1.Optimization of Extraction Process and Inclusion Technology of Volatile Oil in Huangpu Tongqiao Capsules by Orthogonal Design

Objective To optimize the extraction and inclusion process of mixed volatile oil of Rhizoma acori graminei, Fructus alpiniae oxyphyllae and Rhizoma chuanxiong in Huangpu Tongqiao Capsules. Methods With steam distillation method, extraction amount as indicator, the orthogonal experiment was used to optimze the extraction process of volatile oil from three factors such as water amount, soak time and extraction time. With saturated aqueous solution method, co-inclusion compound yield and volatile oil rate as indicators, the orthogonal experiment was used to optimize the inclusion process from three factors such as the proportion of volatile oil and β-CD, inclusion time and inclusion temperature. Results The optimum extraction was:adding eight times amount of water and soak for 2 hours to extract 7 hours. The optimum inclusion process was:the proportion of volatile oil andβ-CD was 1∶10, the inclusion time was 3 hours, the inclusion temperature was 40 ℃. Conclusion Optimal extraction process of oil has higher rate, and the volatile oil inclusion rate of inclusion process was also higher, the process is stable.
2.Study on Quality Control Methods of Yuling Tea

Objective To establish the quality standard of Yuling Tea for its quality control. Methods Astragalus, Fructus cnidii L, Schisandra chinensis and Lycii Fructus in Yuling Tea were identified by TLC. The content of Schisandrin was determined by HPLC. Welch Materials C18 column (250 mm×4.6 mm, 5 μm) with mobile phase of methanol-water (70∶30) was used. The detective wavelength was 250 nm. Results The TLC for identification was simple and special. Schisandrin showed a good linear relationship in 0.222-0.222 0 μg, r=0.999 9. The average recovery was 98.96%, and RSD was 0.74%. Conclusion The method can accurately determine the content of Schisandrin in Yuling Tea. It can be used for quality control of the preparation.
3.Bladder mucosa lymphoma: clinical and pathological characteristics and treatment

Objective To analyze the clinicopathologic characteristics and treatment of primary and secondary bladder mucosa lymphoma. Methods From June 1996 to September 2011,8 cases of bladder mucosa lymphoma patients including primary bladder mucosa lymphoma patients (2 males and 3 females)and secondary bladder mucosa lymphoma patients ( 1 male and 2 females) were treated in our institute.The patient's mean age was 68 years (ranged from 57 to 78 years),and the other 3 secondary bladder mucosa lymphoma patient's mean age was 61 years (ranged from 41 to 81 years).7 of 8 patients accepted transurethral resection of bladder tumor combined with CHOP chemotherapy and radiotherapy,and 1 patient abandoned treatment.CHOP chemotherapy and radiotherapy were performed at 2 weeks post-operatively.Secondary bladder mucosa lymphoma patients accepted 3-dimensional conformal 6000 cGy radiotherapy.Urinary CT examination showed that there were round or oval irregular tumors in the wall of urinary bladder,whose diameters were 1 to 4 cm.Tumor edges were glossy and tumor density were uniform.In addition,the bladder wall with tumor was uneven.In contrasted CT,the tumors appeared obviously enhanced.7 cases had cauliflower-like tumors found by cystoscope,and there were necrosis-calcification area in the center of tumors.1case appeared multiple tumors,which consisted of one big tumor and three smaller ones. Results There were 5 cases of primary bladder mucosa lymphoma, including 3 bladder mucosa associated lymphoma (MAL) cases and 2 diffuse large B cell lymphoma cases.There were 3 secondary bladder mucosa lymphoma cases including 2 diffuse large B cell lymphoma cases and 1 generalized follicular cystic central lymphoma case,which belongs to the tuberous sclerosis Hodgkin lymphoma.Immunohistochemical analysis showed that CD20,CD21,CD43 staining were positive in low grade MAL (3 cases).However,CD3,CD20,CD31 staining were positivein high grade MAL (5 cases). All patients were followed up 3 to 44 months.3 cases of secondary malignant lymphoma patients died after diagnosed after 3 to 13 months.5 cases of primary malignant lymphoma patients were all survived. 1 case recurred 14 months after surgery and accepted TURBT and CHOP chemotherapy again.Pathologic diagnosis showed the transformation from peripheral zone of bladder mucosa associated lymphoma to diffuse large B cell lymphoma.The patient was free of recurrence. Conclusions MAL and diffuse large B cell lymphoma are the most common type of bladder mucosa lymphoma.Primary and secondary diffuse large B cell lymphoma have similarities in histological analysis,but the former prognosis is better than the latter's. Transurethral resection of bladder tumor combined with chemotherapy and radiotherapy can prevent the relapse of MAL in short term and long term with good tolerance,and can be used as ideal treatment of MALT.
4.Predictors for HBeAg negative chronic hepatitis patients with steatosis and its value in diagnosis of hepatic inflammation and fibrosis

