2.Research on calculation of the regional cerebral blood volume based on minimum mean square error method.

In this paper, the Fourier transform based minimum mean square error (FT-based MMSE) method is used to calculate the regional cerebral blood volume (rCBV) in magnetic resonance (MR) perfusion imaging, and the method is improved to handle the existing noise in the imaging process. In the experiments with signal-to-noise ratio (SNR) of 50 dB, the rCBV values were compared with the results using MMSE method. The effects of different SNRs on the estimation of rCBV were analyzed. The experimental results showed that MMSE was a simple way to filter the measurement noise, and could calculate rCBV accurately. Compared with other existing methods, the present method is not sensitive to environment, and furthermore, it is suitable to deal with the perfusion images acquired from the environment with larger SNR.

3.Efficacy and Safety of Intravenous Thrombolytic Therapy with Alteplase in Aged Patients with Cerebral Stroke

Objective: To explore the safety and efficacy of intravenous thrombolysis therapy with tissue plasminogen activator ( tPA) in aged acute ischemic stroke patients. Methods:The patients treated with intravenous tPA were analyzed and divided into≥80-year group (n=16) and <80-year group (n=79), and 30 hospitalized patients with the age above 80 years without thrombolytic therapy were selected as the control group. The prognosis of the three groups was compared. Results:①The incidence of ICH in the two thrombolysis therapy groups was 13. 9% and 18. 7% in 24h, and that of SICH was 5. 1% and 6. 3% with no significant difference (P>0. 05). ② No significant difference was found in favorable prognosis between the two therapy groups 90 days after thrombolysis (P=0. 771), while very bad prognosis rate was higher in the≥80-year group than in the <80-year group (P<0. 05). ③The≥80 years group had an increased favorable outcome compared with the control group (P<0. 05), while the very bad prognosis rate in the two groups showed no significant difference (P>0. 05). Conclusion:It is safe and effective for old patients to receive IV-tPA throm-bolysis therapy.
4.Drainage effects in total knee arthroplasty

BACKGROUND:Drainage has been applied as a routine to hip and knee replacement surgery. However, as a potential channel, drainage could become a portal for bacteria and other pathogens to invade the joint cavity. OBJECTIVE:To evaluate the clinical significance and safety of drainage in total knee arthroplasty.
METHODS:The 80 patients were prospectively randomized into two groups:Group 1 included 38 patients with wound drainage;Group 2 included 42 patients without any drainage. According to a formula calculation, blood loss, the number of subjects requiring blood transfusion, volume of blood transfusion, complications and the recovery of function after total knee arthroplasty were statistical y analyzed.
RESULTS AND CONCLUSION:The subjects were fol owed up for 2 years after discharge. Two patients lost to fol ow up in the non-drainage group. 78 patients were involved in the result analysis. The total blood loss, the number of subjects requiring blood transfusion and volume of blood transfusion were more in the drainage group than those in the non-drainage group after surgery. At 3 days after arthroplasty, active range of motion was larger in the drainage group than that in the non-drainage group. No significant differences in hemoglobin levels and Keen Society Score scores were detected after arthroplasty between the two groups. Results suggested that draining wil increase the blood loss and blood transfusion, but it may contribute to the early recovery of knee function. No significant differences in long-period Keen Society Score scores and postoperative complications were detectable between the two groups.
5.The impact of dietary methionine-restriction on tight junction expression and function in a rat colonitis model

