1.Urodynamic analysis for the causes leading to dissatisfying result after operation therapy for BPH

Objective To discuss the causes leading to dissatisfying result after operation therapy for BPH.Methods 42 patients who had not acquired satisfying result after operation therapy for BPH were underwent the urodynamic examination.Results 42 patients were classified into two groups according to their different symptoms:difficuhy to void,32 case8,including 8 cases with low contractifity of detrusor,and 24 cases with Madder outflow obstruction(BOO);incontinence,10 eases,including 6 cases of urgency incontinence and 4 cases of stress incontinence.Condusion We can see that the causes leading to dissatisfying resuh after operation for BPH are mainly the dysfunction of detmsor and BOO,and the result of urodynamic examine can guide us the causes and the next therapy.
2.The clinical research about the relationship between serum levels of 8-hydroxy-deoxyguanosine (8-OHdG) of type 2 diabetes patients and its macrovascular complications

Objective To investigate the role of oxidative stress in type 2 diabetes patients with macrovascular complications. Methods Serum levels of 8-hydroxy-deoxygnan osine (8-OHdG) and hemoglobin Alc (HbAlc) in 32 cases of type 2 diabetic patients with macrovascular complications and 46 cases of patients without complications were determined. 8-OHdG was detected by enzyme linked immu-nosorbent assay (ELISA). Results The level of 8-OHdG (2.93±1.37) ng/mL in type 2 diabetic patients with macrovascular complica-tions was higher than those without complications, whose 8-OHdG level was (2.67±1.30)ng/mL, but the difference was not significant(P>0.05). Compared with all kinds of macrovascular complications in patients, the 8-OHdG levels were increased, but the difference were not significant (P>0.05). Compared with patients without complications, the duration and age of the patients with macrovascular compli-cations also increased significantly (P<0.05), but the HbAlc levels were not statistical difference between them (P>0.05). Spearman regression analysis showed that 8-OHdG level was positively correlated with the duration (r=0.33, P<0.05), while the age and HbAlc level had no obvious correlation (P>0.05). Conclusion The role of oxidative stress may not be the important reason in type 2 diabetes with macrovascular complications, but the duration and age of the patients may be closely related with the disease.
3.Surface electromyogram denoising using adaptive wavelet thresholding.

Surface electromyogram (sEMG) may have low signal to noise ratios. An adaptive wavelet thresholding technique was developed in this study to remove noise contamination from sEMG signals. Compared with convention- al wavelet thresholding methods, the adaptive approach can adjust thresholds based on different signal to noise ratios of the processed signal, thus effectively removing noise contamination and reducing distortion of the EMG signal. The advantage of the developed adaptive thresholding method was demonstrated using simulated and experimental sEMG recordings.

4.Cross-bridge transplantation of free latissimus dorsi muscular flap and free fibula for repair of complex tissue defect of lower legs

Objective To introduce surgical repair methods of cross-bridge transplantation of free latissimus dorsi muscular flap and free fibula for complex lower leg defect and discuss its clinical feasibility.Methods The study included 12 patients with tibial defect larger than 8 cm (range,9-12 cm) combined with soft tissue defect of 17 cm × 12 cm to 20 cm × 18 cm treated from May 2008 to May 2012.Cross-bridge transplantation of free latissimus dorsi muscular flap and free fibula was performed at the first phase.The flap pedicled with subscapular vessel was anastomosed to posterior tibial artery and vein of normal lower leg.The flap pedicled with anterior serratus muscle of distal thoracodorsal artery was anastomosed to peroneal vessel of fibular flap.External fixators were used to immobilize the bilateral lower legs postoperatively.Results All patients were followed up for 13-32 months (mean 21 months).According to Enneking system,mean leg function was scored 23 points after tandem transplantation of free latissimus dorsi muscle and free fibula,with recovery rate of 77%.Conclusions Cross-bridge transplantation of free latissimus dorsi muscular flap and free fibula tackles the problem of recipient vessel limitation.Further,the technique is effective in repair of large area of complex defect in lower legs.
5.Observation of post-prostatectomy incontinence :supra-pubic transvesical prostatectomy and transurethral resection of prostate

