1.Radiotherapy efficacy of E-Flow imaging on cervical lymph nodes metastasized from nasopharyngeal carcinoma

Objective To explore the effect of E-Flow imaging on the radiotherapy of cervical lymph nodes metastasized from nasopharyngeal carcinoma.Methods A total of 52 nasopharyngeal carcinoma patients with 94 invasion neck lymph nodes were treated with conventional radiotherapy. Lymph node size, hemodynamic parameters, and blood supply were detected by E-Flow imaging and two-dimensional ultrasound before and after the radiotherapy. The relationship between those examinations and radiotherapy effect was analyzed.Results Of the 94 metastatic cervical lymph nodes from nasopharyngeal carcinoma before the radiotherapy, 40 disappeared, 52 reduced, and 2 maintained the original size after the radiotherapy. Blood flow in all residual lymph nodes decreased or disappearaed. Artery flow spectrum in the lymph node was changed from the high-speed high impedance to low-speed low resistance. The size of cervical lymph node and blood flow before the radiotherapy were associated with clinical efficacy (P<0.05). Conclusion E-Flow imaging plays an important role in assessing the efficacy of radiation therapy by changing the blood supply of cervical lymph node and its size before and after the radiotherapy.
2.P300 event-related potential of patients with vascular dementia

BACKGROUND:It is crucial for us to find an objective and sensitive means for the assessment ofthe severitv and development of dementia,so as to make instant diagnosis and examination of therapeutic effect on patientswith vascular dementias(VD).Event related potential endogenetic eomponent P300 is proved closely correlated with the cognitive management of examinees but not with stimulus property,thereby considered as one of the most valuable and objective index for brain superior psychological activity.OBJECTIVE:To study the role and significance of events related potential P300,providing objective basis for clinical diagnosis and therapeutic assessment for patients with VD.DESIGN:Non-randomized simultaneous controlled explorative study based on patients. SETTING:Neurological department and electromyography department in aprovincial hospital and neurological department in a medical college hospital.PARTICIPANTS:Between February 2000 and June 2001,64 patients were of VD at neurological clinic and ward ofthe Second Affiliated Hospital of Chongqing Medical College,including 38 males and 26 females with theaverage age of(66.0±4.6)years(VD group).Culturallevel:19 cases re-ceiyed over senior school education,31 cases of junior school education and14 cases of illiteracy.Meanwhile 60 inpatients without VD were enrolled(non-VD group)including 35 males and 25 females withthe average age of(68.0±3.7)years.Culturallevel:senior school or above,15 cases,junior school, 29 cases and illiteracy,16 cases.METHODS:Mental sate of both VD and non-VD groups were assessed with mini-mental state examination(MMSE),and tested with P300 potential observing the variability of latency and amplitude of P300 in patients.Both methods were compared between VD group and non-VD group, moreover relationship between P300 latency and MMSE scores was also explored.MAIN OUTCOME MEASURES:Variability of P300 latency and amplitude Relationship between P300 latency and MMSE scores.RESULTS:P300 latency was found prolonged in 74% and 7% of VD group and non-VD group respectively,with obvious significance between them,meanwhile prolonged P300 latency was proved closely correlated with the MMSE scores(r=0.609 6,P<0.05).CONCLUSION:P300 latency can be considered as an obiective predictor for the earlier diagnosis and the assessment of therapeutic effects in patients with VD.
3.Effect of Dexamethasone on Bleomycin-induced Pulmonary Fibrosis in Rats

Objective To investigate the protective and cure effects of dexamethasone on bleomycin-induced pulmonary fibrosis.Methods32 rats were randomly divided into the control group (C-group, n=8), injury group (I-group, n=12) and dexamethasone group (D-group, n=12). The acute pulmonary model was established by intratracheal injection of bleomycin with rats of the I-group and D-group; while rats of the C-group injected with distilled water. After that, rats of the D-group were injected with dexamethasone sodium phosphate in intraperitoneal every day, those of the C-group and I-group were injected with saline. The animals were killed on the 3rd, 7th, 14th, and 27th days after treatment, and tests of bronchoalveolar lavage fluid (BALF), total lung collagen content and lung tissue processing were performed.ResultsPathological evidence of the I-group rats demonstrated that the alveolar compartment companied with massive inflammatory cell invasion and a number of myofibroblast proliferation became more thick. However, lung injury in the D-group rats got better than that in the I-group. Neutrophil percentage achieved peak in both I-group and D-group on the 7th day. But the neutrophil ratio in the D-group was significantly lower than that of the I-group on the 7th day ( P<0.05) and the 14th day ( P<0.01). Total lung collagen content achieved peak on the 14th day both in I-group and D-group, but that of the I-group was significantly higher than that of the C-group ( P<0.01) and the D-group was significantly lower than the I-group ( P<0.01).ConclusionDexamethasone plays a protective and cure role in lung fibrosis by efficiently inhibiting the gather and invasion of neutropils and restraining the increase of collagen secreted by proliferous fibroblasts.
4.Flow imaging of residual neck lymph nodes in nasopharyngeal carcinoma by contrast-enhanced ultrasonography after radiotherapy.

