1.Realization of disaster backup of infectious disease net reporting system in Chinese Center for Disease Control and Prevention

Objective:To realize the different disaster recovery of infectious disease net reporting system of Chinese Center for Disease Control and Prevention (China CDC) in the same city.Methods:We adopted B/S/DBMS three-tier architecture to deploy the infectious disease net reporting system in the disaster recovery center, realized data level disaster recovery through the remote replication technology, realized application level disaster recovery through technology of the data asynchronous replicating and transmitted disaster recovery data of the infectious disease net reporting system through asynchronous mode. Results: According to the existing resources, we established different disaster recovery center of the same city, realized the disaster recovery data backup of important information system in the same city, completed the security of software and hardware in information security level protection, and passed the level evaluation of Ministry of Public Security.Conclusion: The establishment of disaster recovery center in the off-sete of same city can make sure the security of the system and data, and ensure the continuity of work efficiency.
2.Radiographic Diagnosis of Pneumomediastinum in Neonates

Objective To analyze radiographic characteristics of pneumomediastinum in neonates,and to improve its diagnostic quality. Methods Supine anteroposterior chest film was taken in 22 neonates with pneumomediastinum, additional supine lateral chest film with horizontal projection simultaneously carried out in 16. One or more radiographic follow-ups was made in 14 cases. Spiral CT scanning was done in 4 patients. The imaging findings were retrospectively analyzed.Results Of the total 22 cases, thymic spinnaker-sail sign was seen in 9,transparent area cross the intermediate line of mediastinum in 9,streak and cystic transparent area scattered in the mediastinum in 4. Pneumothorax was accompanied in 12 cases.Conclusion Pneumomediastinum in newborn has its own radiographic characteristics. In most patients occurred thymic spinnaker-sail sign,or transparent area cross the intermediate line of mediastinum and frequently with pneumothorax. It is best shown by integrating supine anterioposterior films with lateral films of horizontal projection. CT scanning can provide more valuable information.
3.Relationship between serum C-reactive protein concentration and muscle strength, physical performance among the aged in Tianjin

Objective The aim of this study was to investigate the relationship between serum C-reactive protein (CRP) and muscle strength, physical performance in the elderly. Methods A cross-sectional study was conducted in Hangu district of Tianjin from April to June in 2013. Totally 1 131 subjects aged 60 years or older completed the survey. Every subject filled out a standard questionnaire and underwent grip strength test, timed up and go test (TUGT). Serum CRP concentrations were determined using chemiluminescent immunoassay kits. Multivariate analysis was performed to determine the relationship between CRP and muscle strength, physical performance. Results After adjustment for potential confounding factors including age, BMI, smoking status, drinking status, hypertension, diabetes, cardiovascular disease, anemia, cancer and physical activity, the multiple linear regression analysis suggested that CRP levels were inversely related to handgrip strength (β'=-0.11, P<0.01), and the logistic regression analysis showed that compared with subjects in the lowest quintile, the odds ratios and 95%confidence interval of higher quintile of CRP were 1.09 (0.50, 2.39) and 2.70 (1.34, 5.66) (Ptrend<0.05) in males. However, the relationship between CRP and muscle strength, physical performance was not statistically significant in females. Conclusions In males, higher serum CRP levels were independently correlated with lower muscle strength and poor physical performance in older adults. These results suggested that inflammation-signaling pathways may play an important role in the progression of sarcopenia.
4.Imaging Diagnosis of Testicular and Epididymal Tumors or Inflammatory Nodes in Children

Objective To analyzed imaging features of testicular and epididymal tumors or inflammatory nodes in children and to improve its diagnostic accuracy.Methods 13 cases underwent ultrasonography,of them,plain CT scan in 10 cases,enhanced CT scan in6 cases.Its signs were retrospectively analyzed and compared with surgical and pathological results.Results On ultrasonography,theinhomogenous and different echo could be viewed in 13 cases,color Doppler flow imaging was abundant in 8 masses,3 cases with littler,empty blood flow in 2 cases and retroperitoneal lymphatic metastasis were viewed in 2 cases.On plain CT scan,masses were mixed density in 4 cases,calcification could be seen within tumor in 3 cases,masses were soft tissue or main soft tissue density in 6 cases.Contrast-enhancedCT scan displayed obviously and inhomogenously in 4 cases,lightly enhancment in 1 case and no enhanced in 1 case.By surgical andpathological confirmed,3 were mature treatomaes,1 was immaturity teratoma,4 were yolk sactumor,3 were inflammatory nodes,1 wasrhabdomyosarcoma and 1 was cystic lymphangioma.Conclusion Each kind of testicular tumors in children has its owns CT and UScharacteristics.In combination of CT and US can carry high diagnostic accuracy.
5.Effect of catheter drainage combined with stoma bags in the management of fecal incontinence associated dermatitis:a Meta-analysis