Objective To evaluate the morbidity of the post-prostatectomy incontinence of supra-pubic transvesical prostatectomy(SPP) and transurethral resection of prostate(TURP). Methods One hundred and thirtyfive patients were divided into two groups. 82 patients underwent SPP and 53 patients underwent TURP. We ob-served the morbidity and the time lasting of post-prostatectomy incontinence, the severity of post-prostatectomy in-continence as evaluated by Stamey incontinence grading system,patients who had more than one week postoperativeincontinence were received urodynamic tests,then we got the information of their incontinence types. Results We found that patients who underwent SPP had higher morbidity, severity and time lasting than those who underwent TURP. SPP group had much more morbidity of stress incontinence than TURP group, but the same morbidity of ur-gent incontinence after operation as the later. Conclusion TURP may be better than SPP in consideration of the post-prostateetomy incontinence. SPP group has more stress incontinence and it may be caused by complete resection of prostate and damnifieation of the mucous membrane of membranous urethra.
6.A Study on the Decomposition of Surface EMG Signals Based on Second Order Non-stationary Source Separation

Objective To investigate the decomposition method of surface EMG(sEMG)signals based on Blind Source Separation and to detect the the motor unit action potential(MUAP)information.Methods Utilizing the sEMG signals recorded at low muscle contraction force(10% MVC),the methods of second order non-stationary source separation(SEONS)and FastICA were explored to analyze the sEMG signals decomposition.Results The experiment results showed that the MUAP information could be acquired by spike detection and pattern recognition after the decomposition of recorded sEMG signals using the proposed algorithm and FastICA method,but a little difference occurred due to the complexity of sEMG signals.Conclusion The non-stationary characteristic of sEMG signals is considered by the SEONS algorithm,and the proposed method can be applied in the sEMG signals decomposition.
7.In vitro Effects of the Traditional Chinese Medicine,Berberine,Matrine and Baicalin,on the Proliferation and Lipid Synthesis of Immortalized Human Sebocyte SZ95

Objective To determine the in vitro effects of the traditional Chinese medicines, berberine, matrine and baicalin, on the proliferation and lipid synthesis of human immortalized sebocyte SZ95, and to investigate their possible mechanisms of action on sebaceous glands at the cellular level. Methods Inverted microscopy was used to observe cell morphology and determine toxic concentrations of the compounds. The MTT method was adopted to examine the effects of different concentrations of berberine, matrine and baicalin on SZ95 cell proliferation after 24 h, 48 h and 72 h of incubation. Lipid contents in the SZ95 cells were labeled with Nile red and analyzed by flow cytometry. Results The toxic concentrations were 1?10-3 mol/L, 1?10-4 mol/L, 1?10-3 mol/L for baicalin, berberine and matrine, respectively. Berberine reduced sebocyte proliferation in a dose- and time-dependent manner, with an inhibitory concentration 50 (IC50) of 2.9?10-5 mol/L and 1.4?10-5 mol/L after 48 h and 72 h of incubation, respectively. When the concentration of matrine was 0.05). Lipogenesis of SZ95 cells showed a 26.9% increase with 1?10-3 mol/L matrine. Conclusions Our results indicate that berberine and baicalin can inhibit proliferation and lipid synthesis of SZ95 sebocytes in vitro, which suggests a possible clinical role in treating acne.
8.Identification of Model Parameters Basing on Matched Processing between Simulated and Recorded sEMG Signals

