1.Study on the quality standards of Yi Li Kang tablets

Objective To establish the quality control standard for Yi-Li-Kang tablets. Methods Astragalus membranaceus and Herba Epimedh were identified by TLC. The content of Icariin was determined by HPLC. Results The linear range of Icariin was 0.288~2.016 μg. The average recovery was 100.83%, and RSD was 1.82 (n=5). Conclusion The method is simple, accurate and reliable, which can be used to control the quality of Yi-Li-Kang tablets.
2.Formulation Optimization of Indomethacin-loading Solid Lipid Nanoparticles by Box-Behnken Response Surface Methodology

OBJECTIVE:To optimize the formulation of indomethacin-loading solid lipid nanoparticle(SLN). METHODS:Us-ing indomethacin as model drug,glyceryl behenate as oil phase,poloxamer 188 and polyethylene glycol-12-hydroxystearic acid as emulsifier,with turbidity,entrapment efficiency and drug loading amount as index,Box-Behnken response surface methodology was used to optimize the amount of oil phase,emulsifier-oil phase ratio,drug-oil phase ratio. The physicochemical properties of SLNs were characterized by SEM and DSC. RESULTS:The optimal formulation was as follows as oil phase of 0.91%,emulsifier-oil ratio of 1∶1,drug-oil phase ratio of 1∶5. The turbidity,entrapment efficiency and drug loading amount of prepared nanoparticle were 1 025-1 030 NTU,98.94%-99.08%,2.43%-2.46%,respectively;particle size and polydispersity index(PDI)were 181.5-182.3 nm and 0.340-0.341(n=3). The results of DSC showed that indomethacin was not present in crystalline state dispersed into SLNs. CONCLUSIONS:The optimal formulation is screened successfully,and indomethacin-loaded SLNs have been prepared.
3.Exploration of Specification Rationality of Capsules Commonly Used in Children by Capsule Application Analysis in a Children's Hospital

Objective: To study the dosages of capsules commonly used in children to provide reference for the addition of capsule specification for children.Methods: According to the application situation of capsule dosages commonly used for the inpatients in one children's hospital from January 1, 2013 to September 1, 2016, and combined with the usage rates of various drugs with different dosages, the addition of the minimum dosage of capsules was proposed.Results: Totally 10 species of commonly used capsules were selected from the children's hospital, and among them, 4 ones met the requirements of clinics, and the other 8 ones needed the specification addition, including clostridium butyricum capsules (210 mg) and polysaccharide ferric complex capsules (25 mg).Conclusion: The existing capsule specification can not fully meet the clinical requirements in the children's hospital.Therefore, appropriate dosage adjustments are still needed.
4.Optimizing Effect of PDCA Cycle on Emergent Medicine Management of Inpatient Area Based on JCI

Objective: To establish the standardized management mode for emergent medicines of inpatient area based on JCI. Methods:The expiry verification data of emergent medicines in hospital wards were collected from 2013 to 2015. The number and the amount of emergent medicines expired in the last three months in each calendar month during the three years were analyzed, the exist-ing problems in the management mode were found out and gradually optimized using PDCA cycle. Results:From 2013 to 2015, the re-placement amount of emergent medicines expired in the next month was 3497. 37 yuan. The number of emergent medicines expired in current month was 62. A total of 420 times of emergent medicines didn' t meet the requires of expiry verification from 2013 to 2015. After the check-in form redesign for emergent medicines and the standardization of replacement process, the number of expired medi-cines in the rescue carts was reduced, and the validity and quantity of drugs in all the rescue vehicles could be clearly shown in the new form. Conclusion:The standardization of emergent medicine management model can guarantee the safety of emergent medicines used in patients.
5.Optimization of thermosensitive in situ gel system containing berberine hydrochloride for ocular use

Objective To prepare a thermosensitive in situ gel system containing berberine hydrochloride for ocular use. Methods The in situ thermosensitive gel system of berberine hydrochloride was prepared by Poloxamer 407 (P407) and Poloxamer 188 (P188) as thermosensitive materials,the formulation was optimized by determining solution-gelation conversion temperature by using stirring method. The content of berberine hydrochloride was determined by HPLC. Results The gelation temperature of in situ thermosensitive gel containing berberine hydrochloride formulations lowered as the P407 concentration increased,as the P188 concentration increased gradually the gelation temperature initially increased to maximum and then decreased. The gelation temperature all increased after simulated tear fluid (STF) dilution. The fitted equation was established for the gelation temperature with the concentration of Poloxamer solutions after diluted by STF. An optimized formulation by Design-Expert software was freely flowing liquid at 29.7 ℃ and convert to a firm gel at 34.5 ℃ after STF diluted. Conclusion In situ thermosensitve gel system complies with the requirement for ophthalmic application and shows great potential in ocular application.

