1.Clinical application of double stapling technique in anus-preserving operation of low and middle rectal cancer

Objective:To study the clinical application and postoperative complication of double stapling technique (DST) in anus-preserving operation of low and middle rectal cancer. Method:From April 2002 to April 2006, clinical data of 75 cases with low and middle rectal cancer were analyzed retrospectively. Of those 32 cases used DST underwent sphincter-reserving operation(SPO),regard as study group;and 43 cases used SST received SPO, as control group. Clinical pathologic parameters、 postoperative complication and tumor recurrence were compared between the two groups. 2.Simulation of intestinal content sensory nerves with ultrasonic detection system

BACKGROUND:The existed artificial anal sphincter cannot sense whether there are intestinal contents in the bowel and the state of intestinal contents, and the previous studies on the intestinal content signal did not included the different parts of colon and the signals of different intestinal content status.
OBJECTIVE:To compare the differences of ultrasound signals of different intestinal content status in different parts of New Zealand rabbits detected with transmission-type ultrasonic signal detection system.
METHODS:Twenty healthy New Zealand rabbits were used;the rabbits were divided into groups according to distal part of proximal colon, distal colon and distal rectum. Five positions of solid stool particles were selected randomly as the detection point in each group. The voltage signals of stool were measured under solid, fluid, gaseous, and deflation status of the lumen.
RESULTS AND CONCLUSION:In rectum, there were no significant differences in voltage signals between solid and gaseous conditions, while there were significant differences between fluid and deflation conditions (P=0.000). Multiple comparison showed there were significant differences between four conditions in distal part of proximal colon and distal colon (P=0). There were significant differences in voltage singles between rectum, distal colon and proximal colon under solid and fluid conditions (P=0). Multiple comparison showed there were significant differences in any part of large bowel for solid and fluid status (P=0). There were no significant differences in voltage singles between rectum, distal colon and proximal colon under gaseous and deflation conditions. There was interactive effect of intestinal contents between three parts of large bowel under the four conditions (P=0.000). Transmission-type ultrasonic detection system can distinguish artificial intestinal contents sensory nerves of various intestinal contents. And the application of ultrasonic detection system to detect the intestinal contents under different conditions can provide a new effective method for the research on artificial intestinal content sensory nerves.
3.Curcumin reversal multidrug resistance through downregulation of STAT3 signaling in colon cancer cell lines resistant to oxaliplatin

Objective:To investigate the reversal multidrug resistance effects of curcumin on human colorectal cancer cell lines resistant to oxaliplatin( SW620/OxR) and whether its mechanism was involved in downregulation of STAT3 signaling.Methods: The IC50 value(50%cell growth inhibitory concentrations) of curcumin to SW620/OxR cell lines was obtained by WST-1 reagent,which was one kind of cell proliferation assay.Later experiments included in the experimental group and the control group.In the experimental group,SW620/OxR cell lines were exposed to the previous experiment IC50 concentrations of curcumin plus 2 μmol/L oxaliplatin for 48 h,while in the control group,SW620/OxR cell lines were cultured in medium containing in 2μmol/L oxaliplatin.In the two groups:apoptosis was detected by flow cytometry;the protein expression levels of phosphorylated STAT3(P-STAT3) and P-gp were disclosed by Western blot.Results:The IC50 value of curcumin to SW620/OxR cell lines was 18.9 μmol/L.The apoptosis rate of cells in the control group and the experimental group were respectively ( 5.08 ±1.82 )% and ( 30.69 ±2.94 )%, the apoptosis rate of the experimental group was significantly higher than that of the control group ( P<0.05 ) .The protein expression levels of phosphorylated STAT3(P-STAT3) and P-gp in the experimental group was significantly lower than that in the control group(P<0.05).Conclusion:Curcumin can reverse drug resistance in colorectal cancer cell lines resistant to oxaliplatin, its effect may be associated with downregulation of STAT3 signaling pathways.
4.Expression and significance of Oct4 B1 in colorectal cancer stem cells

