The parameters of 797 measurements of electrolytes and blood gases in 191 patients during and after open heart surgery under cardio-pulmonary bypass were studied. It was found that both the anion gap (AG) and lactate levels were markedly elevated in the patients than in the normal controls. It is well known that increased plasma lactate due to hvpoxia is one of the main factors to result in anion gap acidosis, and the therapeutic measures should be directed to increase tissue oxygen supply rather than administer alkali, which will be liable to induce alkalosis. Therefore, it is suggested that concomitant determinations of AG and blood gases bo valuable and helpful to the management of the postoperative cases of open heart surgery.

Colorectal cancer has the propensity to develop liver-dominant metastases.In some of these patients,the liver is the only site of metastatic disease.Thus,surgical resection is the mainstay of treatment for liver-only colorectal cancer metastases,which can produce long-term survival in selected patients,but only 10%-20% of patients are suitable for a surgical approach.In this paper we introduced how to transfer the inoperable colorectal liver metastases to operable colorectal liver metastases through improved chemotherapy with /without molecular target therapy and multidisciplinary treatment approaches,and the results of radiofrequency ablation(RFA),which can also produce long-term survival in a subset of patients with colorectal liver metastasis or prolong survival duration.

Objective To evaluate whether low-dose ropivacaine in combination with sufentanil could produce effective spinal anaesthesia for severe preeclamptic parturients during cesarean section.Methods 40 severe preeclampsia parturients undergoing elective caesarean delivery under combined spinal-epidural anaesthesia were randomly divided into two groups based on a computer-generated grouping number sheet,each group in 20 cases.Group A received intrathecal ropivacaine 15mg (1.5mL)+10% dextrose 1mL with normal saline 0.5mL.Group B received intrathecal ropivacaine 10mg (1mL) + sufentanil 5μg(1mL) + 10% dextrose 1mL.Blood pressure,heart rate and oxygen saturation were documented at 2 min intervals for 10 min after local anaesthetic was administered.Sensory and motor block were assessed at 2.5 min intervals after spinal injection.The side effects such as hypotension,nausea and vomiting during surgery were recorded.Results The onset of motor block was longer[(4.8±2.1) min vs.(4.2±1.5) min,t=-2.83,P<0.01],and the duration was shorter[(75.9±25.2) min vs.(115.4±29.5)min,t=10.75,P<0.01] in group B than those in group A.The number of patients who had Bromage score 3 in group A was more than that in group B.Compared with group A,the duration of complete analgesia[(180±35) min vs.(120±27) min,t=15.48,P<0.001]and effective analgesia[(245±29)min vs.(184±34)min;t=16.21,P<0.001] increased in group B.More patients were sleepy but easily rousable in group B than in group A (9cases vs.0cases,χ2=8.53,P<0.01).The incidence rate of hypotension was significantly higher(45.0% vs.20.0%,χ2=4.29,P<0.05) in group A than that in group B.Compared with group A,the incidence rate of hypotension(45.0% vs.20.0%,χ2=4.29,P<0.05),vomiting(30.0% vs.5.0%,χ2=4.33,P<0.05) and shivering (50.0% vs.10.0%,χ2=7.62,P<0.01) were lower in group B.The incidence of pruritus was significantly higher in group B (χ2=10.16,P<0.01).Conclusion Hyperbaric ropivacaine 10 mg with sufentanil 5μg could produce effective spinal anaesthesia for severe preeclamptic parturients during cesarean section with less hypotension,vomiting and shivering,and longer lasting analgesia than hyperbaric ropivacaine 15 mg.

