1.A research on measuring health status for residents in rural areas

Objective To study the possibility of measuring health status for residents in rural areas by the Chinese SF-36 scale. Methods Health status for 207 residents in rural areas of Fuyang city who were 14 years old and over was measured by the Chinese SF-36 scale. The reliability and validity were analyzed. Results The internal consistency reliability coefficients (Cronbach'α) exceeded 0.50 for all scales except SF and RE scales. Inter-scale correlation coefficients were lower than Cronbach'α coefficients for all scales except SF scale. Factor analysis identified three principal components, which could be used to explain 64.6% of the total variance. The differences of scale scores between the young group and the middle age group weren't significant. There weren't significant differences of scale scores between sexes except RP scale. The differences of scale scores between smokers and no-smokers, drinkers and no-drinkers weren't significant. Nor were the differences between various educational levels. Conclusion Whether the Chinese SF-36 scale is suitable for measurement of health status for residents in rural areas needs further studies to give more evidences.
2.Observation on Microbiological Pollution and Disinfective Effectiveness in Drinking Water Dispensers

Objective To understand the microbiological pollution of drinking water dispensers and find out the effective disinfection method for dispensers. Methods The counts of bacteria, fungi and yeast in water samples collected from the water stored in liners of 52 drinking water dispensers were determined. The total counts of bacteria in water samples of 8 water dispensers were determined before and after disinfection by 1/10 YOUJIE disinfectant for 2 min. Results The medians and over standard rates were 440 cfu/ml and 96.15% for the total count of bacteria, 0 and 30.77% for fungi and yeast in the water samples before disinfection respectively. After 2_min disinfection by 1/10 YOUJIE disinfectant, 5 of 8 drinking water dispensers showed bactericidal rates of 100% and other 3 water dispensers showed bactericidal rates of 99.56%, 99.77% and 99.78% respectively. Bacillus was found in some unkilled bacteria. Conclusion Microbiological pollution in water stored in liners of drinking water dispensers was high, which could be effectively eliminated and disinfected by 1/10 YOUJIE disinfectant for 2 min.
3.Effects of different concentrations of dexmedetomidine on BKCa channels in rat mesenteric arterial smooth muscle cells

Objective To evaluate the effects of different concentrations of dexmedetomidine on large-con-ductance Ca2+-activated K+ (BKca) channels in the rat mesenteric arterial smooth muscle (MASM) cells.Methods Sprague-Dawley rats of both sexes,weighing 180-220 g,were used in this study.Single MASM cell was freshly isolated from mesenteric arteries in two steps.Ten cells were chosen and studied.When holding potential was 40 mV and the concentration of free calcium ions was 10-7 mol/L,inside-out patch-clamp technique was used to record the single BKCa channel current before and after application of different concentrations of dexmedetomidine (0,10-9,10-8,10-7,10-6,10-5 mol/L).Total open probability (NPo),amplitude (Am),mean open time (To) and mean close time (To) of single BKca channel were observed and recorded.Results Compared with the baseline value,dexmedetomidine 10-7,10-6 and 10-5 mol/L increased NPo in a concentration-dependent man-ner,and dexmedetomidine 10-9,10-8,10-7,10-6,10-5mol/L shortened Tc (P <0.05 or 0.01).Compared with the value obtained when the concentration of dexmedetomidine was 10-9 mol/L,Tc was significantly shortened when the concentration of dexmedetomidine was 10-8 mol/L (P < 0.05),and no significant change was found in Am and To obtained when different concentrations of dexmedetomidine were applied (P > 0.05).Conclusion Dexmedetomidine 10-7,10-6 and 10-5 mol/L activate BKca channels in rat MASM cells in a concentration-depen-dent manner,which is one of the mechanisms of decrease in blood pressure by dexmedetomidine.
4.The Situation of Psychiatrists'Job Burnout and its Inspiration to Education of Psychiatry

Objective: To analyze the influential factors of job burnout among psychiatrists.Methods: Psychiatirsts were evaluated by questionnaires.Results: In general,the job burnout of psychiatrists was very serious.There were significant differences in gender in burnout individual level;the person of 31-35 age was prone to job burnout,and the middle-aged staffs’ job burnout was comparatively in high level;There was significantly correlated show between psychiatrists job burnout level and social support.Besides,there was significantly correlated show between psychiatrists job burnout level and coping way.Conclusion: Psychiatrists Burnout is a high-risk population.To better enhance the stability of their profession,we need concern psychiatrists’mental health from the integrated perspective,and improve their level of social support constantly,and prevent occupational burnout from the perspective of medical education early.

6.Protection action of resveratrol on cardiomyocyte hypertrophy mediated by endoplasmic reticulum stress

Aim To explore the role of resveratrol (Res)on cardiomyocyte hypertrophy induced by iso-proterenol (ISO)and the relationship with endoplas-mic reticulum stress (ERS).Methods Hypertrophic model of cardiomyocytes was induced by ISO.Hyper-trophy status of cardiomyocytes was determined by measuring the cell surface area and the gene expression of ANP.The value of apoptosis was measured by flow cytometry,the content of LDH and MDA was measured in different groups,and the gene and protein expres-sions of GRP78 and CHOP were detected by real-time PCR and Western blot.Results Res could attentuate ISO-induced cardiomyocyte hypertrophy and apoptosis by reducing the cell surface area,the gene expression of ANP and the value of apoptosis.Res could inhibit ERS by downregulating the gene and protein expression of GRP78 and CHOP.Meanwhile,the content of LDH and MDA was decreased.Conclusions The results suggest that treatment of Res may protect cardiomyocyte hypertrophy,which is partially mediated by inhibiting the expression of ERS factors GRP78 and CHOP.
7.Role of large-conductance Ca2+-activated K+ channels and protein kinase G in ketamine-induced isolated tracheal smooth muscle relaxation in rats with asthma