Objective To investigate the relationship between clinical presentation and pathological characteristics in HBeAg negative chronic hepatitis B(CHB) patients with steatosis, and to find out the predictors of hepatic inflammation and fibrosis. Methods HgeAg negative CHB patients with (n=56) or without (n=60) steatosis confirmed clinically and pathologically were enrolled in the study. All patients were examined for fasting blood glucose(FBG), fasting insulin (FINS), triglyceride (TG), cholesterol (TC), alanine aminotransferase (ALT), aspartate aminotransferase (AST), gamma-glutamyhransferase (GGT), alkaline phosphatase (ALP) albumin (Alb), globulin(Glb), homeostatic model assessment of insulin resistance (HOMA-IR), HBV-DNA and body mass index(BMI). The association of above parameters with hepatic inflammation, fibrosis and fatty deposition were analyzed statistically. Results It was demonstrated that BMI, FBG, FINS, TG, TC, GGT, ALP , Glb and HOMA-IR were significantly higher in HBeAg negative CHB patients with steatosis than those without steatosis (P<0.05). Whereas the levels of HBV-DNA, Alb, ALT and AST were significantly lower in HBeAg negative CHB patients with steatosis compared with those without steatosis (P<0.05). The hepatic inflammation and fibrosis were aggravated in patients with steatosis. It was implicated that BMI,FBG, FINS, TG, TC, GGT and HOMA-IR(all P values 0.05) were significant predictors for hepatic steatosis, while ALT, AST, Glb and HBV-DNA(all P values <0.05) were significant predictors for hepatic inflammation. And the predictors for hepatic fibrosis were ALT, AST, Alb, Glb and HBV-DNA(all P values <0.05). Conclusions Hepatic steatosis is common in HBeAg negative CHB patients which is positively associated with parameters including BMI, FBG, FINS, TG, TC, GGT, ALP and HOMA-IR. Besides steatosis, the hepatic inflammation and fibrosis are also aggravated in these patients.

7.The correlation analysis between the neutrophil-lymphocyte ratio and systemic lupus erythematosus viscera involvement and disease activity

Objective To investigate the correlation between neutrophil-lymphocyte ratio (NLR) and disease activity of systemic lupus erythematosus (SLE),and the changes of NLR in different organ involvement of SLE patients.Methods A total of 155 SLE patients and 135 healthy controls from the Rheumatology Department of Xiangya Hospital were enrolled in this study from 2010 to 2018.Patients with SLE were divided into lupus nephritis group (LN group) and non-lupus nephritis group (non-LN group),serositis group and non-serositis group,according to whether they had kidney involvement or serositis.According to the SLE disease activity index 2000(SLEDAI-2000),the patients were divided into mild to moderate disease activity group (SLEDAI score < 15) and severe disease activity group (SLEDAI score≥ 15).The NLR values of the above groups were compared.Spearman's correlation analysis was used to analyze the correlation between NLR and SLE patients' laboratory indexes.Multiple linear regression model was used to analyze the relationship between NLR and SLE disease activity.Receiver operating characteristic curve (ROC) was used to evaluate the value of NLR in SLE diagnosis and activity assessment.Results (1)The NLR value of SLE patients was significantly higher than that of healthy control group,and the difference was statistically significant (P < 0.01).(2)The NLR value of SLE patients in the LN group was higher than that in the non-LN group,and the NLR value of SLE patients with serositis was higher than that in the group without serositis,with statistically significant differences (both P < 0.05).(3)The NLR value of SLE patients in the severe disease activity group was higher than that in the mild and moderate disease activity group,and the difference was statistically significant (P < 0.01).(4)NLR of SLE patients was positively correlated with CRP (rs=0.188,P=0.019),SLEDAI score (rs=0.264,P=0.001),and negatively correlated with total serum protein (rs=-0.250,P=0.002) and serum albumin (rs=-0.329,P < 0.001),respectively.(5) Multiple linear regression showed that NLR was independently associated with SLE disease activity (B=0.351,95%CI 0.012-0.690,t=2.047,P=0.042).(6) According to ROC curve,the optimal cut-off value of NLR for SLE diagnosis was 2.17 (sensitivity 60.0%,specificity 83.1%,AUC=0.744),and the best cut-off value for predicting the activity of severe disease activity in SLE patients was 3.28 (sensitivity 58.5%,specificity 78.1%,AUC=0.700).Conclusion NLR is closely related to renal involvement,serositis and disease activity in SLE patients,which indicates that NLR,as a new inflammatory indicator,is of great significance for the assessment of SLE disease activity and organ involvement.
8.Expression and prognostic significance of epithelial cell transforming sequence 2 in invasive breast cancer

