1.A Method of Synthesizing Tinnitus Rehabilitation Sound Based on Pentatonic Scale and Chaos.

Tinnitus is a common clinical symptom and its occurrence rate is high. It seriously affects life quality of the patients. Scientific researches show that listening some similar and none-repetitive music can relieve tinnitus to some extent. The overall music accorded with self-similarity character by the direct mapping method based on chaos. However, there were often the same tones continuous repeating a few times and tone mutations. To solve the problem, this paper proposes a new method for tinnitus rehabilitation sound synthesis based on pentatonic scale, chaos and musical instrument digital interface (MIDI). Experimental results showed that the tinnitus rehabilitation sounds were not only self-similar and incompletely reduplicate, but also no sudden changes. Thus, it has a referential significance for tinnitus treatment.

2.Joint Optimization of Savitzky-Golay Smoothing Models and Partial Least Squares Factors for Near-infrared Spectroscopic Analysis of Serum Glucose

The optimal model for the near-infrared spectroscopic analysis of serum glucose was established by partial least squares(PLS) and Savitzky-Golay(SG) smoothing method. Based on the prediction effect of the optimal single wave number model, a new dividing method for calibration set and prediction set was given. The calibration and prediction models were established by PLS method adopting the combination bands of 10000-5300 cm~(-1) and 4920-4160 cm~(-1) with Savitizky-Golay(SG) smoothing. By extending the number of smoothing points to 5, 7, ..., 87(odd) and polynomial degree to 2, 3, 4, 5, 6, fourteen smooth coefficient tables including 582 smooth modes were calculated. All PLS models corresponding to all smooth modes and all PLS factors(1-40) were constructed. The optimal model was selected by the prediction effect. And the derivation order was 1, the polynomial degree was 3 or 4, the number of smoothing points was 53, the optimal factor was 7 and the optimal RMSEP reach 0.376 mmol/L. The dividing method for calibration set and prediction set, the extending of SG smoothing modes, large-scale optimization combining SG smoothing modes and PLS factors can be effectively applied for the model optimization of near-infrared spectroscopic analysis.
3.Comparison of dual-energy X-ray absorptiometry T-score concordance between the measurement sites of hip and lumbar spine in the elderly

Objective To compare dual-energy X-ray absorptiometry T-score concordance between hip and lumbar spine, and to estimate the value of T-score of the hip for diagnosing osteoporosis in the elderly. Methods From September 2000 to May 2002, 123 males aged 60-89 years (average 72.0?7.4) and 137 females aged 60-88 years (average 67.3?6.1) were selected in the study based on the exclusion of subjects with apparent abnormalities affecting bone mass or taking medications which might affect bone metabolism. Detailed questionnaire included lifestyle factors such as smoking, drinking, exercise and dietary habits, etc. Radiographs of the thoracic and lumbar spine were taken. Patients' BMD were measured by DEXA (DMS, Challenger) at lumbar spine (L 2-4) and left hip in posterior-anterior projection. The right hip was scanned if patients had a history of fractures or any bone disease in the left hip. All data were statistically analysed using SPSS 8.0 system. 4.Therapeutic effects of Tiaozhong Granule on esophagus mucous membrane in rats with mixed reflux esophagitis and the mechanisms

OBJECTIVE: To explore the mechanism of Tiaozhong Granule (TZG), a compound traditional Chinese herbal medicine, in treating rats with mixed reflux esophagitis. METHODS: Fifty-eight SD rats were randomly divided into untreated group (n=12), sham-operated group (n=10), TZG-treated group (n=12), Banxia Xiexin Decoction (BXXXD)-treated group (n=12) and cisapride-treated group (n=12). Mixed reflux esophagitis was induced by esophago-duodenum end-to-side anastomosis. Four weeks later, the rats were orally administered twice daily for 12 days. Pathological changes of esophagus mucous membrane were observed by using HE staining. The expressions of proliferating cell nuclear antigen (PCNA) and p53 in the esophagus tissue were detected by immunohistochemical SABC method. Spectrophotometric method was used to detect the content of malondialdehyde (MDA) and the activities of superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) in serum. RESULTS: Compared with the untreated group, pathological changes of esophagus mucous membrane were relieved in different degrees in TZG-treated group, BXXXD-treated group and cisapride-treated group. Content of MDA and expressions of PCNA and p53 were obviously decreased in the three treated groups (P<0.01), and the activities of SOD and GSH-Px were significantly increased in the three treated groups (P<0.05, P<0.01). TZG had better effects than cisapride in decreasing the content of MDA and increasing the activities of SOD and GSH-Px (P<0.05). TZG was better in aspect of reducing the expressions of PCNA and p53 than BXXXD and cisapride tablets (P<0.05). CONCLUSION: Tiaozhong Granule can treat mixed reflux esophagitis in rats, and its action mechanisms may be associated with decreasing the expressions of PCNA and p53 in esophagus mucous membrane, reducing the content of MDA and increasing the activities of SOD and GSH-Px in serum.

