1.Investigation on the effect of performance training of adverse events

Objective To investigate the application and effect on the performance training of adverse events, and to improve the quality of nursing teaching. Methods Totally 120 new graduate nurses in our hospital were randomly divided into study group(n=60)and control group(n=60). Study group used performance training while control group used traditional training mode and evaluation was made from the adverse event knowledge score , number of adverse events and satisfaction degree of stu-dents. Results Adverse event knowledge score of study group (9.57±0.563) was obviously higher than that of control group(84.90±3.245). Satisfaction degree of students in study group(9.57±0.563) was higher than that of control group(7.35 ±0.917), with statistical differences(P<0.01). Number of ad-verse events(n=3) was obviously lower in study group than in control group(n=15), with statistical differences(P<0.01). Conclusion In adverse events teaching, performance training can be helpful in arousing students' interests in learning. Experiences of self-examination and self-correction also make the students have a deep impression on knowledge, which improves the quality of adverse events teaching and reduces the incidence of adverse events in the new graduates.
2.Changes and its significance of neuron-specific enolase and myelin basic protein in serum after seizure of rats with temporal epilepsy

Objective To explore the dynamic changes of neuron-specific enolase (NSE) and myelin basic protein (MBP) in serum of rats with temporal epilepsy (EP) induced by kainic acid (KA) and to judge the degree of injury of brain neuron and nerve myelin after seizure.Methods KA was injected into rat's hippocampus by stereotactic operation to establish an animal model of temporal EP.The levels of NSE and MBP in serum of rats with temporal EP were measured at the time 3 h,6 h,12 h,24 h,48 h and 72 h after seizure.Results The level of NSE in serum increased gradually and reached its peak at 24 h after seizure, as well as MBP at 72 h.Conclusions There are the nerve cell damage and necrosis after seizure in rats with temporal EP, then brain white matter nerve myelin appear to damage.
3.The effects of antiphospholipid antibodies and EGb761 on the Schwann cells in vitro

Objective To detect the effect of the antiphospholipid antibodies on the Schwann cells in vitro and to find if the Extract of Ginkgo biloba 761(EGb761)has any protective effects.Methods The immunoglobulins(Ig)were extracted from the serum of the patient with autoimmune polyneuropathies and those with controls.Ig only or Ig plus calf serum(served as complement)were added into the culture solutions of the Schwann cells.The density of the cell in one high power field,the cell perimeter and area were detected and compared to those blank and those controls.Results The shape of the Schwann cells in both the Ig and Ig plus calf serum solution were greatly changed.The densities of the cells in them were both lower than the blank ones,with more severe in the cells with calf serum.There was no significant different between the solutions with or without EGb761.Conclusion Ig from the patients with autoimmune polyneuropathies could attack the Schwann cells in vitro,with the existence of complements,the destruction was even more severe.No protection of EGb761 could be found in these situations.
4.Clinical and imaging characteristics of posterior reversible encephalopathy syndrome in children with acute lymphoblastic leukemia after chemotherapy induction

Objective To investigate the clinical features, brain imaging significance and the possible pathogenesis of posterior reversible encephalopathy syndrome (PRES) in childhood acute lymphoblastic leukemia (ALL) followed by chemotherapy induction.Methods The diagnosis and treatment of ALL were performed according to the guidelines of the Pediatric Association of Chinese Medical Association.There were 11 cases of pediatric ALL who developed PRES after chemotherapy induction.The clinical presentations, initial and follow-up radiologic features, and the neurologic outcomes of these 11 cases were investigated for one-year follow-up.All patients were reexamined 1,3,6, and 12 months after first imaging.Results Headache (10/1 1 cases), epileptic seizure (7/11 cases), high blood pressure (4/11 cases) ,visual impairment (6/11 cases) ,disturbance of consciousness (5/11 cases) and walking instability (2/11 cases) were the most common symptoms of these ALL patients with PRES.Magnetic resonance imaging (MRI) scanning revealed that lesions were mainly distributed in occipital lobe (9/11 cases), parietal lobe (8/11 cases), frontal lobe (5/11 cases) ,temporal lobe (3/11 cases), the deep white matter of bilateral periventricular and centrum semiovale (2/11 cases) and hemisphaerium cerebelli (1/11 cases).The radiological findings indicated that lesions had multifocal,symmetrical and posteriorly distributed characteristics in the cerebral hemispheres.After the diagnosis of PRES,patients stopped chemotherapy courses promptly and received symptomatic treatment, and then the clinical and imaging symptoms of most cases gradually disappeared.After 1-year follow-up,9 patients had good prognosis and no sequelae, 1 patient had symptomatic epilepsy (brain magnetic resonance imaging scan showed lesions in the left temporal lobe) ,and 1 patient had slight visual impairment.After the craniocerebral symptoms disappeared clinically ALL chemotherapy continued in all patients and no recurrent PRES was observed.Conclusions Although the clinical and imaging features of PRES may be diverse ,PRES should be recognized as a possible important complication of ALL when neurological symptoms appear.However, PRES is reversible when the patients are diagnosed and treated at an early stage.Thus,the occurrence of PRES should be considered and investigated to optimize the early induction schemes for ALL treatment.
5.Application effect of family integrated care combined with intelligent health education platform in health education of elderly patients with stroke

