Three cases of renal pelvic neoplasm associated with staghorn calculi were enrolled to the study from March 2015 to June 2019. All patients underwent minimally invasive procedures previously including extracorporeal shock wave lithotripsy (ESWL), flexible ureteroscopic lithotripsy(FURL), or percutaneous nephrolithotripsy (PCNL). 3 patients of renal pelvic neoplasm with giant staghorn calculus were diagnosed by resection biopsy during laparoscopic pyelolithotomy intraoperatively and then treated by laparoscopic radical nephroureterectomy (LRNU). Case 1 was identified retroperitoneal lymph node metastasis 16 months after operation. Case 2 and 3 remained disease free. The timely diagnosis of renal pelvic neoplasm associated with staghorn calculi is always difficult, nevertheless, the laparoscopic pyelolithotomy with resection biopsy intraoperatively can be more effective in selected cases compared with routine endoscopic biopsy.

Objective To describe the clinical feature ,therapeutic approach and prognosis of histoplasmosis for improving clinicians’ awareness of this disease .Methods The clinical data of 7 cases of histoplasmosis treated in Shanghai Huashan Hospital from 2001 to 2014 were reviewed retrospectively .Relevant reports about histoplasmosis from 2001 to 2014 in Chinese mainland were comprehensively reviewed .Results The major clinical manifestations of progressive disseminated histoplasmosis included fever ,hepatosplenomegaly ,lymphadenopathy ,and pancytopenia .Skin lesions and pancytopenia were more common in the patients complicated with HIV/AIDS .The patients with local infection were lack of systemic symptoms or signs . Histological examination found Histoplasmacapsulatum in macrophages in bone marrow or biopsy tissues .Amphotericin B was used most frequently to treat histoplasmosis .Itraconazole was appropriate in mild patients .Conclusions Histoplasmosis is caused by H .capsulatum .The golden standard of diagnosis is any culture positive for H .capsulatum .Antifungal treatments such as amphotericin B and itraconazole are very important .

The author carried on bibliometrics statistical analysis of published papers during 2003-2012 by Disease control agencies of Chongqing and put forward some suggestions based on the results of the analysis.The results showed that in 10 years,the number of published papers for a total of 1154 with an average annual growth rate of 10.41％,of which 467 core journals,journal article 687.the difference between the municipal,district and county larger.The quantity and quality of published papers showed an upward trend,but there is still development space and the quality of the paper should be improved.

The framed storehouse and the open rolling shutter door equipment are designed and manufactured for the infrastructure construction.The multipurpose and moved frame operating platform is designed and manufactured according to the demands of loading and unloading as a whole for part medical equipments.In this way,the shortage of the storehouse space is solved and the usual training and moving become easier.

Objective To investigate the effect of allergen-specific immunotherapy on FEV1 and airway responsiveness in patients with idiopathic asthma.Methods 90 patients with idiopathic asthma from January 2013 to August 2015 in Deqing City People's Hospital were selected and randomly divided into control group and study group with 45 cases in each group.Control group were treated with anti-infection, relieving cough and eliminating phlegm, relieving spasm and relieving asthma, the study group was treated with allergen specific immunotherapy based on the control group, and a total of 3 months for a course of treatment.Blood samples were used to measure inflammatory mediators , airway sensitivity index, pulmonary function and airway responsiveness, at the same time, the clinical efficacy and complications were compared.Results Compared with before treatment, the levels of FVC, PEF, FEV1 and FEV1/FVC in the two groups increased, levels of AI, AO, T and WA decreased, levels of serum IL-4 decreased, levels of IL-10, IFN-γincreased, levels of serum IgE, SIgE, TIgE and EOS decreased, the difference was statistically significant (P<0.05), compared with the control group, the levels of FVC, PEF, FEV1 and FEV1/FVC in the study group were higher, the levels of AI, AO, T and WA were lower, serum IL-4 levels were lower after treatment, levesls of IL-10, IFN-γwere higher, levels of serum IgE, SIgE, TIgE and EOS were lower, the difference was statistically significant (P<0.05), and the effective rate in the control group (71.11%) was lower than the study group (88.89%), the difference was statistically significant(P <0.05).Conclusion Allergen-specific immunotherapy was effective for cough variant asthma, it can improve pulmonary function, inflammation and airway sensitivity.

OBJECTIVE:To study the influence of oscillator on the cleanliness of class 100 clean bench in PIVAS. METH-ODS:Using sedimentated bacteria and the number of dust particle as index,in common drug configuration room,antibiotics con-figuration room and risk drugs configuration room including biological safety cabinet and horizontal laminar flow,the cleanliness of class 100 clean bench were monitored when oscillator was set at clean bench and different positions in work and non-working state. RESULTS & CONCLUSIONS:In working and non-working state of oscillator,there was no difference in sedimentated bacteria and the number of dust particle which was in line with the requirements of 2010 edition of GMP,i.e. the application and location of oscillator didn't influence the cleanliness of class 100 clean bench. From a view of safety,it is suggested to place the oscillator in the left(or right)posterior wall of clean table when biological safety cabinets is used to dispense antibiotic and risk drugs.

