1.The analysis of Montreal classification for 102 patients with Crohn's disease

Objective To analysis the features of Montreal classification,serum markers,treatment and prognosis of Chinese patients with Crohn's disease(CD),and to estimate the relationships between clinical classification and anti-sarccharomyces cerevisiae antibody(ASCA),treatment and prognosis.Methods A retrospective study of 102 consecutive definite CD eases were performed and all subjects were classified into subtypes according to Montreal classification.The results of ASCA,extra-intestinal manifestation,treatment and disease behavior at follow-up were recorded and compared among different subtypes.Results The A2 subtype(52.9%)was dominated in Chinese CD patients.Ileocolon location(40.2%)and stricture lesion (53.9%)were common.The complication rate was dependent on disease course(P<0.05),and intestinal fistula was associated with disease location(P=0.074).B1 subtype had higher progressive rate than B2 subtype(P=0.018).ASCA was not associated with disease loeation,disease behavior,treatment and disease progression(P>0.05).Conclusions Crohn's disease mainly attack young people with male predominance.Early-onset CD patients have higher ASCA positive rate and disease progressive rate.The disease behavior progresses associated with disease course,and the rate of complication and the increase of surgery.The penetrating behavior is the main cause for surgery.Montreal classification is useful to predict the disease course,the need for surgery as well as the prognosis.
2.Changes in number and function of peripheral blood Th 17 cells in acute urticaria patients

Objective To study the changes in number and function of peripheral blood T helper cell 17(Th17) in acute urticaria patients .Methods The percentages of Th17 in peripheral blood from 50 acute urticaria patients and 50 healthy persons were ana‐lyzed by immunofluorescence staining and bicolor flow cytometry (FCM ) in vitro .Retinoic acid‐related orphan nuclear receptor γt (RORγt) mRNA from the same research objects were detected by quantitative realtime PCR (qPCR) method .Peripheral blood transforming growth factor‐β(TGF‐β) ,interleukin(IL)‐6 ,IL‐17A and IL‐17F were tested by enzyme linked immunosorbent assay (ELISA) from the same research objects .Results The quantity of percentages of Th17(t= 36 .634 1 ,P< 0 .05) and levels of RORγt mRNA(t=23 .840 1 ,P<0 .05) in urticaria patients were higher than those in healthy persons obviously .Meanwhile ,the levels of TGF‐β(t=15 .521 1 ,P<0 .05) ,IL‐6(t=7 .247 3 ,P<0 .05) ,IL‐17A(t=15 .415 3 ,P<0 .05) and IL‐17F(t=13 .032 1 , P<0 .05) in urticaria patients were higher than those in healthy persons significantly .Conclusion Dysfunction of Th17 in periph‐eral blood may involve in the immunopathogenesis of acute urticaria .
3.Effects of breviscapine on the renal structure, function and PKC-mRNA and its protein expression in brain-dead BA-Ma mini pigs

Objective To investigate the effects of breviscapine on the renal structure, function and PKC-?mRNA and its protein expression in brain-dead BA-Ma mini pigs. Methods Fifteen BA-Ma mini pigs were randomly divided into 3 groups: brain-dead group (group A), breviscapine pretreatment group (group B), and control group (group C), 5 pigs in each group. The brain-dead models were established by increasing intracranial pressure in a modified, slow and intermittent way. At 3, 6, 12, 18 and 24 h after the initial brain death, serum BUN, Cr, TNF-?, IL-1?, and IL-6 were determined. At 3, 6, 12, and 24 h, the changes of renal tissues were observed by HE staining, and the expression of PKC-?mRNA and protein was detected by RT-PCR and immnohistochemistry respectively. The ultrastructural changes of hepatic cells were observed under electron microscopy. 6.Corrective therapy for different kinds of open bite malformations

