced the weight of thymus,decreased the rate of E rosette,and also suppressed T lymphocyte transformation. The weight and the hemolytic plaque of spleen were not influenced by SA at 15. 20mg/kg. At the dose of 25mg/kg,SA reduced the weight of thymus and spleen,decreased the rate of E rosette and suppressed the T lymphocyte transformation. All of the doses of SA didn' t affect the phagocytotic function of peritoneal macrophage.
4.Clinical application of transrectal ultrasound in treatment of prostate cancer

Objective To evaluate the clinical application of transrecral ultrasound in the treatment of prostate cancer.Methods 23 cases of prostate cancer treated with PSA,CT,MRI and postoperative pathological confirmation were analysed respectively and the results of transrectal ultrasound were compared.Results The diag- nostic match rate of transrectal ultrasound for prostate cancer was 77%.Transrectal ultrasound showed hypoecho nodes in prostate.Conclusion Transrecral ultrasound could detect prostate cancer.

7.Nursing for venous catheter during the course of hemofilitration

Objective To know the nursing countermeasures for the catheter associated complications during the course of hemofilitration by observation critical patients who had accepted hemofilitration.Methods Retrospective analyzed the nursing points and treatment effects among 68 critical patients with hemofilitration from June 2005 to October 2008.Results There were no venous catheters droped off,no blocked and obvious infection.6 cases with exudation,57 cases with vitro cogulation,35 cases with hypotension,21cases with hypothermia,2 cases with hypoglycemia.Conclusions Apropriate anticoagulant,observe the body temperature and keep the comfortable temperature can increase efficiency and reduce the venous catheter associated complications during the hemofilitration.

10.Inhibitory effects of bevacizumab on filtering bleb scarring following trabeculectomy in rabbits

Background The primary reason to trabeculectomy failure is fibrosis of conjunctiva and episclera because of progressive fibroblast proliferation and collagen deposition of the filtration bleb.Conventional methods of inhibiting bleb scarring was intraoperative application of mitomycin C (MMC),but many complications occured after surgery.Researches showed that bevacizumab was an antifibrotic agent,and whether it can suppress scarring of filtering bleb after trabeculectomy is concerned.Objective The aim of this study was to evaluate the antifibrotic efficacy of bevacizumab after trabeculectomy in rabbits.Methods Forty New Zealand rabbits were randomly divided into four groups.Trabeculectomy was performed on the right eyes of each rabbits.The rabbits received subconjunctival injection of 0.05 ml bevacizumab (25 mg/ml) at the end of operation in the bevacizumab single injection group.The same dose of bevacizumab was respectively injected at the end of operation as well as 3 days and 7 days after operation in the bevacizumab repitition injection group,and 0.05 ml normal saline solution was used in the same way in the normal saline group.In the MMC group,MMC cotton patch with 0.2 mg/ml was placed under the Tenon caplsule and scleral flap for 3 minutes during operation.The intraocular pressure (IOP),bleb area and shape were evaluated during the 28-day period.The animals were sacrificed on postoperative day 14 and 28,respectively for the histopathologic examination of bleb.The expression of CD31 in the bleb was detected by immunohistochemistry for the calculation of microvessels.All experiments were performed in accordance with the ethics code for animal experimentation and approved by the Institutional Review Board of Tianjin Eye Hospital.Results No significant difference was found in the postoperative IOP among the groups (F =0.88,P =0.47).Compared with the bevacizumab single injection group,MMC group and normal saline group,the shape of bleb was higher and much diffuse with sparse vessels 7 days after operation in the bevacizumab repitition injection group.The survival time of bleb was 27 days,19 days and 13 days in the bevacizumab repitition injection group,the bevacizumab single injection group,MMC group and normal saline group,respectively.The percentage of collagen deposition area was (49.18±1.54)%,(26.41±1.23)%,(50.68±1.87)% and (70.63±1.81)% at day 14 postoperative in the bevacizumab single injection group,bevacizumab repitition injection group,MMC group and normal saline group,respectively,with the largest area in the normal saline group,and percentage of collagen deposition area was significantly reduced in the bevacizumab repitition injection group compared with the bevacizumab single injection group (all at P<0.05).The percentage of collagen deposition area was (66.82±1.53)% at day 28 postoperative in the bevacizumab repitition injection group,while complete scarring was seen in other 3 groups.The number of microvessels was least at postoperative day 14 in the bevacizumab repitition injection group compared with the bevacizumab single injection group,MMC group and normal saline group (all at P < 0.05).The number of microvessels was more in postoperative day 28 in the bevacizumab repitition injection group (3.51 ±0.31) compared with other groups (all at P < 0.05).Conclusions Subconjunctival injection of bevacizumab following trabeculectomy can improve the successful rate of surgery by remaining the survival time of filtering bleb,inhibiting the bleb scarring in rabbits.
