AIM: To observe the curative effect of reconstruction of extensor tendon by transplanting heterologous tendon in the treatment of mallet finger deformity. METHODS: Fifteen cases of mallet finger deformity treated by reconstruction insertion of extensor tendon were enrolled at Department of Orthopedics, Xuzhou Hospital Affiliated to Medical College, Southeast University from January 2003 to March 2006. They all participated the observation voluntarily. Two open injury patients were treated with operation in the first emergency phase. The 13 closed injury patients were treated with operation in the first week phase. ①operation method: After brachial plexus anesthesia, an incision was conducted on the back of distal interdigital joint. A bone tunnel was made at 4 mm distance from the digital interphalangeal joint and deviation to the back of distal phalanx. A suit heterologous tendon was passed through the bone tunnel and was sutured with the lateral tendon after crossing on the back of the digital interphalangeal joint (the distal interphalangeal joint in 10?-15? extension by 1.0 mm diameter keith needle). Fixation with plaster splint, function practice of the distal phalanx could be performed when the plaster splint and the keith needle were removed after 6 weeks. Follow-up was done regularly after operation. ②function assessment: The total extension lag angle of metacarpophalangeal joint, proximal articulations interphalangeae, distal articulations interphalangeae at maximal extension position and the distance between finger tip and transverse striation at finger flexion position were measured. 0?indicated excellent flexion and finger tip was over transverse striation at finger flexion; at most -15? indicated good flexion and finger tip touched transverse striation at finger flexion. RESULTS: Totally 15 patients were involved in the result analysis, no drop-out. Postoperative follow up ranged from 2 months to 3 years, with an average of 19.5 months. The wound of all patients was good and no reject reaction. According to Dargan criteria, the overall effective rate was 93.3%. Excellent result was obtained in 12 cases (80%) and good in 2 cases (13.3%). One case was distal interphalangeal joint stiffness and flexion function was not free. CONCLUSION: Reconstruction of extensor tendon is an effective method for mallet finger deformity.

Objective To evaluate and demonstrate ultrasonographic characteristics of mandibular ameloblastoma with active proliferation. Methods Nineteen subjects with ameloblastomas in the mandibles were examined with ultrasonography. In addition to sizes, internal echoes, the research was focused on blood flow of the tumors and destruction of mandibles. Ultrasonographic appearances of the tumors were compared with histopathological findings. Sensitivity and specificity of Doppler flow signals and destruction of mandibles for prediction of active tumor proliferations were calculated. Results Fifteen cases (79%)showed no or minimal flow signals on color Doppler flow imaging(CDFI), whereas the remaining 4 lesions demonstrated abundant flow signals,in which 3 cases were confirmed histopathologically with active tumor proliferations. The sensitivity and specificity of the Doppler flow signals for prediction of active tumor proliferations were 100% and 94%, respectively. Fourteen cases (74%) showed the mandibular cortices were thin but continuous, among the remaining 5 lesions showed local cortices destruction,3 of them were confirmed histopathologically with active proliferations. The sensitivity and specificity of the destruction of cortical bone for prediction of active tumor proliferations were 100% and 88 %, respectively. Conclusions Ultrasonography can be used as a new effective supplementary diagnostic method for mandibular ameloblastomas. CDFI of tumor vascularity and destruction of cortical bone could be used to predict active tumor proliferations and invasiveness.

BACKGROUND: Human acellular amniotic membrane is a kind of extracellular matrix material with good biocompatibility and biological activity. It has been widely used in various clinical studies because of its low immunogenicity, small rejection and easy preparation.OBJECTIVE: To review the applications of human acellular amniotic membranes in tissue engineering field, such as skin, blood vessel, cornea, cartilage and bone.METHODS: CNKI (from January 2005 to May 2017), CBMdisc (from January 2005 to May 2017), PubMed (from January 1990 to May 2017) and Elsevier (from January 1990 to May 2017) were retrieved for articles addressing the application of human acellular amniotic membrane as a tissue-engineered scaffold in the bone, cartilage, skin, and blood vessels. The keywords were acelluar amniotic membrane, scaffold, material, tissue engineering ECM in Chinese and English, respectively.RESULTS AND CONCLUSION: Human acellular amniotic membrane owns the structure and function of the natural extracellular matrix, which can be combined with stem cells from different sources to differentiate into different tissues and organs, such as bone, cartilage, skin, blood vessel, and corneal tissues. As a tissue-engineered scaffold, human acellular amniotic membrane has good biocompatibility, biodegradability and low immunogenicity, although it has some shortcomings, such as poor strength and post-transplantation rejection reactions. Therefore, the future studies are mainly focused on shortening the adhesion time between cells and scaffolds, increasing the own mechanical strength of human acellular amniotic membrane, optimizing the cell growth microenvironment, and combining human acellular amniotic membrane with other tissue-engineered scaffolds.

Journal of Chinese Physician 2022;24(4):566-572
Objective:To clarify the clinical significance, diagnostic and prognostic values of circular RNA circ_0141633 and circ_0008234 in peripheral blood of pancreatic cancer, and analyze their impact on the biological function of pancreatic cancer cells. Methods:The peripheral blood samples of 97 pancreatic cancer patients and 71 healthy controls were collected, and the expression of circ_0141633 and circ_0008234 was analyzed by real-time quantitative polymerase chain reaction (qRT-PCR). The relationships between the expression of circ_0141633 and circ_0008234 and clinicopathological characteristics and prognosis of pancreatic cancer were analyzed by chi-square test, K-M survival curves and Cox proportional hazards regression model. The area under curve (AUC), sensitivity and specificity of circ_0141633 and circ_0008234 in the diagnosis of pancreatic cancer were analyzed by receiver operating characteristic (ROC) curve. The effects of circ_0141633 and circ_0008234 on the proliferation, migration, invasion and epithelial mesenchymal transformation (EMT) of Bxpc-3 cells were analyzed by methyl thiazolyl tetrazolium (MTT) method, cell scratch test, Transwell invasion and Western blot. Results:The expression of circ_0141633 and circ_0008234 in peripheral blood of pancreatic cancer patients was higher than those of healthy controls (all P<0.05). High expression of circ_0141633 and circ_0008234 was associated with higher clinical stage, lymph node metastasis and venous invasion, and were independent risk factors for poor prognosis in pancreatic cancer (all P<0.05). The overall survival rate of patients with high expression of circ_0141633 and circ_0008234 was significantly lower than that of patients with low expression of circ_0141633 and circ_0008234 (all P<0.05). The AUC of circ_0141633, circ_0008234, the combination of circ_0141633 and circ_0008234, CA19-9 in the diagnosis of pancreatic cancer was 0.70, 0.67, 0.88 and 0.82, with sensitivity of 64.32%, 60.79%, 78.22% and 73.97%, respectively. The specificity was 68.54%, 65.46%, 81.65% and 79.41%, respectively. The diagnostic efficiency of combination was superior to CA19-9, circ_0141633 and circ_0008234 alone (all P<0.05). Interfering with circ_0141633 and circ_0008234 alone could inhibit the proliferation, migration, invasion and EMT of Bxpc-3 cells, and the above inhibitory effect was more obvious after interfering with both of circ_0141633 and circ_0008234 (all P<0.05). Conclusions:The high expression of circ_0141633 and circ_0008234 in peripheral blood could be used as potential diagnostic and prognostic biomarkers of pancreatic cancer, and could promote the progression of pancreatic cancer.