OBJECTIVE:
To explore the value of flow imaging with contrast-enhanced ultrasound for residual neck lymph nodes in nasopharyngeal carcinoma after radiotherapy.
METHODS:
Blood flow imaging of 50 residual cervical lymph nodes in 36 patients with nasopharyngeal carcinoma after radiotherapy was observed and compared by color doppler ultrasound and contrast-enhanced ultrasonography.
RESULTS:
In the 50 remaining lymph nodes in the neck, 40 blood flow signals were increased, 10 did not significant change by contrast-enhanced ultrasonography. The blood flow in residual lymph nodes by contrast-enhanced ultrasonography was richer than that by color doppler ultrasound. There was a significant difference in the blood flow signals between contrast-enhanced ultrasonography and color doppler ultrasound (chi(2)=49.172, P<0.01).
CONCLUSION
Contrast-enhanced ultrasonography can help blood flow imaging of the residual lymph nodes in nasopharyngeal carcinoma after radiotherapy, which may provide the imaging basis for clinical diagnosis and treatment.

5.The facial nevi associated with hydrocephalus and intracranial anomalous venous return of 3 case reports

Objective To study the clinical manifestation of intracranial anomalous venous return associated with the facial nevi and hydrocephalus. Methods Along with reviewing of the literatures, the clinical records of 3 patients suffered from the facial nevi and hydrocephalus were analyzed. Results All of 3 patients also have sinovenous occlusion, which affect cerebral venous return. Conclusions Intracranial anomalous venous return, which is induced by extensive sinovenous occlusion, has been considered to be the main cause of hydrocephalus in the facial nevi. The formation of sinovenous occlusion and anomalous venous return may be due to developmental defect in the embryonic stage.
6.Study on the relationship between the neovascularization of carotid atherosclerotic plaques,plasma myeioperoxidase(MPO)concentration and cerebral infarction

Objective To study the relationship between the neovascula rization of carotid atherosclerotic plaques with real-time contrast-enhanced ultrasonography(CEUS),plasma myeloperoxidase (MPO)concentration and cerebral infarction.Methods Seventy-three cases with eighty-nine carotid atherosclerotic plaques were examined with real-time CEUS.According to the imaging features,the seventythree cases were divided into three groups:soft plaques group,hard plaques group and mixed plaques group.The differences of the incidence rate of cerebral infarction events,plasma MPO concentration and contrast media persistent enhancement time with real-time CEUS in plaques among the three groups were analyzed.The cerebral infarction events were diagnosed by CT or MRI routinely.Results The incidence rate of cerebral infarction events,plasma MPO concentration and contrast media persistent enhancement time of soft plaques group were significantly higher than that in hard plaques group and mixed plaques group.The risk of cerebral infarction was higher in the patients with elevated level of MPO.It showed positive correlation between the contrast media persistent enhancement time and plasma MPO concentration,and the correlation coefficient was 0.838. Conclusions The type of the plaque is correlated with the neovascularization of plaques and plasma MPO concentration.The study of relationships among them can help to evaluate the plaques,stability thoroughly and guide the clinician to prevent the incidence of cerebral infarction events.
7.Three-dimensional culture of human lung fibroblasts in pulmonary fibrosis remodeling process

