1.Studies on relative factors of vascular parkinsonism

Objective To investigate the relative factors of vascular parkinsonism(VP),in order to provide a clinical basis for early diagnosis of VP. Methods The method of prospective cohort study was used. The first cerebral infarction was taken as the initiating event. All the cases were followed up for 4 years. When they suffered from VP or died of VP, the follow-up was termina-ted. Logistic regression model was used to analyze the risk factors for VP. Results Fifty-four pa-tients developed VP in 404 patients with first cerebral infarction,the incidence rate was 13.4%.According to the typing standard of OCSP,in the 404 patients, the incidence of VP was highest in lacunar infarction group(42 cases, 20.4%). The main risk factors for VP included age (P=0.043, OR = 1. 135,95% CI: 1. 010-1. 275),hypertension history (P=0.032, OR=2.019,95% CI: 1.247-4.746), blood viscosity (P=0.041, OR=1.724,95% CI: 1.036-3.058) and fibrinogen (P=0.001, OR=2.241,95% CI: 1.272-5.473). Conclusion During follow-up for 4 years,the incidence rate of VP in patients with first cerebral infarction is 13.4%. Lacunar in-farction is apt to cause occurrence of VP. Age, hypertension history, blood viscosity and fibrinogen are risk factors for VP. VP may be caused by multiple factors and multiple mechanisms.

5.Comparison of Colony Number in Ordinary Tour and Circuit Tour

OBJECTIVE To study the changes in air bacteria content under gradual circuit model and ordinary circuit model in operating room.METHODS Altogether 80 cases with thyroid gland surgery and gastrointestinal surgery were selected,and divided them randomly into two groups: the gradual circuit model and the ordinary circuit model,each group had 40 cases.The period of early morning to the end of operation was divided into 10 time spots to carry on the air bacilliculture.RESULTS Under the ordinary circuit model,air colony showed straight up with time during the two types of surgical operations,and there was no significant difference between the two types of surgery.Under the gradual circuit model,air colony began to increase with time,followed by steady,or even decline,no differences between the two types of surgery. 6.The comparative study of 4D-CTA with DSA in AVM

Objective To compare the clinical diagnostic value of 320 row four-dimensional CT angiography (4D-CTA)and digital subtraction angiography (DSA)in untreated arteriovenous malformation (AVM)patients.Methods Thirty-six patients with AVM diagnosed by DSA were included in this study.Two independent readers blind to the results of all examinations evaluated the find-ings of DSA and 4D-CTA of each patient.All results were then documented using a standardized scoring sheet.Results The diagno-sis results of 4D-CTA for AVM lesions in all 36 patients were accurately consistent with those of DSA,including the position,size and vascular structure.There were 14 cases with the largest diameter <3 cm,20 cases 3-6 cm,2 cases >6 cm.Lesions involved only the anterior circulation in 13 cases,only the posterior circulation in 13 cases,and both the anterior and posterior circulation in 10 cases.Conclusion 4D CTA has excellent diagnostic accuracy in the detection of AVM lesions,including size,location,feeding ar-teries and draining veins,with similar value as DSA in the clinical diagnose and evaluation of AVM.

9.The predictive value of Wells score combined with D-dimer in the diagnosis of acute pulmonary embolism

Objective To evaluate the predictive value of Wells score combined with D-dimer in the diagnosis of acute pulmonary embolism.Methods A total of 540 patients with suspected pulmonary embolism admitted from 2008 to 2011 were enrolled for study.The diagnosis of pulmonary embolism (PE) was confirmed by using computed tomography pulmonary angiography (CTPA).These patients were divided into two groups:PE group and non-PE group.Comparative analysis was carried out in demographics,underlying diseases,chief complaints,physical signs,venous thrombosis risk factors,laboratory findings and Wells scores between the two groups.Results Of 502 patients selected into this study,there were 246 in PE group and 256 in Non-PE group.The incidence rates of history of recent surgery or bed-ridden,recent fracture of pelvis or lower limb,symptoms of hemoptysis,transient disturbance of consciousness,signs of unilateral lower limb swelling,hypoxia and hypocapnia of arterial blood gas analysis,elevated levels of D dimer,high Wells score in PE group were significantly higher than those in non-PE group (P < 0.05).And there were no statistical difference in other variables found between the two groups.The areas under the ROC curve of Wells score,D-dimer and the combination of the two were 0.775 (95% CI:0.719-0.831),0.802 (95 % CI:0.751-0.853) and 0.899 (95 % CI:0.834-0.964),respectively.And the area under the ROC curve of the combination of the two was greater than that of separated application (P < 0.05).When the cut-off value of Wells score was 5 and D-dimer was 1 724 μg/L,the weighted maximum sensitivity and specificity were reached,and these cut-off values were higher than above determined values,the reliability of the diagnosis of PE was obviously increased,and those were lower than these cut-off values,the reliability of excluding PE was also increased.Conclusions Wells score combined with D-dimer showed a higher value in predicting acute pulmonary embolismthan than their separated application.
10.Expression of intrarenal angiotensin Ⅱ is correlated with kidney fibrosis in primary IgA nephropathy patients

Objective To analyze the expression and regulation of components of intrarenal renin-angiotensin system (RAS) and the correlation between intrarenal angiotensin Ⅱ (Ang Ⅱ) expression and clinicopathological injury index in primary IgA nephropathy patients. Methods Expressions of intrarenal RAS components were assessed by immunohistochemistry staining (IHCS). Correlation among intrarenal RAS components and of intrarenal Ang Ⅱ expression with blood pressure, estimated glomerular filtration rate (eGFR), 24-h urinary protein and Katafuchi score in 36 primary IgA nephropathy patients were examined. Results There were positive correlations between positive IHCS area of intrarenal renin and Ang Ⅱ (r=0.43, P<0.01), angiotensiongen and Ang Ⅱ (r=0.34, P<0.05). There was negative correlation between positive IHCS area of intrarenal Ang Ⅱ and eGFR (r=-0.61, P<0.01). There was positive correlation between positive IHCS area of intrarenal Ang Ⅱ and pathological chronicity index (ρ=0.39, P<0.05), index of interstitial cell infiltration (ρ =0.52, P <0.05). Conclusion Expression of intrarenal Ang Ⅱ is positively correlated with expression of intrarenal renin and angiotensinogen, and plays an important role in kidney fibrosis in primary IgA nephropathy.
