1.Observation on the effect of psychological intervention on emotion and postoperative bladder spasm in elderly patients with benign prostatic hyperplasia

Objective To explore the the effect of psychological intervention on emotion and postoperative bladder spasm in elderly patients with benign prostatic hyperplasia. Methods 40 elderly cases with benign prostatic hyperplasia were classified as the control group and were given the routine intervention, the other 40 cases were received psychological intervention at the basis of routine intervention and classified as the observation group. The two groups were treated in Anji county traditional Chinese medicine hospital from September 2015 to January 2017. Results The anxiety and depression in the 2 groups had no significant difference before intervention, and the prognosis in the observation group was better than that in the observation group after intervention. The bladder spasm in the two groups was compared. The third days after the operation, the incidence of bladder spasm in the observation group was 10.0%, which was significantly lower than 45.0% in the control group, and the difference between the 2 groups was statistically significant (P<0.05). Conclusion Psychological intervention has better effect on mood and bladder spasm in elderly patients with prostate hyperplasia. It can help patients to improve their emotional state, reduce anxiety and depression, and ultimately reduce the incidence of bladder spasm, which is worthy of clinical reference.
2.A study of Hand-assisted Laparoscopic Nephrectomy and Kidney Transplantation in Living Donors

Objectives Hand-assisted laparoscopic nephrectomy in living donors was studied in order to investigate the advantages and disadvantages of this technique as well as the feasibility in clinic use.Methods Hand-assisted laparoscopic live-donor nephrectomy and transplantations were done in 6 canine models,the operating time,warm ischemic time,function of the donor kidney and the recovery of donor were all recorded and analyzed.Results All dogs survival postoperation in our experiment;the mean operating time was 132 minutes,the mean warm ischemic time of kidney was 83 seconds,however,the mean time of micturation after graft reperfusion was 74 seconds.Conclusions Hand-assisted laparoscopic live-donor nephrectomy is feasible with short operating time and warm ischemic time; the grafting function was well and donors recovered quickly postoperation,the minimal trauma;this technique is worth to be widely used in living donor nephrectomy.
3.Intraoperative Ultrasound Monitoring in Anterior Dura Sac Decompression Through Posterior Approach for the Treatment of Thoracolumbar Fracture With Paralysis

Objective\ To observe the practicality of sonography for differentiating the compression factors and judging whether thorough decompression anterior to the dura sac has been obtained. Methods Posterior midline incision was used in a total of 16 cases. After the laminae were resected, normal saline was poured into the operation field, and ultrasound examination performed with a 5.0 MHz sectorscanner. Combined with preoperative imaging (CT or MRI),the compression factors were differentiated. The isolated tissue(bone or disc)pieces in spinal canal were removed first,then reduction of approximate and distal parts of the injuried spinal column was obtained with the help of the Dick devices. Posteriorly protruded bone mass was tumbled in carefully supervised with sonography. The sonographic criterion for thorough decompression consisted of the reappearance of the anterior subarachnoid and an even and smooth echo of posterior dura mater. Results\ The sonographic criterion for thorough decompression was achieved in all cases. No bone mass posterior displacement was observed on postoperative rontgenogram or CT examination. Partial or complete recovery of neural function was obtained in 13 cases. Conclusion\ All elements contributing to anterior dura sac compression can be readily recognized by sonography,which offers a reliable criterion for complete decompression.
4.Effects of scaffold material properties on stem cell differentiation in tissue engineering

BACKGROUND: Increasing evidences support that the physical properties, especially stiffness, can regulate the directional differentiation of stem cells.OBJECTIVE: To review the effects of extracellular matrix on stem cell behaviors, and the research progress concerning the influence of physical properties of biomaterials on stem cell differentiation.METHODS: The first author retrieved CNKI and Web of Science databases using the keywords of extracellular matrix, bioscaffolds materials, chemical property, physical property, substrate rigidity, stem cell differentiation in Chinese and English, respectively. Finally, a total of 31 literatures were included for analysis.RESULTS AND CONCLUSION: The physicochemical properties expose effects on the cell proliferation, migration and differentiation, and especially the regulatory effect of stiffness on the cell differentiation has a revelation for tissue engineering and regenerative medicine. Searching for more biochemical and physical factors interacting with the stiffness of extracellular matrix will enable us to control cell behaviors accurately and to prepare an ideal scaffold closely similar to in vivo environment.
5.Development and biomechanical evaluation of interlocking expanding compressive screws for femoral neck fracture

