1.The influence of Clopidogrel combined with Aspirin in treatment of young people with cerebral artery stenosis and blood plasma P-selectin

Objective To study the influence of Clopidogrel combined Aspirin to treat cerebral artery stenosis of young people and P-selectin in blood plasma. Methods 142 cases were randomly divided into therapy group (70 cases) and control group (72 cases). The two groups were given Aspirin ( 0. 1/d, Bayer company) by peroral administration or nasal feeding( deglutation disturbance) and simultaneously given tatin drug, Venoruton injection, Citicoline and Salvia Miltiorrhiza injection. The therapy group were added clopidogrel(75mg/d,Shenzhen Salubris Pharmaceuticals Co, Ltd). Venous blood was taken suction to detect platelet packing fraction, P-selectin and Doppler ultra-sound and carotid artery blood flow velocity before medication,24 hours after medication ,7 days after medication. Results Platelet packing fraction, P-selectin were significantiy decreased in the two groups than before treatment( P <0. 05)and therapy group were sigrificantly lower than control group( P < 0. 01 ). The control group after 7 days of treatment, transcranial Doppler ultrasound and carotid artery blood flow velocity significantly improved ( P < 0. 05 ), but the treatment group more significantly (P < 0. 01 ). Conclusion Blood plate was activated in patients of cerebral artery stenosis of young people, Clopidogrel combined with Aspirin could improve the blood rheology and hemodynamics, reduce the volume of cerebral lesions and improve the prognosis of patients.
2.Clinical Observation of Shenfu Injection Combined with Linezolid in the Treatment of Severe Pneumonia Caused by Methicillin-resistant Staphylococcus aureus

OBJECTIVE:To observe clinical effect and safety of Shenfu injection combined with linezolid in the treatment of severe pneumonia caused by methicillin-resistant Staphylococcus aureus (MRSA). METHODS:In retrospective study,62 MRSA severe pneumonia patients selected from ICU of our hospital during Jun. 2012-Oct. 2015 were divided into observation group (25 cases)and control group(28 cases)according to medication plan. Based on routine treatment,control group was additionally given Linezolid injection 600 mg,ivgtt,bid. Observation group was additionally given Shenfu injection intravenously with initial dose of 60 mL,and then given continuous intravenous infusion of 60 mL at a rate of 20 mL/h,q12 h,on the basis of control group. Both groups were treated for consecutive 2 weeks. Clinical efficacy,bacteriological efficacy,cardiac function indexes and serum levels of inflammatory factors before and after treatment as well as the occurrence of ADR were compared between 2 groups. RESULTS:The clinical response rate of observation group was 88.00%,which was significantly higher than 60.71% of control group,with statistical significance (P<0.05). Bacterial clearance rate of observation group was 72.00%, and that of control group was 64.28%,there was no statistical significance between 2 groups(P>0.05). Before treatment,there was no statistical significance in cardiac function indexes and serum inflammatory factor levels between 2 groups(P>0.05). After treatment,LVEF,SV,CO and CI of 2 groups were increased significantly compared to before treatment,while the levels of TNF-α,IL-6,CRP and PCT were de-creased significantly;the indexes of observation group was significantly better than those of control group,with statistical signifi-cance (P<0.05). No severe ADR was found in 2 groups. CONCLUSIONS:Shenfu injection combined with linezolid effectively improves the cardiac function of patients with MRSA severe pneumonia,enhances the anti-inflammatory effect,and have a very significant clinic effect with good safety.
3.Thin sectional anatomy of the cardiac septum and surrounding structure

Objective To study the sectional anatomy of the cardiac septum to provide the reference for clinical imageology and surgery. Methods Sixteen normal adult hearts without organic lesions were verified macroscopically. After vascular perfusion, the specimens were embedded with gel, fixed with 5% formalin, and cryopreserved for a week, and then were sectioned with the Digital Sectioner. Results A total of 1 608 slices (thickness: 0.2 mm) of the heart were obtained. Cardiac septum and the surrounding structures were shown clearly. The demarcation of connective and muscular tissues was clear. The diameters of the cardiac apex, fossa ovalis, brawny intraventricular septum, left ventricle, right ventricle, left and right fibrous trigone were measured with Photoshop6.0. Conclusion The clear images can display tiny structures that could be measured, which could provide anatomical references clinical imageology and surgery.