Objective To study the impact of methionine restriction (MetR) on mucosal histopathology,permeability and tight junction composition in a dextran sulfate sodium (DSS)-induced colitis model,and to explore its underlying mechanism.Methods SD rats were randomly divided into 4 groups:normal rats fed by a complete amino acid (AA group) diet,normal rats fed by MetR diet (MetR group),DSS treated rats fed by a complete amino acid (DSS + AA group) and DSS treated rats fed by MetR diet (DSS + MetR group),each group had 15 rats.Abdominal aorta blood sampling was taken at day 21 after DSS model been established to analyze blood routine examination,liver and kidney function and level of electrolyte.Morphological changes in colonic mucosa were evaluated and scored by light microscopy.Myeloperoxidase (MPO) activity was measured.The effect of MetR on mucosal permeability of colon strips was detected by Ussing chamber.Claudin2,occludin,claudin3,ZO-1 expression were quantified by Western blot.Results The early clinical manifestation in the DSS treated rats were loose stool or diarrhea,hematochezia positive and bleeding,and weight losing.HE observation showed prominent colitis in distal colon with manifestations of crypt abscess and infiltration of inflammatory cells.Although MPO activity and WBC account between the DSS + MetR and DSS + AA group did not significantly changed,treatment with MetR diet significantly decreased the extent and severity of epithelial injury of DSS + MetR group (10.55 ± 3.62 vs 15.00 ± 4.89,P =0.003).There were no significant difference in PCNA immunohistochemical result between the DSS + MetR group and DSS + AA group.Compared to the rats on AA diet,transepithelial electrical resistance(TEER) in DSS + AA group was obvious lower [(28.40 ± 6.78) Ω · cm2 vs (46.53 ± 4.03)Ω · cm2,P <0.05],and TEER in MetR group were obviously higher[(60.64 ± 8.40)Ω · cm2 vs (46.53 ±4.03)Ω · cm2,P <0.05].However,short-circuit current (Isc) in DSS + MetR group was obviously higher that of DSS + AA group [(35.01 ± 2.19) μA/cm2 vs (29.61 ± 1.19) μA/cm2,P <0.05].Western blot suggested that colon claudin2 expression was not found in colon epithelium of normal rats,and an obviously increase expression of claudin3 protein was found in the MetR group,compared to AA group; and an significantly increase in the abundance of claudin3 was found in the DSS + MetR group,but amount of claudin2 was decreased,compared with the DSS + MetR group.Conclusion The MetR diet has obvious therapeutic effect on ulcerative colitis model rats induced by DSS,and its mechanism may not by regnlating inflammatory cell infiltration and the way of promoting intestinal cell growth to alleviate inflammatory injury,but probably by changing the structure and function of tight junction protein and improve the intestinal mucosal barrier function,and promote the repair of damaged intestinal mucosa.
6.Nanobacteria: isolation from prostatic calculus and identification of its 16S rRNA gene

ObjectiveTo explore the distribution of nanobacteria in prostatic calculus and investigate its role in the formation of prostatic calculus. MethodsThe stones of 40 patients with prostatic calculus was used to isolate and culture the possible bacteria. The genomes of obtained bacteria were extracted, and the 16S rRNA was amplified by PCR followed by sequencing. ResultsThe obtained specific fragment had a 98% resemblance with 16S rRNA of nanobacteria: Score=2 480 bits (1 290), Expect=0.0; Identities=1 387/1 409 (98%), Gaps=4/1 409 (0%); Strand=Plus/Plus. ConclusionNanobacteria is proved existing in the stones of prostatic calculus patients by PCR and sequencing.
7.Bacteriology Study and Choice of Antibiotics for Open Wounds in Patients with Earthquake Injury in Our Hospital

OBJECTIVE:To study the bacteria strains and susceptible drugs in open wound in patients with earthquake injury. METHODS:Bacteriological test and the drug susceptibility test were conducted for earthquake injury patients with open wound and the results of bacterial culture and the drug susceptibility test were analyzed using Excel 2003. RESULTS:Of the 1 172 earthquake injury cases,the patients with trauma / wounds represented 93.9%,the wound had obvious infection in 132 cases; of the infected cases,the proportions of patients with open fractures,soft tissue injuries and traumatic brain injury accounted for 63%,28% and 9%,respectively. The positive rate of the bacterial culture was 69%,and gram-negative of bacteria which accounted for 85.6% took the lead,chiefly Acinetobacter baumanii,Enterobacter cloacae and Escherichia coli. The results showed bacterial drug resistance was on the high side. CONCLUSION:The open wounds in patients with earthquake injury are likely to be infected with gram negative organisms. Debridement and correct choice of anti-infective drugs are needed to reduce the risk of wound infection,
8.Expression of LMP2A, E-Cadherin and fibronectin in nasopharyngeal carcinoma and its clinical significance.