Objective To evaluate the effect and cost-benefit of catheter drainage combined with stoma bags in the management of fecal incontinence associated dermatitis. Methods Cochrane Library, PubMed, Embase, Web of Science, Chinese Biomedical Medical Disc, China National Knowledge Infrastructure, Wanfang database and VIP database were searched up to March 2016 for the relative randomized controlled trials of catheter drainage combined with stoma bags in the management of fecal incontinence associated dermatitis. Two researchers independently extracted data and evaluated the quality of literature. Then, meta-analysis was performed by using RevMan 5.3 software.Results A total of eight RCTs were involved and 788 patients were included in the research. Meta-analysis showed that compared with the traditional method, catheter drainage combined with stoma bags improved the effective rate of management in patients with fecal incontinence associated dermatitis (RR=1.40,95%CI(1.13,1.72),P=0.002); reduced the incidence rate of incontinence associated dermatitis [RR=0.16, 95%CI(0.09,0.31),P<0.000 01];and reduced nursing time [MD=-0.91, 95%CI(-1.31,-0.51),P<0.000 01].Conclusions Meta-analysis showed that, the effect of catheter drainage combined with stoma bags in the management of fecal incontinence associated dermatitis is better than the routine skin care method. However, the included studies is not enough, so a large sample of high-quality RCTs are needed for further verification.
6.Case report of acromesomelic dysplasia Maroteaux type caused by the NPR2 gene mutation

Clinical data of a child with acromesomelic dysplasia Maroteaux type (AMDM) treated in the Department of Pediatrics, Tianjin Medical University General Hospital at November 2018 was retrospectively analyzed.The female child aged 3 years and 3 months old with 83 cm height (-3.84 SD) had clinical manifestations of disproportionate short stature, disproportionate shortening of forearms and forelegs, and stubby fingers and toes.Gene sequencing identified compound heterozygous mutations, c.1640T>A(p.Val547Asp)/c.682G>A(p.Gly228Ser), in the NPR2 gene, which have not been reported in the Human Gene Mutation Database.Their protein function was predicted harmful.The child was diagnosed as AMDM.During the follow-up until 4 years and 8 months old, the child was 90 cm tall (-4.35 SD), with a growth velocity of 4.9 cm/year.She was treated with recombinant human growth hormone (rhGH) treatment for 9 months and regularly followed up.The child was now 98.2 cm height (-3.07 SD) and she had a growth velocity of 10.9 cm/year.This case report enriched the gene mutation spectrum of AMDM.Treatment with rhGH can effectively improve the height of the child, but the long-term effect needs further follow-up and observation.
7.Chinese localization and test on reliability and validity of Skin Condition Assessment Tool

Objective To localize Skin Condition Assessment Tool (SAT) in Chinese and test on the reliability and validity of scales.Methods Totally 50 incontinence associated dermatitis (IAD) patients who were admitted to Tianjin Medical University General Hospital between July and September 2016 were investigated and assessed in terms of their SAT reliability and validity.Results The validity of SAT re-test was 0.775;the correlation coefficient within the group was 0.977 (P<0.01);the Cronbach′s α coefficient which stands for internal consistency was 0.836;the content validity index (CVI) was 0.93;there was one common factor in factor analysis;and the accumulated variance contribution rate was 78.561%.Conclusions The Chinese version of SAT has less entries and is easy to use. Its result has sound reliability and validity,hence applicable to the assessment of IAD severity.
8.Eukaryotic expression,purification and immunoefficacy evaluation of ferritin nan-oparticles of dengue virus type Ⅱ

The E protein of dengue virus type Ⅱ was presented on ferritin of Helicobacter pylori to construct a novel dengue nanoparticle vaccine candidate,and the immunological indexes of the vac-cine were evaluated,aiming to provide new ideas for the development of dengue vaccine.The re-combinant plasmid of E-Ferritin was optimized and synthesized,and then transfected into HEK-293F cells.The recombinant protein was expressed,identified,purified and analyzed.Mice were im-munized with E-Ferritin nanoparticle vaccine by intramuscular injection on the hind limbs on the day 0,14 and 28.ELISA,neutralization test,flow cytometry and lymphocyte proliferation test were used to detect the levels of specific antibodies,neutralizing antibodies,CD3+,CD4+and CD8+T lymphocytes in spleen cells and the proliferation of spleen lymphocytes after specific stimulation.The target protein with a size of about 69 kDa was expressed in the cells with a single band.The purified protein concentration was 0.407 g/L,and the purity was 82.32%.The results from transmission electron microscopy showed that E-Ferritin protein could be recombined into a particle structure with a particle size of about 50 nm.The results of mouse immune experiments showed that E-Ferritin protein had good immunogenicity.The average specific antibody titer of E-ferritin protein in serum was 1∶92 160 after immunization 42 d.The main subclass of antibody was IgGl.The results of flow cytometry showed that E-Ferritin as an immunogen could induce higher levels of CD4+and CD8+T lymphocyte immune response.In lymphocyte proliferation test,the level of specific stimulation in the vaccine group was significantly higher than that in the non-specific stimulation group.In conclusion,the dengue virus envelope protein ferritin nanoparticle vaccine constructed in this study has good immunogenicity,which can provide reference for the de-velopment of new dengue vaccine candidates.
9.Progress of clinical nutrition support management and corresponding data analysis in a tertiary (A) children's hospital in Shanghai in the past ten years

