1.Correlation between multi-drug resisitance and Her-2 gene in breast cancer

Objective To investigate the correlation between the expression of p170、GST-π 、TOPO Ⅱ and the expression of Her-2 in human breast cancer. Methods The expression of Her-2 was detected by fluorescence in situ hybridization.The expression of p170,GST-π,TOPO Ⅱ were tested in 48 breast cancer by immunohistochemical SP method,and their correlations with clinicopathological features and the expression of Her-2 were analyzed.Results The positive rate of p170,GST-π,TOPO Ⅱ were 43.8 %(21/48),39.6 %(19/48),56.3 %(27/48),respectively. The expression of p170, GST-π,TOPO Ⅱ positively correlated with the expression of Her-2(P< 0.05).The levels of p170,GST-π,TOPO Ⅱ expression were not associated with age,primary tumor and lymph node metastasis(P > 0.05).Conclusion There is great possibility of chemotherapy drug resistance in breast cancer with expression of Her-2.Expressions of p170,GST-π,TOPO Ⅱ and Her-2 have an instructive significance for chemotherapy.
2.Effect Evaluation of Clinical Pharmacist Intervention on the Application of Antibiotics in Respiratory De-partment of Our Hospital

OBJECTIVE:To evaluate the effect of clinical pharmacist intervention on the application of antibiotics in patients with respiratory infection from our hospital. METHODS:Each 500 patients were selected from respiratory department of our hospi-tal during Jan. to Jun. in 2016 and Jul. to Dec. in 2016. The patients were divided into pre-intervention group and post-intervention group. The application of antibiotics,the occurrence of ADR and anti-infective effects were compared before and after clinical phar-macist intervention,and the rationality of antibiotics use was evaluated. RESULTS:After intervention,the utilization rate of antibi-otics decreased from 92.60% to 74.20%;the proportion of single drug therapy and triple and more drugs therapy were decreased significantly,with statistical significance(P<0.05). The incidence of ADR reduced from 14.20% to 5.80%;the rate of infection control increased from 95.03% before intervention to 97.04%;antibiotics cost decreased significantly,with statistical significance (P<0.05). The incidence of irrational drug use,nonstandard dosage,irrational drug combination and no etiological support as well as total irrational rate decreased from 7.40%,3.60%,5.80%,7.80%,20.60% to 4.00%,1.40%,2.00%,4.20%,9.60%,with statistical significance(P<0.05). There was no statistical significance in the proportion of double drug therapy or emergency and se-vere rescue rate (P>0.05). CONCLUSIONS:The intervention implemented by clinical pharmacists effectively reduce the utiliza-tion rate of antibiotics and the incidence of ADR,improve the rate of infection control,reduce antibiotics cost and the occurrence of irrational drug use. However,there are still some problems in respiratory department of our hospital,such as irrational drug use, nonstandard dosage,irrational drug combination and no etiological support,etc. Continuous follow-up intervention is needed.
3.Opinions about diagnosis and treatment of multilevel cervical spondylotic myelopathy.

There should be confusion about diagnosis and treatment for multiple segments cervical myelopathy in some respects. The author reviewed the literature and combined with clinical experience, proposed a new classification for cervical myelopathy according to responsibility segment areas, which dividing into single segment,double segments (continues or jumping type), multi-segment (≥ three segments). The responsible segments determination is the premise of diagnosis and also a key to determine surgical decompression segment. Decompression only according to imaging was not desirable, surgical segment should mainly relies on clinical, imaging, electrophysiological and comprehensive analysis to avoid surgery range expansion. Surgical approach and procedures are still the focus and hotspot of cervical myelopathy treatment, and no treatment standards and corresponding guidelines to consult. The author proposes that surgical approach should advocate the individual, and surgical procedure should follow simple instead of complicate, anterior and posterior combined decompression is not necessary in most cases, and anterior and posterior fixation are not need.

