1.A Clinical Study on the Minor Myocardial Injury Associated with Percutaneous Coronary Intervention

In PCI group,the balloon inflation time, the highest inflation pressure and the number of placed stents in the patients with the increased level of cTnI had not significant difference compared with those in the patients without the increased level of cTnI. There were 2 patients with side branch occlusion, whose cTnI level obviously elevated. Conclusion PCI could lead to minor myocardial injury in some patients, the reason of which might be side branch occlusion. The number of placed stents and balloon inflation time were not associated with the minor myocardial injury.

3.Treatmant of no-reflow phenomenon during percutaneous coronary intervention

Objective To evaluate the effect of injecting UK and Herbesser into the target coronary artery where no-reflow happened during percutaneous coronary intervention. Methods Among the 820 acute coronary syndrome (ACS) patients who underwent PCI from Jan. 1990 to Apr. 2004, no-reflow phenomenon occurred in 86 patients. Three hundred cases with acute myocardial infraction (AMI) underwent primary PCI, no-reflow phenomenon occurred in 50 cases. In the other 520 unstable angina pectoris (UAP) cases, no-reflow phenomenon occurred in 36 cases during PCI. All the 86 patients with no-reflow were given intra-coronary arteries nitroglycerin but 78 of them showed no improvement in coronary perfusion and were in turns devided into 3 groups randomly. Group A (n=26) was given intra-vessel UK and Herbesser alternately. The maximan dosage of UK was 2?105 U kIU and 2 mg for Herbesser. Group B (n=24) was given intra-coronary Herbesser 500 ?g each time until the maximum amount reached 2 mg. Group C (n=28) was given UK which maximun dosage was 2?105 U. AIM: To investigate the levels of IL-18, IL-16, IL-8, eotaxin and the chymase activity in the sputum of asthmatics. METHODS: IL-18, IL-16, IL-8 and eotaxin levels were detected with sandwich ELISA procedures and chymase activity was determined spectrophotometrically (410 nm) by the rate of hydrolysis of N-succinyl-L-Ala-L-Ala-L-Pro-L-Phe-p-nitroanilide (SAAPP). RESULTS: The specific chymase activities in the severe and moderate asthmatics were higher than that in controls. Native protease inhibitors ?_1-antitrypsin (?_1-AT) and soybean trypsin inhibitor (SBTI) inhibited 71.9% and 72.1% enzymatic chymase activity, respectively. 5.Study on the quality standards of Yi Li Kang tablets

Objective To establish the quality control standard for Yi-Li-Kang tablets. Methods Astragalus membranaceus and Herba Epimedh were identified by TLC. The content of Icariin was determined by HPLC. Results The linear range of Icariin was 0.288~2.016 μg. The average recovery was 100.83%, and RSD was 1.82 (n=5). Conclusion The method is simple, accurate and reliable, which can be used to control the quality of Yi-Li-Kang tablets.
6.Factors influencing long-distance transportation and medical rescue of trauma patients attributable to China Wenchuan earthquake

Objective To explore the influencing factors during long-distance transportation and medical rescue of trauma patients in Wenchuan earthquake. Methods After Wenchuan earthquake, based on prehospital care scheme, we organized medical coordinators and psychologically trained medical staff in emergency medicine, cardiovascular medicine and logistics to transport trauma patients and pro-vide early psychological intervention during whole course of transportation. Results A total of 162 trauma patients were safely transported from Sichuan province to Chongqing relying on prehospital care scheme, equipment supplies, integrated rescue mode and psychological intervention during the transporta-tion. Conclusions Medical rescue mode and psychological intervention indicate that it is necessary to conduct psychological intervention for better transportation of patients for further medical treatments. In the meantime, we should establish medical rescue system for coping with psychological stress, improve rescue preparation scheme and professional medical care team for long-distance transportation and guaran-tee sufficient medical supplies in station hospitals.
7.A study on screening,treatment and prognosis of congenital hypothyroidism in Xuancheng City

