1.The feasibility studies of using acute normovolemic hemodilution in the elderly patients undergoing knee arthroplasty

Objective To evaluate the effectiveness and safety of acute normovolemic hemodilution(ANH) in the elderly patients undergoing knee arthroplasty.Methods Thirty ASA Ⅰ~Ⅱ patients of both sexes,aged between 65~75 years undergoing elective knee arthroplasty were enrolled in this study.The hemodynamic status,hemostasis,arterial blood gas and hematocrit were monitored at before anesthesia(T1),after anesthesia(T2),after hemodilution(T3),the start of operation(T4),the end of operation(T5),24 hours after operation(T6),48 hours after operation(T7).Results The vital signs were fairly stable during preoperative in both groups.In both groups compared with time point T1 CVP was significantly elevated at point T3 and point T4.The increase in CVP was significantly higher in group Ⅱ than that in group Ⅰ(P
2.Experimental study on coronary restenosis after bone marrow mononuclear cells transplantation

Objective To study the effect of bone marrow mononuclear cells transplantation on restenosis rate and its mechanism. Methods The left anterior descending coronary arteries of swines were obstructed by balloon to create myocardial infarction models. After 3 weeks coronary stents were implanted at the middle part of the left anterior descending coronary arteries followed by the injection of bone marrow mononuclear cells into the coronary arteries. The degree of restenosis were measure by quantitative coronary angiography (QCA) after four weeks. Vascular tissue at both ends of stents were tested by HE and Sirius staining to analyse the mechanism of restenosis. Results At end of the experiment there were 8 swines in the bone marrow mononuclear cells transplantation group and 9 in the control group. After injection of bone marrow mononuclear cells the restenosis rate was similar to the control group (50% vs 44%, P=0.762). The lumen late loss was also similar between the two groups (1.50?1.45 mm vs 1.31?1.07 mm,P=0.736). Conclusion Bone marrow mononuclear cells transplantation does not increase the restenosis rate after percutaneous coronary artery intervention.
3.The investigation of state-trait anxiety among nurses in children's hospital

Objective To explore the current state-trait anxiety among nurses in children's hospital and supply some scientific references for the decision-making to healthy guidance and occupation behavior for the practioners. Methods Investigations were carried out in 410 female nurses in Guangzhou children's hospital by usage of the state-trate anxiety inventory(STAI).The results were analyzed statistically. Results The STAI of nurses in the children's hospital was higher than that of the norreal control (P < 0.01 ).But no difference existed in the aspect of special anxiety (P > 0.05) between them.The special anxiety of female cadre and technical staff was higher than the nurses in the children's hospital (P < 0.01). Conclusion The high level of state-trait anxiety hinted us that we should pay attention to the anxiety status of nurses in the children's hospitals.

OBJECTIVE To observe a time course of keratinocyte growth factor(KGF) and surfactant protein A(SPA) changes in rats with Pseudomonas aeruginosa pneumonia and to find out the significances of them.METHODS Specific pathogen-free male Sprague-Dawley rats were used to study.Standard P.aeruginosa strain ATCC 27853 was instilled into airway by the tracheal route to induce model of pneumonia.Samples of lung tissue and BALF were harvested before infection,and 6 h,9 h and 72 h after infection.Six rats per each point were sacrificed for harvesting samples.Expression of KGF protein in lungs was detected by Western blotting.Western-dot-blot was used to detect SPA expressions in BALF.RESULTS After infected with P.aeruginosa,KGF protein in lungs was markedly increased,reached the peak at 72 h postinfection.KGF protein level at 72 h postinfection was markedly higher than that of before infection(P
6.Inhibitory effects of gossypol on the activation of human T-lymphocytes stimulated with polyclonal activators