BACKGROUND:Recent studies have shown that adipose-derivedmesenchymal stem cells (ADMSCs) not only have multilineage differentiation potential, but also exert an important role in blood sugar balance and hormone production.OBJECTIVE:To observe the differentiation potential of human ADMSCs (hADMSCs) into functional islet-like cells and the therapeutic effect of hADMSCs transplantation in diabetic rats.METHODS:PDX-1 gene was transfected into hADMSCs by adenovirus. Cell differentiation and insulin secretion were identified and detected by dithizone staining and ELISA, respectively. Twenty male Sprague-Dawley rats were randomly divided into control group (n=4), diabetes group (n=8) and transplantation group (n=8). Rats in the latter two groups were subjected to making diabetic models by 65 mg/kg streptozotocin injection. Afterwards, rats in the transplantation group were given PDX-1 transfected ADMSCs via the tail vein.RESULTS AND CONCLUSION:At 15 days after transfection, the number of insulin positive cells and insulin secretion were both increased significantly (P < 0.05). Fasting glucose levels in the transplantation group decreased significantly (P < 0.05), while the body weight increased significantly (P < 0.05). In the diabetic group, the fasting glucose level still maintained at a high level, and the body weight of rats was significantly decreased. These results implicated that PDX-1 gene could induce hADMSCs differentiating into functional islet-like cells. PDX-1 transfected ADMSCs transplantation is effective in treating diabetic rats, but the mechanism needs further study.
6.The value of procalcitonin in the diagnosis and prognosis of stoke-associated pneumonia

Objective To observe the value of procalcitonin (PCT ) in the diagnosis ,treatment and prognosis judgement of stoke-associated pneumonia(SAP) .Methods The serum procalcitonin ,hs-CRP and clinical pulmonary infection scores (CPSI) of two groups were detected :SAP group(including 27 patients of stoke-associated pneumonia) and control group(including 31 healthy ob-jects) .Results The CPSI ,hs-CRP and serum concentration of PCT in SAP group were significantly higher than control group (P<0 .01) ,and decreased significantly when get better after treatment .The serum concentration of PCT in SAP group was significantly higher than control group(P<0 .01) .In SAP group ,the serum concentration of PTC was positively associated with CPSI scorers . Moreover ,the results of ROC analysis showed that the cut-off of serum procalcitonin for the diagnosis of SAP was 0 .9 ng/mL .The sensitivety was 88 .6% and the specifity was 79 .3% ,which was superior to hs-CRP .Conclusion The serum concentration of PCT in SAP patients is higher and it is associated with the prognosis .It might be a better clinical marker for the diagnosis and prognosis of SAP .
7.Studies on cognitive function in patients of first-episode schizophrenia with and without metabolic syndrome

Objective To discuss the difference of cognitive function in patients of first-episode schizophrenia with and without metabolic syndrome.Methods Event related potential detection and Wechsler Memory Scale (WMS) was determined in 57 patients of first-episode schizophrenia with metabolic syndrome (research group) and 57 patients of first-episode schizophrenia without metabolic syndrome (control group),and the psychiatric symptom in two groups was assessed by the application of the positive and negative symptoms scale (PANSS).Results The memory quotient score was (78.51 ± 12.54) scores in research group,and (92.01 ± 17.49) scores in control group,and there was significant difference between two groups (t =-2.130,P=0.047).The short-term memory,instantaneous memory impairment in research group were lower than those in control group(t =2.151-3.054,P=0.007-0.046).The latency of P300 was (325.10 ±22.46) ms in research group and (284.60 ±22.46) ms in control group,and there was significant difference (t =3.454,P =0.003).The amplitude was (6.56 ± 1.61) μ V in research group and (9.78 ±1.73) μV in control group,and there was significant difference (t =-4.307,P =0.000).Conclusion The patients of first-episode schizophrenia with metabolism syndrome has more severe cognitive impairment than those without metabolism syndrome.
8.Effect of computer-assisted cognitive remediation therapy on cognition in recovering patients with major depressive disorder