Objective:To investigate the expression and its possible role of Oct4B1 subtype of Embryonic stem cell transcription factor Oct4 in colorectal cancer stem cells. Methods: 3D microspheres were cultured by suspension culture to human colorectal cancer cell line SW480 cells. The 3D microspheres and SW480 cells were used as the research objects. Whether 3D microspheres were enriched cancer stem cells,we used the methods of cell differentiation experiments,soft agar cloning experiments,and the expression levels of cancer stem cells markers CD133,CD44 detected by flow cytometry. The expression levels of Oct4B1 mRNA were detected by RT-qPCR. Results:3D microspheres could differentiate into normal cancer cells. Compared with the parental SW480 cells,in vitro colony formation was significantly enhanced(P<0. 01),the percentage of positive cells of CD133 and CD44 were significantly increased ( P < 0. 01 ), the expression levels of Oct4B1 mRNA were obviously higher ( P < 0. 01 ) in 3D microspheres. Conclusion: Oct4B1 subtype of Embryonic stem cell transcription factor Oct4 in 3D microspheres enriched human colorectal cancer stem cells,which may be involved in the regulation of colorectal cancer stem cells.
5. Application of anatomical reduction and fixation of lesser trochanter through posterior-lateral approach in treating intertrochanteric fractures in the elderly

Objective:
To observe the feasibility and clinical curative effect of anatomical reduction and fixation of lesser trochanter through posterior-lateral approach for the treatment of comminuted femoral intertrochanteric fractures in elderly patients.
Methods:
A total of 19 cases with comminuted femoral intertrochanteric fracture, aged 60 to 84 years were enrolled in this study.According to Evans-Jensen classification, 13 cases had type Ⅱ B and 6 cases had type Ⅲ femoral fracture.Through the posterior-lateral approach, the lesser trochanter was fully exposed, reduced and fixed by screw.The proximal and distal femur were well fixed with dynamic hip screw(DHS)or proximal femoral locking plate(PFLP). The lesser trochanter reduction was observed by intraoperative vision and postoperative X-ray.The function evaluation after hip surgery was performed by using Sanders hip function score.
Results:
All 19 patients obtained the anatomic reattachment in the lesser trochanter.All patients were followed up for an average of 18 months.After treatment, the Sanders hip function score were excellent in 7 patients(36.8%), fine in 12 cases(63.2%). And curative efficacy was good in all cases(100%). There was no significant difference in the curative effect between patients with different classification and gender.
Conclusions
Through the posterior-lateral approach, the surrounding structures of the lesser trochanter can be fully exposed, and the lesser trochanter can be anatomically repositioned.The posterior-lateral approach to anatomical reduction and fixation of lesser trochanter is an effective and feasible method for the treatment of comminuted intertrochanteric fracture in the elderly.
6.Effects of general anesthesia combined with epidural anesthesia on oxidative stress in patients with cervical cancer after laparoscopic hysterectomy

Objective To investigate the application value of general anesthesia combined with epidural anesthesia in patients with cervical cancer after laparoscopic hysterectomy.Methods From January 2014 to November 2016,64 patients with cervical cancer in Beijing Ditan Hospital were selected and randomly divided into control group and study group,with 32 cases in each group.Both two groups were treated with laparoscopic hysterectomy,and the control group was treated with general anesthesia,the study group was treated with general anesthesia combined with epidural anesthesia.The amount of general anesthesia,the time of surgery and postoperative recovery time,the incidence of adverse reactions,preoperative and postoperative oxidative stress-related indicators[metabolic enzyme monoammine oxidase A (MAOA),enzyme tyrosine hydroxylase (TH),glutathione peroxidase (GSH-Px),5 -hydroxytryptamine (5 -HT)]were statistically compared between the two groups.Results The operative time,postoperative recovery time and doage of atracurium,remifentanil,propofol in the study group were less than those in the control group (t=6.928,11.205,16.966,7.798,16.839,all P<0.01).There were no statistically significant differences in the MAOA,TH,GSH-Px and 5 -HT levels between the two groups before treatment (all P>0.05),but after operation, the MAOA,TH and 5 -HT levels in the study group were significantly lower than those in the control group,the GSH-Px level was significantly higher than that of the control group,the differences were statistically significant (t=28.585,26.346,10.850,21.827,all P<0.01).The incidence rates of cardiac tachycardia[18.75%(6/32)],vomiting and nausea[15.63%(5/32)],and restless[9.38%(3/32)]in the study group were lower than those in the control group[43.75%(14/32),37.50%(12/32),40.63%(13/32)],the differences were statistically significant (χ2=4.655,3.925,8.333,all P<0.05).Conclusion General hysterectomy combined with epidural anesthesia in patients with cervical cancer after laparoscopic hysterectomy can effectively reduce the postoperative oxidative stress,reduce the dosage of narcotic drugs and the time of operation and wake up,and the incidence rate of adverse reactions is low.
