1.Fecal microbiota transplantation therapy on Clostridium difficile infection in children

With the overuse of antibiotics,Clostridium difficile infections are increasing,and the incidence of refractory or recurrent cases increase.Increasing resistance to the traditional treatment(metronidazole and/or vancomycin),relapsing Clostridium difficile infection needs to seek new treatments.Fecal microbiota transplantation has been widely used in the treatment of adult Clostridium difficile infection,but is seldom used in children.In this paper,fecal microbiota transplantation for the treatment of Clostridium difficile infection in children related issues were reviewed.
2.Clinical characteristics and nutritional therapy of pediatric inflammatory bowel disease

Recently, the incidence of pediatric inflammatory bowel disease( IBD) is increasing. The diagnosis and treatment of IBD were focused by pediatric clinicians. The clinical features and treatment of pediatric IBD are dif-ferent,because of the growth and development. Growth deficiency and delayed puberty are unique symptoms. Children with IBD were treated with exclusive enteral nutrition as the first line treatment. The better treatment was formulated in accordance with the clinical features and nutritional therapy of pediatric IBD.
3.Recent progress of dry powder inhalation of proteins and peptides.

To provide theoretical and practical basis for the successful formulation design of physically-mixed inhalation dry powder of proteins and peptides, related references were collected, analyzed and summarized. In this review drug micronization technology and commonly used carriers for inhalation dry powder preparation were introduced. For proteins and peptides, supercritical fluid technology and spray-drying are more suitable because of their capabilities of keeping drug activity. Being approved by U. S. Food and Drug Administration, lactose has been extensively used as carriers in many inhalation products. Formulation and process factors influencing drug deposition in the lung, including carrier properties, drug-carrier ratio, blending order, mixing methods, mixing time and the interaction between drug and carrier, were elucidated. The size, shape and surface properties of carries all influence the interaction between drug and carrier. Besides, influence of micromeritic properties of the dry powder, such as particle size, shape, density, flowability, charge, dispersibility and hygroscopicity, on drug deposition in the lung was elaborated. Among these particle size plays the most crucial role in particle deposition in the lung. Moreover, based on the mechanisms of powder dispersity, some strategies to improve drug lung deposition were put forward, such as adding carrier fines, adding adhesive-controlling materials and reprocessing micronized drug. In order to design physically-mixed inhalation dry powder for proteins and peptides with high lung deposition, it is essential to study drug-carriers interactions systematically and illustrate the potential influence of formulation, process parameters and micromeritic properties of the powder.
4.Experimental Study of Suzijiangqi Decoction on Effect on the Relation between Passage Recombination and the Content of TNF and ET of Asthma Model

Objective To explore the mechanism of Suzijiangqi decoction to passage recombination of asthma model by studying the effect of Suzijiangqi decoction on passage recombination and the relation between it and the content of TNF and ET of asthma model. Methods Asthma model was established by OVA atomized and inhaled, the effect of Suzijiangqi decoction on the lung histomorphometric and the content of TNF and ET in serum or plasma/BALF of asthma model were observed. Results Suzijiangqi decoction obviously improved the lung histomorphometric and significantly inhibited the content of TNF and ET. Conclusion Suzijiangqi decoction can obviously control passage recombination. It is relate to inhibiting the content of TNF and ET in serum or plasma/BALF of asthma model.
5.Effect of lipid emulsion on cardiac arrest induced by Bupivacain intoxication