Objective The incidence rate of pressure ulcer is high in critical patients and off-loading mattresses and reposi-tioning are known as effective interventions for the prevention of pressure ulcers .However, evidence is lacking for selection of the right type of mattresses and suitable interval of repositioning .This study was to compare the effects of two types of off-loading mattresses with two different repositioning intervals in preventing pressure ulcers in critical patients . Methods According to the design of this ran-domized controlled trial , we made a training plan concerning the participants , methods of intervention and comparison , criteria and methods of observation , and methods of recording , and trained 26 nurses from 7 hospitals .Using non-inferiority design and the method of stratified blocked randomization , we divided 1194 patients with the risk of pressure ulcer into a trial group ( n=596) and a control group ( n=598) , a viscoelastic sponge mattress with every-four-hours repositioning used for the former and an automatic aeration mat-tress with every-two-hours repositioning for the latter , both for 7 successive days .We examined the patients every day , recorded the in-cidence and stages of pressure ulcer , and compared the data obtained between the two groups of patients . Results The total inci-dence rate of pressure ulcer was 1.09%(13/1194), significantly lower in the trial than in the control group (0.34%[2/596] vs 1.84%[11/598], P=0.012). Conclusion A viscoelastic sponge mattress with every-four-hours repositioning is superior to an automatic aeration mattress with every-two-hours repositioning and therefore is preferred to the latter in preventing the incidence of pressure ulcer in critical patients in the ICU .
6.Effect of the sonic hedgehog inhibitor GDC-0449 on an in vitro isogenic cellular model simulating odontogenic keratocysts

Odontogenic keratocysts (OKCs) are common cystic lesions of odontogenic epithelial origin that can occur sporadically or in association with naevoid basal cell carcinoma syndrome (NBCCS).OKCs are locally aggressive,cause marked destruction of the jaw bones and have a propensity to recur.PTCH1 mutations (at ~80%) are frequently detected in the epithelia of both NBCCS-related and sporadic OKCs,suggesting that PTCH1 inactivation might constitutively activate sonic hedgehog (SHH) signalling and play a major role in disease pathogenesis.Thus,small molecule inhibitors of SHH signalling might represent a new treatment strategy for OKCs.However,studies on the molecular mechanisms associated with OKCs have been hampered by limited epithelial cell yields during OKC explant culture.Here,we constructed an isogenic PTCH1R135X/+ cellular model of PTCH1 inactivation by introducing a heterozygous mutation,namely,c.403C>T (p.R135X),which has been identified in OKC patients,into a human embryonic stem cell line using the clustered regularly interspaced short palindromic repeats (CRISPR)/CRISPR-associated 9 (Casg) system.This was followed by the induction of epithelial differentiation.Using this in vitro isogenic cellular model,we verified that the PTCH1R135X/+ heterozygous mutation causes ligand-independent activation of SHH signalling due to PTCH1 haploinsufficiency.This activation was found to be downregulated in a dose-dependent manner by the SHH pathway inhibitor GDC-0449.In addition,through inhibition of activated SHH signalling,the enhanced proliferation observed in these induced cells was suppressed,suggesting that GDC-0449 might represent an effective inhibitor of the SHH pathway for use during OKC treatment.
7.The lived experience of patients with mechanical ventilation in intensive care unit: a Meta-synthesis

Objective:To systematically review the qualitative researches on patients′ lived experiences of being mechanically ventilated in intensive care unit.Methods:The Cochrane Library, PubMed, Web of Science, Ovid, CNKI, VIP and Wanfang database were searched to collect qualitative studies on patients′ lived experiences of being mechanically ventilated in intensive care unit, from October 2009 to October 2019. Two reviewers independently screened the literature against the pre-determined inclusion and exclusion criteria, extracting the data, and evaluated the included studies according to JBI Critical Appraisal Tool for qualitative studies in Australia.Results:A total of fourteen studies were included. Thirty-eight complete results were grouped according to their similarities to form seven categories. These categories led to three synthesized findings: results 1: patients suffered from both physical and mental distress; results 2: they were eager for supports; results 3: patients achieved personal growth through self-adjustment and reflection.Conclusion:Discomfort experience during mechanical ventilation reduces patients′ comfort, and to a certain extent, has negative impacts on their physical or mental health and clinical outcome. As the main caregiver of patients with mechanical ventilation, not only should nurses alleviate patients′ physical distress by strengthening communication but also give them adequate psychological support. Eventually, promote the physical and mental recovery of patients.
8.A new method of synthesizing personalized tinnitus rehabilitation sound based on iterative function system algorithm.

