1.The Effect of Cervical Canal Mucosa Ring Excision on Cervical Columnar Eversion in Loop Electrosurgical Excision Procedure

Objective:To study the effect of cervical canal mucosa dng excision on cervical columnar eversion in Loop electrosurgical excision procedure(LEEP).Methods:A prospective randomized control trail was performed in 125 cervical intraepithelial neoplasia(CIN)patients.Cervix and cervical canal were conically excised with triangle electrode in 62 patients in the control group;in the study group,a small ring electrode was put into the cervical canal and the cervical canal mucosa was excised about 0.5~0.8cm,after excision as the control group.Follow-up was performed postoperatively in 1,3,6 months respectively.The operation time,the bleeding volume,the rate of cervical columnar eversion and cervical adhesion or stenosis were compared between the two groups.Results:The rate of cervical columnar eversion in the study group (1/63,1.59%)was lower than that in the control group(9/62,14.52%).There was significantly statistical difference between them(P=0.008).There was no statistical differences between them in the operation time,the bleeding volume,and the rate of cervical adhesion or stenosis (P>0.05).Conclusions:The cervical canal mucosa ring excision in LEEP can effectively prevent postoperative cervical columnar eversion.
2.Therapeutic effect of Marvelon on adenomysis

Objective To evaluate the efficacy of Marvelon on treatment of adenomyosis.Methods A total of 42 women with adenomyosis were recruited in the study.They were given Marvelon on the first day of menstruation,1 piece,qd,continouly for 21 days.With an alternation of seven days a new cycle begins.There are 6 cycles in total.We observe their degree of dysmenorrheal,menstrual blood volume(MBV) ,hemoglobin and the volume of uterus before and after Marvelon used.Results After 6 months,the dysmenorrheal remitted obviously,the score decline from (8.51±1.73 ) to (0.62±0.70) ,P <0.01.The MBV reduced and hemoglobin rise distinctly ,before treatment Hb is (104.71±12.09) while (112.00±8.57)after therapy,P <0.01.After treatment the volume of uterus shrinks a little, but not significant(P > 0.05).Conclusion Marvelon is an effective drug in treatment of adenomyosis in the near future,and is cheap,convcnieat.
3.Dynamic Expression of Nogo-A in Hippocampus of Rats after Carbon Monoxide Poisoning

Objective:To study the dynamic expression of Nogo-A in hippocampus of rats after carbon monoxide poisoning,and to explore the effect and influence of Nogo-A in the damage to nervous system after carbon monoxide poisoning.Methods:Thirty male SD rats were randomly divided into NC group(n=6),CO group(n=6),CO-24 h group(n=6),CO-48 h group(n=6),CO-7d group(n=6).The method of injection CO gas was used to establish the carbon monoxide poisoning model.Then immunohistochemical (IHC) and Western blot (WB) techniques were used to observe dynamic expression of Nogo-A in hippocampus of rats at several time intervals after carbon monoxide poisoning and to analyze its change law.Results:IHC results showed that the average optical density value of expression of Nogo-A in NC group,CO group,CO-24 h group,CO-48h group and CO-7d group were 0.0928± 0.0038,0.01172± 0.0042,0.1452± 0.0056,0.1271 ± 0.0057,0.1088± 0.0055.WB results showed that the expression of Nogo-A in hippocampus after carbon monoxide poisoning was significantly higher than that in NC group(P<0.05),and reached the highest level at 24 h,then had a gradual recovery after 24h.The expression of Nogo-A decreased obviously,but still higher than that of NC group by day 7 (P<0.05).Conclusions:In this study,the increase of expression of Nogo-A was associated with carbon monoxide poisoning.The expression of Nogo-A reached the highest level at 24h,then had a gradual recovery after 24 h.
4.Therapeutic Observation of Ginger-partitioned Moxibustion in Treating Myelosuppression Induced by Chemotherapy for Multiple Myeloma