Objective To investigate the effects of a corrective tr ea tment on open bite malformations, such as gnash their teeth, nail-biting, tongu e thrusting, impacted tooth, cross bite with improper correction, open bite defo rmity resulted from premature shedding of deciduous tooth and molar cysts. Methods We used dental resin technology: acid etching the teeth, adh ering metal clasps to the teeth by means of composite resin, and applying elasti c bands for intermaxillary traction to correct the cross bite. Results We had treated 22 patients, age ranged from 7-30, with the malformatio ns resulted from different kinds of reasons, and obtained significant results. Conclusion With cooperation of the patients, simple corrections for open bite with sticky clasp and occlusion with elastic band are easy to car ry on, with short period of treatment time, and every patient gets prominent out come. It is effective, prior to commonly-used corrective therapy and deserves t o be widely used.
7.Fracture resistances of pulpless teeth restored with two kinds of cast metal post-core or glass-fiber post with a composite core

Objective: To compare the fracture resistance of pulpless teeth restored with three types of post core.Methods: A total of 21 recently extracted sound upper incisors were randomly distributed into 3 groups. In group A the teeth were restored with prefabricated glass fiber post and composite core; in group B with custom cast titanium alloy post and core; and in group C with Ni Cr alloy dowel core. The size and shape of posts were identical in the 3 groups. All teeth were fully covered with Ni Cr alloy cast crown. Fracture resistance was measured by applying a point force at 130 degrees to the long axis of the teeth. Results: Mean fracture threshold was (276.6?15.1)N in group A, (394.7?29.1)N in group B and (373.4? 53.8 )N in group C. In group B and C, however, fracture always affected the tooth itself,whereas in group A,the core always failed first. Conclusion: Fiber reinforced composite(FRC) post may meet the clinical need, FRC post combined with composite core may prevent tooth root from fracture.

Objective To explore the relationship between superior vena cava-jugular vein oxygen saturation-differential value (ScVO2-SjVO2) and injury severity and prognosis in patients suffered from severe traumatic brain injury,finding out the chnical value.Methods Forty-three patients with severe tranmatic brain injury were selected and divided into two groups according to Glasgow Coma Scale (GCS) score,19 cases were enrolled in GCS 3-5 scores group;24 cases in GCS 6-8 scores group;20 contemporaneous cases in control group had conscious and without traumatic brain injury.The changes of ScVO2-SjVO2 at 0,12,24,48 h after admission of ICU among the groups were observed.In addition according to the degree of change in ScVO2-SjVO2,12 cases were enrolled in ScVO2-SjVO2 < 0.05 group;14 cases in ScVO2-SjVO2 0.05-0.15 group; 17 cases in ScVO2-SjVO2 > 0.15 group.The GCS score and mortality rate in patients after brain injury 28 d among groups were observed.Results GCS 3-5 scores group compared with control group,ScVO2-SjVO2 reduced significantly at 0,12,24,48 h after admission of ICU (-0.071 ±0.032 vs.0.093 ±0.049,-0.097 ±0.052 vs.0.081 ±0.052,-0.035 ±0.031 vs.0.089 ± 0.059,0.036 ± 0.033 vs.0.081 ± 0.064),there were significant differences (P < 0.01).GCS 6-8 scores group compared with control group,ScVO2-SjVO2 dramatically increased at 0,12,24 h after admission of ICU(0.173 ± 0.043,0.158 ± 0.067,0.123 ± 0.073),there were significant differences (P < 0.01 or < 0.05).ScVO2-SjVO2 <0.05 group and ScVO2-SjVO2 >0.15 group compared with ScVO2-SjVO2 0.05-0.15group,the mortality rate significantly increased,the mortality rate were 8/12,4/17,1/14,respectively,there were significant differences (P <0.01).ScVO2-SjVO2 <0.05 group and ScVO2-SjVO2 >0.15 group compared with ScVO2-SjVO2 0.05-0.15 group,GCS score decreased significantly[(4.17 ± 1.34),(5.88 ±1.35) scores vs.(6.86 ± 1.35) scores],and there was significant difference (P <0.01 or <0.05).Conclusions When ScVO2-SjVO2 < 0.05,the difference is negatively related to the deg~e of brain injury.When ScVO2-SjVO2 >0.15,the difference is positively related to the degree of brain injury;ScVO2-SjVO2 can assess injury severity objectively in patients who suffered from severe traumatic brain injury,and it also expresses the rate of cerebral oxygen metabolism.It can provide a basis for treatment accurately and timely,and judgment prognosis.
9.Effects of prucalopride combined with polyethylene glycol electrollytes powder in bowel preparation for colonoscopy in 124 elderly patients