AIM: Peripheral blood mononuclear cells (PBMC) were cultured in vitro to study the effect of gossypol, a polyphenolic antifertility agent, on the activation of normal human T cells. METHODS: Double fluorescent staining together with flow cytometry was adopted to analyze the influence of gossypol on expression of the early activation antigen CD69 on T-lymphocytes under stimulation of mitogen or phorbol ester. RESULTS: Analysis of T cell activation in vitro revealed that preincubation of PBMC with 100 μmol/L gossypol could completely inhibit the expression of early activation marker CD69 on CD3+ T cells in response to 10 mg/L PHA, and block T cell activation by 10-7 mol/L PDB as well. The suppression of CD69 expression was dose-dependent and IC50 of gossypol on PDB and PHA were (35.7±2.9) μmol/L and (32.8±1.5) μmol/L(眘), respectively. Besides, gossypol had similar inhibitory effect on CD69 expression of CD3- lymphocytes. However, it did not have any significant effect on T cell surface molecule CD3 down-regulation. CONCLUSION: Gossypol could inhibit T cell activation in vitro in response to polyclonal activators, both PHA and PDB, suggesting that its action site may be at PKC or its downstream and that gossypol possessed potential immuno-regulatory effect.
7.Induction of apoptosis in human leukemic HL-60 cells by trichosanthin

AIM: To study the effect of trichosanthin (TCS), a type I ribosome-inactivating protein, on the induction of apoptosis in human leukemic cell line HL-60 cells and the influence of cycloheximide (CHX) on TCS-induced apoptosis. METHODS: Flow cytometry together with fluorescent microscopy were adopted to investigate the apoptotic cell death in HL-60 cells treated with TCS. RESULTS: Flow cytometric analysis indicated that TCS was able to induce significant apoptosis in HL-60 cells. The rates of apoptotic cells in HL-60 cells treated with TCS (20 mg/L) for 48 h was 48.7%±2.3%(±s), which was significantly higher than that of control (6.3%±1.0%)(P<0.05). Under the same condition, the rate of apoptosis caused by CHX (5 mg/L) was 65.3%±3.9%. TCS-induced apoptosis was further confirmed by fluorescent microscopy observation and DNA gel electrophoresis, in which typical nuclear morphological changes such as chromatin condensation, nuclear fragmentation, were observed in many of the cells treated with TCS, and DNA extracted from these cells displayed typical ladder pattern. Furthermore, the effect of TCS was significantly enhanced with the pretreatment of CHX (0.2 mg/L) which did not induce any significant apoptosis when used at 0.2 mg/L seperately. TCS-induced apoptosis was time- and dose-dependent. CONCLUSION: TCS was able to induce apoptosis in HL-60 cells, which was enhanced by CHX. It was suggested that TCS-induced apoptosis was independent of new protein synthesis.

9.Surgical treatment and perioperative management of gastric cancer with liver cirrhosis

Objective To explore the operative procedure and perioperative management of gastric cancer with liver cirrhosis.Methods Clinical data of 28 cases of gastric cancer with liver cirrhosis who were surgically treated in our hospital during the last seven years were retrospectively analyzed.Preoperatively,a detailed examination,and improvement of their nutritional status,liver function,and prothrombin time were aggressively made,and the possibility of tumor removal was estimated.Seven patients underwent radical proximal subtotal gastrectomy,and 4 of these cases underwent splenectomy and pericardial devascularizaion simultaneously.Five cases underwent total gastrectomy,and 3 of them had splenectomy and pericardial devascularizaion simultaneously.Twelve patients underwent radical distal subtotal gastrectomy,and 5 of these cases had splenectomy and suture of pericardial varices simultaneously.Four patients had palliative distal subtotal gastrectomy.The support of liver function and intense monitoring and management of complications were carried out postoperatively.Results Varying degrees of ascites occurred in all the 28 patients postoperatively,and other complications such as early liver coma occurred in five patients,extensive wound bleeding in three patients,left subdiaphragmatic abscess in one patient and wound infection in two patients.The morbidity rate was 100%(28/28).However,all the complications were recovered by non-surgical treatment.There were no perioperative deaths in this series.Conclusions Gastric cancer patients with liver cirrhosis are at a significant risk of developing postoperative complications.The surgical procedure should the "individual" principle based on liver function,history of upper gastrointestinal bleeding and the location of gastric carcinoma.Good hepatic reserve and perioperative care,meticulous hemostasis during operation,and prevention and treatment of postoperative complications are the likely determinants of operative prognosis.