Objective To investigate the efficacy of the indocyanine green (ICG) clearance test (ICGR15) combined with the model for end-stage liver disease (MELD) for assessing the short-term prognosis of patients with liver failure.Methods Eighty patients with liver failure were analyzed retrospectively.ICGR15 and relevant clinical data within 24 hours of diagnosis were analyzed.Meanwhile,the MELD score and King's College Hospital (KCH) were evaluated.All findings were tested for correlation with 3-month mortality.Quantitative data were analyzed with analysis of variance and Student's t-test.Count data were analyzed with chi-square test.Correlation analysis was performed with Pearson's coefficient test.Results Among 80 patients with liver failure,39 patients survived and 41 died.The mortality rate of all patients was 51.2％.The serum total bilirubin,creainine concentrations,ICGR15,MELD scores and patient number in accordance with KCH criteria of surviving patients were (288.0±109.1) μmol/L,(63.3±24.4) μmol/L,(48.1±10.2)％,20.6±4.4,and 6 cases,respectively,which were lower than those in dead patients [(340.7 ± 108.2) μmol/L,(98.8 ± 59.1) μmol/L,(60.2 ± 10.6) ％,26.9 ± 7.1 and 19 cases,respectively] (P =0.033,P＝ 0.001,P＝ 0.000,P＝ 0.000 and P =0.003,respectively).There was no significant difference of ICGR15 among four types of liver failure.A positive correlation was observed between ICGR15 and MELD score (r=0.289,P=0.009).The ICGR15-MELD model was created by subjecting ICGR15 and MELD scores to Logistic regression analysis.The following ICGR15-MELD model,Logit (P) =0.105 × ICGR15 + 0.178 × MELD score-9.734,was constructed by Logistic regression analysis.The area under the receiver operating characteristic (ROC) curve was 0.860 and the cut offpoint of 0.3 had sensitivity of 85.40％ and specificity of 74.40％.The area under the curve of the ICGR15-MELD model was significantly higher than those of ICGR15 (0.791),MELD score (0.770) and KCH criteria (0.655).Conclusions ICGR15 and MELD scores perform better than the KCH criteria in predicting the prognosis of liver failure.The ICGR15-MELD model is superior to ICGR15,MELD score,and KCH in predicting the short term prognosis of patients with liver failure.

In order to clarify the chemical constituents in Qiliqiangxin capsule, a rapid ultra-performance liquid chromatography/orthogonal acceleration time-of-flight mass spectrometry (UPLC-Q-TOF/MS(E)) method was established. Forty peaks were identified on line using this method. The herbal sources of these peaks were assigned. The results implied that triterpenoid saponins, flavonoid glycosides, C21-steroids and phenolic acids were included in the main components of Qiliqiangxin capsule. The method is simple and rapid for elucidation of the constituents of Qiliqiangxin capsule and the results are useful for the quality control of Qiliqiangxin capsule.

Objective To investigate the cytotoxicity of laser-welded nickel titanium (NiTi) and stainless steel composite archwire.Methods The NiTi and stainless steel composite archwire (CoAW) laser-welded with pure copper inrerplayer was studied with methyl thiazolyl tetrazolium (MTT) test in vitro.The cytotoxicity of CoAW was compared with stainless steel archwire and NiTi archwire.Two tests were carried out.Test 1:the immersed solution of CoAW was diluted to five grades (50％,40％,30％,20％,10％).The cytotoxicity in vitro of these agents was assayed on murine fibroblast cell L929 line with MTT test at 24 and 48 hours.Test 2:the immeresed solution of CoAW,NiTi archwires and stainless steel archwires was diluted to four grads (100％,75％,50％,25％).The cytotoxity of three kinds of material was compared at 48 hours.Results The results of all samples revealed level 0-1 cytotoxicity.In test 1,the same grade solution optical density (except 20％) at 24 hours was statistically lower than at 48 hours.In test 2,the optical density of CoAW solution (1.964 ± 0.122,2.084 ± 0.056,2.056 ± 0.071,2.096 ± 0.050) was statistically lower than the same grade solution of stainless steel archwire (2.168 ± 0.091,2.227 ± 0.160,2.302 ± 0.052,2.301 ± 0.060) and NiTi archwire (2.138 ± 0.105,2.262 ± 0.050,2.271 ± 0.082,2.294 ± 0.056) (P ＜ 0.05).Conclusions The MTT test of CoAW in vitro showed that cytotoxicity was related to concentration and time.The cytotoxicity of the CoAW was more serious than that of stainless steel and NiTi archwires.However,CoAW belonged to secure rang of material toxicity reaction.

Objective To investigate the feasibility of inserting and detaining nasointestinal feeding tube in small bowl guided by bedside ultrasound(US)in critically ill elderly patients.Methods This was a retrospective study.Sixty four aged patients(≥ 60 years)in general ICU,the Second Affiliated Hospital of Jiaxing College,received the US-guided nasointestinal feeding tubes inserting and detaining.Feeding tubes passed through nasal and went into the stomach by manual blind method.Under US-guiding condition,the tube passed through the pyloric sphincter and further into the duodenum or jejunum.Finally the correct position of the tube head was assessed by bedside X-ray examination.Results The US-guided nasointestinal feeding tube-detaining technique was successfully operated in 57 patients(89.1％).The feeding tube heads were in the duodenum in thirty four cases(53.1 ％),and in proximal jejunum in twenty-three cases(35.9％).The untoward reaction included the bleeding of nasal cavity in 1 case,and hypotension in another case.Conclusions Bedside US-guided nasointestinal feeding tube placement is safe and feasible in aged critical patients.