Objective To investigate effect of lipid emulsion on cardiac arrest induced by bupivacain intoxication.Methods Forty SD rats were randomly devided into 2 groups.Group A applied epinephrine (n=20);Group L applied lipid emulsion combinding epinephring(n=20).The rats were administered Lupivacaine 20 mg/kg.Cardiopulmonary resuscitation(CPR) was started. Epinephrine 10 μg/kg were repeated three times followed by epinephrine 10 μg/kg every 5 minutes.Normal saline 5 ml/kg and 1 ml·kg-1·min-1 were administered in group A.20% Lipid emulsion 5 ml/kg and 0.25 ml·kg-1·min-1 were administered in group L.Heart rate,blood pressure, temperature were monitored.Blood gas analysiswere checked at 5 min,30 min after restoration of spontaneous circulation(ROSC).To record the number of successful CPR,time of ROSC and epinephrine dose.Results 11 rats(55%) and 16 rats(80%) were successful resuscitated.The success rate was higher in group L than in group A.Epinephrine dose was higher in group A than in group L.There was no difference in heart rate between the two groups.Systolic blood pressure and PaO2 was higher in group L than in group A(P<0.05).Conclusion There was better effect in lipid emulsion combined epinephrine than soly epinephrine when applied in cardiac arrest induced by bupivacaine intoxication.
6.Preliminary analysis of two-dimensional gel electrophoresis maps in gastric carcinoma

Objective: To establish the two-dimensional gel electrophoresis pH 4-7 maps of human gastric carcinoma and paired paracancerous mucosa,and to analyze differently expressed proteins of human gastric carcinoma and paired paracancerous mucosa.Methods: The total proteins of human gastric carcinoma and paired paracancerous mucosa were extracted,separated and analyzed by two-dimensional electrophoresis(2-DE).Results:By comparing and analyzing pH 4-7 protein expression maps of 5 cases of gastric carcinoma and paired paracancerous mucosa,128 differently expressed protein spots were detected,of which 56 expressed only in gastric carcinoma,27 in normal gastric mucosa tissues,32 more highly and 13 more lowly in gastric carcinoma than in paracancerous mucosa tissues. Conclusion: Proteins express differently in gastric carcinoma from paired paracancerous mucosa.Searching for differently expressed proteins between the two types of tissues may throw light on the molecular mechanism of gastric carcinoma.
7.Comparative study on the mental health status of urban and rural mothers of children with cleft lip and palate

Objective:To compare the mental health between the urban mothers and rural mothers of children with cleft lip and pal-ate.Methods:1 68 mothers(including 1 24 rural and 44 urban)of the children with cleft lip and palate were included in the trial group and 82 mothers of normal children were included in the control group.The subjects were tested by the self-assessment symptom lists of SCL-90 for the evaluation and comparison of mental health.Results:The scores of nine dimensions of trial group in somatization, forcing symptoms,depression,anxiety,etc were significantly higher than those of the control group(P <0.05).The scores of nine di-mensions of rural group in somatization,forcing symptoms,depression,anxiety,etc were higher than those of the urban group,the ri-val,psychotic and total scores showed significant differences(P <0.05);Conclusion:The mental health status of mothers of children with cleft lip and palate is poorer than that of those of normal children.That of the rural mothers is poorer than that of the urban ones. Special attention should be paid to the mental health status of the parents of children with cleft lip and palate.
8.Drug therapy and mechanism of pain in mice with induced adenomyosis

Objective To investigate the effects of different drugs and to explore the mechanism of pain in mice with adenomyosis ( ADM) .Methods The mouse model of adenomyosis was induced by oral administration of tamoxifen. The vaginal smear was examined by cytology, and serum levels of 5-HT, GnRH-R, NGF and NF were determined.Results Higher level of 5-HT was detected in the controls compared with the models.The expression of GnRH-R and NGF in nor-mal endometrium and eutopic endometrium were significantly lower than those in ectopic endometrium.The expression on NF in normal endometrium was significantly lower than that in the eutopic and ectopic endometria.Conclusions Similar with regular dose of GnRH-a, half dose of GnRH-a can slow down the progress of ADM as well as reducing pain.Combina-tion of regular dose of GnRH-a and Diane-35 could consolidate the therapeutic effect and even achieve pathological cure.5-HT may take an active part in the mechanism of pain in ADM.
9.Relationship between placenta HBsAg and hepatitis B serum markers in newborns