Tinnitus is a common clinical symptom. Researches have shown that fractal sound can effectively treat tinnitus. But current fractal sound is usually synthesized based on constant notes via fractal algorithm, which lead to monotony of synthesized fractal sound. So it is difficult to achieve personalized match. Clinical datas have confirmed that it is common to match tinnitus sound with nature sound and it has a good effect on regulating negative emotion and relieving tinnitus via some natural sound. Therefore, a new method of personalized synthesizing tinnitus rehabilitation sound based on iterative function system (IFS) fractal algorithm is proposed in this paper. This method firstly generates personalized audio library based on natural sound, then tinnitus rehabilitation sound is synthesized via IFS fractal algorithm. Simulation results show that rehabilitation sound in this paper can meet the basic requirements of tinnitus therapy sound and can match tinnitus sound by controlling personalized audio library. So it has reference significance to the treatment of tinnitus sound therapy.
9.Clinical manifestation and cytogenetic analysis of 607 patients with Turner syndrome.

OBJECTIVETo explore the correlation between cytogenetic findings and clinical manifestations of Turner syndrome.
METHODS607 cases of cytogenetically diagnosed Turner syndrome, including those with a major manifestation of Turner syndrome, were analyzed with conventional G-banding. Correlation between the karyotypes and clinical features were analyzed.
RESULTSAmong the 607 cases, there were 154 cases with monosomy X (25.37%). Mosaicism monosomy X was found in 240 patients (39.54%), which included 194 (80.83%) with a low proportion of 45,X (3 ≤ the number of 45, X ≤5, while the normal cells ≥ 30). Structural X chromosome abnormalities were found in 173 patients (28.50%). A supernumerary marker chromosome was found in 40 cases (6.59%). Most patients with typical manifestations of Turner syndrome were under 11 years of age and whose karyotypes were mainly 45,X. The karyotype of patients between 11 and 18 years old was mainly 45,X, 46,X,i(X)(q10) and mos45,X/46,X,i(X)(q10), which all had primary amenorrhea in addition to the typical clinical manifestations. The karyotype of patients over 18 years of age were mainly mosaicism with a low proportion of 45,X, whom all had primary infertility. 53 patients had a history of pregnancy, which included 48 with non-structural abnormalities of X chromosome and 5 with abnormal structure of X chromosome.
CONCLUSIONGenerally, the higher proportion of cells with an abnormal karyotype, the more severe were the clinical symptoms and the earlier clinical recognition. Karyotyping analysis can provide guidance for the early diagnosis of Turner syndrome, especially those with a low proportion of 45,X.

Objective To describe the distribution of medication costs of rheumatoid arthritis patients, and to analyze the factors that may affect the costs. Methods Data were obtained from a 12-month retrospective investigation of patients with rheumatoid arthritis (RA) across China. Department of Rheuma-tology of 18 hospitals were randomly selected. The data about their social conditions, clinical conditions, medications associated with RA such as disease-modifying antirheumatic drugs (DMARDs), non -steroidal anti -inflammtory drugs (NSAIDs), steroids, biologic agents were collected, and the costs of drugs were calculated. A non-parameter test and multivariate logistic regression analysis were performed. Results Six hundred and forty six patients were enrolled into the study, 435 completed data were chosen for analysis. The results demonstrated that the average costs per patient for medications in the past year was 8018 . The total medication costs were further subdivided into the following parts: DMARDs, (represented 20% of the total costs), biologic drugs (49%), NSAIDs (4%), herbal drugs (22%), steroids (1%). Data analysis showed that patients with higher education and higher incomes, with medical insurance,better health function status and outpatients paid more on DMARDs. Extra-articular manifestations increased the odds of the high-cost group (OR: 2.180, 95%CI: 1.335~3.558, P=0.002), while poor health function status increased the probability of paying high costs (OR: 1.373, 95%CI: 1.012~1.863, P=0.041). Conclusion High medication costs in RA do exist in RA patients. The costs of medication is associated with health function status and the presence of extra-articular manifestations.
