BACKGROUND:Clinical treatment of tibiofibular syndesmosis separation of unstable ankle fractures can be treated by surgical internal fixation method. The internal fixation process can choose different materials, in the past, most of the operations used metal pul ing nail, but there may be broken nails and other risks. OBJECTIVE:To explore the application effect of traditional metal pul ing nail and macromolecule absorbable screw fixation in tibiofibular syndesmosis separation of unstable ankle fracture. METHODS:The clinical data of 63 patients with tibiofibular syndesmosis separation of unstable ankle fractures who received surgical internal fixation treatment in Affiliated Hospital of Shaanxi University of Chinese Medicine from February 2013 to February 2014 were retrospectively analyzed. Patients were divided into metal pul ing nail group (n=32) and absorbable screw group (n=31) according to the use of different materials. The fracture end was reset by cutting from the outside of the lower section of the fibula. The steel plate was used for fixation. The traditional metal pul screws and macromolecule absorbable screw were used for fixation. During the 12 months of fol ow-up after internal fixation, the ankle function scores of two groups (Baird-Jackson), clinical effect and the incidence of adverse events were observed and compared. RESULTS AND CONCLUSION:The average ambulation time and hospital stay of patients after internal fixation in absorbable screw group were significantly less than those of metal pul ing nail group (P<0.05). The ankle joint function scores (Baird-Jackson) in these two groups were increased compared with those before surgery, but the difference was not significant (P>0.05). The excel ent and good rate of patients in absorbable screw group was significantly higher than that in the metal pul ing nail group (P<0.05). During 12 months of fol ow-up after internal fixation, one patient had screw breakage in metal pul ing nail group. There was no wound infection among patients in these two groups. These results confirm that the use of macromolecule absorbable screw fixation for the treatment of tibiofibular syndesmosis separation of unstable ankle fractures can obtain better application effect compared with the traditional metal pul ing nails.

Objective To present our experience in two-stage procedure immediate breast reconstruction, which aimed to retain local tissue condition for breast reconstruction, overcome the limitation of radiotherapy on implant reconstruction, and simplify the procedure as well as improve cosmetic result. Methods The proceduces of our method were divided into two stages:in the first stage, the round-shaped expander was implanted in the subpectoral major space during the procedure of mastectomy; at the same time as expander implanted, the first saline injection was performed; with 4 to 8 weeks of inflation, the tissue expander offered adequate tissue for breast reconstruction; in the second stage, silicon gel implant, latissimus dorsi muscle flap, extended latissimus dorsi muscle flap and deep inferior epigastic perforatior flap were used for breast reconstruction after the tissue expander was exchanged. Results Thirty-four patients had undergone the two-staged breast reconstruction using tissue expansion. The average time of therapy was 5.5 months. There were no postoperative complications such as implant exposure, additional scar, and flap necrosis. The follow-up time was 6 to 18 months and the result showed excellent contour of the breast at a satisfactory rate of 97.1%.Conclusions Two-staged technique using tissue expansion in breast reconstruction is easily done and the complications are rare. The technique possesses advantages such as avoiding affection of radiotherapy on silicongel implant and elasticity of skin, retaining maximal local tissue for breast reconstruction. There is no necessary for correction of skin defect and additional scarring, thus the patch-like appearance of breast is avoided. The patients need not to experience depression post mastectomy.Two-staged procedure immediate breast reconstruction is a safe and reliable technique that is especially applicable to the patients who need radiotherapy.

Journal of Applied Clinical Pediatrics 2006;0(13):-
Objective To investigate the changes of plasma adrenodullin(ADM) and C-type natriuretic peptide(CNP) level in children with chronic heart failure(CHF) and its clinical implications. Methods Forty-two children with CHF were collected. The patients suffered from dilated cardiomyopathy, congenital heart defects, and other heart diseases.According to a modified scoring system described by Ross and Reithman,16 patients were classified as class Ⅱ,14 as class Ⅲ,and 12 as class Ⅳ.Plasma levels of ADM and CNP were measured by radioimmunoassay assay in these patients and 11 healthy children.Echocardiography was performed to measure left ventricular function and the ratio of E/A.Results Plasma ADM and CNP levels of CHF patients were significantly elevated as compared with those of the control subjects [(218.27?106.53) ng/L vs(74.39?53.99) ng/L,P=0;(190.27?108.38) ng/L vs(92.59?(59.46) ng/L),P0.05).ADM levels were elevated with the advancing severity of CHF determined by a modified scoring system described by Ross. However, the plasma CNP levels in the normal state wasn′t significantly different from those observed in class Ⅱ.Likewise, the plasma CNP level in the class Ⅲ was not significantly different from that observed in class Ⅱ.Conclusions ADM and CNP might play a compensatory and defensive roles in the pathophysiology of the pediatric CHF.ADM may be a biochemical marker for evaluation the severity of the chronic heart failure in children, but also a new prognostic indicator of this syndrome.

Chinese Journal of Neuroanatomy 2005;21(3):252-258
In order to estimate the relationship between iron and the Alzheimer's disease, the behavioral and pathological changes were observed by Morris water maze and immunohistochemical staining respectively after injecting FeC13 into brain ventricle of rats. The apoptosis was tested by flow cytometry and the electron microscopy was used to observe ultrastructural changes. There were significant differences in escape latency of time and distance between normal animals and iron treated rats. Percentage and fluorescence intension (FI) of AnnexinV FITC loaded cells undergoing apoptosis were higher in iron treated rats compared with normal animals. Fawn-coloured products of β amyloid protein were interspersedly distributed in extensive areas of cerebral cortex and hippocampus. Under electron microscope, vacuolate degeneration of neuronal processes with mitochondria degeneration and accumulation of microtubule near vacuolar nucleus were observed in iron treated rats. These results suggest that a local higher concentration of iron in brain may induce Alzheimer-like impairment of intelligence and pathological changes.