Objective To identify the model parameters of surface Electromyography (sEMG) by comparison between simulated and recorded signals. Methods A physiological model of sEMG signal was established basing on several logical hypothetical conditions, such as motor unit action potentials (MUAP), motor unit recruitment and firing behavior caused by excitation, architecture of volume conductor and other simulated factors. According to the matched shapes between the simulated and recorded sEMG signals, a group of model parameters was obtained; according to the similar power spectrum variations of real sEMG signals, decreased muscle fiber conduction velocity (MFCV) was applied to simulate the sEMG signals of the fatigued muscle. Results The experimental results showed that the simulated superimposed MUAP shapes could be matched with the recorded MUAPs satisfactorily by adjusting some proper physiological parameters of the model. When the MFCV of each fiber was assumed to decrease, the mean and median frequency (MNF, MDF) of the simulated sEMG signals declined, and this phenomenon was very similar to that of the recorded sEMG signals and could be used to interpret the muscle fatigue process. Conclusion This model provides an effective approach to simulate real sEMG signals, and the simulated signals can also be used to help the analysis of recorded sEMG signals.
9.A biomechanical study on torque resistance of distal radioulnar joint by extra-and intra-articular reconstruction

Objective To compare the torque resistance effect with different types of reconstruction of the distal radioulnar joint (DRUJ) dislocation,in order to explore biomechanical basis for clinical practice.Methods Six adult cadaver forearm were used,including 4 males and 2 females.The average death age was 64.9 years (range,36-78 years).Three were right side,3 were left side.The DRUJ dislocation model was made after the standardized resection of the palmar and dorsal DRUJ ligament.Clinic reconstruction methods of the DRUJ include trans-bone tunnel renconstruction (intra-articular reconstruction) and Looptie up reconstruction (extra-articular reconstruction).The mechanical properties as follows:torque velocity:5°/min,maximum torque angulation:90°,maximum torsion force:1 N· mm.Each limb was detected for the torque mechanic under four conditions:normal,DRUJ dislocation,intra-articular reconstruction and extra-articular reconstruction.Results Each limb was inspected for intact mechanic baseline and without the visual damage to the bone.The average pronation torque resistance on normal condition was (0.44±0.10) N·mm,(0.37±0.09) N·mm on DRUJ dislocation group,(0.45±0.09) N-mm on intra-articular reconstruction group,and (0.42±0.09) N·nun on extra-articular reconstruction group.Pronation torque resistance of DRUJ dislocation group was apparently lower than the normal group (t=2.043,P=-0.047).The pronation torque resistance reconstruction group had no significant difference with normal group.The average supination torque resistance on normal condition was (0.56±0.16) N·mm,while on DRUJ dislocation group was (0.42±0.11) N-mm,on intra-articular reconstruction group was (0.54±0.16) N·mm,on extra-articular reconstruction group was (0.51±0.13) N·mm.There were no apparent differences among four groups.Conclusion Both internal and external reconstruction could recover the rotation stability and normalize the torque resistance,without statistical difference.
10.Expressions of extracellular matrix protein 1 and matrix metalloproteinase-9 in hepatocellular carcinoma and their clinical significances

Objective To investigate the expressions of extracellular matrix protein 1 (ECM1)and matrix metalloproteinase-9 (MMP-9) in hepatocellular carcinoma (HCC) and to explore their correlations with metastasis and recurrence.Methods Immunohistochemisty was employed to determine the expressions of ECM1 and MMP-9 in 120 HCC and 17 normal liver tissues.Results The positive rates of ECM1 and MMP-9 expression in HCC tissues were 73.3% (88/120) and 65.0% (78/120),respectively.They were significantly higher than that in normal liver tissues.The expression of ECM1 in HCC tissues was significantly correlated with vascular invasion and TNM stage.The expression of MMP-9 in HCC tissues was significantly correlated with tumor size,number of tumor nodules,differentiation of tumor and degree of cirrhosis.They were not correlated with age,gender,serum AFP level,HBsAg,Child-Pugh class and capsulated tumor.Patients with ECM1 and MMP-9 positivity had significantly poorer overall and disease-free survival rates.There was a significant positive correlation between ECM1 and MMP-9 (r=0.585,P<0.001).Conclusion Both ECM1 and MMP-9 were significantly related to invasion and metastasis.Over-expression of both ECM1 and MMP-9 had predictive values for prognosis and recurrence of HCC after surgery.
