1.Effect of different sleeping state and body positions on cardio-pulmonary function and development of preterm infants

Objective To explore the effect of different sleeping state and body position nursing on the surface temperature, heart and lung function, and development of premature infants. Methods A total of 160 cases of premature infants were selected as research objects, divided into active-prone group, active-supine group, quiet-prone group, quiet-supine groupwith random number tables. Each group on the basis of conventional nursing care was given the above position. Respiratory frequency (RR), heart rate (HR), oxygen saturation (SpO2), RR deviation, HR deviation and surface temperature were observed before and after nursing. After continuous nursing for 7 days, the differences of development indexes of each group were also observed. Results Surface temperature of forehead, right flank, right forearm, right leg were (36.4 ± 0.2), (36.4 ± 0.3), (35.6 ± 0.3),(34.4 ± 0.5)℃in group C, which were significantly higher than those in the other groups (F=2.89-3.31, P<0.05). HR, RR, HR-deviation, RR-deviation and its change value in group C were (128.5 ± 1.8) beats/min, (34.6 ± 2.2) beats/min, (9.1 ± 1.2)%, (212.5 ± 31.5)%, which were significantly lower than those in the other groups (F=4.61-4.96,P < 0.05), and SpO2 change value was 0.027±0.001, which was significantly higher than those in the other groups (F=3.65,P<0.05). Weight gain, the length growth, head circumference growth and upper arm around growth in group C and A in preterm infants after 7 days of continuous nursing were (118.6±6.3) g, (11.3±1.2) mm, (6.4±0.4) mm, (4.4± 0.2) mm and (110.3 ± 5.7) g, (11.0 ± 1.1) mm, (6.0 ± 0.5) mm, (4.2 ± 0.3) mm, which were significantly better than (74.5 ± 4.4) g, (6.9 ± 0.8) mm, (4.5 ± 0.7) mm, (3.2 ± 0.2) mm of active-supine group and (78.6 ± 4.9) g, (7.6±0.7) mm, (4.8±0.6) mm, (3.5±0.3) mm of quiet-supine group (F=3.57, 4.98,P<0.05). And the above indicators of quiet-prone group were higher than that of active-prone group (P<0.05). Conclusions The quiet-prone position can improve the heart and lung function and development of premature infants.
2.Preparation and Quality Control of Fluconazole Eyedrops

OBJECTIVE:To study the preparation of fluconazole eyedrops as well as its quality control.METHODS:The content of fluconazole in eyedrops was determined with HPLC.The quality control of eyedrops has done in accordance with ChP.RESULTS:The average recovery was100.4%,RSD=0.54%(n=5).CONCLUSION:The formula of this preparation is reasonable with simple and convenient technology.Moreover,its quality control is easy to handle.
3.Compliance rate of clinic appointments among residents contracting family doctor service and its influencing factors

Objective To investigate compliance rate of clinic appointments among residents contracting family doctor service and its influencing factors.Methods Total 1 442 clinic appointments among residents contracting family doctor service in Yuetan community were investigated by face to face questionnaire.Results Among 1 442 clinic appointments in Yuetan community health service center in June 2015,563 were kept with a compliance rate of 37.2%.There were significant difference in compliance rate among the residents with difference degree of education (OR =1.461,95 % CI:1.296-1.646,P =0.000),between those with and without chronic diseases(OR =2.009,95% CI:1.136-3.550,P =0.016),with and without receiving first contact care at community (OR =1.360,95% CI:1.036-1.785,P =0.027),with and without satisfying with family doctor service (OR =1.315,95 % CI:1.060-1.630,P =0.013),with and without recognizing clinic appointment system (OR =2.765,95% CI:2.142-3.569,P =0.000).Among the 906 failed appointments,the reasons were forgetting the appointment in 525 (57.9%),missing the date in 124 (13.7%),time conflict in 92 (10.2%),not bring reservation form in 85 (9.4%),being hospitalized or engaging in other medical service in 41 (4.5%) andneglecting appointment system in 39 (4.3%).Conclusion Compliance rate of clinic appointments among residents contracting family doctor service is lower,and necessary measures should be taken to increase the compliance rate.
4.Expression of Th1/Th2 cytokine and T lymphocyte subset in rat allogenic corneal transplantation with organcultured graft

