1.Hypoxic preconditioning effects on the ability of bone marrow mesenchymal stem cells tolerant to ischemia and hypoxia

BACKGROUND:According to the previous studies, it is known that bone marrow mesenchymal stem cells live in the physiological environment of lower oxygen concentration compared with the common culture concentration (20%-21%). Hypoxia can promote bone marrow mesenchymal stem cells proliferation and maintain the characteristics of differentiation potential.
OBJECTIVE:To understand the biological characteristics of bone marrow mesenchymal stem cells after hypoxic preconditioning, and search for in vitro basis for bone marrow mesenchymal stem cells therapy in vivo with hypoxic preconditioning strategy.
METHODS:After passage and inoculation, bone marrow mesenchymal stem cells were cultured in the hypoxic incubator (1%O2, 5%CO2). Bone marrow mesenchymal stem cells cultured in the normoxic incubator (20%O2, 5%CO2) served as controls.
RESULTS AND CONCLUSION:Compared with bone marrow mesenchymal stem cells cultured under normoxia (20%O2), bone marrow mesenchymal stem cells cultured under hypoxia (1%O2) began to display exponential growth phase after a longer incubation period. Hypoxia (1%O2) was not the lethal condition for bone marrow mesenchymal stem cells. cellsurvival rates of both groups reduced over time, while those of bone marrow mesenchymal stem cells cultured under normoxia reduced faster (P<0.05). Surface markers expression of bone marrow mesenchymal stem cells, including CD29(+), CD90(+), CD31(-), CD45(-), after hypoxic preconditioning did not change greatly when compared with the results before hypoxic preconditioning. The hypoxia preconditioned bone marrow mesenchymal stem cells did not differentiate into vascular endothelial cells. Hypoxia-inducible factor-1αprotein expression was promoted after 24 hours to 48 hours hypoxic preconditioning, and there was significant difference between the two time points (P<0.05). Vascular endothelial growth factor expression increased at both transcriptional and protein levels after 48 hours hypoxic preconditioning (P<0.05). Culture under hypoxia (1%O2) does not produce a lethal effect on bone marrow mesenchymal stem cells and can be used for hypoxic preconditioning. Surface markers of bone marrow mesenchymal stem cells after 48 hours hypoxic preconditioning exhibit no great changes and the hypoxia preconditioned bone marrow mesenchymal stem cells maintain the characteristics of stemness. Hypoxic preconditioning can promote the ability of bone marrow mesenchymal stem cells to survive under hypoxia/ischemic condition. The activation of the hypoxic response signal transduction pathway through hypoxia-inducible factor-1αmay explain this.
2.Observation of early active training effects after the operation for lumber intervertebral disc herniation

Objective To explore the recent and distant effects of early active training after the operation for lumber intervertebral disc herniation.Method 79 patients after the operation for mono segmental lumber intervertebral disc herniation had been divided into early active training group and routine control group randomly, and accepted training, regular re examination, and follow up of 1~ 6 years respedtively.Results The early active training group had better recent and distant objective effect, and more patients (97.6% ) were satisfied with the operational effects.Conclusions The early active training after the operation for lumber intervertebral disc herniation is positive significant for operational effects.
3.SELECTIVE POSTERIOR RHIZOTOMY (SPR) AND PROGRAMMED TREATMENT FOR SPASTIC LIMBS AFTER SPINAL CORD INJURY

To evaluate the effect of selective posterior rhizotomy and programmed treatment on the relief of limb spasm secondary to the spinal cord injury. The posterior roots of L 2~S 1 were separated from anterior roots and split into funiculi. The funiculi sensitive to lower threshold of electrical stimulation were sectioned. The selective posterior rhizotomy and programmed treatment had been performed on 36 cases, and followed up for 3 years. The results of the treatment were satisfactory. The rate of spasticity relief was 90%, and the rate of functional improvement was 75%. Selective posterior rhizotomy and programmed treatment could be considerered to be the optimal treatment for spasticity of limbs subsequent to spinal cord injury.
4.GAIT ANALYSIS OF SPASTIC LIMBS AFTER TREATMENT WITH SELECTIVE POSTERIOR RHIZOTOMY OF L5-S1 COMBINED WITH SEVERANCE OF ADDUCTOR MUSCLE

The adductor tendon was partially divided. The posterior roots of L 2-S 1 were separated from anterior roots and they were split into fasciculi. The fasciculi showing low threshold to electric stimulation were sectioned. Gait analysis was conducted both preoperatively and postoperatively. This operative strategy was carried out in 16 cases. The result of the treatment was satisfactory after being followed-up for 4 years. Spasticity was reduced by 90 percent and the rate of functional improvement was 80 percent. The results indicated that selective posterior rhizotomy of L 5-S 1, combined with division of addactor muscle was effective in the treatment of spastic lower limbs as a result of brain injury, cerebral palsy, and spinal cord injury.
5.The cloning and sequence analysis of 5′regulatory region of Mal gene of esophageal cancer

Objective To investigate the mechanisms of Mal gene down regulated in human esophageal cancer. Methods We amplified the 5′regulatory region of Mal gene in normal placenta tissue and human esophageal cancer cell lines EC9706、EC8712、EC109 by optimization the PCR methods, cloned and sequenced the PCR products and compare the sequence. Results Human esophageal cancer cell lines EC9706、EC8712、EC109 all have the 654 th of 5′regulatory region of Mal gene T to C changes, corresponding codon changed from TCC to CCC, and the amino acid which it codes changed from serine to proline; EC9706 also has the 657 th of 5′regulatory region of Mal gene T to C changes, corresponding codon changed from TGC to CGC, the amino acid which it codes changed from cysteine to arginine; EC8712 has the 139 th of 5′regulatory region of Mal gene A to G changed, corresponding codon changed from GGA to GGG, the amino acid which it codes remains glycine. Conclusion Our results suggested there are may point mutation in 5′ region of Mal gene in human esophageal cancer.