Objective To review the experience on screening,diagnosis and treatment for neonatal congenital hypothyroidism (CH)in Xuancheng City,Anhui Province.MethodsFrom Jan 2008 to Dec 2014,peripheral blood samples from the heel were collected among 83787 neonates and the blood samples were sent to the screening center to test the level of thyroid stimulating hormone (TSH).Neonates with abnormal level of TSH were admitted to our hospital for further TSH and free tetraiodothyronine (FT4 )tests. Neonates with CH diagnosis were given standard treatments of levothyroxine and their thyroid function, physical and intelligence development were evaluated on a routine basis.Results Among the 106 neonates with abnormal TSH level ,68 were diagnosed CH.The prevalence of CH in Xuancheng City was 0.8 ‰.All 68 patients with CH received levothyroxine treatment and 6 cases were lost during follow-up.44 patients received physical examination and their bone ages were normal. 53 patients received intelligence assessment.Only 1 of them was diagnosed of mental retardation, and 3 with suspicious mental retardation.Conclusion Neonatal screening is necessary for the early diagnosis and treatment of CH. Neonates with CH can have normal physical and mental development if they receive standard treatment as early as possible and followed up regularly.
8.Formulation Optimization of Indomethacin-loading Solid Lipid Nanoparticles by Box-Behnken Response Surface Methodology

OBJECTIVE:To optimize the formulation of indomethacin-loading solid lipid nanoparticle(SLN). METHODS:Us-ing indomethacin as model drug,glyceryl behenate as oil phase,poloxamer 188 and polyethylene glycol-12-hydroxystearic acid as emulsifier,with turbidity,entrapment efficiency and drug loading amount as index,Box-Behnken response surface methodology was used to optimize the amount of oil phase,emulsifier-oil phase ratio,drug-oil phase ratio. The physicochemical properties of SLNs were characterized by SEM and DSC. RESULTS:The optimal formulation was as follows as oil phase of 0.91%,emulsifier-oil ratio of 1∶1,drug-oil phase ratio of 1∶5. The turbidity,entrapment efficiency and drug loading amount of prepared nanoparticle were 1 025-1 030 NTU,98.94%-99.08%,2.43%-2.46%,respectively;particle size and polydispersity index(PDI)were 181.5-182.3 nm and 0.340-0.341(n=3). The results of DSC showed that indomethacin was not present in crystalline state dispersed into SLNs. CONCLUSIONS:The optimal formulation is screened successfully,and indomethacin-loaded SLNs have been prepared.
9.Oral rapamycin prevents artery restenosis after stent implantation by inhibiting mammalian target of rapamycin ( mTOR )

Objective To identify the mechanisms of oral administered rapamycin on the prevention of restenosis after stent implantation with intravascular ultrasound (IVUS) and pathology.Methods Twenty-four New Zealand white rabbits underwent balloon-induced abdominal aortic wall injury and were given a diet of 1 % cholesterol for 8 weeks.Then,the rabbits were divided into three groups:the bare metal stent group(BMS group),the bare metal stent with oral rapamycin group(BMS + RAPA group) and the rapamycin eluting stent group(DES group).Rabbits in the RAPA and BMS + RAPA groups received a daily oral dose of rapamycin(0.5 mg/kg),whereas rabbits in the DES group received no drugs.All the rabbits were euthanized after the 4-week intervention.Serum lipids were measured.IVUS and pathologic studies were performed.The minimal luminal diameter (MLD),external elastic membrane (EEM) area,lumen area (LA),and plaque area(PA),plaque burden(PB) were measured.Mammalian target of rapamycin(mTOR) expression level was examined by immunohistochemistry.Results After the 4-week intervention,there was no significant difference of serum lipid levels among the three groups.IVUS showed that oral administration of rapamycin in the BMS + RAPA groups showed similar effects in reducing PA and PB as the DES group,which all were better than the BMS group.The BMS + RAPA and DES groups showed much more MLD and less lumen reduction,compared with the BMS group( P <0.05).Level of mTOR expression of the BMS + RAPA group and DES group was significantly lower than that of BMS group.Conclusions Oral administration of rapamycin demonstrates the same effect in the reduction of plaque burden and stent restenosis as the rapamycin eluting stent.Inhibition of mTOR by rapamycin involves in the stent restenosis.
10.The effects of levocitirize dihydrochloride on serum IL-13 and IL-18 in patients with cough variant asthma

Objective To explore the effects of Levoeitirize dihydrochloride on interleukin-13(IL-13)and interleukin-18(IL-18)in the serum of the patients with cough variant asthma(CVA).Methods 70 cases with CVA were randomly devided into control group of 35 cases and treatment group of 35 cases.Control group was given Chlort rimeton and the treatment group was given Levocitirize dihydroehloride.The levels of IL-13 and IL-18 in the serum were measured before and after treatment.Results After treatment,the concentrations of IL-13 and IL-18 in patients in the treatment group were(46.7±17.3)ng/L and(145.2±27.1)ng/L,and those in the control group were(98.5±30.7)ng/L and(179.6±30.5)ne/L,which were significantly improved.Conclusion The treatmem of Levoeitirize dihydrochloride could improve the CVA through improving the production of IL-13 and IL-18.