Objective To explore the methods of internal fixation for femoral neck fractures and evaluate the biomechanical characteristics of the interlocking expanding compressive screw (IECS)of Type HZS. Methods Based on the special biomechanical characteristics of the femoral neck fracture, the device of internal fixation was designed to consist of main screw, bifurcate screw, boost screw cap, washer, compressive screw cap, oblique interlocking screw and main screw cap. Its biomechanical performances were evaluated through comparison with those of the dynamic compressive screw (DCS), twin compressive screw (2CS) and three cannulated screws (3CS) respectively. Results The mechanical performances of the femoral neck fracture fixed with the interlocking expanding compressive screw were superior to those of the others in displacement ,axial and level stiffness, torsional strength and stiffness and terminal loading. Conclusion Interlocking expanding compressive screw can efficiently withstand the splitting, torsional, shearing, tensional and compressive forces in the fracture, and is simple, safe and easy to use. 6.Experience of laparoscopic treatment of ureteral calculi

Objective To summary the therapy outcome of retroperitoneal laparoscopic treatment on upper ureteral calculi.Methods The retroperitoneal laparoscopic treatment were performed to all cases from Jun.2011 to Jun.2012 in our hospital.All cases were treated with 3-hold method (two 10 mm and one 5 mm ports).The retroperitoneal space was made by a combination of blunt and balloon dissection,and the space was maintained with CO2 Ureteral longitudinal incision was made to remove the stones,and double J catheter was served as stent drainage.Absorbable suture was used to suture ureteral incision.Results A retroperitoneal approach was performed in 12 patients,and another patient was conducted the open surgery because the stone cower in the kidney calices.Operative periods ranged from 55 to 132 min (average was 85 min).There were no significant postoperative complications.Conclusion It is a minimally invasive and effective approach in the therapy of upper urerteral calculi with laparoscopic.
7.Polymorphism analysis on chemokine SDF-1α transcript being the ligand for HIV-1 coreceptor CXCR4

Objective To determine the polymorphism in CXC chemokine SDF-1α transcript and its effects on HIV infection.Methods Three groups of study subjects lived in Hang Kong were recruited:278 HIV-heahhy donors of Chinese origin.49 HIV+Caucasians and 13 Chinese with high risk behavior to HIV but kept uninfected.Genomic DNA and RNA were extracted from eripheral blood mononucleal cell. The PCR and RT-PCR reaction were set up accordingly.Sequence of the SDF-1α promoter,the open reading frame(ORF)and the 3'untranslate region(3'UTR)were analyzed.Two steps PCR reaction using two reverseprimers with mismatched nucleic acid were employed to screen the frequency of a novel mutation. Results equencing analysis from 100 subjects indicated that non mutation happened in tlle promoter and ORF of SDF-1α.A novel mutation Was detected from 3'UTR of SDF-1α.It is a "GA" insertion in "G" rich region near the stop code of SDF-1α.The mutation Was named as SDF-1-3’GA+and submitted to GenBank (AY874118).The mutation happened in three roups.with allele frequency of 15.1% in the healthy Chinese.of 30.7% in the high risk Chinese group.Conclusions our results confirm that SDF-1 genes arerelatively conserved.None noteworthy mutation is identified in the promoter and ORF regions of SDF-1α However.a novel mutation is identified from the 3'UTR of SDF-1α. It would be worthwhile to etermine effect of the novel mutation on HIV infection.
8.Isolation and identification of nucleus pulposus mesenchymal stem cells from tissues removed by percutaneous endoscopic lumbar discectomy

AIM:To explore a novel method to isolate human nucleus pulposus mesenchymal stem cells (hNP-MSCs) in vitro and to identify their biological characteristics.METHODS:The explant culture method was employed to isolate hNP-MSCs from nucleus pulposus tissue obtained by percutaneous endoscopic lumbar discectomy (PELD).The isolated cells were passaged for purification and cultured in vitro followed by morphological observation.The cell proliferation ability was detected by CCK-8 assay.Growth curves of the cells were drawn and surface antigens were detected by flow cytometry.The cells at the 3rd~6th passages were induced for adipogenic, osteogenic and chondrogenic differentiation, and examined by oil red O staining, alizarin red staining and Alcian blue staining.RESULTS:The cells with self-renewal were obtained from nucleus pulposus tissue obtained by PELD.The results of flow cytometry analysis revealed that the cells were positive for CD29, CD44, CD90, CD73 and CD105, but negative for CD34 and CD45.The proliferative capacity was consistent with the growth characteristics of MSCs and multilineage differentiation potential was identified.CONCLUSION:A novel method to efficiently isolate and culture hNP-MSCs,PELD combined with explant culture method,was established, which would promote the study of regenerative medicine based on hNP-MSCs.
9.Establishment and evaluation of predictive diagnostic equation for smear negative tuberculosis meningitis