BACKGROUND:Contraction of three-dimensional collagen gels has been used as a model for the contraction which characterizes both normal wound healing and the development of fibrosis in the tissue. Several factors and cytokines,such as tumor necrosis factor alpha (TNF-α), interleukin-1, prostaglandin E2 and insulin have been proved to play important roles in collagen remodeling in vitro as well as serum extravasation during the fibrotic progress.OBJECTIVE: To observe extracellular collagen matrix contraction and apoptosis of fetal lung fibroblasts in TNF-α,interleukin-1, insulin, prostaglandin E2, albumin and globum under three-dimensional culture, and investigate the effects of cytokines, insuin, serum and serum protein on the remodeling and fibrotic formation of lung tissue.DESIGN: A randomized controlled experiment.SETTING: Department of Respiratory Medicine, Lanzhou General Hospital of Lanzhou Military Area Command of Chinese PLA.MATERIALS: The experiment was carried out The experiments were carried out in the respiratory laboratory of Lanzhou General Hospital of Lanzhou Military Area Command of Chinese PLA from August 2005 to January 2006. Human fetal lung fibroblasts (American Type Culture Collection), Dulbecco's modified Eagle's medium (DMEM) and fetal bovine serum, insulin, transforming growth factor (TGF) (R&D), prostaglandin E2, type Ⅰ collagen was extracted from rat-tail tendons.METHODS: In order to investigate the effect of initial collagen concentration in the gels on the contractility of fibroblasts,the appropriate amount of collagen was mixed with distilled water, four fold-concentrated DMEM, and cells were suspended so that the mass concentration was 0.75-2.0 g/L, with a physiological ionic strength and the desired cell concentration. In order to investigate the effect of cell number in the gels on the contraction, the cellular concentration fibroblasts in the gels were prepared to 0.2×107-4×107 L-1. The areas of floating gels were measured daily and the contraction was calculated by contrasting the initial size (% of initial area). Different serum concentrations (0.01%-0.5%)in the medium were prepared, the serum albumin (0.1%) or globulin (0.1%) were added to the serum-free culture medium to observe the gel contraction. TGF (10 mg/L), interleukin-1 (10 mg/L), insulin (1 mg/L) and prostaglandin E2 (0.1 μmol/L) were added to observe the effects of cytokines and insulin on fibroblasts-mediated collagen gel contraction.The DNA content and cellular survivability in gels in collagen were determined.MAIN OUTCOME MEASURES: Effect of lung fibroblasts on collagen contraction with or without the presence of cytokines in three-dimensional culture; Effect of collagen with different concentration on the proliferation and apoptosis.RESULTS: ① Collagen gel contraction showed a dependence on the number of fibroblasts. ② Collagen gel contraction was augmented by increasing serum concentration in culture medium, and albumin increased the contraction dramatically. ③TGF and insulin significantly increased the contraction, whereas prostaglandin E2 and interleukin-1 significantly inhibited the gel contraction. ④ The lower the initial collagen concentration was, the more gel contracted and smaller final size were observed, and cell apoptosis increased.CONCLUSION: During the fibrotic process, fibroblast population migrated into the injured lung tissue may play an important role in the development of pulmonary fibrosis and serum infiltrating into injury lung tissue may play an role in stimulating the fibrotic progress. Infiltrating fluids and edema result in the dilution of the collagen concentration in the pulmonary interstitial which may lead to stronger contraction and serious fibrosis. In the dense fibrosis area, cells were hard to survive. In consequence, the final structure of fibrotic lung could not been changed and lung fibrosis progressed.
8.Clinical application of the 3-dimensional speckle tracking imaging in coronary artery disease.

OBJECTIVE:
To observe the 3-dimensional speckle tracking imaging (3D-STI) in clinical diagnosis of coronary artery disease by quantitatively evaluating left ventricular segmental and global systolic function.
METHODS:
According to the selective coronary artery angiography, 150 inpatients suspected of coronary artery disease were divided into 5 groups according to the distribution of the affected coronary arteries: a left anterior descending coronary artery group (LAD, n=46), a left circumflex coronary artery group (LCX, n=25), a right coronary artery group (RCA, n=23), a multivessel group (n=26), and a normal group (n=30). Gensini score was calculated according to coronary artery angiography. Conventional echocardiography was applied in each patient and wall motion score index (WMSI) was calculated. 3D-STI was applied in each patient to evaluate the segmental and global systolic function in the left ventricle.
RESULTS:
Compared with the normal group, ejection fraction was decreased in the multi-vessel group alone (P<0.05). Peak systolic 3D strains in all segments were decreased in the multi-vessel group (P<0.01), while peak systolic 3D strains in the main blood-supply segments were decreased significantly in the rest of the groups (P<0.05). Global 3-dimensional strain (G3DS) was negatively correlated with Gensini score in patients with coronary artery disease (r= -0.76, P<0.01). Receiver operating characteristic curve analysis demonstrated that G3DS was more sensitive and accurate in clinical diagnosis of coronary artery disease.
CONCLUSION
3D-STI can quantitatively evaluate the segmental and global systolic function of the left ventricle, and reflect the extent of coronary artery stenosis in patients with coronary artery disease.

9.Perioperative complications of stenting with symptomatic intracranial artery stenosis

Objective To investigate the perioperative complications of stenting with symptomatic intracra-nial artery stenosis and study the mechanism and prevention of complications. Methods 63 patients were collect-ed from Stroke Center of Guangxi. They were proved intracranial artery stenosis and performed intracranial stents. Patients′ age,with hypertension,diabetes and hyperlipidemia or not,smoking or not,types and occurrence time of complications were registered. Results 63 patients were registered and 2 patients terminated operation due to blood vessels circuity or serious vessel spasm. Operation success rate reached 96.83%. 5 patients had complications among 63 cases,with complication incidence of 8.20%. 3 patients experienced cerebral hemorrhage and two cere-bral infarction in peri-operation period. 2 patients died of complications and mortality rate was 3.28%. Conclu-sions The incidence rate of complications of intracranial stenting with symptomatic intracranial artery stenosis is relatively high and it can be reduced by preoperative sufficient assessment and prudent selection ,careful operation and strict management after operation.

SOAP is widely used in various aspects of medicine, and there are still some problems in the course of TCM. The use of SOAP model for teaching and assessment has important practical significance and value in the course of Chinese medicine internal medicine practice, and SOAP emphasize the practice of the post competence oriented teaching, which is worth promoting.