OBJECTIVE:
To investigate the expression of EBV-encoded latent membrane protein 2A (LMP2A) and epithelial-mesenchymal transformation(EMT) associated markers (E-cadherin and fibronectin) in nasopharyngeal carcinoma (NPC) and its clinical significance.
METHOD:
The expression of LMP2A, E-cad-herin and fibronectin proteins in 32 cases of chronic nasopharyngeal inflammation, 56 cases of NPC and 18 cases of NPC lymph node metastasis were examined byimmunohistochemical SP method.
RESULT:
(1)The positive rates of LMP2A in NPC and its lymph node metastasis were significantly higher than those of chronic nasopharyngeal inflammation (89. 3%vs 37. 5%o and 77. 8% vs 37. 5%) respectively (both P<0. 01); The normal expression rates of E-cadherin in NPC and its lymph node metastasis were significantly lower than those of chronic nasopharyngeal inflammation (33. 9% vs 90. 6% and 5. 6% vs 90. 6%) respectively (both P<0. 01); The positive rates of fibronectin in NPC and its lymph node metastasis were significantly higher than those of chronic nasopharyngeal inflammation (83. 9% vs 28. 1% and 72. 2% vs 28. 1%) respectively (both P<0. 01). (2) ZLMP2A expression were negatively correlated with normal expression of E-cadherin (r= -0. 387, P<0. 01), and were positively correlated with fibronectin (r= 0. 421, P<0. 01). (3)LMP2A, E-cadherin and fibronectin expression were significantly correlated with N stage and clinical stage (both P<0. 05), but the three proteins were not significantly correlated with M stage (both P> 0. 05). In addition, LMP2A and E-cadherin expression were significantly correlated with T stage (both P<0. 01).
CONCLUSION
LMP2A and fibronectin expressions were increased in NPC, but normal expression of E-cadherin were decreased. LMP2A may promote lymph node metastasis and malignant progression of NPC by induce EMT through downregulation of E-cadherin and upregulation of fibronectin.

9.Comparison of the effective optical zone after small incision lenticule extraction measured by corneal topography and modulation transfer function in myopia

Objective:To measure the effective optical zone (EOZ) after small incision lenticule extraction (SMILE) by using corneal topographic map and modulation transfer function (MTF), and to analyze the related factors affecting the EOZ.Methods:A retrospective observational case series study was performed.Sixty-two myopic patients (62 eyes) who underwent SMILE between December 2015 and July 2017 in Jinan Mingshui Eye Hospital were enrolled.The EOZ was measured by using tangential corneal curvature topographic map and MTF pre- and 6-month post-operatively.The repeatability of data was determined by intraclass correlation coefficient (ICC) and Cronbach Alpha coefficients; the agreement of data was identified by using Bland-Altman plot; the correlations between EOZ and surgical parameters were analyzed by utilizing Pearson correlation coefficient.This study protocol was approved by the Ethics Committee of Jinan Mingshui Eye Hospital(No.2015[013]). Written informed consent was obtained from each patient before surgery.Results:The ICC and Cronbach Alpha coefficients of EOZ measured by corneal topographic map were greater than 0.9, which showed high intraobserver repeatability.The Bland-Altman plots displayed relatively good agreement between the two methods.The 95% limits of agreement was -0.49 to 0.89 μm.Six months after SMILE, the EOZ measured by tangential corneal curvature gradient topographic map was (5.32±0.25)mm, which was (1.18±0.25)mm smaller than predicted optical zone, and the EOZ measured by MTF method was (5.07±0.32)mm, which was (0.20±0.35)mm smaller than corneal tomographic EOZ, and the difference was significant ( t=-4.487, P<0.01). A negative correlation was found between the EOZ and attempted refractive correction ( r=-0.364, P=0.004). The positive correlations were found between the EOZ and ΔKm or ΔQ 6 months after SMILE ( r=0.367, 0.514; both at P<0.01). Conclusions:The EOZ measured by tangential corneal curvature topographic map after SMILE is of high repeatability and is consistent with the result calculated by MTF method.The EOZ is significantly reduced after SMILE.
10.A Sampling Survey on Prevalence and Risk Factors of Overweight and Obesity among Inhabitants in Shenchi County, Shanxi

Objective To comprehend the status and risk factors of overweight and obesity among urban and rural inhabitants of Shenchi county.Methods With stratified cluster sampling,two residents" committees were selected as investigation spots from each town of Longquan,Yijing,Bajiao and Hezhi of Shenchi county.Permanent inhabitants aged 20 to 74 yrs were investigated on their status of overweight and obesity and its realted risk factors.Results In this study,2231 inhabitants were investigated,among which male 871 and female 1 360.The prevalence rate of overweight and obesity were 28.6%(standardized rate was 26.7%) and 5.1% (standardized rate was 4,5%).There was a statistic significance in prevalence rate of overweight between male and female(X~2=4.595, P0.05).It was discovered that the age,the degree of education, the annual income of family,drinking,exercise status,alimentary control status,family history and so on had the relation with BMI, the exercise and diet control status assumed negative association by multiple linear regression.Conclusion There is a high prevalence rate of overweight and obesity among residents in Shenchi county in Shanxi province.We should make the comprehensive intervention measures according to its status and risk factors,and make the prevalence rate decreased.