Objective To evaluate the colon cleaning efficacy and safety of prucalopride combined with polyethylene glycol electrollytes powder in bowel preparation for colonoscopy examination and treatment under colonoscopy in elderly patients.Methods Elderly patients who prepared for colonoscopy were enrolled from October 2014 to January 2015.They were divided into group A,B and C.Patients of group A only took polyethylene glycol electrolyte powder (137.12 g dissolved in 2 000 mL warm water,stired until completely dissolved),patients of group B took polyethylene glycol electrolyte powder (same usage as group A) combined with prucalopride 2 mg/d for one day,patients of group C took polyethylene glycol electrolyte powder (same usage as group A) combined with prucalopride 2 mg/d for three days.The bowel preparation effects,especially ascending colon and ileocecal junction and cardiovascular adverse effects in all the patients and constipation patients of three groups were compared.Student t test,chisquare test and analysis of variance were performed for statistical analysis.Results A total of 124 elderly patients were divided into group A (64 cases),group B (23 cases) and group C (37 cases).Bowel preparation scores of total colon and ascending colon-ileocecal junction of group A,B,C were 8.34±0.90 and 2.56±0.56,8.96±0.21 and 2.96±0.21,8.86±0.35 and 2.86±0.35,respectively.The cleaning effects in total colon and ascending colon-ileocecal junction of group B were better than those of group A (t=5.52 and 5.15,both P<0.05).The cleaning effects in total colon and ascending colon-ileocecal junction of groupCwere better than those of group A (t=4.69,3.65,both P<0.05).There was no statistically significant difference between group B and group C in cleaning effects in total colon and ascending colon-ileocecal junction (both P>0.05).There were 33,11 and 29 constipation cases in group A,B and C,respectively.The bowel preparation scores for total colon and ascending colon and ileocecal junction of group A,B,C were 7.85±0.94 and 2.24±0.56,8.91±0.30 and 2.91±0.30,8.83±0.38 and 2.83 ± 0.38,respectively.The cleaning effects in total colon and ascending colon-ileocecal junction of group B were better than those of group A (t=6.72 and 5.49,both P<0.05).The cleaning effects in total colon and ascending colon-ileocecal junction of group C were better than those of group A (t=104.04 and 59.65,both P<0.05).There was no significant difference between group B and group C in cleaning effects in total colon and ascending colon ileocecal junction (both P> 0.05).No obvious cardiovascular adverse events were observed in the patients of three groups.Conclusions The cleaning effects of prucalopride combined with polyethylene glycol electrolytes powder are good for colonoscopy examination in elderly patients,including elderly patients with constipation,especially in ascending colon and ilecocecal junction.No obvious cardiovascular adverse events are observed.
10.The protection effects of glycine pretreatment on brain dead donor liver

Objective To evaluate the protection effects of glycine on the brain dead donor liver. Methods 42 male Wistar rats were randomized into 3 groups of liver transplantation: brain dead donor (BDD) group (group B), glycine pretreatment group with BDD (group G), strychnine pretreatment group with BDD (group S). For groups B, G and S, the brain death model was established in the donor rats and then liver transplantation was performed utilizing microsurgical techniques. After establishment of brain death state, and during liver cold rinse of donors or liver reperfusion of the recipients, rats in group B were treated with glycine at a dose of 0.6 mmol, 25 ?mol and 25 ?mol in group G, and rats in group S were given the same dose of glycine and strychnine ( 1 000 ∶1), and rats in group B were not treated. Before the cold rinse, at 2 h and 6 h after the portal vein (PV) reperfusion, blood samples were taken from IHVC to determine the levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), tumor necrosis factor alpha (TNF ?) and hyaluronic acid (HA). At 6 h after PV reperfusion, graft samples were fixed for morphological observation and the apoptosis of hepatocytes was detected by using TUNEL method. Results At the time points before liver cold rinse or at 2 h and 6 h after PV reperfusion, serum levels of ALT, AST, TNF ?, HA and apoptosis index (AI) in groups B and S were significantly higher than those in group G ( P 0.05 ). Electron microscopy showed that Kupffer cells were activated and hepatic cells injured more obviously in groups B and S than in group G. Conclusion Glycine may alleviate the injury of the graft from the brain dead rats.
