1.Vitamin D deficiency and critical illness

[Summary] There is growing awareness of the importance of vitamin D beyond maintenance of musculoskeletal health.Vitamin D has been shown to have extensive biological functions,including modulating immune function,inhibiting cellular proliferation,inducing differentiation,promoting apoptosis.Vitamin D deficiency is highly prevalent in critically ill patients,and it may impact the severity of illness and contribute to lessthan-optimal outcomes.Vitamin D plays an important role in preventing the development of critical illness based on its pleiotropic actions,thus holding a profound prospective with respect to an adjunctive therapy in critically ill patients.
2.ULTRASTRUCTURAL OBSERVATION OF MOUSE MAMMARY ADENOCARCINOMA CELL LINE (MA782/5S-8102)

The ultrastructural characteristics of the mouse mammary adenocarcinoma cell line (MA782/5S-8102) have been studied by transmission electron microscope. The results show that the cells were irregular in the shape, with abundant microvilli on cell surface. The nucleus was abnormal in shape with the nuclear membrane sunk or swollen, the nucleus/cytoplasmic ratio (N/C) was high. There were many free ribosomes, abundant mitochondrias, dilated rough endoplasmic reticulum in the cytoplasm. Golgi apparatus, lysosomes and lipid droplets were present. Moreover, many cytoplasmic type-A particles and a few immatural type-B particles at the surface of the cell were observed.
3.Effect of abducens orthosis combined with walker on developmental dysplasia of the hip

Objective To evaluate the effect of abducens orthosis combined with walker on de-velopmental dysplasia of the hip ( DDH ). Methods A total of 126 patients (224 hips ) with DDH aged 6~36 months in Xiangya Hospital was randomly divided into 2 groups: an orthosis combined with walker group and an improved hip frog cast fixation group. Seventy patients (130 hips) were treated by the orthosis combined with walker and 56 patients (94 hips) were treated by the improved hip frog cast fixation. We compared the effect and complications of the 2 groups. Results The fine-ness rates of the orthosis combined with walker group and the improved hip frog cast fixation group were 89.2% and 90.4% , respectively, with no significant difference (P>0.05). The rate of femoral head osteonecrosis in the orthosis combined with walker group was significantly lower than that in the improved hip frog cast fixation group (1.5 % vs. 5.3 % , P<0.05) , but the re-dislocation rate in the former was significantly higher than that in the latter (6.9 % vs. 1.1% , P<0.05). Conclusion Both methods are effective for DDH. Orthosis combined with walker has a lower propor-tion of femoral head osteonecrosis, but a higher proportion of re-dislocation.
4.The prevalence of vitamin D deficiency in intensive care unit and its influence on prognosis

Objective To investigate the prevalence of vitamin D deficiency in medical intensive care unit (ICU) and its relationship with severity of disease and prognosis.Methods A prospective study was performed to evaluate vitamin D status in 216 patients admitted to the medical intensive care unit.The incidence of hypovitaminosis D was observed.Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ) score,days kept in ICU and on ventilator,main laboratory findings,and mortality rate were compared among patients with different serum 25-hydroxyvitamin D [25 (OH) D] levels.Potential risk factors for mortality were analyzed by multivariate logistic regression analysis.Results One hundred and fifty-three patients (70.8%) developed hypovitaminosis D.25 (OH) D deficiency was identified in 95 (44.0%),25 (OH) D insufficiency in 58 (26.8%),and 25 (OH) D sufficiency in 63 (29.2%) patients.APACHE Ⅱ score,positive blood culture,the incidence of multiple organ dysfunction syndrome(MODS),and mortality rate were higher in deficiency group compared with the other two groups [APACHE Ⅱ score:deficiency group 25 (20,28) score,insufficiency group 22 (17,26) score,sufficiency group 19 (18,20) score,P<0.01 ; positive blood culture:deficiency group 18.9%,insufficiency group 13.8%,sufficiency group 3.2%,P=0.015 ; MODS:deficiency group 48.4%,insufficiency group 43.1%,sufficiency group 25.4%,P=0.025; mortality rate:deficiency group 40%,insufficiency group 24.1%,sufficiency group 15.9%,P =0.003].25 (OH)D levels were negatively correlated with APACHE Ⅱ score and mortality rate (r =-0.325,P<0.01 ; r=-0.276,P<0.01,respectively).Analysis by multiple logistic regression demonstrated that 25 (OH) D deficiency (OR =3.005,95 % CI 1.321-5.875,P =0.008) was independent risk factor for mortality.Conclusions This study demonstrates that vitamin D deficiency is highly prevalent among patients admitted into ICU.Vitamin D deficiency is associated with disease severity,and seems to be an independent risk factor for mortality.
5.Progress in the role of specific signaling pathways in bone repair and regeneration