Objective:To investigate the effect of computer-assisted cognitive remediation therapy (CACR) on the improvement of cognitive functioning in patients with major depressive disorder during the recovery period.Methods:Sixty-seven patients with major depressive disorder, during the recovery period, were included according to the inclusion criteria. All of them were from Hangzhou Seventh People’s psychiatry department and were selected during the September 2016 to September 2018 time period and were randomized into the CACR group and the observation group. The previously used conventional drug treatment was continued for 8 weeks in both groups, but an additional 8-weeks was given to the CACR group, mainly for intensive attention training and memory. The CACR training time was 8 weeks, once a day each for attention and memory training for 20 minutes. Before treatment and at the end of the 8-week treatment, the Wisconsin Card Sorting Test (WCST), Trail Making Test A, Trail Making Test B, and Stroop test were used to measure cognitive function.Results:67 patients with depression during the remission period were included. The control group included 32 patients, 18 males (57%) and 14 females (43%), aged (29.0±9.2) years old; 35 patients in the intervention group, 20 males (56%) and 15 females (44%), aged (28.0±9.2) years old. There were no significant differences between the two groups in terms of gender, age, years of education, scores on the Chinese version of Webster's adult intelligence scale, scores on Hamilton depression scale, and course of illness. There were no significant differences between the groups in terms of WCST performance, TMT performance, and scores on the Stroop test at baseline ( P>0.05). After the 8-week treatment, the CACR group demonstrated better performance on the mean number of trials [(76.8±14.3) vs.(83.6±14.6)], the number of correct classifications [(27.9±1.8) vs.(26.6±2.6)], perseverative errors [(24.4±3.3) vs.(27.4±4.8)],non-perseverative errors [(17.97±3.1) vs.(22.2±4.3)], and the mean time for completing part B of TMT [(86.1±15.6) vs.(119.6±16.2)]. However, there were no significant differences between the groups on mean number of categories completed, mean time for completing part A of TMT, the right numbers in Stroop-C and Stroop-CW, and the completed numbers in Stroop-C and Stroop-CW ( P>0.05). Conclusion:CACR can effectively improve the cognitive function in patients with major depressive disorder who have stabilized.

Objective:To study the expression of ULBP2 protein in the brain tissues of patients with drug-refractory epilepsy and its clinical significance.Methods:Gene-chip,immunofluorescence and Western blot were used to test expression of ULBP2 in the surgically removed brain tissue of patients with drug-refractory epilepsy from the brain bank of our department(n=42),and the results were compared with that of normal controls (n=12).Results:The relative increasing expression of ULBP2-gene in the brain of patients with drug-refractory epilepsy,and ULBP2 protein expression was significantly increased in temporal lobe cortex of patients with drug-refractory epilepsy as compared with the same regions of the controls specimens.Conclusion:The results indicate that the overexpression of ULBP2 may be involved in the pathophysiology of drug-refractory epilepsy.

Objective To investigate expression of extracelluar signal regulated protein kinase(ERK)and phosphorylation ERK(p-ERK)in the temporal lobe from patients with DR-TLE so as to explore the possible roles of ERK in the pathogenesis of DR-TLE.Methods Expression of ERK was detected with Western blot and immunohistochemistry in 32 patients with DR-TLE(24 temporal lobe,8 hippocampi),as compared with 12 controls(9 temporal lobe,3 hippocampi).Results ERK and p-ERK expression in DR-TLE was significantly higher(0.2266?0.0613,0.2097?0.0183 and 0.1924?0.0054,respectively)than those of controls(0.1840?0.0023,0.1974?0.0056 and 0.1825?0.0063,respectively,all P
