2.Osteoporosis in patients with ankylosing spondylitis

Objectives To study the frequency and risk factors of osteoporosis,and the relationship between bone turnover makers and bone mineral density (BMD) in patients with ankylosing spondylitis (AS).Methods BMD in the spine and femoral neck was measured with dual-energy x-ray absorptiometry (DEXA) in 30 AS patients. Enzyme linked immunoassay was used to measure serum bone gla protein (BGP), bone-specific alkalin phosphtase (BAP), and urine deoxypyridinoline crosslinks (D Pyr).Results The patients in early stage lost the bone mass already.They had lower volume of BMD in both hip and spines than that of controls. But in the later stage,the BMD of spines was higher than that of the control group because of the calcification of the surrounding tissues of the spine.While even the BMD of femoral neck was still lower than that of normal subjects. Osteoporosis was observed in 6 patients, osteopenia in 10. Osteoporosis in AS correlated negatively with the disease duration, ESR, CRP, and X ray grades. The reduction in BMD in premenopausal women with early AS was slighter than that in men. The BMD had no significant differences in HLA B 27 (+) patients and HLA B 27 (-)patients. BGP and BAP which reflect bone formation had no differences in AS patients and controls. D Pyr which reflects bone resorption was significantly increased compared with controls.Conclusions Osteoporosis in AS is common and may be related to disease duration and severity. A significant increase in bone resorption is observed in AS with osteoporosis.

4.Clinical pathology of autoimmune cholangitis

Objective To investigate clinical and pathological features of autoimmune cholangitis (AIC) in antimitochondrial antibody (AMA) positive and negative patients.Method According to AMA presence or absence,37 primary biliary cirrhosis (PBC) patients were assigned to AMA positive group and AIC group.Clinical and pathological differences between two groups were compared.Results No differences were found in the level of liver function,liver cirrhosis,frequency of cholestasis and bile duct lesions between two groups.However,lower serum level of alkaline phosphatase,severer hepatocellular damage in pathology and higher AIH score were found in AIC group.Therefore the frequency accompanied by Sjgren′s syndrome (SS) in AIC group was significant higher.Conclusion AIC group is companied by slighter cholestasis and severer damage of hepatocytes indicating more autoimmune findings of AIC patients.AIC may have some correlation with SS in etiology and pathogenesis.

AIM: To study the potential effects of lipoteichoic acid (LTA)-induced delayed preconditioning (PC) on cardioplegic arrest/reperfusion injury in donor rat heart. METHODS: The rats were pretreated with LTA (1 mg/kg, ip) 24 h before the experiment, and the isolated hearts were subjected to arrested by cardioplegic solution and stored in Eurocollin's solution for 4 h by the Langendorff method, and to evaluate the changes of cardiac function at the reperfusion for 30 min and 60 min, to measure the amounts of MB isoenzyme of creatine kinase (CK-MB), lactate dehydrogenase (LDH) and total nitric oxide (NO) oxidation products in the coronary effluent, and to detect myocardial apoptosis on tissue samples of left ventricle at the end of reperfusion by TUNEL staining. Aim To explore the effects of lipoteichoic acid (LTA) induced delayed preconditioning (PC) on hypoxia-reoxygenation (H/R) injury of cultured human coronary artery endothelial cells (HCAECs), and to investigate the potential role of endogenous nitric oxide (NO) participated in the protective mechanism. Methods HCAECs were incubated for 2 h in a hypoxic atmosphere and reoxygenated for 4 h in a normoxic atmosphere. The delayed PC was induced by pretreatment with LTA assessed by the percentage of cellular injury with Trypan blue exclusion and by the amount of lactate dehydrogenase (LDH) in culture media. The NO level of the culture media was measured detect the expression of eNOS mRNA by RT-PCR method after cells were recovered from different points.Results LTA pretreatment significantly decreased the percentage of the killed cell and the concentration of LDH in media. Also, LTA pretreatment obviously raised the concentrations of NO in culture media. The protective effects of LTA were abrogated by pretreatment with N-monomethyl-L-arginine (L-NMMA).Moreover, the expression of eNOS mRNA was significantly upregulated after HCAECs exposure to LTA for 4 h following 2 h or 4 h recovery. Conclusion LTA could induce the delayed protection against H/R induced endothelial injury and dysfunction of cultured HCAECs. NO produced by eNOS acts initially as a trigger and subsequently as a mediator of delayed PC.
7.The proliferative effect on silencing of endogenetic mdr1 gene expression by RNA interference in ovarian cancer resistant strain SKOV3/TAXOL

