1.The diagnosis and treatment of 120 cases of pediatric tracheo-bronchiai foreign body

Objective To analyze the clinical diagnosis and treatment of pediatric tracheo-bronchial foreign body cases and therapeutic effects. Methods A retrospective review of the clinical manifestations, the feature of ima- geology and the methods of removing foreign body of 120 cases of pediatric tracheobronchial foreign body, which were confirmed with tracheobronchial foreign body by direct laryngoscope or rigid bronchoscope. Results History of aspiration was present in 90.8 % (109/120) of all cases, penetration syndrome was present in 100 % (120/120) of all cases. Among 39 cases with tracheal foreign body, there were 26 cases with audible flapping sound in trachea, the fre- quency was 66.7 %. With the X-ray chest fluoroscopy showing media-stinal swaying,59 cases were confirmed to be with foreign body by bronchoscope. The accurate rate of diagnosing traeheobronehial foreign body via CT was 86.2 % (25/29). All cases were confirmed and treated by direct laryngoscope and rigid bronchoscope. The foreign bodies of 119 cases were successfully removed. 1 patient was turned into the department of chest surgery. Conclusion History of aspiration, penetration syndrome, audible flapping sound in trachea, X-ray chest fluoroscopy showing mediastinal swaying are important evidence in diagnosing tracheobronchial foreign body. The accurate rate of diag- nosing tracheobronchial foreign body via chest CT is relatively high. Direct laryngoscope and bronchoscopy are con- firming methods in diagnosing tracheobronehial foreign body; it is effective methods to remove traehco-bronchial for- eign body by direct laryngoscope and rigid bronchoscope.
2.Construction of immune inhibitor of new rhTNF-?

Objective:To construct an immune inhibitor of new TNF-?,C-terminal sequence of new rhTNF-? was replaced with sequence of T-help cell of hen egg-white lysozyme(HEL).Methods:The rhTNF-? mutant was cloned 、expressed and purified.Results:The DNA sequencing analysis showed that the C-terminal sequence of new rhTNF-? mutant was correct.The mutant was inserted into pBV220 expression vector .After the recombinant bacteria was incubated at 42℃ for 4 h,a new band of the protein with relative molecular weight of 1.7 kD was shown on the gel.The band amounted to 30 % of total bacteria protein.Western blot showed that the mutant protein could associated with anti-TNF-? antibody.After the protein was purified by through a column of Q-Sepharose Fast Flow, the purity of the protein was above 90%.The biological activity of the protein was measured with L929 cells.The result showed that biological activity of the protein was totally lost.Conclusion:The experimental evidence demonstrated that the construction of new rhTNF-? mutant was successful.The mutant not only can be associated with anti-TNF-? antibody, but also lose the biological activity of original TNF-?. 4.The kinetic mechanism of gallstone formation

Objective To probe into the kinetics of gallstone formation.Methods Fifty seven rabbits were divided into five groups: (1) normal control with standard fodder, (2)1 2% cholesterol was added into the fodder,(3)1 2% cholesterol plus indomethacin in the fodder,(4)1 2% cholesterol plus erythromycin,(5) 1 2% cholesterol plus Dong Li San, a Chinese herb compound. All animals were feed four weeks before measurement.Results Gallstone developed in 0 out of 13 in group 1, in 12 out of 14 rabbits in group 2, in 4 out of 10 rabbits in group 3, in 0 out of 10 in group 4, and in 2 out of 10 in group 5. Compared with that in group 1 rabbits in group 2 had higher level of cholesterol and mucin in bile,much higher common bile duct pressure and cystic duct resistance,much lower gallbladder emptying rate ( P
5.Double-antigen sandwich ELISA for detection of total antibodies against hepatitis C virus

Objective To establish a double-antigen sandwich ELISA(S-ELISA) for detection of total antibodies against hepatitis C virus(HCV).Methods Recombinant HCV proteins fusion-expressed with His-tag and GST-tag were separately used as coating and HRP-labeling antigen of the S-ELISA.Serum samples were tested with both the S-ELISA and another commercial indirect ELISA(I-ELISA) kit(Beijing Wantai Biological Pharmacy Enterprise Co.Ltd.).HCV RNA in some of the samples were tested by RT-nested PCR.Results Among 1 968 tested samples,190(9.7%) were total anti-HCV-positive while 1 761(89.5%) were negative by both of the S-ELISA and I-ELISA,with a resultant concordance rate of 99.1% of the two ELISA assays.However,the results of 17(0.9%) were not consistent in the two assays,in which 14 were S-ELISA negative but I-ELISA positive and 3 were S-ELISA positive but I-ELISA negative.One of the 14 samples(0.7%) with S-ELISA negative was detected as HCV RNA-positive,while 2 of the 3(66.7%) samples with S-ELISA positive were detected as HCV RNA-positive.In addition,HCV RNA was detectable in 23 of 31(74.19%) samples which were total anti-HCV positive in both S-ELISA and I-ELISA.Taking the PCR data together into account,the sensitivity of the S-ELISA and I-ELISA were 99.48% and 98.96% and specificity were 99.94% and 99.27%,respectively.Conclusions The established S-ELISA in this study may provide a novel means for detection of HCV antibody with high sensitivity and specificity.
6.Association between polymorphisms of CYP3A5,MDR1,COX-2 and chemotherapy outcomes of advanced NSCLC