Objective To observe the clinical efficacy of ginger-partitioned moxibustion in treating myelo- suppression induced by chemotherapy for multiple myeloma (MM), and to discuss the effect of ginger-partitioned moxibustion in preventing and treating the adverse reactions of chemotherapy.Method Forty-four MM patients with myelosuppression induced by chemotherapy were randomized into a treatment group and a control group, 22 cases each. The treatment group was intervened by ginger-partitioned moxibustion, while the control group was intervened by oral administration of Leukogen tablets, both for 14 d. The white blood cell (WBC) count was observed before the treatment, on treatment day 7 and 14, and 1 week after the intervention; the incidence of nausea and vomit and clinical efficacies were compared between the two groups.Result The total effective rate was 81.8% in the treatment group versus 63.6% in the control group, and the difference was statistically significant (P<0.05). The WBC count on the 7th treatment day was significantly different from that before the treatment in both groups (P<0.05); the WBC count in the treatment group was significantly different from that in the control group on the 7th treatment day (P<0.05). The WBC count on the 14th treatment day was significantly different from that on the 7th day in both groups(P<0.05); the WBC count in the treatment group was significantly different from that in the control group on the 14th treatment day (P<0.05). The WBC count 1 week after the treatment was insignificantly different from that on the 14th treatment day in both groups (P>0.05); there was no significant difference in comparing the WBC count 1 week after the treatment between the two groups (P>0.05). The incidence of nausea and vomit in the treatment group was significantly lower than that in the control group after the treatment (P<0.05).Conclusion Ginger-partitioned moxi- bustion can effectively treat myelosuppression due to chemotherapy for MM, and it can significantly increase the WBC count.
5.Analysis of chromosome karyotype characteristics in patients with chronic myeloid leukemia

Objective To explore the characteristics of chromosome karyotypes in patients with chronic myeloid leukemia (CML),and to provide help to individualized treatment.Methods The date of chromosome karyotypes of 313 patients and FISH of 45 of these patients with CML excluding Ph chromosome negative (Ph-) after treatment were collected from January 2014 to June 2015.Karyotypes were detected by R-banding.Results In the 313 cases,307 cases (98.08 %) were Ph chromosome positive (Ph+) and 6 cases (1.92 %) were Ph-.In the Ph+ patients,288 cases (93.81%) were classical Ph+,and 19 cases (6.19 %) were variant rearrangements.There were 48 cases (15.34 %) with additional chromosome changes in all patients,including 41 cases (13.10 %) with classical Ph+ and 7 cases (2.24 %) with variant rearrangements.The most common additional chromosome changes were in the following order:+der(22) Ph (35.42 %),+8 (33.33 %) and +21 (12.50 %).The most frequent pattern of combination was +der(22) combined with +8 (16.67 %),followed by +8 combined with +21 (10.42 %).The proportion of pure Ph+ patients in chronic phase was higher than that of advanced phase,but proportion of classical Ph+ patients with additional chromosome changes in chronic phase was lower than that in advanced phase (x2 =1 11.55,P < 0.01).The proportions of chronic phase and advanced phase patients with simple variant rearrangements were not different from those with complex variant rearrangements (P =0.582).The results of FISH in 45 cases were all positive,including 5 cases with 2 GIR1Y.Conclusion Karyotype analysis can reveal the instability of genetic and the characteristics of disease progression by identifying the evolution of Ph,which provides the basis for clinical doctors to choose suitable treatment.
6.Clinical effect of pulmonary rehabilitation therapy including respiratory exercise and vibration expectoration on patients with pulmonary infection after abdominal surgery