BackgroundAllogenic immunological rejection is still the common cause of keratoplasty failure.Organ culture is a good choice for gradually attenuating corneal antigeniciy.However,the complicated regulatory mechanism of such allogenic rejection with organ culture gratts is unclear until now.ObjectiveTo investigate the changes of several Thl/Th2 cytokines and T lymphocyte subsets in the rat corneal allogenic transplantation rejection with organ culture grafts.MethodsThirty-six SPF Wistar rats served as donors,and 72 SPF SD rats served as recipients in this study.Seventy-two SD rats were divided into two groups randomly.Organ culture and fresh grafts from Wistar rats were performed on 36 recipients SD rats,respectively.Six normal SD rats were served as the normal control.After transplantation,graft survival time was observed ; Aqueous humor levels of IL-2,IFN-γ,IL-4 and TNF-α were measured by enzyme linked immunosorbent assay.Immunohistochemistry was performed to examine CD25 expressions in grafts.Levels of TNF-αmRNA,IFN-γmRNA,IL-4mRNA and CD25mRNA in grafts were detected by using reverse transcription polymerase chain reagction.The levels of CD28subsets in peripheralblood were determined by Flow Cytometry.The use of experimental animals followed the Statement of ARVO.Results The mean rat graft survival time was longer in the organ culture grafts group( 13.78 d) than that with fresh gratts( 10.56 d)(t=14.945,P =0.000 ).The aqueous humor levels of IL-2,IFN-γ,IL-4 and TNF-α were higher in two allograft groups at day 6,day 13 and day 24 after transplantation than that in the normal control,which peaked at day 13,and the fresh grafts group had the highest level( F=324.891,416.416,240.661,364.533,P=0.000).At day 13,CD25 was weakly expressed in frozen section of two operation groups.The expression of TNF-α and IFN-γmRNA in organ culture grafts group was markedly less than in fresh grafts group at day 13 after surgeries (t =2.464,P =0.039;t=5.438,P=0.001 ),whereas there was no significient difference in IL-4 and CD25 mRNA levels between them (t=-0.782,P =0.457,t =0.712,P =0.497).The percentages of CD28 of the fresh graft group increased significantly and more weakly expressed in organ culture grafts group (P =0.016 ). Conclusions Th1/Th2 cytokines and CD28 lymphocyte subsets may play important roles during corneal allograft rejection with organ culture grafts.
5.Restenosis after vertebral artery origin stenting

Stenosis or occlusion of vertebral artery origin is one of important reasons of posterior circulation ischemia. Vertebral artery stenting can reduce posterior circulation ischemia in patients with vertebral artery origin stenosis. Restenosis after stenting is an important reason influencing the long-term efficacy.This article reviews treatment approaches of vertebral artery origin stenosis, restenosis after stenting and the related factors.
6.Fetal inflammatory response syndrome and neonatal brain diseases

Fetal inflammatory response syndrome is a sub-clinical state that cause fetal immune sys-tem could be activated and released large amounts of proinflammatory cytokines.Either caused by infection of factors such as chorioamnionitis,fetal sepsis or non-infectious factors such as asphyxia,chronic lack of oxy-gen,which are likely to cause neurological damage in preterm or full-term children .This article reviewed the progress on the mechanism of neonatal encephalopathy caused by fetal inflammatory response syndrome.
7.Drug Resistance Status and Nosocomial Infections of Klebsiella pneumoniae in Our Hospital

OBJECTIVE To investigate drug resistance status of Klebsiella pneumoniae and offer scientific basis for reasonable usage of antibiotics. METHODS Drug resistance was analyzed by using K-B method.The(ESBLs) producing strains detected by disc confirmed test. RESULTS The frequency of(ESBLs)-producing K.pneumoniae isolates was 25.8% in our hospital.All of the 128 strains were susceptible to imipenem.And the resistant rates to ciprofloxacin and amikacin were about 10.9%,the resistant rate to other 6 antibiotics were all above 40%. CONCLUSIONS It is important to supervise drug resistance of clinical isolates in hospital and we should try our best to prevent nosocomial outbreak and surveillance.
8.Observation of curative effect of hemorrhoids lotion on pain, edema and bleeding after anorectal surgery.

The purpose of this study was to evaluate the value of Chinese herbal fumigation in the postoperative anal disease. The authors randomly divided 348 patients into treatment group and control group with 174 cases in each group. The treatment group was given to the Chinese herbal medicine hemorrhoids lotion for fumigation based on conventional anti infective therapy, routine dressing change and relaxing bowel. The control group was given to 1 000 mL 1: 5 000 potassium permanganate solution for sitz bath, fumigation based on conventional anti infective therapy, routine dressing change and relaxing bowel. The pain score, edema score, bleeding score, granulation tissue growth score and wound healing time of two groups were compared after operation. The results showed that the postoperative 6 h pain scores were higher in the two groups, the postoperative 3,5,7 d pain scores gradually decreased, the difference was statistically significant (P < 0.05). The difference of postoperative 6 h pain scores was no significant difference between the two groups, while postoperative 3,5,7 d pain scores in the treatment group were significantly lower than those in the control group (P < 0.05). 7 days after operation, anal margin of edema score and blood in the stool score in the treatment group were lower than those in control group, meat medicine growth score was higher than that of the control group, the difference had statistical meaning (P < 0.05). The healing time of two groups was respectively (13.89 + 2.78), (18.45 + 1.65) d (P < 0.05). This study suggested that Chinese herbal fumigation and washing could reduce the pain degree of patients, the anal margin of edema, and the blood in the stool, also could promote granulation tissue growth and shorten the time of wound healing, deserve the clinical expansion.