Objective To examine how work stress influences somatization and work burnout, especially concerning for the difference between work load and interpersonal relationship stress in the correlation with work burnout. Methods Using cross-sectional design and questionnaire survey method, participants were 455 female nurses recruited from nine hospitals in Nanjing, China. Results The results revealed that work load and interpersonal relationship stress positively predicted somatization (βwork load=0.33,βinterpersonal relationship stres =0.22, P<0.01), emotional exhaustion (βwork load=0.57,βinterpersonal relationship stres =0.22, P<0.01) and cynicism (βwork load=0.49,βinterpersonal relationship stres =0.29, P<0.01), and negatively predicted personal accomplishment (βwork load=-0.18,βinterpersonal relationship stres =-0.21, P<0.01). Specially, the influence of work load on emotional exhaustion and cynicism was significantly higher than that of interpersonal relationship stress (Z=6.37, 3.57, P < 0.05), but there were no differences between them in personal accomplishment and somatization (Z=1.79, 0.47, P > 0.05). Conclusions There are significant differences between work load and interpersonal relationship stress in the correlation with work burnout.

Objective To explore the effect and mechanism of curcumin on rat cerebral ischemia reperfusion injury.Methods The rat model of cerebral ischemia reperfusion injury was constructed by the suture-occluded method.The effects of curcumin on cerebral infarction range,cerebral water content,neurological symptoms,cerebral histopathological morphology and expressions of PI3K,AKT,p-AKT,m-TOR,MDA,CAT,GPX,SOD,Bcl-2,Bax,Caspase-3 and Cleavage-Caspase-3 were evaluated.Results Cur-cumin had the protective effect on cerebral ischemia reperfusion injury,could alleviate the neurological symptoms,decreased the cer-ebral tissue pathological morphological changes and cerebral water content,in addition,which could alleviate the expressions of MDA,Bax,Cleavage-Caspase-3,IL-6,MCP-1 and TNF-αand increased the expressions of PI3K,p-AKT,mTOR,Bcl-2,Caspase-3, CAT,GPX and SOD.Conclusion The curcumin pretreatment has the significantly protective effect on cerebral ischemia-reperfusion injury,which may be associated with activating PI3K/AKT/mTOR signal pathway,while suppressing inflammation,apoptosis and oxidative stress.

Sepsis is the most common cause of death in infants and children in the world.Common infection sites leading to sepsis include respiratory tract,bloodstream,urinary tract,alimentary tract,central nervous system,and endocardia.Infection site is hypothesized to have the significant susceptibility.There is little epidemiological or national research data on the site of sepsis in China.In this article,we discussed the susceptibility of infection site in sepsis by literature review.Identifying infection site early would help guide clinical decision making in diagnosis, treatment and prognosis.

It has some advantages that high absorptivity and bioactivity, low toxicity, which contain broad pharmacological functions such as dropping high blood pressure, reducing blood fat, anti-inflammatory, anti-cancer, anti - aging, anti - fatigue and preventing cardiovascular and cerebrovascular diseases. In this paper, we summarized its physicochemical properties, pharmacologic action and the ophthalmological studies which mainly in keratopathy, cataract, glaucoma, uveitis, retinopathy, traumatic optic neuropathy and asthenopia. With the deeper research and more unequivocal in the pharmacological mechanism of proanthocyanidins, it should make the most of its values in medicine. And some more economical and effective drugs are expected to be provided for the clinical ophthalmology in the near future.

1727 arterial blood gas samples of 341 cases of acyanotic congenitla heart diseases,79 cases of cyanotic congenital heart diseases and 88 cases of acquired heart diseases during cardio-pulmonary bypass surgery were analyzed.It was found that there were various degrees of acid-base disorders right before the bypass was on.During the speration,acid-base imbalance was aggravated because of anesthesia,fluid infusion,hypothermia,surgical manipulation,etc.Respiratory alkalosis was most common to occur,and metabolic acidosis was the secodn.After the cessation of bypass,acid-base disorders could be significantly alleviated or imporved.So it is belived that it is very important to monitor the changes of blood gases and acid-base balance and to manage timely the complications during cardiopulmonary bypass.Fluid infusion,administration of agents to improve the microcirculation.and replacement of alkaline electrolytes are helpful to maintain acid-base balance.