Objective To investigate the clinical effect of pulmonary rehabilitation therapy including respiratory exercise and vibration expectoration on patients with pulmonary infection after abdominal surgery.Methods A retrospective case control study was conducted.Seventy-six patients with pulmonary infection after abdominal surgery admitted to the First Affiliated Hospital of Hunan Normal University from September 2015 to September 2016 were enrolled.According to whether accept the pulmonary rehabilitation therapy or not,the patients were divided into two groups.In the control group (n =35),the convemional expectoration method was adopted.The patients in pulmonary rehabilitation group (n =41) received both methods of the control group and pulmonary rehabilitation treatment,including respiratory exercise (effective cough,lip reduction breathing),respiratory exercise device (respiratory exerciser tri-ball),and vibrated expectoration.The 24-hour sputum volume,degree of comfort,inflammatory and pulmonary function parameters,and recovery situation were recorded in the two groups.Results ① There were no significant differences in the parameters of inflammation and pulmonary function before treatment between the two groups.After treatment,the white blood cell (WBC) and C-reactive protein (CRP) in both groups were significantly decreased,and the forced expiratory volume in 1 second (FEV1) and FEV1/forced vital capacity (FVC) were significantly increased.The above changes in pulmonary rehabilitation group were more significant than those of the control group [WBC (× 109/L):11.12 ± 2.88 vs.13.42 ± 2.62 at 3 days,8.22 ± 1.48 vs.9.27 ± 1.92 at 5 days;CRP (mg/L):13.47 ± 4.77vs.16.03±4.94 at 3 days,9.69±1.56 vs.11.77±1.41 at 5 days;FEV1 (L):2.48±0.14 vs.2.29±0.16 at 3 days,FEV1/FVC:0.78±0.04 vs.0.75±0.04 at 3 days;all P < 0.05].② The 24-hour sputum volume within 3 days of pulmonary rehabilitation group were significantly higher than that of the control group (mL:30.51 ± 4.15 vs.18.30 ± 3.64at 1 day,31.08±3.22 vs.20.37±3.20 at 2 days,29.03±2.55 vs.19.03±2.51 at 3 days,all P < 0.01].③ In the pulmonary rehabilitation group,the recovery time of pulmonary infection symptoms (days:5.44 ± 1.45 vs.6.20 ± 1.55),the days of antibiotic use (days:12.61 ± 3.15 vs.15.03 ± 3.78),the time of getting out of the bed (days:4.05 ± 0.74vs.4.51±0.89),and the hospital days (days:19.95±3.90 vs.22.00±4.42) were significantly shorter than those of the control group (all P < 0.05),and the degree of comfort was significantly better than that of the control group (comfort score:2.71 ±0.90 vs.2.14±0.91,P < 0.01).Conclusion The application of pulmonary rehabilitation including respiratory exercise and vibration expectoration in abdominal surgery patients with pulmonary infection can promote recovery,and it has a good clinical and practical application value.
7.Stem cells transplantation for experimental colitis in rats

Objective To investigate the effect of transplantation of allogeneic bone marrow hematopoietic cells(HCs)and mesenchymal stem cells(MSCs)on experimental colitis(EC)in rats.Methods The HCs and MSCs obtained from SD male rats were cultured and expanded in vitro.In experiment 1 and 2 groups,HCs were labeled with bromodeoxyuridine(BrdU)and MSCs were obtained using the tube wall attach technique,respectively.Seventy-two female rats were infused with trinitrobenzene sulfonic acid(TNBS)to induce EC models.After 24 hours,HC or MSC suspensions were injected into the rats in experimental 1(n=18)and 2(n=18)groups via caudal veins,respectively.Control animals were injected with isotonic saline.The whole colon was removed on day 7,14 and 21 after transplantation and examined histopathologically.BrdU labeled HCs were tested with immunohistochemical staining and MSCs were detected for sex-determining gene(sry)by PCR.Results EC models were successfully established.The HCs or MSCs grew rapidly in the culture suspension.On day 7,14 and 21 after transplantation,the BrdU immunoreactive cells were detected in the colon(6/6),and the positive expression of the sry gene was found in 1/6,2/6 and 3/6,respectively.No positive labeled cell was found in controls.There was no significant improvement in histopathological scores on the colon in two experimental groups compared with the controls.Conclusions Allogeneic HCs and MSCs may localize in the colon of EC models.The ability of localization is higher in HCs than MSCs.The transplantation of HCs and MSCs can not obviously improve histopathologically.
8.The effects of external treatment with honey for acute cellulitis on local symptoms and C-reactive protein

