4.Prevention and treatment of chemotherapy-induced intestinal barrier dysfunction

Chemotherapy has been shown to be effective in the treatment of patients malignancies, however, anticancer drugs, particularly high-dose, also damage other chemosensitive tissues such as the intestinal mucosa, and these adverse effects can impair intestinal mucosal barrier. The derangement in the barrier function plays a central role in the pathophysiology of systemic infection, shock, and sepsis syndrome. The chemotherapy-induced intestinal barrier dysfunction and its prevention and treatment had been reviewed.
5.Developing the PLA critical care medicine is critical for advancing the level of battle wound treatment in the new era

Critical care medicine is an emerging unique specialty developed from the later 20th century,since then,it has been enriched with theoretical and practical experiences and becomes the most active subject in the field of clinical medicine.Critical care medicine of the PLA has attained significant achievements in the treatment and research of severe trauma,sepsis,severe heat stroke,multiple organ failure and severe acute pancreatitis.Besides,it stands in the leading position in the organ function maintenance of critically ill patients,continuous hemofiltration and nutrition support in China.Furthermore,critical care medicine plays an important role in the rescue of critically ill patients,medical support and disaster relief.As the relationship between battle wound rescue system and critical care medicine has been increasingly close,transition in the form of war in the new period brings new tasks to battle wound treatment constantly.Combined with the characteristics of information-oriented war condition in the future,developing the PLA critical care medicine and advancing the level of battle wound treatment in the new period point out the direction for the future work of critical care medicine.

The disadvantages of total parenteral nutrition (TPN) in critically ill patients were increased infection rate and hepatic dysfunction.Enteral nutrition (EN) can overcome the shortcomings of TPN and cost less.However,EN may not provide enough energy and protein because of the limited gut function.The best mode of the nutritional support in the critical ill patients is PN+EN.The non protein calorie and protein needs can be decided by the measured energy expenditure and overfeeding should be avoided.To further improve the critically ill patient nutritional status,immunonutrition and ecoimmunonutrition should be considered.
8.Hyperlipemia in sepsis —A part of non-adaptive host immune response

Bacterial endotoxin elicits dramatic responses in the host including elevated plasma lipid levels due to the increased synthesis and secretion of triglyceride (TG) rich lipoproteins by the liver and the inhibition of lipoprotein lipase. This cytokine induced hyperlipoproteinemia, clinically termed the lipemia of sepsis, was customarily thought to represent the mobilization of lipid stores to provide the fuel for the host’s response to infection. However, since lipoproteins can also bind and neutralize LPS, TG rich lipoproteins (VLDL and chylomicrons) are also components of non adaptive host immune response to infection. The studies are reviewed about the capacity of lipoproteins to bind LPS, protect against LPS induced toxicity and modulate the host response to the bacterial toxin.
9.Clinical significance of enhanced recovery after surgery is not only to shorten the duration of hospital stay

Enhanced recovery after surgery (ERAS) has developed continuously in China for 10 years,including optimizing step by step the perioperative analgesia regimens and goal-oriented fluid therapy.The ERAS principle has been accepted and expanded extensively in the surgery field,meanwhile,ERAS for perioperative care have been proved valueble in shortening hospital stay,reducing complications,improving organ function,reducing surgical stress reaction,accelerating recovery and increasing survival rate.ERAS is in the ascendant,and promotes development of multidisciplinary training and education program,supples of high-quality research evidences and translational clinical applications seem to be the direction of ERAS.And therefore,the content that this paper talks about is of great significance to deepen the research and understanding of ERAS.

s:Primary chronic intestinal pseudo-obstruction (PCIPO)is a rare digestive syndrome characterized by derangement of gut propulsive motility which resembles mechanical obstruction, in the absence of any obstructive process. Treatment of intestinal pseudo-obstruction involves nutritional, pharmacological and surgical therapies, but it is often unsatisfactory and the long-term outcome is generally poor in the majority of cases. This article is aimed at reviewing the current knowledge on etiology, clinical features and management of patients affected by PCIPO.
