Objective:To study the relationship among serum uric acid (SUA)level,brachial-ankle pulse wave veloci-ty (baPWV)and severity of coronary disease.Methods:A total of 121 inpatients,who hospitalized in our depart-ment of cardiology from Jan 2011 to Jul 2012,were enrolled.According to results of coronary angiography,they were divided into coronary heart disease (CHD)group (n=59)and normal control group (n=62),CHD group was further divided into single vessel coronary disease group (n=30,single vessel group),double-vessel coronary disease group (n=14,double-vessel group)and multi-vessel coronary disease group (n=15,multi-vessel group).SUA level and baPWV were compared among all groups,and their relationship with degree of coronary disease was analyzed. Results:SUA concentration gradually rose along with number of diseased coronary vessels increased,it's (349.26± 96.58)μmol/L,(400.37±70.96)μmol/L,(517.57±85.26)μmol/L and (602.60±77.03)μmol/L in normal con-trol,single vessel,double-vessel and multi-vessel group respectively,P<0.05 or <0.01;baPWV gradually rose a-long with number of diseased coronary vessels increased,compared with normal control group,there was significant increase in baPWV [(1499.04±193.82)cm/s vs.(1885.32±319.73)cm/s,(2036.00±406.40)cm/s,(2171.03± 348.53)cm/s]in single vessel,double-vessel and multi-vessel group,that of double-vessel group was significantly higher than that of single vessel group,P<0.05~<0.01.Logistic regression analysis indicated that SUA and baP-WV were independent risk factors for CHD morbidity (OR=13.011,14.008,P=0.000).Conclusion:Serum uric acid level and pulse wave velocity possess important clinical application value for predicting CHD and its severity.

OBJECTIVE:To observe short-term efficacy and safety of early use of tirofiban combined with different lipid-regu-lating drugs percutaneous coronary intervention(PCI)in elderly patients with coronary heart disease. METHODS:562 elderly pa-tients with coronary heart disease underwent selective PCI were selected and randomly divided into atorvastatin group and probucol group,with 281 cases in each group. Both group received routine treatment;atorvastatin group was additionally given tirofiban combined with Atorvastatin tablet,and probucol group was additionally given irofiban combined with Probucol tablet. Clinical indi-cators and main adverse cardiac event(MACE)were compared between 2 groups before and after treatment. RESULTS:After treat-ment,3 level of thrombolysis in myocardial infarction(TIMI)in atorvastatin group accounted for 69.4%,which was significantly lower than 95.7% of probucol group,with statistical significance(P<0.05). After 12h treatment,CK,2 groups had lower CK-MB and LVEF and higher cTnI;LVEF decrease of atorvastatin group was more significant,with statistical significance(P<0.05);af-ter 8 weeks of treatment,NO,NO/ET-1,GSH,GSH/GSSG of probucol group were increased significantly,and ET-1,GSSG and Eh decreased significantly;above indexes of atorvastatin group had no significant change,there was statistical significance between 2 groups(P<0.05). The incidence of MACE in atorvastatin group and probucol group were 21.7% and 21.4%,there was no sig-nificant difference (P>0.05). CONCLUSIONS:Tirofiban combined with different lipid-regulating drugs can effectively improve the prognosis of elderly patients with coronary heart disease after PCI operation with good safety,but tirofiban combined with pro-bucol has more obvious therapeutic effect.

Objective To observe the effect of bronchial artery infusion chemotherapy in the treatment of central non-small cell lung cancer (central NSCLC)with obstructive pneumonia and its efficacy clinical factors.Methods Retrospective analysis method was applied to the 64 cases of central NSCLC with obstructive pneumonia.All patients were confirmed by pathology.We main contrasted the efficacy between intravenous infusion of antimicrobial agents and bronchial artery infusion chemotherapy (BAI).we focused on the efficacy between treatment group using standard systemic vein chemotherapy/radiotherapy and primary group with no chemotherapy/radiotherapy.Results In control group,the improvement rate was 43.33%.The improvement rate in treatment group was 70.59%.In treatment group,the curing rate was 50.00% for the patients who had ever taken standard sys-temic chemotherapy/radiotherapy.But the curing rate was 88.89% for the primary group.Conclusion For the patients who have the central NSCLC with obstructive pneumonia,intravenous infusion of antimicrobial agents and bronchial artery infusion chemo-therapy (BAI)can obviously increase the curing rate of obstructive pneumonia.

Objective To explore the efficacy of nasal septal perferation with three layers of grafts under nasal endoscope. Methods From June 2002 to June 2007,15 cases of nasal septal perforation were repaired by three laryers of grafts using the perpendicular plate of the ethmoid bone or quadrangular cartilage covered by fascia lata of the thigh. The course of the disease ranged from 6 months to 5 years (mean,1.8 years) and the diameter of the perforation ranged from 0.8 to 1.6 cm (mean,1.2 cm). A 1-2 mm new surface was made along the margin of the perforation,and then the nasal septum was separated by means of submucos correction. Afterwards,a perpendicular plate of the ethmoid bone or quadrangular cartilage covered by fascia lata of the thigh was used to form a three-layer graft for seal of the perforation. Then,the nasal cavities were filled with expansive sponge,which was expanded by injecting lyophilized recombinant bovine basic fibroblast growth factor for external use. Results The surface of the graft was completely covered by nasal mucosa in 6 to 12 weeks after the surgery.Afterwords,the patients were followed up for 6 to 12 months (mean,8.2 months),during which no recurrence occurred. Conclusions It is effective to repair nasal septal perforation with three-layer graft under nasal endoscope.

