Objective To reveal the trend of change incidence of hepatitis A(HAV)in Shandong province and explore dynamic analysis methods.Method Dynamic analysis was made on data of hepatitis A in Shandong province from 1990 to 2006.Results The reported incidence of hepatitis A patients dropped from 53.97/100,000 to 1.14/100,000 in Shandong province from 1990 to 2006,which showed a marked downward trend;the groups under the age of 10 dropped from 55.60% to 5.12%,the average age of incidence moved from 14.40 years old to 40.19 years old.Conclusion The incidence of hepatitis A has been changed from prevalent to sporadic in Shandong province,and there was a further downward trend;the age of the high-risk population has been shifted to older age group;the sex ratio of incidence between males and females in the population above 10 years of age had an expansion trend,the age distribution curve differences between males and females was also expanded.

Objective To evaluate the clinical effectiveness and pseudoaccommodation in the early stage after AcrySof ReSTOR multifocal intraocular lens (IOL) and monofocal IOL implantation.Methods A total of 138 patients (158 eyes) with age-related cataract underwent phacoemulsification and IOL implantation were chosen,73eyes of 60 patients with a multifocal IOL implantation as observation group and 85 eyes of 78 patients with monofocal IOL implantation as control group.The uncorrected visual acuity,best corrected visual acuity was evaluated at 1 week,1 month,and 3 months after operation,pseudoaccommodation and quality of life was evaluated at 3 months after operation.Results The patients of both observation group and control group were able to obtain a satisfactory distance vision at 1 week,1 month,and 3 months.The uncorrected intermediate visual acuity of observation group were 0.49 ± 0.19,0.72 ± 0.21,0.77 ±0.23,which of control group were 0.24 ± 0.21,0.27 ± 0.22,0.28 ± 0.24,there were statistical differences between two groups (all P ＜ 0.05).The uncorrected near visual acuity of observation group were 0.47 ± 0.20,0.70 ± 0.22,0.80 ± 0.24,which of control group were 0.21 ±0.23,0.23 ±0.19,0.26 ±0.18,there were statistical differences between two groups (all P ＜ 0.05).There was no significant difference in the uncorrected near visual acuity,best corrected distance,intermediate and near visual acuity between observation group and control group (all P ＞ 0.05).At 3 months,the distance and near pseudoaccommodation of observation group were (2.56 ± 0.82) D,(3.19 ±1.13)D,which of control group were (0.87 ±0.57)D,(1.03 ±0.59) D,there were statistical differences between two groups (all P ＜ 0.05).At 3 months,VF-14 scale score and wearing glasses as near score of observation group were 92.21 ±4.22,4.23 ±0.85,which of control group were 71.23 ± 3.96,2.01 ± 0.71,there were statistical differences between two groups (all P ＜ 0.05).Conclusion AcrySof ReSTOR multifocal intraocular lens implantation can provide a higher level of visual acuity,pseudoaccommodation and satisfaction compared with monofocal intraocular lens.

Objective To explore the influence of Lipo PGE1 on endothelin (ET-1) and interleukin (IL-6) in exhaled breath condensate(EBC) of patients with chronic obstructive pulmonary disease (COPD) and pulmonary hypertension(PH) and its clinical significance.Methods A total of 40 cases of patients with COPD and PH were randomly divided into thecontrol group and the treatment group,20 cases in each group.The control group was administered with the conventional treatments such as anti-infection,bronchodilator,antiasthma,expectorant and oxygen therapy;the treatment group was administered with Lipo PGE1 (10 μg/d,iv,for 10 days) besides the conventional treatments.ET-1 and IL-6 in EBC,pulmonary artery systolic pressure(PASP),arterial blood PaO2 and PaCO2,lung function FEV1/FVC,FEV1%pred in both groups were assayedbefore and after the treatment.Results There were no statistical difference between the two groups in the ET-1 and IL-6 in EBC,PASP,arterial blood PaO2 and PaCO2,FEV1/FVC,FEV1% pred before the treatment.After the treatment,the ET-1,IL-6 in EBC,PASP and arterial blood PaCO2 of the treatment group were lower than thoseof the control group (P<0.05);the arterial blood PaO2,FEV1/FVC,FEV1% pred of the treatment group were higher than those of the control group (P<0.05);The levels of ET-1 and IL-6 in EBC wereboth positivelycorrelated with PASP in the two groups.Conclusion Lipo PGE1 can reduce the levels of ET-1 and IL-6 in EBC of patients with COPD and PH.ET-1 and IL-6 may become a curative effect judgment index of COPD and PH,which has a certain clinical significance.