Objective:To investigate the expression and prognostic significance of epithelial cell transforming sequence 2 (ECT2) in invasive breast cancer.Methods:The clinical data of 165 cases of invasive breast cancer treated in the 909th Hospital of Joint Logistics Support Force from January 2018 to June 2021 were retrospectively analyzed, the tumor tissues and corresponding normal tissues adjacent to the cancer were collected. The tumor tissues of breast cancer and adjacent normal tissues were stained with ECT2 immunohistochemistry. The relationship between the expression of ECT2 protein in different tissues and pathological and clinical data was analyzed. Multivariate Logistic regression was used to analyze the factors affecting the high expression of ECT2 protein. Kaplan-Meier method was used to compare the total survival time of breast cancer patients with high expression of ECT2 and low expression of ECT2 by Kmplot online survival analysis of public databases.Results:The expression of ECT2 in breast cancer tissues was significantly higher than that in adjacent normal tissues: 57.58% (95/165) vs. 12.73% (21/165) ( χ2 = 76.19, P<0.01). The patients with high expression of ECT2 had worse histopathological grade: grade G 1 30.53% (29/95) vs. 57.14% (40/70), grade G 2 40.00% (38/95) vs. 28.57% (20/70), grade G 3 29.47% (28/95) vs. 14.20% (10/40), higher lymph node metastasis: 71.58% (68/95) vs. 7.14% (5/70), worse TNM stage: stage Ⅰ 15.79% (15/95) vs. 34.29% (24/70), stage Ⅱ 27.37% (26/95) vs. 27.14% (19/70), stage Ⅲ 41.05% (39/95) vs. 28.57% (20/70), stage Ⅳ 15.79% (15/95) vs. 10.00% (7/70), higher proportion of menopause: 60.00% (57/95) vs. 44.29% (31/70) and higher Ki-67 proliferation index: 74.74% (71/95) vs. 58.57% (41/70) ( P<0.05). Lymph node metastasis ( OR = 2.764, P = 0.038), histopathological grade G 3( OR = 1.942, P = 0.010), TNM stage Ⅳ ( OR = 2.586, P<0.001), Ki-67 proliferation index greater than 14% ( OR = 1.376, P = 0.006) were the influencing factors of high expression of ECT2 protein in breast cancer patients. The overall survival of ECT2 patients with high expression of breast cancer was even worse ( P<0.05). Conclusions:The expression of ECT2 protein is increased in invasive breast cancer. The prognosis of breast cancer patients with high expression of ECT2 is even worse. ECT2 may be a potential molecular target for breast cancer treatment.
9.Analysis on Material Basis of Anti-COPD Effect of Euphorbia helioscopia Based on Serum Pharmacochemistry and Network Pharmacology

