1.COMPARISON OF EXPERIMENTAL MICROSUTURE OF EPINEURIUM AND PERINEURIUM IN RESTORING PERIPHERAL NERVES FUNCTION

Under the operating microscope an experiment of epineurium suture by comparison with perineurium suture was performed in sciatic nerves of 20 rabbits. "Strength-duration curve" of tibialis posterior muscle and axons growth rate of distal myelinated fiber were used as the viewing indexes. The end result was showed that there were no obvious differences between both methods in restoring peripheral nerves function. Funicular orientation by direct stimulation was advocated. A new instrument of measuring "Strength-duration curve" was produced.
2.The effect of ketamine on the contents of the excitatory amino acid transmitters and calcium in rabbit brain tissue after rabbit cerebral ischemia and reperfusion

Cerebral ischemia models of rabbits were induced by four-artery obstruction method. The changes of excitatory amino acid (EAA) transmitters,brain-tissue calcium (TCa) and intramitochondria calcium (MCa) of the cerebral cortex were measured and the effects of ketamine were observed. The results revealed that 20 minutes after ischemia,the amoumts of the EAA transmitters (Glu and Asp) in brain tissues were decreased, and decreased further after reperfusion. But after 20 minutes ischemia,the levels of TCa and MCa were elevated and higher than that of ischemia and contral groups significantly after the reperfusion. Ketamine could increase the Glu and Asp contents and decrease the TCa and MCa levels during ischemia and reperfusion.
3.Cell growth and alkaloid synthesis in suspension cultures of Datura metel

Object To investigate the synthesis of alkaloids by cell suspension cultures of Datura metel L. Methods Cultured cells derived from leaves of D. metel were m aintained in Murashige and Skoog (MS) liquid medium. The effects of different c arbohydrated, different inoculum sizes, and different pH values of medium as aff ecting facters on cell growth and alkaloid formation were studied. Re sults The optimum carbohydrate was sucrose, inoculum size (1.5 - 2. 0 g/L) for fresh weight and initial pH values (5.5-6.0) of the medium were benefit for cell growth and alkaloid synthesis in the suspension cultures. The synthesis of alk aloids was improved when exogenous phenylalanine added to the culture. Conclusion Suspension-cultured cells of D. metel c an be used to synthesize alkaloids.

5.Morphologic effect of huangjiao granule on apoptosis of cerebral neurocytes in elderly rats with middle cerebral arterial occlusion

BACKGROUND: Huangiao granule is pure Chinese drugs pharmaceu-tics, which can eliminate heat by purgatingfu-organs, clear unconscious-ness and stop endogenous wind as well as promote blood flow and removeblood stasis etc.OBJECTIVE: To observe the effect of huangjiao granule on apoptosis ofneurocytes in rat models with middle cerebral arterial occlusion, and probeinto the possible mechanism of huangjiao granule in the treatment of acutecerebral infarction.DESIGN: Complete randomized grouping design, control animal experi-ment.SETTING: Department of Neurology, Affiliated Hospital of TraditionalChinese Medicine Combined with Western Medicine, Tongji Medical Col-lege of Huazhong University of Science and Technology.MATERIALS: The experiment was conducted in the Central Laboratory,Hubei College of Traditional Chinese Medicine. Sixty old Wistar rats aged22 months were selected. After 3-day-feeding for adaptability, rats wererandomized into 6 groups according to body mass: Normal control group,model group, nimodipine gorup, 10 g/kg huangjia ogranule group and 5 g/kghuangjiao granule group with 10 rats in each group.METHODS: ①Rats were made into cerebral ischemic models with mid-dle cerebral arterial occlusion method. Thirty minutes before modeling, ratsin the 10, 5, 2.5 g/kg huangjiao granule (main gredients were radix et rhi-zoma rhei, cornu bubali etc. provided by Manufacturing Laboratory ofWuhan First Hospital with the batch number of 021120) groups respec-tively received gastric perfusion of 1, 0.5, 0.25 g/mL huangjiao water so-lution containing crude drug at the dose of 10, 5, 2.5 g/kg with each rats perfused once; Rats in the nimodipine group received gastric perfusion of1 mg/mL nimodipine water solution, containing crude drug at the dose of 10 mg/kg with each rats perfused once; Rats in the model group and normal control group received gastric perfusion of normal saline at the dose of 10 mL/kg with each rats perfused once. ② Apoptotic stainning sections in situ were observed under light microscope, 5 fields were observed under high power (20×10). Observed the number of mean positive cells of each highpower field, and then caculated the percentage of apoptosis. Histologic changes of local and peripheral ischemic cortexes were observed under light microscope. Changes of ultramicrostructure of cells in cerebral cortexes of rats were observed under electron microscope. ③Differences of measurement data were compared with F test and Q test.MAIN OUTCOME MEASURES: Morphologic effects of huangjiao granule on apoptotic rate of neurocytes in brain tissues of old rats with middle cerebral arterial occlusion (MACO) and cells in cerebral cortexes.RESULTS: A total of 60 Wistar rats were involved in the analysis of results. ①Number of apoptotic positive cells in brain tissues and apoptotic rate in rats of model group were significantly higher than those in the normal control group (P < 0.01); Those in each huangjiao granule groups and nimodipine group were obviously lower than those in the model group (P < 0.01); Differences in numbers of apoptotic positive cells in brain tissues and apoptotic rates in rats of 10 g/kg huangiao group and nimodipine groupwere not obvious (P < 0.05). Number of apoptotic positive cells in brain tissues and apoptotic rate in rats of 10 g/kg huangjiao granule group were markedly lower than those in the 5, 2.5 g/kg huangjiao granule groups (P < 0.05, 0.01).②Observation with light microscope and electron microscope showed that there were obvious apoptosis in local cells of brain tissues in rats of model group, which can significantly ameliorate by haungjiao granule.CONCLUSION: Huangjiao granule has some antagonistic effects on acute ischemic apoptosis of neurocytes, which may be one of the possible mechanisms of huangjiao granule in the treatment of acute cerebral infarction.
6.Arterial angiographic embolization in treatment of severe pelvic fractures