Objective:To explore the effect of family integrated care (FIC) combined with intelligent health education platform in health education for elderly patients with stroke.Methods:Eighty patients with stroke were included from the Department of Neurology of the Second Affiliated Hospital of Chongqing Medical University from June 2019 to May 2020. They were divided into the experimental group and the control group ( n=40, respectively) by the random number table method. Patients in the control group received routine FIC in the Department of Neurology. Additional intelligent health education platform was provided for the experimental group to carry out intensive education. The modified Barthel index scale, self-care compliance scale and hospital anxiety and depression scale (HADS) were used before and after the intervention to evaluate patient′s self-care ability, self-care compliance and the degree of anxiety and depression. Results:Finally, 39 patients in the experimental group (1 case lost to followed-up) and 37 patients in the control group (3 cases lost to followed-up) completed the study. After the intervention, the scores of the modified Barthel index and self-care compliance scale of the experimental group were significantly higher than those of the control group [(65.9±12.6) vs (60.1±11.9) points, (78.2±13.6) vs (71.4±14.6) points], the HADS score was significantly lower than the control group [(15.18±2.46) vs (19.46±2.40) points] (all P<0.05). The scores of the modified Barthel index scale and self-care compliance scale of the experimental group after the intervention were significantly higher than those before the intervention [(65.9±12.6) vs (40.8±12.5) points, (78.2±13.6) vs (54.6±15.9) points] (all P<0.05), the HADS score was significantly lower than before the intervention [(15.18±2.46) vs (21.74±3.52) points] (all P<0.05). Similarly,the scores of the modified Barthel index and self-care compliance scale of the control group after the intervention were significantly higher than before the intervention [(60.1±11.9) vs (41.6±9.72) points, (71.4±14.6) vs (54.3±14.8) points], the HADS score was significantly lower than before the intervention [(19.46±2.40) vs (21.38±3.09) points] (all P<0.05). Conclusion:FIC combined with intelligent health education platform can significantly improve the self-care ability and self-nursing compliance of elderly stroke patients, and markedly improve the degree of depression and anxiety.
6.Clinic research of Iloprost in patients with congenital heart disease associated with pulmonary hypertension

Objective To explore the effect of inhaled iloprost (Ventavis) for patients with congenital heart disease associated with pulmonary hypertension.Methods Twenty-two patients with congenital heart disease associated with pulmonary hypertension were selected to inhale Ventsvis by atomizer before surgery ,the required dosage is 25-30ng/(kg.min),aerosolization of iloprest was inhaled by PARI Junior BOY N after diluted with 2ml physiological saline at 3-hour intervals .The change of hemodynamic effects was measured by Doppler ultrasonography after 60min,120min,and 180min.Results The left and fight vemfieular cardiac output at 60min,120min and 180rain after therapy were markedly higher than that before inhaled iloprost ,respirator therapy time was shorter in patients inhaled iloprost.Conclusion inhaled iloprost can increase cardiac output and improve patients cardiac function.Thus,the therapy with inhaled iloprost is effective and feasible.
7.Perioperative nursing of pulmonary complication for infants with congenital heart disease

