1.THE SIGNIFICANCE OF PERITONEAL EXFOLIATIVE CYTOLOGY IN GASTRIC CANCER

Exfoliated cells in abdominal cavity were studied in 142 patients with gastric cancer. The cells were mainly obtained by irrigation with normal saline during operation. In this series, 53.5%(76/142) of the patients have malignant exfoliative cells. The relationships between the localization and the pathological types of cancer and the positive rate of the exfoliative cells in the abdominal cavity were discussed. No significant differences were found with regard to the TNM staging of the gastric cancer, the positive rate of stage I was 4/5, while those of the stage II,III,IV were 36%, 42.6% and 66.2%, respectively. The five-year follow-up study showed that in the positive exfoliative group, none lived more than 3 years, including 4 cases of TNM stage I, 9 cases of stage II and 63 cases with advanced cancer. However, in the 66 cases with negative cytology, 1 case of stage I patient was alived more than 7 years after operation, and in the 26 cases of stage II, 10 survived more than 5 years. This method is recommanded to detect the early metastasis of gastric cancer in the abdominal cavity. It may be useful in planning a reasonable management for the patient and may give a better understanding about the prognosis.
2.Ultrasonic appearance and diagnostic criteria of peripheral neurilemmoma

Objective To investigate the ultrasonic appearance of peripheral neurilemmoma.Methods Ultrasonic appearances of 43 patients with 46 neurilemmomas confirmed by surgery and pathology were analyzed retrospectively and compared with their MRI and pathological features.Results Forty-five neurilemmomas were with regular shape and clear boundary,except one at the root of nerve C4 that was irregular in shape and unclear in boundary,like adumbbell.Sonography showed that there were 30 neurilemmomas displaying as solid tumors,15 as cystic and solid,and 1 as entirely cystic.Posterior echoenhancement,target sign,rat tail sign and vessels accompanying sign were found in 39,21,23 and 17 neurilemmomas,respectively.There were 4 neurilemmomas with ultrasound blood flow grade 0,9 with grade Ⅰ,22 with gradeⅡ,and 11 with grade Ⅲ.The target signs were found in 21 cases (21/46,45.6%) and 26 cases (26/46,56.5 %) by ultrasound and MRI,respectively.The capability of ultrasound finding the target sign in neurilemmoma was comparable to MRI(Kappa =0.527,P <0.001).The diagnostic accuracy rate of neurilemmoma by ultrasonography was 73.9%(34/46).Conclusions The ultrasound has a relatively high detection of the target sign in neurilemmoma,which may provide a new basis for the ultrasonic diagnosis of neurilemmoma.
3.Application of Total Quality Management in project management of National Natural Science Foundation of China

Objective:To explore a new model of Total Quality Management in project management of National Natural Science Foundation of China (NSFC).Methods:The theory of Total Quality Management is applied to the management of NSFC funded project, that is, a complete and comprehensive process formed which covered the different stages of project cultivation, application, identification, implementation, completion and post-completion tracking; and the management concepts of whole process personnel engagement and comprehensive quality management are penetrated into those different stages, furthermore, corresponding key points should be focused are also analyzed.Results:The hospital has made great achievements in scientific research management, project quantity, outputs, and discipline construction.Conclusions:The introduction of Total Quality Management theory into NSFC management is conducive to continuously improve the level of fund management, and enhance the scientific and technological innovation capacity building of the hospital.
4.Transcranial direct current stimulation can improve the effectiveness of robot-assisted rehabilitation of a hemiplegic upper limb

Objective:To observe any improvement in hemiplegic upper limb functioning when transcranial direct current stimulation (tDCS) is combined with robot-assisted upper limb treatment, and analyze the potential mechanism of neural plasticity through diffusion tensor imaging (DTI).Methods:Twenty stroke survivors with hemiplegia were randomly divided into a treatment group and a control group, each of 10, according to a random number table. Both groups were treated with conventional medication and rehabilitation training using an upper limb robot, while the treatment group also received tDCS daily, with the current increasing from 0 to 1mA over 10 seconds, and then decreasing to 0 over twenty minutes. The experiment lasted for 15 days. The upper extremity portion of the Fugl-Meyer rating scale (UE-FMA) and the Wolf Motor Function Rating Scale (WMFT) were used to evaluate motor functioning before and after the treatment. DTI was also conducted for both groups.Results:After the treatment, the average UE-FMA and WMFT scores of the two groups were significantly higher than before the treatment, with the average UE-FMA score of the treatment group (35.32±13.25), significantly higher than that of the control group (21.80±13.93). After the treatment there were significant differences between the groups in their average FA, rFA and FAasy of the posterior limb of the internal capsule, as well as in FA and the CST length of the central anterior gyrus.Conclusion:tDCS combined with robot-assisted upper limb rehabilitation training can significantly improve the motor functioning of hemiplegic upper limbs, possibly due to neuroplasticity mechanisms that promote CST integrity and symmetry changes. tDCS can be an important adjunct therapy in clinical neurorehabilitation.
5. Discussion on early warning, prevention and control of emerging infectious diseases from a macroscopic perspective based on big data and effective distance model: enlightenment of COVID-19 epidemic data in China

Objective To provide a system for warning, preventing and controlling emerging infectious diseases from a macroscopic perspective, using the COVID-19 epidemic data and effective distance model. Methods The dates of hospitalization/isolation treatment of the first confirmed cases of COVID-19 and the cumulative numbers of confirmed cases in different provinces in China reported as of 23 February, 2020 were collected. The Location Based Service (LBS) big data platform of 'Baidu Migration' was employed to obtain the data of the proportion of the floating population from Wuhan to all parts of the country. Effective distance models and linear regression models were established to analyze the relationship between the effective distance and the arrival time of the epidemic as well as the number of cumulative confirmed cases at provincial and municipal levels. Results The arrival time of the epidemic and the cumulative number of confirmed cases of COVID-19 had significant linear relationship at both provincial and municipal levels in China, and the regression coefficients of each linear model were significant ( P <0.001). At the provincial level, the effective distance could explain about 71% of the variation of the model with arrival time along with around 90% of the variation for the model in the cumulative confirmed case magnitude; at the municipal level, the effective distance could explain about 66% of the variation for the model in arrival time, and about 85% of the variation of the model with the cumulative confirmed case magnitude. Conclusions The fitting degree of the models are good. The LBS big data and effective distance model can be used to estimate the track, time and extent of epidemic spread to provide useful reference for early warning, prevention and control of emerging infectious diseases.