ObjectiveTo analyze the migrating components absorbed into blood of the aqueous extract of Euphorbia helioscopia, and to explore the pharmacodynamic material basis of the aqueous extract of E. helioscopia against chronic obstructive pulmonary disease(COPD). MethodUltra-performance liquid chromatography-quadrupole-time-of-flight mass spectrometry(UPLC-Q-TOF-MS/MS) was used to detecte the migrating components absorbed into blood of rats after intragastric administration of aqueous extract of E. helioscopia. An Agilent RRHD SB-C18 column(3 mm×100 mm, 1.8 μm) was used with 0.1% formic acid aqueous solution(A)-acetonitrile(B) as the mobile phase for gradient elution(0-15 min, 5%-30%B; 15-20 min, 30%-50%B; 20-30 min, 50%-95%B; 30-35 min, 95%-5%B), and the detection wavelength of 190-800 nm, column temperature of 40 ℃, flow rate of 0.3 mL∙min-1 and injection volume of 4 μL. The electrospray ionization(ESI) was used in positive and negative ion modes, and the detection range was m/z 50-1 250. Network pharmacology was used to screen out the key components and the key targets of COPD through the interaction analysis. Metascape database was used to predict the molecular function, biological process, cellular composition and signal pathways mainly involved in the anti-COPD effect of E. helioscopia. Molecular docking technique was used to determine the affinity of key targets with key components. ResultA total of 29 migrating components absorbed into blood of rats were identified after intragastric administration of aqueous extract of E. helioscopia, 9 of which were prototype components and 20 were metabolites. Network pharmacological analysis showed that luteolin, quercetin, apigenin, naringenin and helioscopinolide C were the key components of E. helioscopia against COPD, and vascular endothelial growth factor A(VEGFA), albumin(ALB), protein kinase B1(Akt1), tumor necrosis factor(TNF) and interleukin-6(IL-6) were the key targets. Molecular docking results showed that one diterpene lactone(helioscopinolide C) and three flavonoids(naringenin, luteolin, apigenin) in the migrating components absorbed into blood all had strong binding activity to the key targets of E. helioscopia against COPD. ConclusionNaringenin, helioscopinolide C, luteolin and apigenin may be the main anti-COPD active substances of E. helioscopia.
10.Identification of Dalbergia odorifera and Its Counterfeits by HS-GC-MS

ObjectiveTo screen the differential markers by analyzing volatile components in Dalbergia odorifera and its counterfeits, in order to provide reference for authentication of D. odorifera. MethodThe volatile components in D. odorifera and its counterfeits were detected by headspace gas chromatography-mass spectrometry(HS-GC-MS), and the GC conditions were heated by procedure(the initial temperature of the column was 50 ℃, the retention time was 1 min, and then the temperature was raised to 300 ℃ at 10 ℃ for 10 min), the carrier gas was helium, and the flow rate was 1.0 mL·min-1, the split ratio was 10∶1, and the injection volume was 1 mL. The MS conditions used electron bombardment ionization(EI) with the scanning range of m/z 35-550. The compound species were identified by database matching, the relative content of each component was calculated by the peak area normalization method, and principal component analysis(PCA), orthogonal partial least squares-discrimination analysis(OPLS-DA) and cluster analysis were performed on the detection results by SIMCA 14.1 software, and the differential components of D. odorifera and its counterfeits were screened out according to the variable importance in the projection(VIP) value>2 and P<0.05. ResultA total of 26, 17, 8, 22, 24 and 7 volatile components were identified from D. odorifera, D. bariensis, D. latifolia, D. benthamii, D. pinnata and D. cochinchinensis, respectively. Among them, there were 11 unique volatile components of D. odorifera, 6 unique volatile components of D. bariensis, 3 unique volatile components of D. latifolia, 6 unique volatile components of D. benthamii, 8 unique volatile components of D. pinnata, 4 unique volatile components of D. cochinchinensis. The PCA results showed that, except for D. latifolia and D. cochinchinensis, which could not be clearly distinguished, D. odorifera and other counterfeits could be distributed in a certain area, respectively. The OPLS-DA results showed that D. odorifera and its five counterfeits were clustered into one group each, indicating significant differences in volatile components between D. odorifera and its counterfeits. Finally, a total of 31 differential markers of volatile components between D. odoriferae and its counterfeits were screened. ConclusionHS-GC-MS combined with SIMCA 14.1 software can systematically elucidate the volatile differential components between D. odorifera and its counterfeits, which is suitable for rapid identification of them.
