OBJECTIVE:To explore the use of antibiotics in the department of respiratory diseases and pediatric outpatient department after the introduction of initiative intervention stragety on the rational use.METHODS:A field investigation was carried out on the international RDU index of antibiotics in the department of respiratory diseases and pediatric outpatient department by self-control test.Initiative intervention on physicians' prescribing practices was carried out at four stages:befter intervention, following first intervention, second intervention, and third intervention, respectively.Data statistics was analyzed by SPSS11.0 software.RESULTS:The percentages of patients treated with antibiotics and injcetable antibiotics in the department of respiratory diseases dropped from 61.6% and 19.2% before intervention to 30.0% and 14.0% after third intervention(P

Objective To investigate the safety of infusion and effective care of cryopreserved umbilical cord blood. Methods Side effects happened during transfusion of cord blood in 78 patients of 111times of transfusion were observed and nursed. Results During intravenous transfusion way:1 case with acute hemolytic reaction; 1 case with sudden loss of consciousness and convulsions; 4 cases with severe headache, difficulty with breathing, rapid increase of blood; 46 patients with high blood pressure, heart rate or blood pressure, heart rate, 1 patient had symptoms of frequent premature beats; most of the cases have hemoglobinuria after the infusion. Transfusion of bone marrow injection of ways: 11 patients have radioactive soreness in leg, hemoglobinuria hemoglobinuria mild symptoms or no symptoms during the process of infusion. Conclusions Systemic reactions were the main side effects during intravenous transfusion of cord blood, conditions change fast and complex, Nursing should focus on preventive care and preparation, close observation and active and effective symptomatic treatment; reaction of bone marrow injection of umbilical cord blood was local pain, the focus of nursing was aseptic operation and injection time.

Objective To evaluate the effect of exercise on the relationship between body mass index (BMI) and health-related quality of life (HRQOL).Methods A total of 1 114 participants were recruited from the Physical Examination Center of two hospitals.SF-36 was used to measure HRQOL,and the classification of sports level was done according to the subjective evaluation of participants. Results The total score of HRQOL,physical component summary and mental component summary were 76.26± 16.10,75.97 ± 16.67 and 76.55± 18.33 respectively. The results of correlation analysis were as following:the positive correlation between BMI and general health (r=0.109),vitality (r=0.116),social functioning (r=0.092),role emotional (r=0.122),mental health (r =0.145) were significant (P< 0.01).The negative correlation between BMI and PF(r=-0.07,P=0.019)was significant. The mediation effects of exercise in was relationship between BMI and general health, vitality, mental health were significant, and the mediating effect were 5.84%,9.14% and 6.58%.Conclusion This study demonstrates that mediation effects of the exercise are significant in some dimensions of HRQOL,but the direct influence of BMI on the HRQOL plays a leading role. And BMI is a protective factor under certain conditions.

Objective To explore the intervention effect of TCM constitution health education on constitution and relevant health indicators. Methods The research design was adopted with nocontrol group before and after the test evaluation.A total of 595 subjects were recruited.Interventions of emotion, diet, sleeping and living, exercise, and meridian and acupoint massage were concluded in the TCM constitution health education interventions.Investigated their constitution, health related quality of life(MOSSF-36)and laboratory indexes before and after the intervention of 3 months and 6 months. Results 509 subjects were available. After 3 months intervention, the score of gentleness type was sig-nificantly increased by 3.67 points (P<0.05)while scores of pathological constitutions except yang-de-ficiency type were significantly decreased (P<0.05).All of the absolute values of standardized response mean (SRM)ranged from 0.10 to 0.5 1 .After 6 months intervention, the score of gentleness type was significantly increased by 3.67 points (P<0.05)while scores of all pathological constitutions were sig-nificantly decreased (P<0.05).All of the absolute values of SRMranged from 0.23 to 0.75.Physical component summary (PCS )scores and mental component summary (MCS )scores of MOS SF-36 were significantly increased (P<0.05 )after 3 months and 6 months intervention. After 3 months interven-tion,systolic pressure (SP),diastolic pressure (DP),2-hour postprandial glucose (2hPG),and total cholesterol (TC)decreased significantly (P<0.05),but high density lipoprotein-cholesterol (HDL-C) was significantly increased (P<0.05 ).After 6 months intervention,SP,DP,2hPG,TC,triglyceride (TG)and low density lipoprotein-cholesterol (LDL-C)were significantly decreased (P<0.05 ),but HDL-Cwassignificantlyincreased(P<0.05).Conclusion TCMconstitutionHealtheducationinter-vention can improve the constitutional conditions, clinical abnormal indexes, and the quality of life resul-ting in the improvement of health. However, the intervention effects were generally low, which indicates that the constitution is relatively stable. Therefore,a long-term of TCM constitution intervention is neces-sary for health promotion.

Journal of Traditional Chinese Medicine 2024;65(23):2427-2433
ObjectiveTo explore the longitudinal measurement invariance of standardized Constitution in Chinese Medicine Questionnaire (CCMQ) among Chinese adult populations, and to provide evidence for the longitudinal effect analysis of this questionnaire. MethodsA total of 509 adults who voluntarily received 26 weeks of comprehensive traditional Chinese medicine (TCM) constitution fitness intervention were included (3 time points: before intervention, 13 weeks intervention, and 26 weeks intervention), evaluated by CCMQ with 9 subscales. A single-group confirmatory factor analysis was performed to establish a single-group baseline model with a well-fitting model, and then nested models, that is multiple groups of confirmatory factors analysis, are used to analyze the longitudinal measurement invariance, followed by configural invariance, metric invariance, scalar invariance, and strict invariance. Comparative Fit Index (CFI), Root Mean Square Error of Approximation (RMSEA), and Standardized Root Mean Square Residual (SRMR) were used as model fit indicators for the analysis. ResultsThe single-group confirmatory factors analysis proved that the 9 subscales of CCMQ fit well at the three time points (CFI: 0.933~0.992, RMSEA: 0.027~0.080, SRMR: 0.022~0.045). The multiple-group confirmatory factors analysis showed that the configural, metric, scalar, and strict invariance across time of qi-stagnation and special diathesis constitution were established (ΔCFI<0.01, ΔRMSEA<0.01); the configural, metric, scalar, and partial strict invariance across time of phlegm-dampness constitution subscale were established (ΔCFI<0.01, ΔRMSEA<0.01); the configural, metric, partial scalar and partial strict invariance of 6 subscales, including gentleness, qi-deficiency, yang-deficiency, yin-deficiency, dampness-heat, and blood-stasis constitutions, were all established (ΔCFI<0.01, ΔRMSEA<0.01). ConclusionThe CCMQ met at least metric invariance at 3 time points, which can be applied to the study of whether different time points have the same unit or meaning. The measurement invariance of the 3 subscales of phlegm-dampness, qi-stagnation and special diathesis, special diathesis have longitudinal measurement invariance, so the mean comparison over time could be made; while the 6 subscales of gentleness, qi-deficiency, yang-deficiency, yin-deficiency, dampness-heat, and blood-stasis constitutions meet partial scalar and partial strict invariance, which could be explained partially for the difference in the comparison of the mean over time.