Objective To study the relationship between placenta HBsAg in HBsAg positive pregnant women and serum hepatitis B virus (HBV) markers,HBV DNA levels in newborns.Methods Placenta HBsAg was detected by immunohistochemical affinity hormone-biotin complex (ABC) method in 155 HBsAg positive pregnant women.Serum HBV markers in newborns were detected by enzyme-linked immunosorbent assay (ELISA).Serum HBV DNA levels of newborns were detected by real-time fluorescence quantitative polymerase chain reaction (PCR).The positive rates were compared using x2 test.Results HBsAg was expressed with different levels in various types of cells of placenta in 155 pregnant women.The total placenta HBsAg positive rate was 37.4% (58/155),and those in decidual cells,trophoblastic cells,villous mesenchymal cells and villous capillary endothelial cells were 37.4% (58/155),25.8% (40/155),18.7% (29/155) and 7.1% (11/155),respectively.The HBsAg positive rates of placenta gradually decreased from decidual cells of the maternal surface to villous capillary endothelial cells of the fetal surface (tendency x2 =43.01,P=0.00).The positivity of placenta HBsAg was associated with both HBsAg and HBeAg in newborns (x2 =4.88,P<0.05 and x2 =3.86,P<0.05,respectively),while that was not associated withanti-HBe and anti-HBc in newborns (x2 =3.36,P>0.05 and x2 =0.00,P> 0.05,respectively).The risk of HBsAg positive in newborns was higher when HBsAg was positive in villous capillary endothelial cells and villous mesenchymal cells (OR=5.31,95 %CI=1.38-20.40 and OR=3.33,95%CI=1.16-9.52,respectively).The risk of HBeAg positive in newborns was higher when HBsAg was positive in trophoblastic cells and villous mesenchymal cells (OR=3.04,95 %CI=1.45-6.39 and OR=3.05,95 % CI=1.32-7.03,respectively).However,placenta HBsAg positive was not associated with HBV DNA positive in newborns (x2 =0.09,P>0.05).Conclusion The risk of neonatal HBV serological markers positive is higher when the HBsAg positive placental cells are closer to fetal surface,which indicates that HBsAg enters fetal blood circulation by means of cell transferring layer by layer.
10.The safety of Habib VesOpen bipolar radiofrequency ablation catheter used in the treatment of portal vein tumor thrombus:an experimental study in miniature pig models

Objective To explore the safety of Habib VesOpen bipolar radiofrequency ablation (RFA) catheter used in the treatment of portal vein tumor thrombus (PVTT). Methods A total of 10 miniature pigs were randomly divided into 3 groups. Group A(n=6):RFA of normal portal vein was directly performed;group B (n=2): balloon obstruction of the portal vein was performed first, which was followed by RFA for the fresh thrombus in the portal vein; group C (n=2): PVTT model was established first, and RFA of the portal vein was carried out when the portal thrombus became organized. MRI examination was employed at one, 3 and 4 weeks after RFA; the animals were sacrificed 4 weeks after RFA and pathological examination of portal vein was performed. Results Pigs of group A received portal vein RFA under the condition of 5 W power for 0.6-3.6 min. No obvious abnormality was detected by MRI and pathological examination , which were performed one month after the treatment. In the pigs of group B , MRI performed after RFA showed that the damage of portal vein area was more serious than that in the pigs of group A;abdominal MRI examination performed at one, 3 and 4 weeks after RFA showed that the portal venous edema was gradually decreased;pathological examination at one month after RFA demonstrated serious injury of adjacent liver tissue. Pigs of group C received portal vein RFA under the condition of 7 W power for 1.5 min; no obvious edema of the ablated area was observed on MRI performed after RFA , and pathological examination revealed organized thrombus necrosis and va scular endothelial cell damage. Conclusion When Habib VesOpen bipolar RFA catheter is used for the treatment of PVTT, the RFA power and time should be properly selected according to the severity of PVTT. In order to ensure a safer procedure, high power and short ablation time should be used when the severity of PVTT is mild, while low power and longer ablation time are recommended when the PVTT is more severe.