Objective To construct the short hairpin RNA recombinant plasmids targeting mdr1 gene which expresses highly in ovarian cancer resistance strain SKOV3/TAXOL to silence endogenefic mdr1 gene expression and investigate the role of mdr1 gene in the development of resistant ovarian cancer. Methods The pGPU6/GFP/Neo-mdr1 were constructed by gene clone technology. The influence on proliferation and apoptosis were investigated by CCK-8 in SKOV3/TAXOL after transfected pGPU6/GFP/Neo-mdr1. Results The expression against mdr1 proteins were inhibited by pGPU6/GFP/Neo-mdr1. The cell proliferation were inhibited after transfected pGPU6/GFP/Neo-mdr1 by CCK-8. The apoptosis were observed in DAB experiments and the apoptosis rate increased. Conclusion mdr1 plays an important role in proliferation of resistant ovarian cancer and the short hairpin RNA of mdr1 can efficiently suppress mdr1 expression and enhance the apoptosis in SKOV3/ATAXOL.
8.Value of the expression of drug-resistance gene and in vitro drug-sensitivity test in tissue of patients with ovarian carcinoma by individualized treatment

Objective To study the relation between the expression of drug-resistance gene and in vitro drug sensitivity test in ovarian carcinoma and clinical biological behavior.Methods In 60 primarily treated patients with epithelial ovarian carcinoma,the expression of COX-2,Connexin43(CX43)and P-gp was detected using flow cytometry.Sensitivity of cells of short term in vitro culture to anticancer drugs was also examined by the game techniques.Remits The expression level of COX-2 was higher in tissue with DDP-resistance patients than those with sensitivity patients but the expression level of CX43 was lower in tissue with DDP-resistance and Taxol-resistanee patients than those with sensitivity patients.The expression level of P-gp was higher in tissue with VP16-resistance patients than those with sensitivity patients.The sensitivity of DDP,Taxol and VP16 was lower in tissue with higher expression of COX-2 and P-gp than those with lower expression of COX-2 and P-gp.The sensitivity of DDP and Taxol was higher in tissue with higher expression of CX43 than those with lower expression of CX43.Conclusion The in vitro sensitivity of some of the drugs is effected by tissues with expression of COX-2,P-gp and CX43.It is not only useful but also individualized treatment for application of sensitive drug test and detecting the expression of COX-2,P-gP and CX43 to increase the response rate of chemotherapy in patients with epithelial ovarian carcinoma.
9.Effects of lipoteichoic acid induced delayed preconditioning on ischemia-reperfusion injury in isolated rat hearts.

To explore the potential of lipoteichoic acid (LTA) induced cardioprotection against ischemia-reperfusion (I/R) injury in isolated rat hearts and whether endogenous nitric oxide (NO) participates in the protection, the rats were pretreated with LTA (1 mg/kg, i.p.) 24 h before the experiment, and the isolated hearts were subjected to 30 min no-flow normothermic global ischemia and 60 min reperfusion after a 20-min stabilization period by the langendorff method. Cardiac functions were evaluated at the end of stabilization, and at 30 min, 60 min of reperfusion. The amounts of MB isoenzyme of creatine kinase (CK-MB), lactate dehydrogenase(LDH) and total NO oxidation products in the coronary effluent were measured spectrophotometrically at the end of reperfusion. It was revealed that pretreatment with LTA could significantly improve the recovery of cardiac function, reduce the release of CK-MB and LDH, and increase the concentrations of NO in coronary effluent. The protective effects were abrogated by pretreatment of the rats with L-NAME. It was concluded that LTA could induce the delayed cardioprotection against I/R injury, and endogenous NO may be involved in the mechanisms.

10.Minimally invasive percutaneous fixation based on ISO-C3D computerized navigational system for fracture of the acetabulum

BACKGROUND:Percutaneous hol ow screw under X-Ray fluoroscopy has been shown to treat fracture of acetabulum of the pelvis, but the time of internal fixation was long, and the amount of radiation exposure to the patients and physicians was large.
OBJECTIVE:To test the application of the ISO-C3D METHODS:Thirty-one patients with fracture of the acetabulum were treated with percutaneous hol ow screw under a fluoroscopy-based ISO-C computerized navigational system for fracture of acetabulum. 3D computerized navigational system. The interval from injury to operation was 4 to 13 days. Al patients were fol owed up for one year.
RESULTS AND CONCLUSION:The average bleeding volume during operation was only 18 mL, except that the bleeding volume of only one patient, who suffered from the sacroiliacjoint injury and received open reduction and internal fixation, was up to 300 mL. The total number of screws used in the operation was 42, among which 24 were screws for acetabular anterior column fracture and 18 for posterior column fracture. Al screws were implanted once precisely. The average time of internal fixation was 59 minutes, and the mean time for fluoroscopy was 39 seconds. The 31 patients were pain-free one week after the operation and no complication (infection, vascular nerve injury or implant breakage) was noted post-operatively. When the fol ow-up ended, radiography revealed fracture union with satisfactory screw fixation (no screw breakage or loosening). According to Matta functional scoring, results were excellent in 23 cases, good in 8 cases, with an excellent and good rate of 100%. According to Majeed functional scoring, the results were excellent in 22 cases, good in 6 cases and average in 3 cases, with an excellent and good rate of 90%. These results indicated that ISO-C3D computerized navigational system can supply stable internal fixation without an increase of complication.