7.Correlation between Empty Bottle Volume, Negative Pressure and Gas Production Volume of Freeze-dried Powder Commonly Used in Children

Objective: To study the correlation between the empty bottle volume, negative pressure and gas production of the freeze-dried powder in the out-patient pharmacy intravenous admixture center of a children' s hospital in order to provide reference for the drug production. Methods:20 ml Syringes were used to measure the volume of empty bottles, negative pressure and produced gas. The relationship between the theoretical drug dissolution volume and the actual dissolution volume was compared, and the precautions for the drug production were put forward. Results:Among the tested 30 drugs, 6 ones were with the actual dissolution volume half of the theoretical dissolution volume, 8 ones were with negative pressure in the bottles, and 3 ones were with produced gas after dissol-ving. It was appropriate that the empty bottle volume be 4 ml larger than the theoretical dissolution volume, and it was appropriate that the negative pressure volume of drugs was slightly larger than the theoretical dissolution volume. Negative pressure should be still kept in the bottles after the gas production. Conclusion:The design of part of freeze-dried powder injection needle shows defects resulting in drug mixing difficulties to a certain extent.
8.Endovascular treatment of abdominal aortic aneurysm and aortic bifurcation stenosis by unibody bifurcation stent graft.

OBJECTIVE:
To sum up experience in unibody bifurcation stent graft in the treatment of abdominal aortic aneurysm with aortic bifurcation stenosis.
METHODS:
Clinical data of 19 cases of abdominal aortic aneurysm and aortic bifurcation stenosis received endovascular treatment using unibody bifurcation stent graft in Zhejiang Provincial People's Hospital during March 2009 and March 2018 were collected. The clinical characteristics, surgery procedure and follow-up results were reviewed.
RESULTS:
Stent graft was successful in all patients, and the average operation time was (70.0±2.3) min. Leakage was found in 3 patients, in which 2 patients with type Ⅰ leakage and 1 patient with type Ⅱ leakage. All leakage disappeared 15 days after surgery. The 19 cases were followed-up for 9-48 months with the median follow-up time of 27 months, and no displacement, leakage and lower limb ischemia was observed.
CONCLUSIONS
Unibody bifurcation stent graft is of satisfactory long-term effect for patients with abdominal aortic aneurysm and aortic bifurcation stenosis, and can avoid displacement of stent graft after operation.

9.Study on the Preparation of Melatonin Solid Lipid Nanoparticles and Their Transcutaneous Absorption in vitro

OBJECTIVE: To prepare Melatonin solid lipid nanoparticles (MT-SLNs), and to investigate in vitro transcutaneous absorption characteristics of them in isolated skin of mice. METHODS: Using melatonin (MT) as model drug, glycerin sorbate as oil phase, poloxamer 188 (F188) as emulsifier, MT-SLNs was prepared by melt-emulsion method. The morphology of MT-SLNs was observed by TEM; the particle size distribution of MT-SLNs was measured by laser particle size analyzer, and the infrared spectrum characteristics were measured by infrared spectroscopy. The entrapped efficiency and drug loading amount were determined by HPLC and ultrafiltration centrifugation method. The transcutaneous absorption characteristics in vitro of MT-SLNs and MT raw material were compared using Franz diffusion cells method. RESULTS: The formulation of MT-SLNs included 2.5 mg/mL MT raw material, 25 mg/mL glycerin sorbate and 1%F188. The obtained MT-SLNs were spherical, and no aggregation was observed. Average particle size was (67.88±0.17)nm, and polydispersity index was 0.188±0.001. The characteristic absorption peaks of N—H stretching and bending vibration were not found in infrared spectra, and the characteristic absorption peaks of C—H stretching vibration of benzene ring shifted red to 1 750 cm-1. The entrapped efficiency and drug loading amount of MT-SLNs were (87.54±5.31)% and (8.42±0.78)%. The transcutaneous absorption behavior of MT-SLNs and MT raw material conformed to zero-order kinetics release rule. The steady-state transmission rate of MT-SLNs [(31.71+2.78) μg/(h·cm2)] was significantly higher than that of its raw material [(10.32±3.24) μg/(h·cm2)], and its lag time [(0.17±0.01) h] was significantly shorter than that of its raw material [(1.57±0.37) h] (P<0.01). CONCLUSIONS: Prepared MT-SLNs have small particle size and distributed evenly, and can promote in vitro transcutaneous absorption.

During coronavirus disease 2019 epidemic, the treatment of severe trauma has been impacted. The Consensus on emergency surgery and infection prevention and control for severe trauma patients with 2019 novel corona virus pneumonia was published online on February 12, 2020, providing a strong guidance for the emergency treatment of severe trauma and the self-protection of medical staffs in the early stage of the epidemic. With the Joint Prevention and Control Mechanism of the State Council renaming "novel coronavirus pneumonia" to "novel coronavirus infection" and the infection being managed with measures against class B infectious diseases since January 8, 2023, the consensus published in 2020 is no longer applicable to the emergency treatment of severe trauma in the new stage of epidemic prevention and control. In this context, led by the Chinese Traumatology Association, Chinese Trauma Surgeon Association, Trauma Medicine Branch of Chinese International Exchange and Promotive Association for Medical and Health Care, and Editorial Board of Chinese Journal of Traumatology, the Chinese expert consensus on emergency surgery for severe trauma and infection prevention during coronavirus disease 2019 epidemic ( version 2023) is formulated to ensure the effectiveness and safety in the treatment of severe trauma in the new stage. Based on the policy of the Joint Prevention and Control Mechanism of the State Council and by using evidence-based medical evidence as well as Delphi expert consultation and voting, 16 recommendations are put forward from the four aspects of the related definitions, infection prevention, preoperative assessment and preparation, emergency operation and postoperative management, hoping to provide a reference for severe trauma care in the new stage of the epidemic prevention and control.