Bone regeneration strategy to repair bone defects shows great potential.Although the skeleton has a strong self-healing and repair capabilities, for huge bone defect caused by trauma, congenital malformations,infection and surgery,the skeleton is difficult to complete the self-repairing process.In order to promote bone regeneration,bone growth must be induced by biologically active implants,cell and intracellular molecular signaling pathways.Mesenchymal stem cells(MSC)play an important role in bone repair and remodeling.Understanding the role of MSC and signaling pathways in bone repair and regeneration is critical for the study and development of bone repair materials.
6.Research progress of MSC in vitro expansion of platelet lysate instead of animal serum

The regulator does not encourage the use of fetal bovine serum (FBS) as a cell culture supplement to reduce the risk of zoonosis and heterogeneous immune responses.In addition,FBS production is subject to strict examination because of animal welfare principles.Mesenchymal stem cells (MSC) in the platelet lysate (PL) showed a faster growth rate,while maintaining good osteogenic differentiation,PL also have the key factors of MSC attachment,biological safety and immune regulation ability after MSC amplification.PL was proposed as an altemative to MSC in vitro expansion of FBS.This paper briefly introduces the latest research on the field of PL amplification in vitro.
7.Diagnostic value of fine needle aspiration cytology combined with serum calcitonin for medullary thyroid carcinoma

Objective To investigate the value of fine-needle aspiration cytology (FNAC) combined with serum calcitonin in the diagnosis of medullary thyroid carcinoma (MTC).Methods Retrospective analysis of the serum calcitonin and FANC results in 17 cases of MTC confirmed by pathological examination.The diagnostic value of the two methods alone and in combination for MTC were compared.Results 13 of the 17 MTC patients (13/17,76.5%) underwent fine needle aspiration biopsy before surgery,11 cases (11/13,84.6%) diagnosed as thyroid malignant tumor,7 of whom were diagnosed or suspected of MTC (7/13,53.8%).Serum calcitonin levels increased in different degrees in 16 patients.The sensitivity of the two methods combined in the diagnosis of MTC was 92.3% (12/13).Conclusions FNAC is an important diagnostic method for the evaluation of thyroid nodules,but it is not sensitive to the diagnosis of MTC.Serum calcitonin is highly sensitive but not universally available.Compared with FNAC alone,FNAC combined with the serum calcitonin examination may significantly improve the detection rate of MTC preoperative and reduce missed diagnosis.
8.Anatomic data of the proximal femur and its clinical significance.

OBJECTIVE:
To measure the anatomic data of the proximal femur and to design an internal fixation instrument aiming at subtrochanteric fracture.
METHODS:
We measured the anatomic data of 56 pairs of the matching proximal femur specimens: the diameter of femoral head (HD), the axis length of femoral head (HAL), 135 degree femoral head-neck axis length (HNAL), 135 degree femoral head-neck axis upper length (HNAUL), 135 degree femoral head-neck axis underside length (HNADL), the anterior-posterior axis diameter of femoral neck (NAPD), the upper-underside diameter of femoral neck (NUUD), femoral neck-shaft angle (NFA), femoral shaft lateral cortex-greater trochanter angle (SLGA), the medial-lateral diameter of lesser trochanter level's femoral shaft (LSMLD), the anterior-posterior diameter of lesser trochanter level's femoral shaft (LSAPD), the medial-lateral diameter of 5 cm below lesser trochanter femoral shaft (5 cm MLD), and the anterior-posterior diameter of 5 cm below lesser trochanter femoral shaft (5 cm APD). Part of the data was analyzed and compared.
RESULTS:
HD was (46.69+/-3.73) mm, HAL was (39.22+/-4.17) mm, HNAL was (95.45+/-8.16) mm, HNAUL was (84.02+/-7.11) mm, HNADL was (99.95+/-9.34) mm, NAPD was (26.27+/-3.15) mm, NUUD was (32.24+/-3.31) mm, NFA was 126.21 degree+/-7.13 degree, SLGA was 16.38 degree+/-4.04 degree, LSMLD was (31.05+/-3.57) mm, LSAPD was (27.63+/-2.96) mm, 5 cm MLD was (26.36+/-3.22) mm, and 5 cm APD was (25.59+/-2.75) mm. NFA was positively correlated with SLGA (r=0.396, P=0.003).
CONCLUSION
It is necessary to design internal fixator to fit the anatomical feature of Chinese femur for the treatment of subtrochanteric fracture, and we should thoroughly consider the angle of the SLGA.

9.Applications of marine-derived chitosan and alginates in biomedicine.

Marine-derived biopolymers are excellent raw materials for biomedical products due to their abundant resources, good biocompatibility, low cost and other unique functions. Marine-derived biomaterials become a major branch of biomedical industry and possess promising development prospects since the industry is in line with the trend of "green industry and low-carbon economy". Chitosan and alginates are the most commonly commercialized marine-derived biomaterials and have exhibited great potential in biomedical applications such as wound dressing, dental materials, antibacterial treatment, drug delivery and tissue engineering. This review focuses on the properties and applications of chitosan and alginates in biomedicine.