4.Expression and implication of Survivin and its correlation with p27~(kipl) in gallbladder cancer

The process of protein synthesis is terminated by one of the three stop codons which are recognized by classⅠ polypeptide release factors.Subsequently, it could promote the hydrolysis of the ester bond of peptidy-tRNA, resulting in release of the nascent polypeptide.Recent results from cryoelectron microscopy, crystallography, NMR, molecular dynamic and biochemical experiments have shed considerable light on the function and structure of the classⅠ release factors.The progress in these aspects were summarized.

9.Progress on genetics and therapy in the spinal muscular atrophy

Spinal muscular atrophy (SMA) is a group of neuromuscular disorders, caused by degeneration of the motor neurons in the anterior horn of the spinal cord, with prevalence of about 1 in 6000 to 1 in 10000 in newborn. The gene carrying frequency is about 1 in 40 to 1 in 50 all over the world. SMA is one of the most common autosomal recessive diseases causing infant death. SMA mainly refers to SMN1 dependent caused by SMN1 gene mutations. Noninvasiveness and specificity make genetic testing a recommended method for diagnosis of SMA. In addition to conventional methods such as neural nutrition, muscle exercise, etc., there is no specific treatment for SMA up to now. Nevertheless, HDAC inhibitors deserve attention as they are the only drugs completed Phase Ⅲ clinical trials to date. Furthermore, other ways as small-molecule SMN enhancers, induced pluripotent stem cell (iPSC), antisense oligonucleotides to correct SMN2 splicing, etc, were still on the way of in vitro stage at present.
10.Study on the Relationship between UGT1A1 Gene Polymorphisms and Irinotecan-induced ADR

OBJECTIVE:To study the correlation of UGT1A1 gene polymorphisms with the incidence and severity of irinote-can-associated ADR in the patients with irinotecan-based chemotherapy. METHODS:56 patients with advanced gastroenteric tumor and small cell lung carcinoma were selected from our hospital and treated with irinotecan-based chemotherapy. The occurrence of ADR was observed during chemotherapy. Gene DNA were collected from peripheral blood sample,and UGT1A1 gene polymor-phisms was determined. The relationship of genotypes with ADR was analyzed. RESULTS:TA sequence of UGT1A1*28 genetic lo-cus was as follows:wild-type genotype TA6/6(42 cases,75.0%),heterozygous mutation-type TA6/7(13 cases,23.2%)and ho-mozygous mutation-type TA7/7(1 cases,1.8%);that of UGT1A1*6 genetic locus was as follows:wild-type genotype(44 cases, 78.6%),heterozygous mutation-type(10 cases,17.9%)and homozygous mutation-type(2 cases,3.6%). In UGT1A1*28 genetic locus,6 wild-type genotype patients and 3 mutation-type patients suffered from Ⅲ degree or above hypoleukemia and/or neutrope-nia (14.3% vs. 21.4%,P>0.01),among which only one homozygous mutation-type patient suffered from hypoleukemia and/or neutropenia(100%);6 wild-type genotype patients and 2 mutation-type patients suffered from Ⅲ degree or above diarrhea(14.3%vs. 14.3%,P>0.01),among which only one homozygous mutation-type patient suffered from Ⅲ degree or above diarrhea (100%). In UGT1A1*6 genetic locus,3 wild-type genotype patients and 8 mutation-type patients suffered from Ⅲ degree or above neutropenia (6.8% vs. 66.6%,P<0.01),and 2 wild-type genotype patients and 7 mutation-type patients suffered from Ⅲ degree or above diarrhea(4.5% vs. 58.3%,P<0.01). CONCLUSIONS:Among patients with advanced gastroenteric tumor and small cell lung carcinoma,UGT1A1 gene wild-type gene promoter is most common,followed by heterozygous mutation-type,and homozy-gous mutant rare. For TA7/7 homozygous mutation-type patients,irinotecan-based chemotherapy increase the risk of Ⅲ degree or above hypoleukemia and/or neutropenia and diarrhea. For TA6/7 heterozygotes patients and TA6/6 wild-type patients, irinote-can-based chemotherapy doesn't affect the incidence of Ⅲ degree or above neutropenia and diarrhea. For UGT1A1*6 genetic locus mutation-type patients,irinotecan-based chemotherapy significantly increase the risk of Ⅲ degree or above neutropenia and diar-rhea.