Objective:To analyze the paradigm and changes of enteral nutrition support for hospitalized children in Shanghai Children's Medical Center affiliated to School of Medicine of Shanghai, Jiao Tong University in last ten years, so as to provide guidance for nutritionists and dietitians on the application and management of enteral nutrition.Method:Relevant data were collected and analyzed from 2011 to 2020 in the Clinical Nutrition Department of Shanghai Children's Medical Center, including the implementation status of parenteral and enteral nutrition as well as the types and distribution of different enteral nutrition formulas and diet.Results:The Clinical Nutrition Department of our hospital provided seventy-five types of diet for hospitalized children. The utilization of ordinary diets and specialized diets has been stable at about 76,000 cases per year. In the past decade, twenty-one ordinary formulas and special formulas were provided every year by the Clinical Nutrition Department in our hospital and the utilization has been stable at 46,000 cases per year. The total consumption of ordinary formulas and the proportion of ordinary formulas users showed a descending trend. The proportion of specialized formulas users among discharged patients remained at 10%, with an upward trend in the proportion of high-calorie formulas and extensively-hydrolyzed formulas.Conclusions:The enteral nutrition formulas and inpatient diets are important components of nutritional treatment. The evolution of its clinical application to some extent reflects the changes of disease spectrum and the development of clinical disciplines. In short, the dietitians play a significant role in the nutrition support team.
10.Risk factors analysis and prediction model construction of major adverse cardiovascular events in pregnant women with valvular heart disease

Objective:To analysis the risk factors for major adverse cardiovascular event (MACE) in pregnant women with valvular heart disease (VHD) and to construct a risk prediction model.Methods:The clinical data of 245 pregnant women with VHD who were hospitalized in Beijing Anzhen Hospital from January 1, 2012, to June 1, 2023 were retrospectively analyzed, including general information, pre-pregnancy and pregnancy-associated cardiac conditions, and MACE. Univariate analysis and logistic regression models were employed to identify risk factors for MACE during pregnancy among pregnant women with VHD. Furthermore, a predictive model was constructed and internal validation was conducted using bootstrap techniques.Results:(1) Among 245 pregnant women with VHD, the incidence of MACE was 18.0% (44/245), and the most common MACE was heart failure (61.4%, 27/44). The mitral valve was the most frequently affected valve (64.9%, 159/245). Prior to pregnancy, the most common type of valve surgery undertaken was mechanical valve replacement, representing 31.4% (77/245) of surgeries. In contrast, among those pregnant women who did not undergo valve surgery before pregnancy, the most common lesion type was mitral regurgitation (17.6%, 43/245). (2) Comparing the maternal and infant outcomes of warfarin, low molecular weight heparin (LMWH) and LMWH sequential with warfarin, the fetal loss rate (36%, 15/42) and malformation rate (7%, 3/42) were the highest, but the MACE rate (12%, 5/42) was the lowest in warfarin group. The fetal loss rate (1/19), malformation rate (1/19) and artificial valve thrombosis rate (0) of LMWH sequential with warfarin were the lowest, and the fetal loss rate and artificial valve thrombosis rate of the three anticoagulation methods were statistically significant (all P<0.05). (3) There were no significant differences in gestational age, age of diagnosis of heart disease, weight at delivery, pre-pregnancy body mass index, proportion of multiparous women and chronic medical history between women with MACE and those without MACE (all P>0.05). (4) Binary logistic regression analysis identified the following as risk factors for MACE during the second trimester of pregnancy among pregnant women with VHD: pre-pregnancy cardiac symptoms, history of corrective surgery for congenital heart disease, pregnancy risk grade Ⅴ, anticoagulation with LMWH during pregnancy, and arrhythmia (all P<0.05). Based on the results of multivariate analysis, a receiver operating characteristic curve was constructed, with an area under the curve of 0.837, indicating good discriminative ability. The calibration plot demonstrated a close alignment between the standard curve and the calibration prediction curve, suggesting excellent calibration of the model. Conclusions:Pregnant women with VHD are at a high risk of experiencing MACE during gestation. Five risk factors, including pre-pregnancy cardiac symptoms, history of corrective surgery for congenital heart disease, pregnancy risk grade Ⅴ, anticoagulation with LMWH, and arrhythmia, could aid in identifying high-risk pregnant women.