Objective: To compare and observe the clinical efficacy of ALP and PFNA in treatment of intertrochanteric fractures, to provide clinical evidence for such patients. Methods:The clinical data of 122 cases patients with intertrochanteric fractures admitted to our department from January, 2012 to January, 2013 was analyzed retrospectively. 72 patients treated with PFNA were regarded as the study group and 50 patients with ALP as controls group. Two groups were compared with surgical incision, blood loss, operative time, healing time, hip functional recovery and postoperative complications. Results: The incision length, blood loss, operative time, postoperative weight-bearing activity time, fracture healing time, Harris score and complications of study group were significantly better than the control group (P<0.05), with statistical significance. Conclusion: The long-term efficacy of PFNA and ALP has no difference, but PFNA has smaller wound, less bleeding, sooner fracture healing time and weight-bearing activities and better early results.

ObjectiveTo explore the relationship between the changes of the blood platelet counts (BPC) in children with cerebral palsy (CP) and pather mechanism of CP in traditional Chinese medicine.MethodsBPCs of CP children and healthy children were analyzed with statistical method.ResultsThe BPC of CP children was higher than that of the healthy children ( P<0.05).ConclusionThe BPC of CP children is higher than that of the healthy children, and this is consistent with the theory of traditional Chinese medicine about pathogenic mechanism of CP that CP is related to blood stasis.

Objective To evaluate the differentiation diagnostic value of acoustic radiation force impulse imaging (ARFI) technique to the benign and malignant breast lesions .Methods A total of 87 breast masses in 84 patients were selected and per‐formed the routine ultrasound ,virtual touch imaging(VTI) and virtual touch quantification(VTQ) examination .The shear wave ve‐locity was obtained .The average optical density of the tumor was obtained by processing the VTI images by using the image J soft‐ware .The pathological results served as the gold standard and the ROC curve was drawn .Finally ,the significance of ARFI was e‐valuated .Results All 87 pathologically verified breast tumors included 31 malignant lesions and 56 benign lesions .In malignant le‐sions ,there were 21 lesions with VTQ value≥3 .49(threshold value) and 10 lesions with VTQ value<3 .49 .In benign lesions ,there were 3 lesions with VTQ value≥3 .49 and 53 lesions with VTQ value<3 .49 .In malignant lesions ,there were 28 lesions with the mean optical density value≥202 .25(threshold value) and 3 lesions with the mean optical density value<202 .25 .In benign lesions , there were 6 lesions with the mean optical density value≥202 .25 and 50 lesions with the mean optical density value<202 .25 .There were statistically significant differences in the shear wave velocity measured by VTQ and average optical density measured by VTI between malignant and benign lesions(P=0 .000) .The sensitivity ,specificity ,positive predictive value ,negative predictive value ,ac‐curacy rate and Youden index of conventional ultrasound were 96 .8% ,67 .8% ,62 .5% ,97 .4% ,78 .2% ,0 .645 ,respectively ;which for ARFI were 93 .5% ,98 .2% ,96 .7% ,96 .5% ,96 .6% and 0 .917 ,respectively .Conclusion The ARFI is superior to the conven‐tional ultrasound in the specificity and accuracy rate in diagnosing breast tumor .

BACKGROUND:An increase in activity of muscle contraction can induce transcription of a variety of signaling molecules to activate a large number of gene expression within the nucleus through proprietary signaling pathway OBJECTIVE:To review studies related the peroxisome proliferator-activated receptor-gamma coactivator 1(PGC-1)and exercise induced skeletal muscle adaptations.METHODS:A computer-based online search of Pubmed was performed for relevant English articles published from January 1995 to January 2009 with the keywords of"PGC1,skeletal muscle,exercise,adaptations"The relevant articles about PGC-1 and exercise induced skeletaI muscle adaptations were included.and repetitive contents were excluded.PGC1 and mitochondrial oxidative metabolism,and exercise-induced skeletal muscle adaptation and PGC1s changed served as the evaluation indicators.RESULTS AND CONCLUSION:Of 57 collected articles,34 were classified and sorted according to the criteria.Endurance training results in profound adaptations for skeletal muscle,including mitochondrial biogenesis,capillary density,and fiber composition.Transcription factors are highly dependent on coactivator molecules to regulate training physiologic adaptation processes.The majority of transcription factors target genes are involved in mitochondrial biogenesis and metabolism.These transcriptional patterns may provide a basic framework for understanding the integration of mitochondrial biogenesis and function with signaling events that exercise induced energetic properties Post-transcription of PGC-1 alpha protein,there are a variety of protein modifications,with a variety of biological processes closely related to the possible adaptive mechanism of exercise-induced skeletal muscle.