group was (1.20 ± 1.09),(1.09 ± 1.04), (0.86 ± 0.42), (1.89 ± 1.30) and that of the control group was (2.40 ± 1.87), (2.34 ± 1.71), (1.60 ± 1.33),(3.49 ± 1.72) respectively, which had statistical significance between the two groups (t=3.139-4.390, P<0.01). the Blood CRP values and CRP positive rate in the observation group, were (6.34±2.74) and 17.14%(6/35) respectively and which of the control group were (10.35±7.37) and 42.86%(15/35), the difference of the two groups was statistically significant (t=2.881, P<0.01 andχ2= 5.250, P < 0.05). Conclusions Topical treatment of acute cellulitis with honey can effectively improve the clinical symptoms, reduce the level of c-reactive protein, which is better than magnesium sulfate wet compress.
9.The expression of wnt1 in oral submucous fibrosis

Objective:To observe the expression of wnt1 in patients with oral submucous fibrosis(OSF) before and after treatment.Methods:40 patients with OSF were treated with triamcinolone acetonide combined with salvia miltiorrhiza,Before and after 4 weeks treatment,pain score of VAS and mouth opening(MO) were examined.wnt1 protein in saliva and gingival crevicular fluid(GCF) was examined by ELISA,wnt1 mRNA expression in buccal mucosa tissue was examined by real-time fluorescent quantitative PCR.20 healthy subjects were served as the controls.Results:The expression of wnt1 in OSF group[buccal tissue RT-PCR (36.89 ± 10.40) × 10-5,saliva ELISA (61.61 ± 4.45) ng/L,GCF ELISA (56.20 ± 3.65) ng/L] were significantly higher than that of control group [buccal tissue RT-PCR (4.63 ± 1.53) × 10-5,saliva ELISA (40.26 ± 3.00) ng/L,GCF ELISA (53.45 ± 1.74) ng/L)] (P < 0.01).In OSF group,after treatment VAS was decreased(P <0.01),MO increased(P <0.01)),Buccal mucosa wnt1 mRNA level was positively correlated with wnt1 protein in saliva and GCF,negativity with MO (P < 0.05),saliva wnt1 was positively correlated with VAS and GCF wnt1,negitively with MO(P < 0.05).Conclusion:Wnt1 might take part in the occurrence and development of OSF.The detection of wnt1 in saliva and GCF might be a noninvasive method for the evaluation of OSF treatment.
10.Impact of needle size and sonographic feature on accuracy of ultrasound-guided breast biopsy.

OBJECTIVETo assess the accuracy of ultrasound-guided 16G and 18G core needle biopsy for detecting ultrasound visible breast lesions with different sonographic features.
METHODSA total of 955 sonographically detected breast lesions examined with ultrasound-guided core needle biopsy (US-CNB) and subsequently surgically excised from July 2005 to July 2012 were retrospectively reviewed. Histological findings of US-CNB and the surgical specimens were analyzed for agreements, sensitivities, false negative rates, and underestimate rates according to different sonographic features.
RESULTSThe pathological results of the US-CNB showed malignant lesions in 84.1%, high-risk lesions in 8.4%, and benign lesions in 7.5% of the samples. The overall agreement rates were 92.4% for 16G CNB and 92.8% for 18G CNB; their complete sensitivities and false negative rates were both 98.6% and 1.4%, respectively; the high-risk underestimate rates and DCIS underestimate rates were 48.0% and 46.2% for 16G CNB vs 53.3% and 41.2% for 18G CNB, showing no significant difference between the two groups (P>0.01). For both 16G and 18G CNB, the agreements were better for mass lesions than for non-mass lesions (P<0.01). For the mass lesions with a diameter no greater than 10 mm, the agreement rates were lower than the overall data (P<0.01). Calcification in the lesions did not affect the agreement rates (P>0.01).
CONCLUSIONUltrasound-guided 16G and 18G CNB are both accurate methods for evaluating ultrasound visible breast mass lesions with a diameter larger than 10 mm.