Objective To discuss related factors of perioperative pulmonary complications in infants with congenital heart disease and provide a scientific basis to improve the quality of intensive care in in-fants with congenital heart disease after surgery. Methods Tracking survey was carried out in 225 cases of infants with congenital heart disease in our city to observe the perioperative lung condition from March 2005 to May 2007. Every process that might occur pulmonary complications,such as preoperative care of children,anesthesia and cardiopulmonary bypass surgery (CPB) management and postoperative monitoring was closely observed. Statistical analysis was conducted according to emerging problems and summarized the relevant factors and nursing methods. Results 225 infant patients passed the operation with no seri-ous complications.Conclusions Effective treatment and nursing during every process of perioperation was the key factor to reduce pulmonary complications.
8.Standardized early medical care of Lushan earthquake victims in army hospitals

Objective To summarize the experiences with early treatment of the wounded associated with 4 · 20 Lushan earthquake in army hospitals.Methods Emergency response programs were started and tent hospitals were set up.According to the basic principle of wartime wound treatment regulation,emergency triage,emergency medical care,transferring,specialized treatment and evacuation were performed for the wounded in this earthquake.Results After the earthquake,the emergency rescue programs were started in the hospital.Twenty minutes later,emergence medical team with full equipment were set out for the epicenter and the tent hospitals were set up.A total of 181 earthquake cases were admitted within two weeks,of which 68 (37.6%) were transferred from the scene of earthquake and 113 (62.4%) were transferred after the primary treatment (including cases evacuating from other hospitals).Forty-nine surgeries had been done,including 30 emergence ones.Thirty-four cases (including 14 severe wounded) were evacuated to superior hospitals as their vital signs turned stable.There was no death or surgical wound infection.Conclusions Peacetime complete emergence programs and materials reserves,unified and well-organized command as well as normative grading treatment system in army hospitals assure the standardized medical care with high efficiency and high quality for earthquake victims.However,more researches are needed over diagnosis methods and standards for wound severity,equipment improvement and standards for casualty evacuation.
9.Application of QCC in Improving the Compliance of Asthma/COPD Outpatients Handling Inhaler Devices

OBJECTIVE:To evaluate the effects of quality control circle(QCC)in improving the compliance of asthma/COPD outpatients handling inhaler devices. METHODS:By simple random sampling method,90 asthma/COPD outpatients receiving Tiotropium bromide powder for inhalantion,Salmeterol xinafoate and fluticasone propionate powder for inhalantion and Budesonide and formoterol fumarate powder for inhalantion were selected from our hospital during Apr. to Nov. 2015. The reasons for poor com-pliance of handling inhaler devices were analyzed by QCC. The countermeasures were formulated to improve the compliance,in-cluding clinical pharmacist education,follow-up,issuing education manuals,developing WeChat consulting. Operation step score of inhaler devices,compliance score and activity ability score of QCC members were analyzed statistically before and after QCC. RESULTS:After the development of QCC,operation step score of inhaler devices and compliance score in 90 asthma/COPD outpa-tients increased from 7.7 and 7.7 before activity to 8.8 and 9.3 after activity,the achievable rate of compliance was 114.3%,and the improvement rate was 20.8%. The sense of honor,responsibility,self-confidence,QCC techniques,communication and coordi-nation,team cohesion of all members were all improved obviously. CONCLUSIONS:QCC activity improves the compliance of asthma/COPD outpatients handling inhaler devices and shows the professional service level of clinical pharmacists,which is of sig-nificance to the improvement of pharmaceutical care.
10.Possible signal pathway of the apoptosis in gastric cancer cell lines induced by arsenic trioxide

Objective To investigate the possible signal pathway of the apoptosis in gastric cancer cell lines(SGC 7901 and MKN 45)induced by arsenic trioxide. Methods TUNEL method was used to observe the influence of calcium antagonist, inhibitors of protein kinase C (PKC) and protein tyrosine kinase(PTK) on the apoptosis of gastric cancer cells induced by arsenic trioxide. Levels of cAMP, PKC and PTK were detected before and after the treatment with arsenic trioxide. Results Both PKC and PTK inhibitors could induce apoptosis of gastric cancer cell lines, also both of them had a cooperative action with arsenic trioxide in inducing apoptosis of gastric cancer cells, while calcium antagonist had no any effect on the apoptosis of gastric cancer cell lines.