Objective To investigate whether genotypes of CYP3A5,MDR1 and cyclooxygenase-2 are associated with the sensitivity of vinorelbine-platinum to NSCLC.Methods The genotypes of CYP3A5(*3),MDR1 (2677G>T at exon 21 and 3435C>T at exon 26 and their haplotypes),cyclooxygenase-2 (-1 195G>A) were determined by RFLP-PCR and chemotherapy responses were analyzed in 69 non-small-celllung cancer (NSCLC) Chinese Han patients.They received a combination chemotherapy of vinorelbine-cispla-tin.Chi-square test was used to investigate the potential association of genotype with chemotherapy response.OR and 95% C1 were calculated.Results The 3435 CC genotype was associated with a significantly betterchemotherapy response compared with the combined 3435 CT and 1Tr genotypes(P=0.033).The 2677 GG genotype was also associated with a significantly better chemotherapy response compared with the combined 2677 GT and IT genotype(P=0.012).Moreover.patients with the 2677 G-3435 C haplotype seemed to have a better response to chemotherapy compared with those with the other haplotypes(P=0.063).CYP3A5*3 was not likely to correlate with sensitivity of vinorelbine-platinum to NSCLC.Cyclooxygenase-2-1195G>A was likely to have better response to vinorelbine but not statistically significant(P=0.067).Conclusion Polymor-Dhisms of MDR1 3435 C>T and MDR1 2677 G>A/T can be used for predicting treatment response to vinorel-bine-cisplatin chemotherapy in NSCLC patients.
7.Detection of Sodium Caprylate in Human Serum Albumin by Precolumn Derivation Reversed Phase High Performance Liquid Chromatography

A method was developed for the determination of sodium caprylate in human serum albumin by reversed phase high performance liquid chromatography(RP-HPLC) after pre-column derivatization. The caprylic acid, extracted from human serum albumin by hexane, was treated with ω-bromoacetophenone and 18-crown-6 for 30 min at 50 ℃, and analyzed on a Nova-Park C_(18)(150 mm×3.9 mm, 4 μm) column with methanol-water(75∶ 25, V/V) as the mobile phase. The internal standard was enanthic acid, the flow rate was 1.0 mL/min and the detection wavelength was 262 nm. The extraction yield of caprylic acid was 97.9% and that of enanthic acid was 98.2%. The linear range of sodium caprylate was 9.00×10~(-4)-1.44×10~(-2) mol/L(r=0.9995). The average recovery of caprylic acid was 99.7%, RSD was less than 0.9%. The present method is reliable and relatively simple and can be used for the determination of sodium caprylate in human serum albumin.
8.Advances in anti-atherosclerosis mechanisms of traditional Chinese medicine

Cardiovascular and cerebrovascular diseases, including coronary heart disease, periph-eral vascular disease and atherosclerosis, are the first cause of death worldwide. The pathogenesis of atherosclerosis is a complex process that involves a number of cellular processes and molecular mecha-nisms, such as disorder of lipid metabolism, inflammatory cell infiltration, oxidative stress, vascular en-dothelial cells injury and activation of smooth muscle cells. Their interaction eventually leads to plaque rupture and thrombus formation, causing serious cardiovascular and cerebrovascular events. Chinese medicine has displayed rich anti-AS activities and clinical applications. This review summarizes the anti-atherosclerosis effects and possible mechanisms of Chinese medicine in regulating lipid metabolism, anti-inflammation and antioxidation, protecting endothelial cells, inhibiting the proliferation and migration of smooth muscle cells, improving coagulation and fibrinolysis systems and stabilizing the plaque.
9.Patent applications in hospitals of Guangdong Province since 2000

The relation between patent applications and published papers in hospitals of Guangdong Province since 2000 on China Intellectual Property Network was analyzed , which showed that the quality and quantity of patents were lower in hospitals of Guangdong Province than in those of Shanghai and Beijing , the quality of patents was re-lated with the medical service level in hospitals, the patent applications were centralized in large class A hospitals of Guangzhou , and inventive patents were those of medical preparations .Suggestions were thus put forward for hos-pitals of Guangdong Province to improve their awareness of patent application, the quality of patents, transformation of patents, cooperation with patent service organizations, research of patent application, and balance of patent dis-tribution in different areas.
