1.Comparative analysis of clinical characters between acute focal bacterial nephritis and acute pylonephritis

Objective To improve standards of diagnosis and therapy for acute focal bacterial nephritis bY comparing the characters of acute focal bacterial nephritis and acute pylonephritis.Methods Thirty-five patients of upper urinary tract infection whoever accepted ultrasonographic and computed tomographic (CT) examinations in Beijing Hospital from January 2007 to January 2013 were studied retrospectively.Eighteen patients were diagnosed as acute focal bacterial nephritis (AFBN) according to CT imaging features,the other 17 patients were diagnosed as acute pyelonephritis (non-AFBN).The clinical feature,laboratory and imaging examination were compared between the two groups.Results Leukocyte count,percentage of neutrophil,fasting blood-glucose,ESR,NAG/Cre of urine,the combination of more than one kind of antibiotics and duration of antibiotic treatment were higher or much more in AFBN group patients than non AFBN patients,the differences between two groups were statistical significance(P < 0.05).Urine microorganism culture was mainly Escherichia coli no matter in AFBN group or non-AFBN group,microorganism species of AFBN patients were more complex than non-AFBN patients.The most common contrast-enhanced CT feature of AFBN included local distribution of inflammatory area,wedge-shaped or unregular-shaped hypodense lesions on bilateral or single kidney,and mass-like hypodense lesions in more severe cases.NAG/CRE of urine was higher in AFBN group patients with bilateral kidney lesions than with single kidney lesions and non AFBN patients.The remission time of AFBN and non-AFBN was (3.4 ± 2.9) d,(3.0 ± 1.8)d respectively.The relapsing rate of AFBN and non-AFBN within half a year was 2/18,2/17,respectively.Conclusions The symptoms of acute focal bacterial nephritis are not specific.Abdominal contrast-Enhanced CT,ESR,leukocyte count,urine NAG/CRE are probably a predictor of AFBN.Antibiotic treatment with appropriate usage and sufficient duration could bring satisfactory outcomes and prognosis in acute focal bacterial nephritis and pyelonephritis patients.
2.Teaching reform of preventive medicine course for clinical medicine speciality

Objective To meet the needs of the development of modern medicine and train health technical talent for basic level. Methods Using heuristic approach,and multi-form discussions,case teaching method,reform teaching methods,optimizing teaching content. Result Survey results showed that the satisfaction degree was high and it effectively enhanced the concept of preventive medicine and the actual working capacity. Conclusion Through teaching reform,the students’ ability to prevente and cure diseases,to analyze and solve problem has been markedly enhanced,and this approach can be widely used.
3.Application of absorbable anti-bleeding gauze in nasal packing after endoscopic sinus surgery in elderly patients

Objective To investigate the effect of anti-bleeding gauze (TISTAT) in nasal packing after endoscopic sinus surgery in elderly patients.Methods 72 elderly patients with chronic sinusitis and nasal polyps for endoscopic sinus surgery were divided into two groups:TISTAT group (use TISTAT,n=38) and calcium alginate group(use calcium alginate,n=34).TISTAT and calcium alginate were respectively used as packing materials in TISTAT and calcium alginate group.Hemorrhage in the packing process and after the packing materials removed were observed and compared.The symptoms such as headache、nasal blockage and nose pain during the packing process and 24 hours after the packing materials removed were compared.The recovery of surgery cavity in 4 weeks after surgery which reviewed once a week were checked and compared.Results The tamponade during hemorrhage rates of TISTAT group and calcium alginate group were 31.6% (12 cases)、58.5% (20 cases),after removing the tamponade,the total bleeding rates were 15.8%,41.2 %,two groups had significant difference (P<0.01) ;TISTAT group nasal pain symptoms in the packing process,packing materials were 12 h,stuffing out after 24 h were (2.8±0.6)、(1.3±0.5),calcium alginate group nasal pain symptoms in the packing process,packing materials were 12 h.stuffing were 24 h were (5.2±0.4)、(4.3±0.6),two groups of nasal pain symptom difference was statistically significant (P<0.05),but the process of tailing and calcium alginate group blocked nose were (5.4±0.9) 、(8.3±0.6),the two groups had significant difference (P<0.05).Conclusions The anti-bleeding gauze(TISTAT) has safety and active role in nasal packing after endoscopic sinus surgery in elderly patients with less symptoms and a better surgical cavity recovery.It is an ideal packing material.
4.Sancai Lianmei Particle Regulates Apoptosis of Liver Cells Based on ROS-ASK1-JNK/NF-κB Pathway in Type 2 Diabetes Mellitus Combined with Non-alcoholic Fatty Liver Disease Model Rats

Objective To study the action and mechanism of Sancai Lianmei Particle on cell apoptosis of liver cells in Type 2 Diabetes Mellitus(T2DM)combined with Non-alcoholic Fatty Liver Disease(NAFLD).Methods High fat and sugar + STZ induced diabetic with fatty liver rats used as models,intervention with Sancai Lianmei Particle,intraperitoneal glucose tolerance test(IPGTT)to assess insulin resistance,ELISA method to detect the mice serum biochemistry,insulin levels;ELISA method to detect inflammatory factors in liver homogenate;SOD and MDA levels were monitored to assess the degree of oxidative stress;ASK1/JNK/NF-κB mRNA expression in liver tissue was monitored by Real-time PCR;apoptotic-related proteins were detected by Western blot,apoptosis of hepatocytes was assayed by TUNEL;HE staining was conducted to observe the liver tissue.Results Sancai Lianmei Particle can obviously reduce the body weight of T2DM with NAFLD model rats;reduce the levels of GHb,INS,TC,TG,LDL-C,ALT,AST,IL-1β,IL-6,TNF-α,MDA;improve insulin resistance and oxidative stress.HE staining of liver tissue showed that Sancai Lianmei Particle could alleviate the vacuolar degeneration of liver and deposition of lipid droplets.Sancai Lianmei Particle can effectively down-regulate the expressions of ASK1,JNK and NF-κB mRNA in liver tissues of model rats.Western Blot results exhibited that Sancai Lianmei Particle could significantly regulate the expression of apoptotic proteins of Bax,Caspase-3,and Bcl-2 and inhibit the apoptosis of hepatocytes.Conclusion This study proved that Sancai Lianmei Particle can improve hepatic insulin resistance and oxidative stress,slow the progression of NAFLD by regulating liver cell apoptosis based on ROS-ASK1-JNK/NF-κB pathway.
5.Research progress on electrical impedance tomography in pulmonary perfusion.

Electrical impedance tomography (EIT) is an emerging technology for real-time monitoring based on the impedance differences of different tissues and organs in the human body. It has been initially applied in clinical research as well as disease diagnosis and treatment. Lung perfusion refers to the blood flow perfusion function of lung tissue, and the occurrence and development of many diseases are closely related to lung perfusion. Therefore, real-time monitoring of lung perfusion is particularly important. The application and development of EIT further promote the monitoring of lung perfusion, and related research has made great progress. This article reviews the principles of EIT imaging, lung perfusion imaging methods, and their clinical applications in recent years, with the aim of providing assistance to clinical and scientific researchers.