Objective To investigate the role of large-conductance Ga2+-activated K+ (BKCa) channels and protein kinase G (PKG) in ketamine-induced isolated tracheal smooth muscle relaxation in rats with asthma.Methods Healthy Sprague-Dawley rats,weighing 250-300 g,were used in this study.Asthma was induced with egg albumin.Thirty-six tracheal rings of 15 rats in which asthma model was successfully established were randomly divided into 3 groups (n =12 each):ketamine treatment group (group AK),IBTX (BKCa channel blocker) +ketamine treatment group (group AKI),and KT-58232 (PKG inhibitor) + ketamine treatment group (group AKK).Tracheal rings were suspended in an organ bath filled with oxygenated Kreb's solution at 36.5-37.5 ℃.In group AK,the tracheal rings were precontracted with acetyleholine 0.1 mmol/L,and the rings were then exposed to ketamine 0.4 g/L for 15 min.In group AKI,before acetyleholine and ketamine were added to the solution,the rings were pretreated with IBTX 3μmol/L for 30 min.In group AKK,before acetyleholine and ketamine were added to the solution,the rings were pretreated with KT-5823 2μmol/L for 30 min.The tension of rings was measured by using a force-displacement transducer.Results The amplitude of relaxation of isolated tracheal smooth muscle was significantly decreased in groups AKI and AKK as compared with group AK (P < 0.05).Conclusion Ketamine induces isolated tracheal smooth muscle relaxation through activating BKCa channels and PKG signaling pathway in rats with asthma.
8.Survey on de-escalation of empiric broad-spectrum antibiotics treatment for patients in intensive care unit

Objective To investigate de-escalation of empiric broad-spectrum antibiotics treatment for patients in intensive care unit (ICU).Methods Data of the patients discharged from ICU in the Second Affiliated Hospital of Zhejiang University from July 1 to December 31 of 2012 and from July 1 to December 31 of 2013 were retrospectively reviewed.Patients with initial use of empirical broad-spectrum antibiotics within 3 d after ICU admission were included in the study.Clinical data including status of infection,the initial empiric antimicrobial therapy,pathogens culture and adjustment of antibiotics in 5 days were analyzed.Results A total of 841 patients were discharged from ICU during the study periods and antibiotics were used in 786 (93.5%) patients.Among 786 patients,389 (49.5%) were treated empirically with broad-spectrum antibiotics,but only 269 (69.2%) had evidences of bacterial infections.Of the 389 patients with empiric antibiotics use,de-escalation of antibiotics was applied only in 6 (1.54%) patients within 5 days after the initiation of treatment.In 269 patients with evidence of infection,specimen sampling and culture were performed in 248 (92.2%) patients within 3 days,among which 165 samples were positive,and the clinical isolates were mainly multi-drug resistant gram negative bacilli and colonized bacteria in oropharyngeal cavity.De-escalation was applied only in 4 (1.49%,4/269) patients with evidences of bacterial infections.Conclusion Broad-spectrum antibiotics as initial empiric therapy is common for patients in ICU,however de-escalation of empiric therapy is rarely applied even in patients with positive results in pathogen isolation and culture.

Aim To study the effects of difluoromethy-lornithine (DFMO)on cardiomyocytes hypertrophy and apoptosis induced by isoproterenol (ISO).Methods The cardiomyocytes were divided into four groups ran-domly:Control group,ISO group,ISO+DFMO group and (ISO +DFMO +Putrescine)group.The hyper-trophic model of cardiomyocytes was induced by ISO, the cardiomyocytes were pretreated with 0.5 mmol · L-1 DFMO and 0.5 mmol·L-1 putrine,the gene ex-pression of ANP,the surface area of cardiomyocytes and the contents of LDH and MDA were measured. Apoptotic value of cardiomyocytes was observed by An-nexin V/PI,the gene and protein expressions of Bcl-2 and Bax were detected by real-time PCR and Western blot.Results Compared with ISO group,the cell sur-face area,the gene level of ANP,the apoptosis value of cardiomyocytes and the contents of LDH and MDA were decreased in ISO +DFMO group (P <0.05 ). Meanwhile,DFMO pretreatment upregulated the gene and protein expressions of Bcl-2 (P <0.05 ), in-creased the ratio of Bcl-2/Bax (P<0.05 ),downregu-lated the gene and protein levels of Bax (P<0.05 ). Conclusion The cardiomyocyte hypertrophy and ap-optosis induced by ISO can be protected by DFMO pre-treatment,which may be associated with the expression of apoptotic gene Bcl-2 and Bax.

Objective:
To explore the effect of simvastatin combined with cyclosporin A treatment on the development of obliterative bronchiolitis in a murine heterotopic tracheal transplantation model.
Methods:
Murine tracheals were heterotopically transplanted from BALB/c donors into C57BL/6 recipients. Transplanted animals received either control chow, chow containing simvastatin, chow containing cyclosporine A, or chow containing simvastatin and cyclosporine A. beginning immediately after transplantation.