BACKGROUND: Puerarin functions to relieve the injury caused by ischemia reperfusion, improve the microcirculation, and inhibit the agglutination of platelet. But it is not clear yet that whether its protection on neurons relates to the apoptosis of cells.OBJECTIVE: To observe the protection of puerarin on neurons injured by mimic ischemia reperfusion in vitro.DESIGN: A random control study. SETTING: Laboratory of Biochemistry and Moleculobiology Department of Preclinical Medicine College of Tongji Medical College of Huazhong University of Science and Technology.PARTICIPANTS: The experiment was done on March 11, 2002. The cultured N2a cells of rat neuromablast were observed.METHODS: Ninety minutes after the cultured N2a cells were put into the 37 ℃ incubator with 0.05 CO2 and 0.95 N2(v/v), puerarin 0.5 mmol/L was added for cuiture for 24 hours. The methyl thiazolyl tetrazolium (MTT)method was used to observe the surviving ability of cells. The Annexin-V staining was adopted to exam the severity of apoptosis in the early stage and the supernate was collected to analyze the activity of lactate dehydrogenase (LDH) to reflect the permeability of cytomembrane. The immunoblotting method was applied to analyze the expression of caspase-3, at the same time, its activity was measured.MAIN OUTCOME MEASURES: The severity of cell apoptosis, the activity of LDH, the expression and activity of caspase-3.RESULTS: Puerarin could improve significantly the surviving rate of N2a cells 24 hours after reperfusion, decrease remarkably the activity of LDH in the culture fluid, lessen obviously the N2a apoptosis (P ＜ 0.01), at the same time, reduce tremendously the activity and expression of caspase-3 induced by ischemia reperfusion (P ＜ 0.01).CONCLUSION: Puerarin can protect nerves, inhibit obviously the N2a apoptosis induced by ischemia reperfusion. The mechanism is that it inhibits greatly the expression and activity of caspase-3.

Sphincter of Oddi manometry (SOM) is considered the gold standard for the diagnosis of sphincter of Oddi dysfunction (SOD),especially the type Ⅰ SOD.The dysfunction of sphincter of Oddi can lead to other comorbidity.Therefore,SOM is of special importance for such patients.When performing SOM,we need to intubate into the papilla,so it requires high technique and is associated with a relatively higher risk of post-ERCP pancreatitis.Recently,the equipment of SOM is being updated constantly.The use of modified catheter and high resolution gastrointestinal dynamic system for recording and analyzing has made the technology more accurate and safe than ever before.Due to the widespread use of ERCP,SOM is expected to be one of the routine examinations in clinical practice and its application has a promising prospect.This article reviews the latest research progress on SOM in recent years.

The carboxyl-terminal amino acids 272-299-truncated apoE4 (delta272-299) is the main fragments of apoE4 hydrolysate in neurons. The effects of truncated-ApoE4 (delta272-299) overexpression on tau phosphorylation in cultured N2a cells were investigated. The truncated-apoE4 (delta272-299) cDNA was subcloned into pEGFP-c3 to form recombinant pEGFP-T-apoE4. pEGFP-c3, pEGFP-T-apoE4 and pEGFP-apoE4 were transfected into N2a cells respectively by lipofectamine 2000 method. After 24--48 h, tau phosphorylation was detected by Western blot assay and glycogen synthase kinase-3 (GSK-3) activity by using GSK-3 activity assay. The results showed that the overexpression of both full length-apoE4 and truncated apoE4 fragments in N2a cells induced a dramatic increase in phosphorylation of tau at Ser202 sites and the activation of GSK-3 as compared with untransfected cells, most significantly in the cells transfected with pEGFP-T-apoE4 (P < 0.05). It was concluded that in vitro overexpression of truncated-ApoE4 (delta272-299) can result in tau hyperphosphorylation in N2a cells by activating GSK-3, suggesting truncated-ApoE4 (delta272-299) might contribute the pathogenesis of Alzheimer disease.

Objective To analyze the surgical operation and prognosis of gastric stump carcinoma in elderly patients. Methods The clinical data of 25 cases of elderly gastric stump carcinoma in our department from 1993 to 2003 were analyzed retrospectively. Results In this group of cases, TNM stages Ⅰ, Ⅱ, Ⅲ and Ⅳ was 3, 5, 8 and 7 cases, respectively. Among them, radical resection was performed in 12 cases(48.0%), palliative operation in 9 cases(36.0%), exploratory laparotomy in 2 cases, and 2 cases lost the chance of resection due to extensive tumor dissemination. Nutritional jejunostomy was performed in all the cases received resection operation. 1, 3 and 5 years survival rates in the patients who underwent radical dissection were 81.2%, 41.5% and 28.6%, respectively. Conclusion Endoscopy examination plus gastric mucosa biopsy were major means for diagnosis of gastric stump carcinoma in elderly patients. Perioperative managements should be emphasized. Radical resection was a reasonable therapeutic procedure of elderly gastric stump cancer, and was also an important factor affecting prognosis.
