1.Use of Omaha system in community nursing and transitional care

The authors have applied the Omaha system in six different researches conducted in Hong Kong and mainland China in the last ten years. These researches have been conducted using different client groups,specialty nurses,and in the hospital,clinic and community settings. The Omaha system was found to be instrumental in serving as a conceptual framework for study,designing the intervention protocol,identifying clients' health problems,defining nursing interventions and evaluating research outcomes. The Omaha system is a valuable tool for research and practice and is worth exploring further for its application in mainland China,particularly in the areas of community nursing and transitional care.
2.Dynamic changes in ATP-binding cassette transporter A1 in peripheral blood monocytes of patients with type 2 diabetes mellitus

Objective To analyze the dynamic changes in blood lipids and ATP-binding cassette transporter A1 (ABCA1) in peripheral blood monocytes of the patients with type 2 diabetes, and explore the correlations between ABCA1 expression and blood lipids levels and glycometabolism. Methods Human monocytes were isolated from the patients with type 2 diabetes (n=40) and from the healthy individuals (n=40) serve as normal control. ABCA1 was labeled by immune reaction PE fluorescence, then the changes in ABCA1 expression before and after being incubated with the ox-LDL were investigated by flow cytometer. The blood lipid levels, fasting plasma glucose (FPG) and glycosylated hemoglobin (HbA1c) levels in the patients of type 2 diabetes group were determined, and the correlations between ABCA1 and age, blood lipid, and glycometabolic levels were analyzed. 3.Effect of ox-LDL on the expression of ATP-binding cassette transporter A1 in monocyte of patients with coronary heart disease

Objective To investigate the expression level of monocytes ATP-binding cassette transporter A1(ABCA1) stimulated by oxidized low-density lipoprotein (ox-LDL) in patients with coronary heart disease (CHD) for exploring the mechanism of ABCA1 in the pathogenesis of atherosclerosis (AS). Methods Human monocytes of patients in CHD group (n=42) and normal control group (n=40) were isolated by density gradient centrifugation with separating medium (1.077). ABCA1 was labeled by immune reaction PE fluorescence, then the expression of ABCA1 before and after being incubated with the ox-LDL (30mg/L) for 24 hours was investigated by flow cytometer, and the changes in ABCA1 expression were observed. The differences between the two groups and the correlation between ABCA1 expression and blood lipids were analyzed. Results The high-density lipoprotein cholesterol (HDL-C) and apolipoprotein A1 (apoA1) levels in CHD group (1.15?1.40 and 1.19?0.17, respectively) were significantly lower than that in control group (1.36?0.40 and 1.34?0.22, respectively, P0.05). Conclusions ABCA1 expression in the patients with CHD may be inhibited by high concentration of ox-LDL. The lowered expression of ABCA1 and the abnormal lipids spectrum may decrease the reverse cholesterol transport (RCT), and contribute to the development of AS and CHD.

Objective:To explore the SPECT detection method of quality control in detection of main performance index. Methods: according to the 2007 edition of SPECT performance measurementof the NEMA (the United States Electrical Manufacturers Association) standard, and the corresponding model of intrinsic spatial resolution, intrinsic spatial non-linearity, intrinsic non-uniformity, characteristic count rate, system planar sensitivity, system spatial resolution, fault spatial resolution and other indicators. Results: after quality control testing, have deviation more index hospitals SPECT results. Conclusion: the hospital should detect quality control as far as possible in nuclear medicine and nuclear medicine imaging equipment, ensure the normal operation of equipment, and improve the quality of image.

AIM: To investigate the pentanucleotide repeat(PNR) polymorphism of apolipoprotein(a) in patients with coronary heart disease(CHD) in Hubei area, and evaluate the association of polymorphism of apo(a) PNR with the level of serum lipid. 6.Comparison of radiobiological models for evaluation of radiotherapy treatment planning of breast cancer

Objective To find an appropriate r3diobiological model for analyzing the biological effect of the radiotherapy for breast cancer by comparing different results computed by various types of radiobiological models. Methods DVHs database simulating breast conserving radiotherapy was set up,based on clinical DVHs data of the heart.the lung and PTV of 22 patients with early breast cancer given conventional tangential radiotherapy.Two models assessing NTCP of radiation pneumonitis and cardiac mortality and four models assessing TCP were compared by analyzing the effects of the parameters and DVH database input methods on the results. Results When mean irradiation dose of the whole lung was 30 Gy.the incidence of radiation pneumonitis was 32%and 54%predicted by NTCP-RSM model and NTCP-Lyman model,respectively.When 1%cardiac mortality of radiation was assumed,28 Gy and 40 Gy isodose should cover the heart assessed by the two models.The mean TCP were 21.1%.80.8%.38.4%and 41.0%when assessed by LQ-Poisson-TCP,Zaider-TCP,Poisson-TCP and Logit-TCP models,respectively.Setting various differential DVH(dDVH)bins had very few effect on the NTCP/TCP results in a certain model.Adopting physical dose or LQED2 affected the results with greater resu]ts for physical dose.Variation in α or β value,tumor cell density and D50 had significant effect upon TCP results in LQ-Poisson-TCP(P:0.000). Conclusions NTCP-Lyman model is better for predicting the incidence of radiation pneumonitis while NTCP-RSM model is better for predicting radiation-induced cardiac mortality.LQ-Poisson-TCP is the best TCP model for clinical application.Parameters selected for model can significantly affect the results.It is imporrant to understand the distinct characteristics of different models.
7.Comparison of isolation ways of MSC and VEGF and SDF-1αconcentration in respective culture medium