Objective To explore a rapid and accurate method for the diagnosis of smear negative tuberculosis meningitis (TBM). Methods Sixty-seven patients with TBM were selected from Tianjin Haihe Hospital from June 2014 to June 2016, and 118 patients with non-tuberculous meningitis (NTBM) in the same period were chosen as control group, including bacterial meningitis (BM) group (n=61) and viral meningitis (VM) group (n=57). The laboratory routine, biochemical and immune indicators were tested with the specimens of both the blood and cerebrospinal fluid of all the patients. The Logistic regression equation was established for the diagnosis of TBM, and the diagnostic efficacy of which was evaluated by the receiver operating characteristic curve (ROC). Results The predictive regression equations of the TBM with BM, VM and NTBM (BM + VM) were obtained when BM group was used as a control: PRE_BM=1/1 +e-(-5.298+0.196 × ESAT-6+ 0.119 × CFP-10-2.968 × PCT+2.206 × ADA_CSF+0.705 × GLU_CSF+0.093 × LDH_CSF), PRE_VM=1/1+e-(-6.907+0.394 × ESAT-6-0.120 × Na+2.633 × ADA_CSF-0.088 × Cl_CSF) and PRE_NTBM=1/1+e-(0.683+0.099×ESAT-6+0.063×CFP-10-2.645×PCT+1.393×ADA_CSF+1.342×TbAb_CSF)respectively. When BM group was served as a control, the sensitivity, specificity, positive and negative predictive values of the regression for the diagnosis of TBM were 97.01%(89.63%-99.64%), 98.36%(91.20%-99.96%), 98.48%(91.84%-99.96%) and 96.77%(88.83%-99.61%), respectively.When VM group was served as a control, which were 94.03%(85.41%-98.35%), 94.74%(85.38%-98.90%), 95.45%(87.29%-99.05%) and 93.10%(83.27%-98.09%), respectively. When NTBM group was served as control, which were 94.03%(85.41%~98.35%), 90.68%(83.93%-95.25%), 85.14%(74.96%-92.34%) and 96.40%(91.03%-99.01%), respectively. Conclusion The predictive regression equation could be used as early diagnostic TBM with high sensitivity and specificity, which should be popularized in clinical practice, while, according to the higher negative predictive value, the negative results of which could be used to rule out of the TBM and non-empirical medication.
10.Effects of liquid crystal/PU composite substrate on osteogenic differentia-tion of rBMSCs

[ABSTRACT]AIM:Toexploretheeffectoftheelasticmodulusandsizesofliquidcrystal(LC)phasesonosteo-genic differentiation based on OPC/PU composite substrate by mimicking the microenvironment in rat bone mesenchymal stem cells (rBMSCs).METHODS: A series of composite substrates with different elastic modulus were constructed via modulation of LC content in the composites .The surface phase structure was observed by polarized microscopy , and the mechanical property was measured by a universal material testing machine .Furthermore, the laser confocal microscope was employed to observe the spreading , polarization and the cytoskeleton arrangement of the rBMSCs .The proliferation of rBM-SCs was evaluated by CCK-8 assay.The specific mRNA expression of osteogenic differentiation such as collagen Ⅰ, and osteopontin on the composite membranes was detected by real-time PCR.RESULTS:The size and number of LC phase in-creased and the elastic modulus of the composite substrates decreased with the increase of the LC content .The rBMSCs ex-hibited better characteristics of initial adhesion , spreading and proliferation on the OPC 10-PU and OPC30-PU in the early and medium culturing .The rBMSCs displayed higher expression of collagen Ⅰ and osteopontin on the OPC10-PU in the early and medium osteogenic induction , while the high expression of these osteogenic genes occured on the OPC 30-PU and OPC50-PU in later osteogenic induction .The emphasis of genetic expression was switched from collagen Ⅰin the early and medium osteogenic induction to osteopontin in the later stage .CONCLUSION:When the content of LC remained low in the composite substrates , rBMSCs mainly responded to the mechanical stimuli induced by substrate stiffness and exhibited distinguished cellular behaviors;with the increase in the LC content , rBMSCs had strong interactions with LC by sensing the viscoelasticity of LC , probably resulted from the contribution of both substrate stiffness and the viscoelasticity of LC phase .