7.Effect of bacailin on the angiogenic regulating factor and migration of laryngeal cancer cells

Objective To explore the effects of bacailin on the angiogenic regulating factor and migration activity of laryngeal cancer cells.Methods Hep-2 was cultured in vitro,(1)in the groups adding phorbol ester,gelatin-zymogram and Transwell assay were used to observe the effects of bacailin on the gelatinase activity of matrix metalloproteinase (MMP) and the migration ability of Hep-2,respectively; (2)in the group without phorbol ester,enzyme-linked immunosorbent assay (ELISA) was used to detect the effects of bacailin on the protein level of endothelial growth factor (VEGF) and basic fibroblast growth factor (bFGF).Results Among the control group,the low-dose group,the middle-dose group,and the high-dose group,the activity of MMP-3 of each corresponding group was (100.0 ±0.0) %,(95.0 ±4.9) %,(93.1± 6.2)%,and (90.7 ± 7.3)% with statistically significant difference (t =2.123,2.309,2.412,P <0.05) ; the activity of MMP-9 of each corresponding group was (100.0 ± 0.0) %,(92.3 ± 7.5) %,(89.5 ± 9.3) %,and (85.6 ± 6.1) % with statistically significant difference (t =2.253,2.302,3.708,P <0.05 or P <0.01) ;the protein level of VEGF of each corresponding group was (242.7 ±9.5)ng/L,(230.6 ± 12.7)ng/L,(212.1 ± 15.9)ng/L,and (184.2 ± 23.5)ng/L with statistically significant difference (t =2.408,3.733,4.146,P <0.05 or P <0.01) ; the protein level of bFGF of each corresponding group was (190.7±8.2) ng/L,(181.2±13.0) ng/L,(169.9±17.3)ng/L,and (140.7±9.2)ng/Lwith statistically significant difference (t =2.330,2.922,5.145,P <0.05 or P <0.01) ; Cellular migration rate of each corresponding group was (38.3 ± 5.6) %,(32.9 ± 7.1) %,(27.9 ± 4.4) %,and (23.3 ± 6.0) % with statistically significant difference (t =2.141,3.365,4.027,P < 0.05 or P <0.01).Conclusions Bacailin could suppress the migration of Hep-2 through inhibiting expression of angiogenic regulating factor,which might has clinical value in the treatment of laryngeal cancer.
8.The Measurement of Mandibular Thickness and Its Clinical Significance Relative to the Mandibular Plasty by Three-dimensional CT

Objective To measure the mandibular thickness relative to the osteotomy line of mandibular angle plasty so that to provide the anatomical basis for operation.Methods 37 youth women patients mandible were scanned by spiral CT,then the three-dimensional reconstruction was done,the thickness of the mandible around osteotomy line were measured on the planes corresponding to the posterior margin of mandibukar ramus,the middle part of the mandibular ramus,the posterior line of the third molar,the line between the second and third molar,the line between the second and first molar and the line between the first molar and the second premolar.The data were analyzed by Spss 11.5.Results The thichest bone around the osteotomy line was under the second and third molar,then the bone thickness became thinner forward and backward.The thinnest bony was on the middle part of the mandibular ramus.Conclusion The result of this study is of significant for guiding operation and reducing the complications.

BACKGROUND:Standing bending, supine bending, lateral pressing, pronation compression films and traction radiography are indeed effective to predict the correction outcome of surgeries by Harrington's method and Lugue's method,however, they are discontented for the correction of three-dimensional pedicle screw of vertebral arch.OBJECTIVE: To evaluate the role of preoperative X-ray films of gravity-suspending traction, supine bending and fulcrum bending in patients undergoing spinal three-dimensional correction and fusion for adolescent idiopathic scoliosis (AIS).DESIGN: Contrast observation.SETTING: Department of Orthopaedics, the 211 Hospital of Chinese PLA (Military Orthopaedics Center).PARTICIPANTS: From January 2003 to December 2005, 63 cases aged 10-21 years were recruited from Department of Orthopaedics in the 211 Hospital of Chinese PLA, including 18 males and 45 females. They were diagnosed as AIS by clinical examinations and X-ray film, with Cobb angle≥40°. Informed consents were obtained from all the subjects.METHODS: All of the 63 AIS cases with 79 structural curves were studied by preoperative X-ray films of standing,gravity-suspending traction, supine bending and fulcrum bending, which were compared with standing X-ray films one week postoperatively.MAIN OUTCOME MEASURES: Cobb angles of preoperative fulcrum bending film and postoperative standing film.RESULTS: All of the 63 cases were involved in the result analysis. There was no significant difference in the Cobb angle between preoperative fulcrum bending X-ray film and postoperative standing X-ray film (P > 0.05). Gravity-suspending traction and supine bending films were significantly different from postoperative standing film (P < 0.05).CONCLUSION: The fulcrum bending X-ray films are superior to gravity-suspending traction and supine bending films in predicting the postoperative correction of AIS patients.