Pelvic fractures are high energy injuries involving severe trauma and potential intra pelvic bleeding. Immediate control of hemorrhage, fixation of pelvic fractures and inter department cooperation are important for their treatment. Transcatheter angiographic embolization can be used to identify bleeding sites, control hemorrhage and improve mortality and complications, and it has been proved to be a valuable technique in the diagnosis and treatment of pelvic vascular injuries. Still there is a considerable controversy on the fracture patterns, the sequence and the timing of emergency angiography and possible embolization in these patients. In this paper, we reviewed the role of angiographic embolization and we thought more attention should be paid on angiographic embolization in the control of hemorrhage.
7.Effect of ischemia-reperfusion on calcium accumulation and ultrastructure alteration in rabbit brain.

Using the model of the rabbit brain ischemia the intracellular calcium accumulation and ultrastructure alteration of cerebral cortex were observed. The results indicated that the longer the ischemia reperfusion time,the more was the calcium accumulation and ultrastructure alteration in cerebral cortex. It is revealed that calcium plays an important role in the brain Injury induced by ischemia-reperfusion. The mechanism of brain injury related to calci- um was diseussed.
8.Impact of the implementation of Malpractice-handling Regulations on hospitals and strategies in response

The implementation of the Malpractice handling Regulations, which has set new and higher demands on medical institutions and medical workers, will have a certain impact on hospital administration and patient care. Medical institutions and medical workers must be highly aware of this. The entire staff of each hospital must study the Regulations carefully and try to obtain an objective understanding of it. Management of medical institutions must be strengthened, weak links identified, measures formulated and regulations perfected. Medical workers ought to strive for better professional skills, and supervision and management of medical records ought to be intensified.
9.Recurrent lumbar disc herniation after discectomy alone:long-term results

[Objective]To evaluate the long-term results of recurrent lumbar disc herniation between different kinds of reoperation.[Method]Seventy-four patients of recurrent lumbar disc herniation after discectomy alone were treated surgically from May 2001 to Augest 2005.They were divided into three groups:nonfusion,posterolateral fusion and intervertebral fusion.After a long term follow-up(≥2y),their clinical data were reviewed to compared their improvement rate of lower back pain,JOA score,subside rate of disc height and chang rate of superior intervertebral space angle.[Result]The long-term results revealed that the improvement rates of lower back pain JOA score in the three groups were increased in sequence.The differences in subside rate of disc height and change rate of superior intervertebral space angle between discectomy and posterolateral fusion groups were insignificant,but much higher than those of posterior lumbar interbody fusion group.[Conclusion]Posterior lumbar interbody fusion is effective in increasing the improvement rates of lower back pain JOA score and maintaining the disc height and the superior intervertebral space angle.
10.Lumbar Spinal Stenosis Associated with Disc Protrusion

Sixty-eight patients with a surgically proven lumbar stenosis associated with disc protrusion were studied retrospectively. According to the essential clinical feature the patients were divided into three groups: Group 1, intermittent claudication (n=21), Group 2, low back pain associated with unilateral aciatic pain (n=42), and Group 3, intermittent claudication coexisting with bilateral or unilateral leg pain (n=5). The final diagnosis prior to operation was difficult to make. Surgical decompression of the spinal canal required removal of the portion of hypertrophied superior articular faced and prolapsed disc. Fifty-eight patients were followed up from 6 months to 6 years and 3 months. In 41 patients preoperative neurogenic symptoms completely disappeared, 14 still an occasinal bachache, existed and 3 moderate symptoms persisted postoperatively.