Objective To choose one protocol that can quickly ,safely and effectively provide amount enough of bone marrow derived mesenchymal stem cells(MSCs) for use of clinical or experimental test through comparison of their growth characteristics and growth factors levels in culture solution .Methods Cells extracted from bone marrow of C57BL/C mice respectively underwent two different isolation protocols :whole bone marrow adherent culture(WBMAC) or gradient density separation(GDS);characteris‐tic surface antigens of MSCs were identified by flow cytometry on cells isolated in different ways ;the distinct growth curve of pri‐mary stem cells cultured in vitro described their different proliferation rate;levels of vascular endothelial growth factor(VEGF) and stromal cell‐derived factor‐1α(SDF‐1α) in culture medium were detected by ELISA .Results Primary MSCs obtained by WBMAC proliferated at higher speed and exhibited shorter growth cycle than those separated by GDS ;on MSCs from both groups ,surface antigens CD29 were detected positively ,and antigens including CD31 ,CD34 and CD45 were assayed negatively ;concentration of VEGF and SDF‐1αin both two nutrient solution primarily keep at low levels ,comparatively ,level of VEGF and SDF‐1αin the di‐shes which contain MSCs by WBMAC was higher than the one in the dishes which contain MSCs by GDS .Conclusion MSCs ex‐tracted by WBMAC shows unimpaired cell function ,can build automatically more suitable microenviroment for their growth;this classic method was qualified for clinical and experimental use in a safe ,rapid ,effective way .
8.Mechanism of rosiglitazone in promoting the reverse cholesterol transport in atherosclerotic rabbits

Objective To investigate the effects and anti-atherosclerotic mechanism of rosiglitazone on the expression of ATP-binding cassette transporter A1 (ABCA1) and reverse cholesterol transport (RCT) in atherosclerotic rabbits. Methods Twelve rabbits were randomly divided into two groups (6 each): control group (only high cholesterol diet for 6 weeks), rosiglitazone group [high cholesterol diet plus rosiglitazone 0.5mg/(kg?d) for 6 weeks]. ABCA1 expression and [3H] cholesterol efflux rates were evaluated by flow cytometry and liquid scintillation spectrometry, respectively. Enzymatic methods were used to assay serum lipids levels and cholesterol contents in tissues, and the atherosclerotic area of aorta was calculated by professional image analysis software. Results For the rabbits of both control and rosiglitazone group, the serum levels of high-density lipoprotein cholesterol (HDL-C), non-high-density lipoprotein cholesterol (NHDL-C) and apolipoprotein A1 (apoA1) significantly went up when they took their cholesterol rich diet for 6 weeks (P0.05). OBJECTIVE:
To investigate the treatment for benign tumor of external auditory canal by carbon dioxide laser under microscope.
METHOD:
Ten cases of benign tumor of external auditory canal were treated by carbon dioxide laser under microscope. The curative effects and complications were observed.
RESULT:
Ten cases of benign tumor of external auditory canal were satisfied after operation without any complications. There were no recurrences during 3 months to 2 years of follow up.
CONCLUSION
The operation for benign tumor of external auditory canal by carbon dioxide laser under microscope was easy, safe and effective.

10.Effects of moxibustion on behaviors and tryptophan metabolism-related products in mice with irritable bowel syndrome

Objective: To observe the effect of moxibustion on behaviors and related products of tryptophan (Trp) metabolism in the colon of mice with irritable bowel syndrome (IBS), and to explore the mechanism of moxibustion in the IBS treatment.Methods: Twenty-four mice were randomly divided into a normal group, a model group, a moxibustion group, and a probiotic group, with 6 mice in each group. The visceral pain model of IBS was established by enema with 2,4,6-trinitrobenzene sulfonic acid (TNBS) solution. Mice in the moxibustion group were treated with mild moxibustion at bilateral Zusanli (ST36), and those in the probiotic group were treated with probiotics such as Bifidobacterium by gavage. Abdominal withdrawal reflex (AWR) test, elevated plus-maze (EPM) test, and forced swimming test (FST) were performed after treatment. The expression levels of 5-hydroxytryptamine (5-HT) and tryptophan hydroxylase 1 (TPH1) in the colon were detected by immunofluorescence, and the expression levels of Trp, kynurenine (Kyn), and indole-2,3-oxygenase (IDO) in the colon were detected by enzyme-linked immunosorbent assay. Results: Compared with the normal group, the AWR scores were increased significantly in the model group under different pressure values (P<0.01), the open-arm staying time and open-arm entries in the EPM test were decreased significantly (P<0.01, P<0.05), the motionless time in the FST was increased significantly (P<0.01), and the expression levels of colonic Trp, TPH1, IDO, 5-HT, and Kyn were increased significantly (P<0.01) in the models. Compared with the model group, the AWR scores were differently decreased (P<0.05 or P<0.01), the open-arm entries in the EPM test were increased (P<0.05), the motionless times in the FST were decreased (P<0.05), and the colonic expression levels of Trp, TPH1, IDO, and 5-HT were decreased (P<0.01 or P<0.05) in the moxibustion and probiotic groups; the open-arm staying time was significantly increased in the moxibustion group (P<0.01), and the colonic expression level of Kyn was significantly decreased in the probiotic group (P<0.01). Conclusion: Moxibustion at Zusanli (ST36) improves visceral pain and pain mood and down-regulates the expression levels of colonic TPH1, IDO, Trp, 5-HT, and Kyn in IBS mice.
